Total parathyroidectomy combined with partial autotransplantation for the treatment of secondary hyperparathyroidism

Background Drug treatment for secondary hyperparathyroidism caused by chronic renal failure may be available at the early stage of the disease, but it is not as effective for serious patients. The aim of the study was to evaluate the effect of total parathyroidectomy combined with forearm autotransplantation in the uremic patients with secondary hyperparathyroidism.Methods From September 1999 through September 2006, parathroidectomy and autotransplantaUon was performed in 20 patients. The coherence between the results of preoperative parathyroid ultrasonography and surgical exploration were compared. The serum calcium concentration and intact parathyroid hormone （iPTH） were monitored preoperatively, intraoperatively, and postoperatively.Results A total of 71 hyperplastic parathyroid glands were resected in the 20 patients. The accordance rate of parathyroid localization between B-ultrasonography and intraoperaUve exploration was 94.4%. The average iPTH value was （110.90±67.42） ng/L, （433.80±243.72） ng/L, （48.80±42.69） ng/L, （229.04±172.68） ng/L and （232.39±224.05） ng/L at day 1, 2, 3, 7, 30 after operation respectively. The clinical symptoms were ameliorated and the levels of serum calcium concentration were controlled within the normal range after operation. Recurrent secondary hyperparathyroidism had happened in 1 case, 4 years postoperatively because of the development of autograft hyperplasia, and in another case 2 years postoperatively due to remnant of neck parathyroid glands. The clinical symptoms were all alleviated after re-operation. No surgical complication had occurred in any of the patients.Conclusions The total parathyroidectomy with forearm autotransplantation is feasible, safe, and effective for patients with secondary hyperparathyroidism in the short term. The long-term effects should be further investigated.

Dual-phase heterointerface electrocatalysts(DPHE)constructed by oxygen reduction reaction(ORR)-and oxygen evolution reaction(OER)-active elements exhibit excellent bifunctional activity and long-term durability due to the abundant interface exposure and synergistic catalytic ***,low-dimensional N-doped graphene nanoribbons(N-GNRs)coupling with ultrathin CoO nanocomposites(N-GNRs/CoO)were controllably fabricated through a facile two-step approach using synthesized Co(OH)_2 nanosheet as CoO *** functional theory(DFT)calculations and experimental characterizations prove that the formation of interface between N-GNRs and CoO can induce local charge redistribution,contributing to the improvement of catalytic activity and *** optimal N-GNRs/CoO DPHE possesses hierarchically porous architectures and presents outstanding bifunctional activities with a small potential gap of 0.729 V between the potential at 10 mA·cm^(-2)for OER and the halfwave potential for ORR,which outperforms Pt/C+IrO_(2)and the majority of noble-metal-free bifunctional ***-and solid-state rechargeable Zn-air batteries assembled with N-GNRs/CoO as the cathode also display high peak power density and fantastic cycle stability,superior to that of benchmark Pt/C+IrO_(2)*** is anticipated to offer significant benefits toward high activity,stability and mechanical flexibility bifunctional oxygen electrocatalysts for rechargeable Zn-air batteries.

Testosterone is closely associated with lipid metabolism and known to affect body fat composition and muscle mass in ***,the mechanisms by which testosterone acts on lipid metabolism are not yet fully understood,especially in *** this study,cyp17a1-/-zebrafish(Danio rerio)exhibited excessive visceral adipose tissue(VAT),lipid content,and up-regulated expression and activity of hepatic de novo lipogenesis(DNL)*** assay for transposase accessible chromatinwithsequencing(ATAC-seq)results demonstrated that chromatin accessibility of DNL genes was increased in cyp17a1-/-fish compared to cyp17a1+/+male fish,including stearoyl-CoA desaturase(scd)and fatty acid synthase(fasn).Androgen response element(ARE)motifs in the androgen signaling pathway were significantly enriched in cyp17a1+/+male fish but not in cyp17a1-/-*** androgen receptor(ar)-/-and wildtype(WT)zebrafish administered with Ar antagonist flutamide displayed excessive visceral adipose tissue,lipid content,and up-regulated expression and activity of hepatic de novo lipogenesis *** Ar agonist BMS-564929 reduced the content of VAT and lipid content,and down-regulated acetyl-CoA carboxylase a(acaca),fasn,and scd ***,the rescue effect of testosterone on cyp17a1-/-fish in terms of phenotypes was abolished when ar was additionally ***,these findings reveal that testosterone inhibits lipid deposition by down-regulating DNL genes via Ar in zebrafish,thus expanding our understanding of the relationship between testosterone and lipid metabolism in teleosts.

Recently,low-cost desktop three-dimensional(3D)printers,employing the fused deposition modeling(FDM)technique,have gained widespread ***,most users cannot test the strength of printed parts,and little information is available about the mechanical properties of printed high-impact polystyrene(HIPS)parts using desktop 3D *** this study,the user-adjustable parameters of desktop 3D printers,such as crisscross raster orientation,layer thickness,and infill density,were *** experimental plans were designed using the Box-Behnken method,and tensile,3-point bending,and compression tests were carried out to determine the mechanical responses of the printed *** prediction models of the process parameters were regressed to produce the optimal combination of process *** experimental results showcase that the crisscross raster orientation has significant effects on the flexural and compression strengths,but not on the tensile *** an increase in the layer thickness,the tensile,flexural,and compression strengths first decreased and then increased,reaching their minimum values at approximately 0.16 mm layer *** addition,they all increased with an increase of infill *** was demonstrated that when the raster orientation,layer thickness,and infill density were 13.08°/–76.92°,0.09 mm,and 80%,respectively,the comprehensive mechanical properties of the printed HIPS were *** results can help end-users of desktop 3D printers understand the effects of process parameters on the mechanical properties,and offer practical suggestions for setting proper printing parameters for fabricating HIPS parts.
