Esophageal duplication cysts:Endosonographic findings in asymptomatic patients

Esophageal duplication cysts are rare inherited lesions usually diagnosed in early childhood.Most of them are found in the mediastinum and manifest themselves as separate masses along or in continuity with the native esophagus.Their prevalence remains unknown and they are treated either surgically or endoscopically.In this report we describe a series of four adult patients in whom esophageal duplication cysts were localised intramurally as masses pressing on the esophageal lumen and who were diagnosed with endoscopic ultrasonography.All patients were initially referred to other centres for upper gastroduodenoscopy due to non-specific dyspeptic symptoms.After finding suspicious lesions in the esophagus their endoscopists referred them for endoscopic ultrasound examination at our centre.In two of the cases lesions mimicked esophageal varices and the other two submucosal tumours.In all four patients endoscopic ultrasonography has shown esophageal duplication cysts.Patients had no symptoms suggesting disease of the esophagus and required no treatment.As the true prevalence of esophageal cysts is unknown,it is very likely that in many patients,like in these four described by us,they may cause no symptoms,remain undetected and require no intervention.Increasing availability of new diagnostic modalities such as endoscopic ultrasonography may change the current view regarding the prevalence of esophageal duplication cysts and prove that they may,in fact,not be such rare findings.

Heterozygous single nucleotide variants(SNVs)or copynumber variant(CNV)deletions,involving the mesenchymal forkhead box family transcription factor gene,FOXF1,or its distant lung-specific enhancer,are responsible for 80%e90%of cases of alveolar capillary dysplasia with misalignment of pulmonary veins(ACDMPV).1 ACDMPV is a lethal lung developmental disorder with severe progressive respiratory failure and persistent pulmonary arterial hypertension(Supplemental Material).Intriguingly,in contrast to point mutations in FOXF1,the ACDMPV-causative CNV deletions arise de novo almost exclusively on the maternal chromosome 16q24.1.

Quantum dynamical maps are defined and studied for quantum statistical physics based on Orlicz spaces.This complements earlier work[26]where we made a strong case for the assertion that statistical physics of regular systems should properly be based on the pair of Orlicz spaces(Lcosh-1,L log(L+1)),since this framework gives a better description of regular observables,and also allows for a well-defined entropy function.In the present paper we"complete"the picture by addressing the issue of the dynamics of such a system,as described by a Markov semigroup corresponding to some Dirichlet form(see[4,13,14]).Specifically,we show that even in the most general non-commutative contexts,completely positive Markov maps satisfying a natural Det ailed Balance condition canonically admit an action on a large class of quantum Orlicz spaces.This is achieved by the development of a new interpolation strategy for extending the action of such maps to the appropriate intermediate spaces of the pair(L∞,L1).As a consequence,we obtain that completely positive quantum Markov dynamics naturally extends to the context proposed in[26].

Objective To check whether health risk impacts of exposure to airborne metals and Benzo（a） Pyrene during episodes of high PM10 concentrations lead to an increased number of lung cancer cases in Poland. Methods In this work, we gathered data from 2002 to 2014 concerning the ambient concentrations of PM10 and PM10-bound carcinogenic Benzo（a）pyrene [B（a）P] and As, Cd, Pb, and Ni. With the use of the criterion of the exceedance in the daily PM10 mass concentration on at least 50% of all the analyzed stations, the PM10 maxima’s were selected. Lung cancer occurrences in periods with and without the episodes were further compared. Results During a 12-year period, 348 large-scale smog episodes occurred in Poland. A total of 307 of these episodes occurred in the winter season, which is characterized by increased emissions from residential heating. The occurrence of episodes significantly （P 〈 0.05） increased the concentrations of PM10-bound carcinogenic As, Cd, Pb, Ni, and B（a）P. During these events, a significant increase in the overall health risk from those PM10-related compounds was also observed. The highest probability of lung cancer occurrences was found in cities, and the smallest probability was found in the remaining areas outside the cities and agglomerations. Conclusion The link between PM pollution and cancer risk in Poland is a serious public health threat that needs further investigation.

The goal of the work was to investigate the concentrations of the 16 US EPA priority polycyclic aromatic hydrocarbons（PAH） bound to submicrometer particles（particulate matter, PM1） suspended in the air of university teaching rooms and in the atmospheric air outside. Two teaching rooms were selected in two Polish cities, Gliwice, southern Poland,and Warsaw, central Poland, differing with regard to the ambient concentrations and major sources of PM and PAH. The variabilities of indoor and outdoor 24-hr concentrations of PM1-bound PAH, the ratio（I/O） of the indoor to outdoor 24-hr concentrations of PM1-bound PAH, probable sources of PAH and the level of the hazard from the mixture of the 16 PAH（ΣPAH） to humans at both sites were analyzed. In both Warsaw and Gliwice, the mean concentrations of PM1-bound ΣPAH were slightly higher in the atmospheric air than in the rooms. The indoor and outdoor concentrations of individual PAH in Gliwice were correlated,in Warsaw – they were not. Most probably, the lack of the correlations in Warsaw was due to the existence of an unidentified indoor source of gaseous PAH enriching PM1 in phenanthrene, fluorene, and pyrene. Although the ambient concentrations of PM1-bound PAH were low compared to the ones observed earlier at both sites, they were much higher than in other urbanized European areas. However, because of low mass share of heavy PAH in ΣPAH, the various indicators of the health hazard from the 16 PAH mixture were low compared to other regions.

28-Carboxymethoxy lupane tritepenoids 3 and 4 were synthesized by alkylation of betulin with the THP protected 2- hydroxyethyl iodide followed by oxidation and reduction. Direct reaction of betulin （5） or betulone （10） with ethyl bromoacetate led to 28-O-acylation, instead of 28-O-alkylation. The targeted compounds 3 and 4 were not cytotoxic at the highest concentration tested （75 μmol/L）, suggesting that elongation of the chain length at the 28-position in both betulinic acid （1） and betulonic acid （2） was detrimental to the cytotoxicity. The acylation products 28-O-bromoacetates （Sa, 8b and 11） and 28-O-methoxyacetate 13 exhibited cytotoxicity against several cancer cell lines tested.

Based on the real case of a frontal precipitation process affecting South China, 27 controlled numerical experiments was made for the effects of hydrostatic and non-hydrostatic effects, different driving models, combinations of initial/boundary conditions, updates of lateral values and initial time levels of forecast, on model predictions. Features about the impact of initial/boundary conditions on mesoscale numerical weather prediction (NWP) model are analyzed and discussed in detail. Some theoretically and practically valuable conclusions are drawn. It is found that the overall tendency of mesoscale NWP models is governed by its driving model, with the initial conditions showing remarkable impacts on mesoscale models for the first I0 hours of the predictions while leaving lateral boundary conditions to take care the period beyond; the latter affect the inner area of mesoscale predictions mainly through the propagation and movement of weather signals (waves) of different time scales; initial values of external model parameters such as soil moisture content may affect predictions of more longer time validity, while fast signals may be filtered away and only information with time scale 4 times as large as or more than the updated period of boundary values may be introduced, through lateral boundary, to mesoseale models, etc. Some results may be taken as important guidance on mesoseale model and its data a.ssimilation developments of the future.
