Advantage of Computed Tomography (CT) Virtual Bronchoscopy in the Evaluation of Children with Suspected Foreign Body Airway

Background & Objectives: Foreign body aspiration (FBA) into the tracheo bronchial tree is a frequent and serious cause of respiratory distress and visit to the pediatrics emergency, principally in patients under 3 years of age. Most foreign bodies are not radio opaque and approximately one third of the children admitted will show normal chest X-ray. Virtual bronchoscopy is a relatively new and non-invasive procedure that provides a three dimensional view of the internal walls of the tracheobronchial tree through the reconstruction of axial images. The objectives of the study are, 1) to study the diagnostic accuracy and advantage of virtual bronchoscopy over rigid bronchoscopy in the evaluation of children with suspected FBA and to plan for early management as it is a non-invasive technique;2) to study the clinical spectrum of children attending with suspected FBA. Methods: An observational study of 37 patients of age 12 yrs and below who underwent Virtual Bronchoscopy for suspected foreign body aspiration in the department of pediatrics and Otorhinolaryngology at MGM Hospital, Warangal is carried out for a period of 1.5 years during 2012-2013. Results: The common age of presentation was 1 - 3 years with male preponderance. History of foreign body was obtained in 64.86% of cases. Normal X-ray was found in 27.02% of cases. In 75.67% FB detected on rigid bronchoscopy was also revealed on virtual bronchoscopy. False positive percentage was 5.40% and false negative was 2.70%. Sensitivity and specificity was 96.5% and 75% respectively. Ground nuts were the commonest foreign bodies aspirated. Conclusion: Virtual bronchoscopy should be considered in cases with suspected foreign body aspiration, when chest X-ray is normal, to avoid needless rigid bronchoscopy. Virtual bronchoscopy is useful in screening cases of occult foreign body as it has sensitivity, specificity, and validity.

A Rare Case of Reparative Granuloma of Nasal Cavity

Giant cell reparative granuloma (GCRG) is a rare, benign tumour that mostly involves the maxilla and mandible. It is rare in the nasal cavity and paranasal sinuses. Here we report a case of GCRG of nasal cavity in 24-year-old man who came with unilateral nasal obstruction, epistaxis and proptosis. CT scan showed soft tissue hypodensity lesion of left nasal cavity. Endoscopic surgical excision and biopsy were done. The pathology was consistent with GCRG. There was no recurrence seen on follow-up for 2 years. A short literature review about diagnosis, clinical behavior, radiological findings and treatment of this tumor entity are given.

Primary Tuberculosis of Tonsil in a Diabetic Patient—A Case Report

Though Tuberculosis is one of the most common causes of ill health and death worldwide, people with diabetes have 2 - 3 times higher risk of tuberculosis when compared with people without diabetes. Though tuberculosis can affect any part of the body, oral and oropharyngeal tuberculosis is rare, but reported. And it’s association with diabetes mellitus because of the decreased host mechanism, is still rare, but reported. A 50-year-old female diabetic who was on insulin therapy came to OPD of MGM hospital, Kakatiya Medical College, Warangal with complaints of severe pain in throat for the last 6 months. On examination, right tonsil was enlarged with granular surface. Left tonsil and rest of the oropharynx were normal. Examination of chest was normal and there was no evidence of pulmonary tuberculosis. Punch biopsy revealed tuberculosis of tonsil. Isolated cases of primary tuberculosis of tonsil without evidence of pulmonary tuberculosis are rare. Presence of diabetes mellitus makes patients 2 - 3 times more vulnerable for tubercular infection.

Clinical Application of a Microbiological Study on Chronic Suppurative Otitis Media

Our prospective study conducted over a period of 9 months includes 100 samples of ear discharge collected from 90 patients suffering from tubo-tympanic type of chronic suppurative otitis media (CSOM). The ear discharge which is collected with sterile swabs is subjected to Gram’s staining and culture of the causative organism. Antibiotic sensitivity test of cultured bacterial growth is undertaken to know the susceptibility of the causative organism. Of the 100 samples, 62 were culture positive where Pseudomonas aeruginosa was the most common pathogen followed by Staphylococcus aureus. Most of the cultured organisms in our study were sensitive to drug Ciprofloxacin. The outcome of our study enabled us to set an empirical medical treatment for an early resolution of ear discharge and inflammation in our patients with CSOM as we could understand the aetiological pathogens and their susceptibility pattern. Effective medical treatment in obtaining a discharge free ear prior to surgical treatment led us to improve the surgical outcome in our patients with CSOM.

A Pierre Robin Syndrome with Absent Anterior 2/3 Tongue—A Case Report

The triad of micrognathia, glossoptosis and airway obstruction originally described in 1923 by Pierre Robin, is known as Robin sequence (or Pierre robin sequence “PRS”). PRS is characterized by micrognathia (small and symmetrical receded mandible), glossoptosis (tongue of variable size falls backwards into the post pharyngeal wall), and cleft palate (U or V shaped). We report a case of 2 hours old newborn presented with micrognathia, retrognathia, and glossoptosis and absent anterior two thirds of tongue.

Comparison of Binding Parameter Constants between Sevelamer HCl Tablets and Renagel Tablets by a Validated Inductively Coupled Plasma-Optical Emission Spectrometry (ICP-OES) Method

Sevelamer Hydrochloride is a crossolinked polymeric amine;it is the active ingredient in Renagel Tablets. Sevelamer Hydrochloride is indicated for the control of hyperphosphatamiea in patients with end-stage renal disease. The binding parameter constants of Sevelamer Hydrochloride were determined using the Langmuir approximation for the dosage form at pH 4.0 and 7.0 by Inductively Coupled Plasma-Optical Emission Spectrometry. An ICP-OES method has been developed to estimate free phosphate in In-Vitro phosphate binding study of Sevelamer HCl Tablets. The method is selective and capable of detecting phosphate in the presence of placebo matrix. The method has been validated with a lower limit of quantitation of 0.2 mM for Phosphate. A linear response function was established for the range of concentrations 0.2 - 25.0 mM (r > 0.99) for Phosphate. The intra and inter day precision values for Phosphate met the acceptance as per Food and Drug Administrations guidelines. Phosphate was stable in the set of stability studies viz. bench-top and autosampler. The developed method was applied to in-vitro phosphate binding studies of Sevelamer HCl Tablets.

Evaluation of Impact Strength and Micro-Hardness of Denture Base Acrylic Resin （PMMA） Part Ⅱ

Poly （methyl methacrylate） is widely used as denture base material. During fabrication of a denture, the physical and mechanical properties are influenced by cure condition. Each cure cycle or fabrication technique is attempts to optimize the properties for a given application. The aim of this study was to compare two types of commercially available denture base materials （heat-cure and self-cure） in their mechanical properties. The samples were prepared according to the daily routine work for sample preparation in dental laboratories. After reaching dough stage the mix packed into dumbbell shaped of stainless steel mould and pressed in a hydraulic bench press for 25 mins at room temperature. For heat cure the polymerization cycle was carried out using water bath, while self cure was done at room temperature. The impact strength was evaluated using Charpy impact test. The hardness test was conducted using a calibrated Vickers hardness tester machine. The lowest impact strength was observed in self-cure denture base material （self cure material 6.2 kJ/m^2 while heat cure 12.69 kJ/m^2. It appears that the tendency of heat cure to fracture was lower than self-cure denture base materials. Heat cure denture base material has significantly higher hardness test values than self-cure denture base material. The observed VHN value of the heat-cure was 20.09 g/mm^2 while the self-cure value was 12.7 g/mm^2. This is may be due to the plasticizer effect of residual monomer which was higher in self curing material as reported in previous work. Generally, the heat cure material showed better properties compared to self cure material.

Degradation Pathway for Pitavastatin Calcium by Validated Stability Indicating UPLC Method

Degradation pathway for pitavastatin calcium is established as per ICH recommendations by validated and stability indicating reverse phase liquid chromatographic method. Pitavastatin is subjected to stress conditions of acid, base, oxidation, thermal and photolysis. Significant degradation is observed in acid and base stress conditions. Four impurities are studied among which impurity-4 is found prominent degradant. The stress samples are assayed against a qualified reference standard and the mass balance is found close to 99.5%. Efficient chromatographic separation is achieved on a BEH C18 stationary phase with simple mobile phase combination delivered in gradient mode and quantification is carried at 245 nm at a flow rate of 0.3 mL min-1. In the developed UPLC method the resolution between pitavastatin calcium and four potential impurities is found to be greater than 4.0. Regression analysis shows an r value (correlation coefficient) of greater than 0.998 for pitavastatin calcium and four potential impurities. This method is capable to detect the impurities of pitavastatin calcium at a level of 0.006% with respect to test concentration of 0.10 mg/mL for a 2-μL injection volume. The developed UPLC method is validated with respect to specificity, linearity & range, accuracy, precision and robustness for impurities determination and assay determination.
