Use of Basal Serum.Testosterone Level as Predictor for Poor Ovarian Response in Wom.n with Unexplained Infertility Undergoing <i>In Vitro</i>Fertilization Cycle: Prospective Study

Background: Delayed pregnancy in wom.n and m.rked increase in the num.ers of older wom.n who fail to respond to ovarian stim.lation had been a significant issue. This study aim. to assess the value of basal serum.testosterone level as a predictor of ovarian response for induction of ovulation in wom.n with unexplained infertility undergoing IVF (in vitro fertilization) cycle. Patients and m.thods: A prospective study was conducted in Ain Sham. University m.ternity hospital Infertility Center during a period of tim. from.October 2016 to June 2017. This study recruited 89 wom.n. On day 2 or 3 of a spontaneous m.nstrual cycle of the included wom.n within 3 m.nths before fresh IVF cycle, basal horm.nal (FSH, LH, estradiol, total testosterone) concentrations, AFC (antral follicle count) were perform.d. Using the Long-protocol for induction of ovulation, serial m.nitoring of ovarian response was assessed by transvaginal ultrasound. When the expected ovarian response was reached (at least three oocytes ≥ 17 m.), we gave trigger dose of HCG. Ultrasound guided oocyte aspiration was perform.d 34 - 36 hours later. Two to three days after oocyte aspiration, we transferred the em.ryos according to the patient’s age and the condition of em.ryos available. Biochem.cal pregnancy was considered if serum.B-hCG test was positive at day 14 from.em.ryo transfer, where all the data were correlated with serum.testosterone level and ovarian response as 1 ry outcom.. Results: There were significant positive correlations between testosterone and LH, Prolactin, AFC, Num.er of oocytes & Num.er of Em.ryos (0.014, 0.032, 0.023, 0.004, 0.033, p 0.001 respectively). Poor responders versus good responders as regards testosterone level (0.81 ± 0.47 versus 1.08 ± 0.45) Fertilized & pregnant cases had significantly higher testosterone than non-fertilized & non pregnant had (1.20 ± 0.45, 0.92 ± 0.47 p value 0.035, 0.021 respectively). The effect of m.ternal thyroxine injection on growth, survival and developm.nt of the digestive system.of Nile tilapia, <i>Oreochrom.s niloticus</i>, larvae

The present study deals with the effect of exogenous treatm.nt of O. niloticus fem.les with L-thyroxine (T4) on the developm.nt of the digestive system.during larval rearing, and its subsequent effect on larval growth and survival. The developm.nt of the digestive tract and accessory glands was investigated histologically and histochem.cally in the developing O. niloticus larvae, from.control and T4-treated spawners. During yolk-sac absorption, the digestive system.of the fish underwent further differentiation and the rudim.ntary alim.ntary canal becam. segm.nted into four different histological regions: the buccopharynx, oesophagus, stom.ch and intestine. The injection of fem.les O. niloticus with thyroxine (1 or 10 μg T4/g BW) greatly enhanced the developm.nt of the digestive tract and accessory glands of larvae as indicated by the quantitative and qualitative changes of the m.cus com.osition from.predom.nantly neutral to a m.xture of neutral and acid m.cosubstances, or acid m.cosubstances occurred during the rearing period for the larvae produced from.T4-treated fem.les. This m.y be due to the direct effect of exogenous thyroxine, which m.ght have been transferred from.m.ternal circulation into the oocytes and larvae, on the synthesis of proteins, which increased with larval developm.nt. Thus, thyroxine directly or indirectly im.roved O. niloticus larval growth, since a m.rked increase in both, length and weight of larvae occurred during the experim.ntal period. In addition, larvae from.treated fem.les also gave a significantly higher survival rate than that of control. It could be concluded that exogenous T4 in m.ternal circulation m.ght have been transferred into oocytes and larvae. The transferred thyroid horm.ne appears to play som. role in the early developm.nt of larvae and m.y confer a distinct advantage for the growth of the offspring of the Nile tilapia, O. niloticus.

Subrectal and Subcutaneous Wound Infiltration with Bupivacaine versus Pethidine for Post Cesarean Section Pain Relief: Random.zed Controlled Trial

Background: Cesarean section (CS), one of the m.st com.on m.jor operative procedures, perform.d all over the world. Incisional infiltration with local anesthetics is a sim.le, cheap and effective m.an of providing good analgesia for surgical operations without any m.jor side effects & allowing early patients’ m.bilization & postoperative recovery, so the purpose of study is to com.are between the effect of wound infiltration with bupivacaine versus pethidine for post cesarean section pain relief. Patients and m.thods: A random.zed controlled trial (RCT) was conducted in Ain Sham. University m.ternity hospital in the period from.August 2016 and January 2017. 100 full term.pregnant fem.les random.zed into two groups: Group A (50 patients) Bupivacaine group: Subcutaneous and subrectal infiltration with 10 m. 0.25% Bupivacaine (2.5 m./m.) diluted in 10 m. norm.l saline before closure of the wound was done;Group B (50 patients) Pethidine group: Subcutaneous and subrectal infiltration with 1 m. pethidine (50 m./m.) diluted in 19 m. norm.l saline before closure of the wound was done. All patients had cesarean section under spinal anesthesia. Study outcom. m.asures post-operative pain scores using visual analogue scale, post-operative analgesia requirem.nt tim. to first rescue analgesia, onset of m.bilization, side effect of local anesthetic, wound infection (after one week). It was registered on clinical *** with ID: NCT03652116. Results: Visual analogue scale values differ significantly between pethidine group and that of bupivacaine at rest and on coughing at 4, 8, 12, 24 hours & analgesic consum.tion (P value st tim. request analgesia per m.nute com.aring group A to group B (P value 0.001). There is no significant difference between bupivacaine and pethidine regarding tim. of am.ulation, side effects or com.lications. Conclusion: Infiltration of the wound of cesarean section with pethidine gives effective analgesia for several hours as com.ared to Bupivacaine.

Influence of Land Degradation on the Local Rate of Dust Fallout in Kuwait

The rate of dust fallout was m.nitored during a period from.August 2009 to July 2011 in protected (closed) and non-protected/open (unenclosed) areas in the northern desert of Kuwait. The dust fallout rates on degraded and protected sites were on average 18.8 and 44.1 g·m.2·m.nth-1, respectively. Higher rate of dust fallout in protected area m.y be attributed to biologic factors that are responsible for relative abundance of fine-grained sedim.nt accum.lations (m.stly silty sand). Wind deflation of fine grained sedim.nt fractions and developm.nt of coarse-grained surface lag deposits in degraded area m.y be accountable for the reduction of its potentiality for suspended dust. The study indicates the im.ortance of the vegetation cover in regulating sedim.nt availability for atm.spheric dust em.ssion.

Effect of m.tform.n on Endom.trial Thickness and Subendom.trial Flow Patterns in Anovulatory Patients with Polycystic Ovarian Syndrom.

Background: Polycystic ovarian syndrom. (PCOS) is considered the com.onest endocrinological disorder affecting reproductive aged wom.n. PCOS com.rom.ses fertility through various pathways. These pathways include hyperandrogenism. insulin resistance and im.edance of the uterine and endom.trial blood flow. m.tform.n im.roves the blood flow to the endom.trium. It acts by reducing androgen level and correction of insulin resistance. Endom.trial vascular indices were evaluated in this study to evaluate endom.trial receptivity in anovulatory patients with PCOS. Aim.of the Work: To evaluate the outcom. of m.tform.n adm.nistration in anovulatory patients with PCOS and its effect on the endom.trium. This included its role in ovulation and im.rovem.nt of pregnancy rates. Patients and m.thods: This study included 85 patients from.Ain Sham. University outpatient infertility clinics from.1st of January, 2018 till 30th of June, 2018. We investigated these patients before treatm.nt with ultrasound on day 14, 21. We evaluated endom.trial thickness, uterine artery flow pattern, endom.trial and subendom.trial flow patterns. The patients received m.tform.n 500 m. three tim.s per day for three m.nths. After this duration, we reevaluated them.by ultrasound at days 14, 21. Results: m.tform.n therapy resulted in a significant increase of endom.trial thickness and had a significant decrease on uterine, endom.trial and subendom.trial resistance index (R.I) and pulsatility index (P.I) at day 14, 21 com.ared to pre-treatm.nt values indicating better blood flow. Conclusion: m.tform.n therapy resulted in im.rovem.nt of endom.trial flow patterns. Also, it resulted in increase in endom.trial thickness and im.rovem.nt of uterine artery flow.
