BACKGROUND A patient with advanced tuberculosis of the hip joint combined with Crowe type IV developmental dysplasia of the hip(DDH)and a drainage sinus is a rare *** are no previous reports of this condition,and it is a complex challenge for surgeons to develop a treatment *** SUMMARY We report a 73-year-old male patient with severe hip pain and drainage sinus of the left hip for one *** 40 years ago,a drainage sinus occurred at the lateral left hip was healed at the local hospital with anti-infectious *** the sinus healed,gradual pain occurred in the left hip for 40 *** one month prior,hip pain was sharply aggravated,and a drainage sinus reoccurred in the left *** X-ray and computed tomography examinations showed destruction of the head and neck of the left femur,as well as an acetabular *** results of Mycobacterium tuberculosis antibody and Xpert were ***,the patient was diagnosed with advanced TH combined with Crowe type IV *** 22 d of treatment with anti-tuberculosis chemotherapy,the sinus healed,and the patient underwent one-stage total hip arthroplasty(THA)surgery consisting of debridement,osteotomy,and joint *** surgery,the patient received anti-tuberculosis chemotherapy drugs for nine months,with no recurrent *** one year of follow-up,the Harris score of the patient increased from 21 pre-THA to *** Although drainage sinuses are a contraindication to one-stage THA,one-stage THA is still an effective and safe surgical method after the sinus heals.

Failure wave generations on both impacted surface and internal surface inside the sample have been observed for K9 glass under planar shock wave loading,which demonstrates that formation of failure wave is a process related to the surface microcracks nuclei *** on these observations,a hypothesis is suggested that the large local strains resulting from the rearrangement of SiO4 tetrahedral within the permanent densification region behind the shock wave front and then strains releasing due to the surface microcracks developing could be responsible for the failure wave generation.

Portfolio optimization based on network centralities:Which centrality is better for asset selection during global crises?

We construct correlation-based networks linking 86 assets(stock indices,bond indices,foreign exchange rates,commodity futures,and cryptocurrencies)and analyze the impact of asset selection on portfolio optimization using different centrality measures(including degree,eigenvector,eccentricity,betweenness,PageRank,and hybrid centralities).In times of a global crisis,peripheral assets located in cross-market networks are more suitable for *** comparing portfolio performance based on different centrality measures,we find that(i)hybrid,eigenvector,and PageRank centralities can best improve portfolio performance;(ii)degree centrality is suitable for larger portfolios;and(iii)eccentricity and betweenness centralities are unsuitable for network optimization *** response,we explain them based on the construction principle of centrality ***,our optimal portfolios suggest that investors pay more attention to the role of emerging countries,which are less exposed to external shocks and whose financial markets are more likely to remain stable.

A Pair of Novel Primers for Universal Detection of the NS1 Gene from Various Bluetongue Virus Serotypes

Twenty five serotypes of Bluetongue virus (BTV) have been identified worldwide. Rapid and reliable methods of virus universal detection are essential for fighting against bluetongue (BT). We have therefore developed and evaluated a pair of primers which can detect various serotypes of BTV by RT-PCR. Analysis of the viral protein 7 (VP7) and the non-structural protein (NS1) gene from different serotypes of BTV by DNAstar showed that the 5' end of the NS1 gene is the most conserved region. The primer pairs (P1 and P2) were designed based on the highly conserved region of NS1. The novel primers were evaluated by detecting BTV serotypes 1, 3, 5, 8, 10, 11, 21 and 22. The specificity of the primers was estimated by comparing to gene sequences of viruses published in GenBank, and further assessed by detecting BTV serotype 1-12 and Epizootic hemorrhagic disease virus (EHDV) serotype 1-4. The sensitivity and repeatability of PCR with the novel primers were evaluated by successfully detecting the recombinant plasmid pGEM-T121 containing the diagnosed nucleotide sequence. Our results suggest that these unique primers can be used in high throughout and universal detection of the NS1 gene from various BTV serotypes.

针对目前借助复杂的车轨动力学仿真计算铁路线路曲线段轮轨力存在计算量大的问题,在分析铁道车辆曲线通过时运动学关系基础上,忽略车辆摇头和点头运动的影响,构建仅包含垂向、横向和侧滚运动3个自由度的刚体模型;利用质心运动定理和对质心的动量矩定理,建立车辆动力学基本方程组,由此推导出任意超高、任意线型条件下,以线路参数(平面曲线曲率、超高和轨距)和车辆结构参数(车辆质心到轨顶面距离和车辆对质心的回转半径)表达的轮轨力计算式及近似解,从而能较为高效地实现不同线路条件下铁道车辆曲线运动轮轨力预测。

Objective To evaluate the long-term prognosis of patients with ST-segment elevation myocardial infarction(STEMI)treated with different reperfusion strategies in Chinese county-level hospitals Methods A total of 2,514 patients with STEMI from 32 hospitals participated in the China Acute Myocardial Infarction registry between January 2013 and September *** success of fibrinolysis was assessed according to indirect measures of vascular *** primary outcome was 2-year *** Reperfusion therapy was used in 1,080 patients(42.9%):fibrinolysis(n=664,61.5%)and primary percutaneous coronary intervention(PCI)(n=416,38.5%).The most common reason for missing reperfusion therapy was a prehospital delay>12 h(43%).Fibrinolysis[14.5%,hazard ratio(HR):0.59,95%confidence interval(CI)0.44–0.80]and primary PCI(6.8%,HR=0.32,95%CI:0.22–0.48)were associated with lower 2-year mortality than those with no reperfusion(28.5%).Among fibrinolysistreated patients,510(76.8%)achieved successful clinical reperfusion;only 17.0%of those with failed fibrinolysis underwent rescue *** was no difference in 2-year mortality between successful fibrinolysis and primary PCI(8.8%vs.6.8%,HR=1.53,95%CI:0.85–2.73).Failed fibrinolysis predicted a similar mortality(33.1%)to no reperfusion(33.1%vs.28.5%,HR=1.30,95%CI:0.93–1.81).Conclusion In Chinese county-level hospitals,only approximately 2/5 of patients with STEMI underwent reperfusion therapy,largely due to prehospital *** 30%of patients with failed fibrinolysis and no reperfusion therapy did not survive at 2 *** improvement initiatives are warranted,especially in public health education and fast referral for mechanical revascularization in cases of failed fibrinolysis.

Interleukin-8, a promising predictor for prognosis of pancreatic cancer

AIM: To investigate the value of interleukin-8 (IL-8), a pro-inflammatory chemokine, in predicting the prognosis of pancreatic cancer. METHODS: Expression of IL-8 and its receptor CXCR1 was assessed by immunohistochemistry in pancreatic cancer and chronic pancreatitis samples. Enzyme-linked immunosorbent assay was used to detect the serum IL-8 levels in pancreatic cancer patients. Human pancreatic cancer tissues were heterotopically transplanted to the immune-deficiency mice to evaluate the effect of serum IL-8 on the tumorigenesis of the cancer ***: IL-8 and CXCR1 proteins were both overexpressed in pancreatic adenocarcinoma samples (55.6% and 65.4%, respectively) compared with the matched para-cancer tissues (25.9% and 12.3%, P < 0.01), or chronic pancreatitis (0% and 25%, P < 0.05). Serum IL-8 levels in pancreatic cancer patients (271.1 ± 187.7 ng/mL) were higher than in other digestive system tumors, such as gastric cancer (41.77 ± 9.11 ng/mL, P = 0.025), colorectal carcinoma (78.72 ± 80.60 ng/mL, P = 0.032) and hepatocellular carcinoma (59.60 ± 19.80 ng/mL, P = 0.016). In vivo tumorigenesis analysis further proved that tumor tissues from patients with higher serum IL-8 levels grew faster than those with lower IL-8 levels. CONCLUSION: IL-8 can be a fine serum marker for predicting the prognosis pancreatic cancer.

U-Pb Zircon and Re-Os Molybdenite Geochronology of the W-Mo Mineralized Region of South Qinling, China, and their Tectonic Implications

A W-Mo mineralized region is located along the northern margin of the South Qinling tectonic belt of China. WMo mineralization occurs mainly in Cambrian–Ordovician clastic and carbonate rocks, and the ore bodies are structurally controlled by NW–SE-and NNE–SSW-striking faults. Evidence for magmatism in the area is widespread and is dominated by intermediate–felsic intrusives or apophyses, such as the Dongjiangkou, Yanzhiba, Lanbandeng, and Sihaiping granitic bodies. Quartz-vein-type mineralization and fault-controlled skarn-type mineralization dominate the ore systems, with additional enrichment in residual deposits. At present, there are few or insufficient studies on(1) the age of mineralization,(2) the relationship between intermediate–felsic granite and W-Mo mineralization,(3) the source of ore-forming materials, and(4) the metallogenic and tectonic setting of the mineralized area. In this paper, we present geochronology results for numerous intrusive granitic bodies in the South Qinling tectonic belt. U-Pb zircon geochronology of the Lanbandeng monzogranite and Wangjiaping biotite monzogranite yields ages of 222.7 ± 2.3 and 201.9 ± 1.8 Ma, respectively. In contrast to the Late Triassic age of the Lanbandeng monzogranite, the age of the newly discovered Wangjiaping biotite monzogranite places it at the Triassic–Jurassic boundary. Re-Os molybdenite geochronology on the Qipangou W-Mo deposit yielded a model age of 199.7 ± 3.9 Ma, indicating the deposit formed in the early Yanshanian period of the Early Jurassic. Granitoid intrusions in the mineralized area are characterized by composite granite bodies that crystallized at ca. 240–190 Ma. While there were multiple stages of intrusion, most occurred at 210–220 Ma, with waning magmatic activity at 200–190 Ma. Blood Pressure Targets in the Hypertensive Elderly

Objective： Hypertension is an important risk factor of cardiovascular disease and increases mortality in the elderly. However, the available medical evidences are both inconsistent and insufficient regarding establishing credible and useful blood pressure （BP） targets in the hypertensive elderly. This review summarizes the existing evidences used for establishing optimal BP targets for this patient population and points out some data inconsistencies which have added to the uncertainty. Data Sources： We conducted a search for the articles published in English in the PubMed database up to March 2017, with the keywords ＂hypertension,＂＂elderly,＂＂blood pressure,＂ and ＂antihypertensive.＂ Study Selection： Articles that related to BP targeting in the hypertensive elderly were selected for this review. Results： The selected studies indicated that antihypertensive therapy can substantially reduce the risk of cardiovascular events and mortality, for a subset of the elderly （60 years or older） with systolic BP 〉 160 mmHg. Studies regarding more strict targets yielded mixed findings. For the very old and frail patients （80 years or older）, there is a lack of evidence that optimal BP targets and intensive antihypertensives are helpful but in fact may be harmful. Conclusions： There are solid evidences that patients who are 60-80 years old and in good health have benefited from lowering their BP to below 150/90 mmHg. If well tolerated, the BP target can be further lowered to below 140/90 mmHg. However, for the very old and frail, individualized and careful assessment is crucial. Antihypertensive treatment should be cautious and the adverse effect of drugs requires close monitoring as such treatment can be counterproductive.

A new model for predicting irreducible water saturation in tight gas reservoirs

The irreducible water saturation(Swir) is a significant parameter for relative permeability prediction and initial hydrocarbon reserves ***,the complex pore structures of the tight rocks and multiple factors of the formation conditions make the parameter difficult to be accurately predicted by the conventional methods in tight gas *** this study,a new model was derived to calculate Swir based on the capillary model and the fractal *** model incorporated different types of immobile water and considered the stress *** dead or stationary water(DSW) was considered in this model,which described the phenomena of water trapped in the dead-end pores due to detour flow and complex pore *** water film,stress effect and formation temperature were also considered in the proposed *** results calculated by the proposed model are in a good agreement with the experimental *** proves that for tight sandstone gas reservoirs the Swir calculated from the new model is more *** irreducible water saturation calculated from the new model reveals that Swir is controlled by the critical capillary radius,DSW coefficient,effective stress and formation temperature.
