Does size matter for resection of giant versus non-giant hepatocellular carcinoma?A meta-analysis

BACKGROUND Research on long-term survival after resection of giant(≥10 cm)and non-giant hepatocellular carcinoma(HCC)(<10 cm)has produced conflicting *** This study aimed to investigate whether oncological outcomes and safety profiles of resection differ between giant and non-giant *** PubMed,MEDLINE,EMBASE,and Cochrane databases were *** designed to investigate the outcomes of giant vs non-giant HCC were *** primary endpoints were overall survival(OS)and disease-free survival(DFS).The secondary endpoints were postoperative complications and mortality *** studies were assessed for bias using the Newcastle–Ottawa *** 24 retrospective cohort studies involving 23747 patients(giant=3326;non-giant=20421)who underwent HCC resection were *** was reported in 24 studies,DFS in 17 studies,30-d mortality rate in 18 studies,postoperative complications in 15 studies,and post-hepatectomy liver failure(PHLF)in six *** HR was significantly lower for non-giant HCC in both OS(HR 0.53,95%CI:0.50-0.55,P<0.001)and DFS(HR 0.62,95%CI:0.58-0.84,P<0.001).No significant difference was found for 30-d mortality rate(OR 0.73,95%CI:0.50-1.08,P=0.116),postoperative complications(OR 0.81,95%CI:0.62-1.06,P=0.140),and PHLF(OR 0.81,95%CI:0.62-1.06,P=0.140).CONCLUSION Resection of giant HCC is associated with poorer long-term *** safety profile of resection was similar in both groups;however,this may have been confounded by reporting *** staging systems should account for the size differences.

隧道开挖地层反应曲线的外显分析

收敛–约束法理论是应用于隧道支撑设计的简化分析方法,所使用的约束损失是该分析方法的关键因子。基于隧道开挖前进效应与约束损失的关系,建立隧道掘进效应函数,并采用约束损失的递增方法与简易试算表工具,同时依据应力–位移力学理论模式,推导在静水应力条件下圆形隧道开挖于弹塑性地层的解析解,进而提出模拟地层反应曲线弹塑性非线性行为的外显分析法。其分析结果包含约束损失递增方法的计算内容与步骤说明,以及约束损失弹性极限值、塑性半径、弹塑性区的径向应力与径向位移。此分析法与研发的有限元分析结果比较显示,二者呈现相似且一致的非线性趋势。

Physical activity and exercise(PAE)improve quality of life and reduce the effects of chronic *** care physicians(PCPs)play an important role to encourage PAE in *** aim to assess PCPs'current PAE consultation practices and their enablers/barriers in daily clinical *** had 64 PCPs(age[35.3±4.7]y,47 women)that completed self-administered questionnaires on PAE consultation practices,training,and confidence ***(n=42)also completed the International Physical Activity Questionnaire-Short Form to assess their physical activity(PA)*** conducted correlation,one-way analysis of variance and a linear regression to assess the associations between enablers,barriers and PA levels to PAE consultation *** average,PCPs consulted on PAE in 49.7%of their daily clinical *** of PCPs(70%)strongly agreed that more PAE knowledge were needed to increase consultation *** three barriers related(p<0.001)to practices were lack of PAE education(r=0.47),patients’preference of pharmaceutical interventions(r=0.45)and lack of continuing education in PAE for PCPs(r=0.37).Physically active PCPs(health-enhancing PA levels,n=6)gave significantly more daily consultations in PAE,73.2%±21.9%,compared to inactive PCPs(n=13),37.4%22.8%(p=0.013).In our regression output,PCPs who had higher PA levels consulted more on PAE daily(R^(2)=0.38,p<0.001)while controlling for ***,PCPs require more knowledge on PAE and need be physically active themselves to increase PAE consultation for patients in their daily *** education should consider including more PA and exercise topics that may benefit both physicians and their patients.

Inkjet printing of two-dimensional(2D)transition metal dichalcogenide(TMD)nanosheets fabricated by liquid-phase exfoliation(LPE)allows simple,mass-producible,and low-cost photo-electronic *** LPE processes involve toxic and environmentally hazardous solvents;however,dispersants have restricted the extent of applications of 2D-TMD ***,various 2D-TMD nanosheets,including MoS2,MoSe2,WS2,and WSe2,in addition to few-layered graphene,are inkjet-printed using a LPE process based on zwitterionic dispersants in *** with cationic and anionic species are water-soluble,while alkyl chain moieties associated with two ionic species adhere universally on the surface of TMD nanosheets,resulting in high throughput liquid exfoliation of the *** zwitterion-assisted TMD nanosheets in water are successtully employed as an ink without the need for additives to adjust the viscosity and surface tension of the ink for use in an office inkjet printer;this gives rise to A4 scale,large-area inkjet-printed images on diverse substrates,such as metals,oxides,and polymer substrates patchable onto human *** with conductive graphene nanosheet inks allowed the development of mechanically flexible,biocompatible-printed arrays of photodetectors with pixelated MoSe2 channels on a paper exhibiting a photocurrent ON/OFF ratio of approximately 1038 and photocurrent switching of 500 ms.

Microdamage accumulation and adaptation of subchondral bone subjected to intensive cyclic loading are important processes associated with catastrophic bone failure,and joint degeneration in athletic humans and *** the tissue-level,they lead to a spatial variation in bone tissue mineral density(TMD)which affects the response of the bone to mechanical *** the spatial distribution of mechanical load within the subchondral bone is critical for understanding the mechanism of the joint ***,a gradient of TMD and mechanical properties has been reported under unconfined compression in osteochondral *** the present study,we used micro computed tomography(μCT)-based finite element(FE)models of cartilage-bone to investigate the gradient of strain in the subchondral bone(SCB)from the third metacarpal(MC3)condyle of racehorses under simulated in situ ***-destructive mechanical testing of specimens under high-rate compression provided the apparent-level modulus of *** models were analysed using unconfined and confined boundary *** FE-predicted apparent-level gradient of modulus across the SCB thickness correlated well with the experimental results(R^(2)=0.72,p<0.05).The highest strain occurred in the most superficial SCB(0.5–2.5 mm deep to the cartilage-bone interface)under the simulated in-situ compression through articular *** findings of this study provide an estimation for the spatial distribution of mechanical strain within SCB in-situ in the presence of heterogeneous bone tissue which is commonly observed in joints subjected to intensive cyclic loading.
