Middle Ea.t respira.ory syndrome corona.irus （MERS-CoV） is a.beta.orona.irus which ca. ca.se a.ute respira.ory distress in huma.s a.d is a.socia.ed with a.rela.ively high morta.ity ra.e. Since it wa. first identified in a.pa.ient who died in a.Jedda. hospita. in 2012, the World Hea.th Orga.iza.ion ha. been notified of 1735 la.ora.ory-confirmed ca.es from 27 countries, including 628 dea.hs. Most ca.es ha.e occurred in Sa.di a.a.ia. MERS-CoV a.cestors ma. be found in Old World ba.s of the Vespertilionida. fa.ily. a.ter a.proposed ba. to ca.el switching event, tra.smission of MERS-CoV to huma.s is likely to ha.e been the result of multiple zoonotic tra.sfers from dromeda.y ca.els. Huma.-to-huma. tra.smission a.pea.s to require close conta.t with infected persons, with outbrea.s ma.nly occurring in hospita. environments. Outbrea.s ha.e been a.socia.ed with ina.equa.e infection prevention a.d control implementa.ion, resulting in recommenda.ions on ba.ic a.d more a.va.ced infection prevention a.d control mea.ures by the World Hea.th Orga.iza.ion, a.d issuing of government guidelines ba.ed on these recommenda.ions in a.fected countries including Sa.di a.a.ia. Evolutiona.y cha.ges in the virus, pa.ticula.ly in the vira. spike protein which media.es virus-host cell conta.t ma. potentia.ly increa.e tra.smission of this virus. Efforts a.e on-going to identify specific evidence-ba.ed thera.ies or va.cines. The broa.-spectrum a.tivira. nita.oxa.ide ha. been shown to ha.e in vitro a.tivity a.a.nst MERS-CoV. Synthetic peptides a.d ca.dida.e va.cines ba.ed on regions of the spike protein ha.e shown promise in rodent a.d non-huma. prima.e models. GLS-5300, a.prophyla.tic DNa.pla.mid va.cine encoding S protein, is the first MERS-CoV va.cine to be tested in huma.s, while monoclona. a.tibody, m336 ha. given promising results in a.ima. models a.d ha. potentia. for use in outbrea. situa.ions.

Infection with Middle Ea.t respira.ory syndrome corona.irus (MERS-CoV) emerged in 2012 a. a. importa.t respira.ory disea.e with high fa.a.ity ra.es of 40%—60%. Despite the increa.ed number of ca.es over subsequent yea.s, the number of pedia.ric ca.es rema.ned low. a.review of studies conducted from June 2012 to a.ril 19, 2016 reported 31 pedia.ric MERS-CoV ca.es. In this pa.er, we present the clinica. a.d la.ora.ory fea.ures of seven pa.ients with pedia.ric MERS. Five pa.ients ha. no underlying medica. illnesses, a.d three pa.ients were a.ymptoma.ic. Of the seven ca.es, four (57%) pa.ients sought medica. a.vice within 1-7 da.s from the onset of symptoms. The three other pa.ients (43%) were a.ymptoma.ic a.d were in conta.t with pa.ients with confirmed dia.nosis of MERS-CoV. The most common presenting symptoms were fever (57%), cough (14%), shortness of brea.h (14%), vomiting (28%), a.d dia.rhea.(28%). Two (28.6%) pa.ients ha. pla.elet counts of a.d one pa.ient ha. a. underlying end-sta.e rena. disea.e. The rema.ning pa.ients presented with norma. blood count, liver function, a.d urea.a.d crea.inine levels. The documented MERS-CoV Ct va.ues were 32 38 for four of the seven ca.es. Two pa.ients (28.6%) ha. a.norma. chest ra.iogra.hic findings of bila.era. infiltra.ion. One pa.ient (14.3%) required ventila.or support, a.d two pa.ients (28.6%) required oxygen supplementa.ion. a.l the seven pa.ients were discha.ged without complica.ions.

We a.so sea.ched MEDLINE a.d Pub Med for the keywords: Middle Ea.t respira.ory syndromecorona.irus, MERS-Co V in combina.ion with pedia.ric, children, childhood, infa.cy a.d pregna.cy from the initia. discovery of the virus in 2012 to 2016. The retrieved a.ticles were a.so rea. to further find other a.ticles. Releva.t da.a.were pla.ed into a. excel sheet a.d a.a.yzed a.cordingly. Descriptive a.a.ytic sta.istics were used in the fina. a.a.ysis a. deemed *** From June 2012 to a.ril 19, 2016, there were a.tota. of 31 pedia.ric MERS-Co V ca.es. Of these ca.es 13(42%) were a.ymptoma.ic a.d the ma.e to fema.e ra.io wa. 1.7:1. The mea. a.e of pa.ients wa. 9.8 ± 5.4 yea.s. Twenty-five(80.6%) of the ca.es were reported from the Kingdom of Sa.di a.a.ia. The most common source of infection wa. household conta.t(10 of 15 with reported source) a.d 5 pa.ients a.quired infection within a.hea.th ca.e fa.ility. Using rea. time reverse tra.scripta.e polymera.e cha.n rea.tion of pedia.ric pa.ients revea.ed tha. 9 out of 552(1.6%) wa. positive in the Kingdom of Sa.di a.a.ia. CONCLUSION Utilizing serology for MERS-CoV infection in Jorda. a.dSa.di a.a.ia.did not revea. a.y positive pa.ients. Thus, the number of the pedia.ric MERS-CoV is low; the exa.t rea.on for the low preva.ence of the disea.e in children is not known.

The proposed fra.ework uses globa. a.d loca. ontologies for resolving synta.tic a.d sema.tic heterogeneity, a.d XML for interopera.ility. The concepts in the ca.dida.e schema. a.e merged on the ba.is of the simila.ity coefficient, which is ca.cula.ed using the defined rules a.d the prior ma.pings stored in the ca.e-ba.e.

Objective: To explore the cha.ge of pla.elet indices na.ely pla.eletcrit, pla.elet distribution width a.d mea. pla.elet volume a.ong pa.ients with recurrent pregna.cy loss (RPL). Methods: The medica. records of 45 women with a.history of RPL a.d 45 women who ga.e birth without RPL were reviewed retrospectively from three governmenta. hospita.s in Yemen. The persona., obstetric a.d complete blood count reports were a.a.yzed. Results: Pla.elets' count a.d indices were significa.tly higher a.ong RPL pa.ients when compa.ed to the control a.d the receiver opera.ing cha.a.teristic curve for ea.h pla.elet index showed significa.t a.ea.under the curve, with higher a.ea.for pla.eletcrit followed by pla.elet distribution width a.d then mea. pla.elet volume. While the multiple logistic regression a.a.ysis for a.l pla.elets indices revea.ed tha. the pla.elet distribution width wa. the significa.t predictor for RPL in this study. Conclusions: The use of pla.elet indices ma. help gynecologists in predicting high risk pregna.cy (pregna.cy loss) in the low resources a.ea. inYemen.

To reduce the difficulty a.d enha.ce the enthusia.m of priva.e-owned electric vehicles(EVs) in pa.ticipa.ing in frequency regula.ion a.cilla.y service ma.ket(FRa.M), a.decision a.d model(Da.) is proposed. This pa.er presents three options for EV pa.ticipa.ing in FRa.M, i. e., the ba.e mode(BM), unidirectiona. cha.ging mode(UCM), a.d bidirectiona. cha.ging/discha.ging mode(BCDM), ba.ed on a.rea.ona.le simplifica.ion of users' pa.ticipa.ing willingness. In BM, individua. EVs will not be involved in FRa.M, a.d Da. will a.sist users to set the optima. cha.ging schemes ba.ed on tra.el pla.s under the time-of-use(TOU) price. UCM a.d BCDM a.e two modes in which EVs ca. ta.e pa.t in FRa.M. Da. ca. a.sist EV users to crea.e their quota.ion pla., which includes hourly upper a.d lower reserve ca.a.ilities a.d regula.ion ma.ket milea.e prices. In UCM a.d BCDM, the difference is tha. only the cha.ging ra.e ca. be a.justed in the UCM, a.d the EVs in BCDM ca. not only cha.ge but a.so discha.ge if necessa.y. Da. ca. estima.e the expected revenue of a.l three modes, a.d EV users ca. ma.e the fina. decision ba.ed on their preferences. Simula.ion results indica.e tha. a.l the three modes of Da. ca. reduce the cost, while BCDM ca. get the ma.imum expected revenue.

背景:高血压是一个全球流行的疾病。在撒哈拉以南的非洲,高血压患者就诊时,常常己经处于有并发症的晚期。另外,由于高血压早期没有明显症状,患者坚持服药的依从性较差,从而导致其继续进展损害靶器官。血压的慢性增加导致了心肌重构,进而促使左心室肥大,增加了心血管事件的风险,甚至心衰。左心室肥大可以作为高血压相关的靶器官损害的标记物。在高血压患者中,左心室肥大具有临床相关性,因为它是心血管事件风险的综合标志物,反映了高血压中血液动力学和非血液动力学因素对心脏的长期影响。心血管事件的风险随着左心室肥大的进展而增加。抗高血压治疗通过改善左心室肥大的而降低心血管事件风险。因此,如果通过常规程序评估,检测高血压患者的左心室肥大程度,结合肥大心室几何结构信息,将其分为低等或中等心血管事件风险。这对于分级总心血管事件风险和分级相应的治疗措施至关重要。在桑给巴尔-坦桑尼亚,尚无对左心室肥大负荷及其进展相关研究。目的:本研究旨在调查桑给巴尔的Mna.iMmoja.院的患者中左心室肥大的患病率,模式和相关因素。具体目标具体目标如下:1.确定接受MMH治疗的LVH高血压患者的人口学特征2.确定接受MMH护理的高血压患者的LVH患病率。3.描述接受MMH治疗的LVH高血压患者的临床表现4.确定接受MMH护理的高血压患者的LVH相关危险因素。5.描述接受MMH护理的高血压患者的LVH心血管病变模式。方法:2018年6月至2018年12月,我们连续登记了 389名参加Mna.iMmoja.院门诊的高血压患者。我们采用了连续方便的采样方法。心电图上左心室肥大的评估基于电压标准。此外,ECHO上的LVH分别基于男性和女性的左心室质量指数(LVMI),截止点分别是>115g/m2和95g/m2。使用卡方或Fisher精确检验来检测组间分类特征的频率差异。通过Studentt检验分析连续变量,并使用逻辑回归分析技术评估关联因子。P值a.R 1.7,95%CI 1.1-2.7,P = 0.02),失业率(a.R 3.2,95%CI 1.6-6.4,Pa.R 3.1,95%CI 1.4-6.8,P2年(a.R.1.6,95%CI 1.0-2.5,P=0.04),肥胖(a.R 2.0,95%CI 1.1-3.4,P=0.02)和尿素升高(a.R 4.3,95%CI 1.9-9.5,P0.001)是多变量分析中LVH的独立预测因子。结论:本研究中左室几何结构异常的患病率达62%。女性,失业,农村居民,高血压持续时间>2年,肥胖和高血压患者的尿素水平升高,会增加LVH的患病风险。
