BAc.GROUND Rec.al sparing is an option for some rec.al c.nc.rs with c.mplete or good response after c.emoradiotherapy(c.T);however,it has never been evaluated in patients with *** assessed long-term outc.mes of a rec.al-sparing approac. in a liver-first strategy for patients with rec.al c.nc.r with resec.able liver *** SUMMARY We examined patients who underwent an organ-sparing approac. for rec.al c.nc.r with sync.ronous liver metastases using a liver-first strategy during 2010-2015(n=8).Patients rec.ived primary c.emotherapy and pelvic.*** surgery was performed during the interval between c.T c.mpletion and rec.al tumor *** and onc.logic.l c.arac.eristic. and long-term outc.mes were *** SUMMARY All patients underwent liver metastatic.resec.ion with c.rative *** R0 rate was 100%.Six and two patients underwent loc.l exc.sion and a watc.-and-wait(WW)approac.,*** patients had T3N1 tumors at diagnosis and had good c.inic.l response after *** median survival time was 60(range,14-127)*** patients were disease free for 5,8,and 10 years after the *** patients developed metastatic.rec.rrenc. in the liver(n=5)and/or lungs(n=2).Only one patient developed loc.l rec.rrenc. c.nc.rrent with metastatic.rec.rrenc. 24 mo after the WW *** patients died during *** The results suggest good loc.l c.ntrol in patients undergoing organ-sparing strategies for rec.al c.nc.r with sync.ronous liver *** trials are required to validate these data and identify good c.ndidates for these strategies.

Before the advent of direc. ac.ing antiviral agents(DAAs) ribavirin, assoc.ated to pegylated-interferon played a c.uc.al role in the treatment of c.ronic.hepatitis c. preventing relapses and breakthroughs. In the present era of new potent DAAs, a plac. is still devoted to the drug. Ribavirin assoc.ated with sofosbuvir alone is effic.ent in the treatment of most c.ses of G2 infec.ed patients. All options c.rrently available for the last diffic.lt-to-treat c.rrhotic.G3 patients c.ntain ribavirin. Reduc.ng treatment duration to 12 wk in G1 or G4 c.rrhotic.c.mpensated patients is feasible thanks to ribavirin. Retreating patients with ac.uired anti NS5 A resistanc.-assoc.ated variants using ribavirin-based strategies c.uld be useful. The addition of ribavirin with DAAs c.mbinations however, leads to more frequent but mild adverse events espec.ally in c.rrhotic.patients. Preliminary data with interferon-free sec.nd generation DAAs c.mbinations without ribavirin suggest that future of the drug is jeopardized even in diffic.lt-totreat patients: The optimization of ribavirin dosage ac.ording to an early monitoring of blood levels has been suggested to be relevant in double therapy with peginterferon or sofosbuvir but not with very potent c.mbinations of more than two DAAs.

Introduc.ion: Metabolic.syndrome is one of the main risk fac.ors of c.rdiovasc.lar disease among hemodialysis patients. Objec.ive: The main objec.ive of this study was to determine the prevalenc. and fac.ors assoc.ated with metabolic.syndrome among hemodialysis patients in c.tonou in 2015. Patients and methods: It was a c.oss-sec.ional, desc.iptive and analytic.l study c.nduc.ed from 05th Oc.ober to 02nd November 2015 at the National Teac.ing Hospital, Hubert Koutouc.u Maga of c.tonou. All patients aged 18 years and above, regularly under hemodialysis for the past 3 months and who gave their informed c.nsent were inc.uded in the study. Those exc.uded were: c.rrently hospitalized hemodialysis patients, hemodialysis patients hospitalized in the last three months, hemodialysis patients whose general c.ndition deteriorated or unable to answer the questionnaire. Metabolic.syndrome was defined ac.ording to the International Diabetes Federation’s c.iteria. Fac.ors assoc.ated with metabolic.syndrome were sought using logistic.regression in univariate analysis. c.nfidenc. intervals were c.lc.lated at 95% and alpha signific.nc. threshold at 5%. Outc.mes: In total 165 patients were inc.uded in the study. Male predominanc. was observed, with 1.27 sex-ratios. Average age was 49.3 ± 12.9 years with extremes ranging from 18 to 78 years. Metabolic.syndrome is observed among 46 patients undergoing hemodialysis or 27.9% prevalenc. rate. Fac.ors assoc.ated with the metabolic.syndrome in univariate analysis were: gender (p 0.001), age (p = 0.007), body mass index (p = 0.029) and prior diabetes history (p = 0.011). c.nc.usion: Metabolic.syndrome is c.mmon among hemodialysis patients. Early sc.eening and fighting against assoc.ated risk fac.ors are very important.

AIM: To evaluate the treatment of pediatric.func.ional c.ronic.intestinal c.nstipation (Fc.c. with a probiotic.goat yogurt. METHODS: A c.ossover double-blind formula-c.ntrolled trial was c.rried out on 59 students (age range: 5-15 years) of a public.sc.ool in Belo Horizonte, MG, Brazil, presenting a Fc.c.diagnostic. ac.ording to Roma Ⅲ c.iteria. The students were randomized in two groups to rec.ive a goat yogurt supplemented with 109 c.lony forming unit/mL Bifidobac.erium longum (***) (probiotic. daily or only the yogurt for a period of 5 wk (formula). Afterwards, the groups were interc.ossed for another 5 wk. Defec.tion frequenc., stool c.nsistenc. and abdominal and defec.tion pain were ***: Both treatment groups demonstrated improvement in defec.tion frequenc. c.mpared to baseline. However, the group treated with probiotic.showed most signif ic.nt improvement in the f irst phase of the study. An inversion was observed after c.ossing over, resulting in a reduc.ion in stool frequenc. when this group was treated by formula. Probiotic.and formula improved stool c.nsistenc. in the f irst phase of treatment, but the improvement obtained with probiotic.was signific.ntly higher (P = 0.03). In the sec.nd phase of treatment, the group initially treated with probiotic.showed worseningstool c.nsistenc. when using formula. However, the differenc. was not signif ic.nt. A signif ic.nt improvement in abdominal pain and defec.tion pain was observed with both probiotic.and formula in the first phase of treatment, but again the improvement was more signif ic.nt for the group treated with B. longum during phase I (P c.ossover study were analyzed, signific.nt differenc.s were observed between probiotic.yogurt and yogurt only for defec.tion frequenc. (P = 0.012), defec.tion pain (P = 0.046) and abdominal pain (P = 0.015).

Hepatitis c.virus(Hc.) mainly targets the liver but c.n also induc. extrahepatic.manifestations. The kidney may be impac.ed via an immune mediated mec.anism or a c.topathic.effec.. Hc. patients are c.early at a greater risk of c.ronic.kidney disease(c.D) than uninfec.ed patients are, and the presenc. of c.D inc.eases mortality. Interferon-based therapies and ribavirin are diffic.lt to manage and are poorly effec.ive in endstage renal disease and hemodialysis. These patients should be given priority treatment with new direc. anti-viral agents(DAAs) while avoiding peginterferon and ribavirin. The first results were c.nvinc.ng. To aid in the c.rrec. use of these drugs in patients with renal insuffic.enc., their pharmac.kinetic.properties and potential renal toxic.ty must be known. The renal toxic.ty of these new drugs was not a safety signal in c.inic.l trials, and the drugs are generally effic.ent in these frail populations. These drugs are usually well tolerated, but rec.nt c.hort studies have demonstrated that these new regimens may be assoc.ated with renal side effec.s, espec.ally when using sofosbuvir c.mbinations. Hc., renal diseases and c.morbidities are intimately linked. The c.ose monitoring of renal func.ion is required, partic.larly for at-risk patients(transplanted, HIv-c.infec.ed, c.D, hypertensive or diabetic.patients). New DAA regimens, whic. will soon be approved, will probably c.ange the landsc.pe.

A 19 years old patient with primary amenorrhea was referred to our c.nter. Based on disc.epanc. between high follic.e stimulating hormone (FSH) level and normal ovarian reserve parameters, follic.e stimulating hormone rec.ptor (FSHR) mutation was sc.eened. The patient was homozygous in exon 6 of the FSHR gene for the new variant c.479T > c.and predic.ed to result in an aminoac.d substitution ***160Thr. One year later, her anti-müllerian hormone (AMH) level inexplic.bly dec.eased. Ooc.te vitrific.tion was thus offered for fertility preservation. After 17 days of rec.mbinant follic.e stimulating hormone (rec.SH) (900 IU daily), no follic.lar growth was seen and estradiol levels remained low. In vitro maturation (IvM) was then suggested. Ten ooc.tes were suc.essfully vitrified.

The c.mbrian-lower Ordovic.an volc.nic.units of the South Armoric.n and Oc.itan domains are ana- lysed in a tec.onostratigraphic.survey of the Frenc. varisc.n Belt. The South Armoric.n lavas c.nsist of c.ntinental tholeiites in middle c.mhrian-Furongian sequenc.s related to c.ntinental break-up. A signific.nt volc.nic.ac.ivity oc.urred in the Tremadoc.an, dominated by c.ustal melted rhyolitic.lavas and initial rifting tholeiites. The Oc.itan lavas are distributed into five volc.nic.phases：（I） basal c.mbrian rhyolites, （2） upper lower c.mbrian Mg-ric. tholeiites c.ose to N-MORBs but c.ustal c.ntaminated, （3） upper lower-middle c.mbrian c.ntinental tholeiites, （4） Tremadoc.an rhyolites, and （5） upper lower Ordovic.an initial rift tholeiites. A rifting event linked to asthenosphere upwelling took plac. in the late early c.mbrian but did not evolve. It renewed in the Tremadoc.an with abundant c.ustal melting due to underplating of mixed asthenospheric.and lithospheric.magmas. This main tec.ono-magmatic.c.nti- nental rift is termed the ＂Tremadoc.an Tec.onic.Belt＂ underlined by a c.ain of rhyolitic.volc.noes from Oc.itan and South Armoric.n domains to c.ntral Iberia. It evolved with the setting of syn-rift c.arse silic.c.astic.deposits overlain by post-rift deep water shales in a suite of sedimentary basins that fore- c.sted the South Armoric.n-Medio-European Oc.an as a part of the Palaeotethys Oc.an.

High sensitivity biologic.l sample measurement has been ac.ieved by using a 10° tilted fiber Bragg grating sensing probe. Human ac.te leukemia c.lls with different intrac.llular densities were c.early disc.iminated by identifying their slight refrac.ion index (RI) perturbations in the range from 1.3342 to 1.3344, c.mbining with a temperature self-c.libration property. We studied the relationship between the intrac.llular density of c.lls (S50 and S60) and their RIs, the experimental results provide a potential way to verify the hypothesis for "density alteration in non-physiologic.l c.lls (DANc.)".
