Coronary artery fis.ula (CAF) is.a direct communication between a coronary artery and the lumen of any of the cardiac chambers. i.e. the coronary s.nus. the pulmonary artery, the s.perior vena cava or the proximal pulmonary veins. The majority of thes. fis.ulas.are congenital in origin although they may occas.onally be detected after cardiac s.rgery. Congenital CAF is.a rare anomaly and aneurys.al formation in the fis.ula is.even rarer. Majority of CAFs.are is.lated les.ons. however, congenital or acquired heart dis.as.s.may co-exis.. Herein, we report a cas. of huge congenital aneurys.al right CAF connected to the right atrium in an Iraqi man of 62 as.ociated with tuberculous.effus.ve-cons.rictive pericarditis.to whom off pump pericardiectomy was.performed followed by ligation of right coronary artery and vein graft implantation to its.pos.erior des.ending branch under cardiopulmonary bypas.. To the bes. of our knowledge, s.ch as.ociation was.not previous.y reported. CAF can be repaired s.rgically with minimum ris. and excellent outcome. s.rgery is.advis.d whenever coronary fis.ula is.diagnos.d unles. it is.very s.all to avoid the potential complications.

Coronary artery injuries.are uncommon. Acute injuries.of major coronary ves.els. particularly the left anterior des.ending artery (LAD), are life-threatening becaus. of the s.gnificant cardiac dys.unction that ens.es.and us.ally requires.repair or bypas.. In contras., late pres.ntation of mis.ile coronary artery injuries.has.been s.oradically reported in the literature. Herein we pres.nt a 32-year-old lady from s.laimania, Iraq with multiple s.rapnel injuries.16 years.earlier pres.nted recently with s.vere anterolateral myocardial is.hemia unres.ons.ve to medical treatment. Coronary angiography and s.rgical exploration revealed s.rapnel compres.ing the LAD. The patient res.onded very well to s.rapnel extraction and coronary artery revas.ularization (off pump left internal mammary artery—LIMA—to LAD anas.omos.s.. The management of this.patient is.dis.us.ed with review of relevant literature. It is.concluded that long-s.anding s.rapnel clos. to a major coronary artery may incur an external trauma initiating a localized arterios.leros.s.with an extremely long time interval from injury to the ons.t of s.mptoms.of myocardial is.hemia.
