BACKGROUND Diverting stoma(DS)is routinely proposed in intersphincteric resection for ultra-low rectal cancer,but it is associated with increased stoma-related complications and economic *** patient selection and operative strategies to avoid stoma formation need further *** This study enrolled 505 consecutive patients,including 84 who underwent stoma-free(SF)intersphincteric *** matching,patients were divided into SF(n=78)and DS(n=78)*** primary endpoint was the anastomotic leakage(AL)rate within 6 months and its protective factors for both the total and SF *** secondary endpoints included overall survival and disease-free *** The AL rate was greater in the SF group than in the DS group(12.8%vs 2.6%,P=0.035).Male sex[(odds ratio(OR)=2.644,P=0.021],neoadjuvant chemoradiotherapy(nCRT)(OR=6.024,P<0.001),and tumor height from the anal verge≤4 cm(OR=4.160,P=0.007)were identified as independent risk *** of the left colic artery(LCA)was protective in both the total cohort(OR=0.417,P=0.013)and the SF cohort(OR=0.312,P=0.027).The female patients who did not undergo nCRT and had preservation of the LCA experienced a significantly lower incidence of AL(2/97,2.1%).The 3-year overall survival or disease-free survival did not significantly differ be-tween the *** Female patients who do not receive nCRT may avoid the need for DS by preserving the LCA without increasing the risk of AL or compromising oncological outcomes.

污染密集型产业发展状况可以揭示城市绿色发展过程,本文利用2000—2020年长江干支流沿岸64个城市的工业产值和地理区位数据,探索污染密集型产业空间格局演化及转移特征。结果表明:(1)污染密集型产业产值在工业总产值中占比逐步下降,2015—2020年产值增速出现负增长,反映产业结构进入转型调整期;(2)从流域特征来看,污染密集型产业自长江下游向中上游转移和扩散的趋势愈发显著,支流沿岸城市成为产业转移的主要承接地;从行业特征来看,污染密集型产业总体上分布较为集中,但多数行业产业集中度随时间变化呈波动下降趋势。(3)产业结构、外向度、劳动力成本和基础设施是驱动长江干支流沿岸污染密集型产业空间布局演化的主要因素。

The reform stems from honesty and determination. Since 2005, organ donation and transplantation in China has undergone thorough reform, which complies with legislation requirements and ethical principles established by the World Health Organization(WHO). Reform in China has demonstrated the unwavering confidence and utmost determination of the Chinese government and the Chinese transplantation community. The year 2015 marked a historic turning point when voluntary donations from Chinese citizens became the sole legitimate source for organ transplantation. Since 2015, China has gradually established and refined the “Chinese Mode” and “China System” for organ donation and transplantation, fulfilling its political pledge of reform, and has garnered international recognition, and fostered a social culture which promotes organ donation. This article reviewed the history of reform on organ donation and transplantation in China, presented a new pattern of establishment of organ donation system in the new era of the country, and the direction of advances in the future.

A mass transport model was developed to predict the transport rate of ions within biofilms, which was experimentally verified using the fluxes of NH4^＋ and Ca^2＋ through the heterotrophic biofilms with the thickness varying from 230 to 1430μm under the effect of external field in the range of-20 V/m to 60 V/m. It is found that the result predicted by the model is in agreement with the experimentally obtained one, with the error less than 5 percent for the thin biofilms. The error increases with the increase of the biofilm thickness. The transport rate of ions caused by electric migration is affected by the charges, field strength, and biofilm thickness and so on.

目的:探讨功能性电刺激联合新Bobath技术在脑卒中后肩痛患者康复中的效果。方法:选取2022~2023年在上海市第五康复医院接受治疗的脑卒中后肩痛患者90例,采用随机数字表法分为试验组(功能性电刺激联合新Bobath技术治疗)和对照组(新Bobath技术治疗)各45例,并比较二者的临床治疗效果、疼痛视觉模拟量表(VAS)评分和运动功能评分量表(Fugl-Meyer)评分、肩关节活动度评分、生活质量评分及患者治疗满意度,探讨其应用价值。结果:试验组治疗总有效率(84.44%)高于对照组(64.44%),差异有统计学意义(P<0.05)。治疗4周后,试验组VAS评分低于治疗前和对照组,Fugl-Meyer评分高于治疗前和对照组,差异均有统计学意义(P<0.05)。治疗4周后,试验组肩关节屈曲、外展、外旋的活动度均大于对照组,差异均有统计学意义(P<0.05)。治疗4周后,两组生活质量评分均高于治疗前,且试验组高于对照组,差异均有统计学意义(P<0.05)。试验组总满意度均高于对照组,差异有统计学意义(P<0.05)。结论:功能性电刺激联合新Bobath技术在脑卒中后肩痛患者康复中的效果较好,治疗后患者满意度较高。

Impai red axonal deve lopment and degeneration underlie many debilitating diseases,including hereditary spastic paraplegia(HSP).HSPs are a heterogeneous group of neurogenetic disorders characterized by axonopathy of cortical motor neurons(Fink,2006;Blackstone et al.,2010).The axonal degeneration of these cortical projection neurons(PNs)in HSP patients disrupts the signals from brain to spinal motor neurons,leading to muscle weakness and *** the discovery of the first HSP gene(SPAST)in 1999,over 80 distinct genetic loci associated with HSP have been *** the mutations of these divergent genes specifically result in axonal degeneration of cortical PNs remains largely unclear,which contributes to the lack of effective treatment to ameliorate,stop,or reverse axonal defects in HSPs.
