Tree community composition,structure and diversity along an elevational gradient in an Andean forest of Northern Ecuador

The Andean forests of northern Ecuador are known for their high levels of plant diversity relative to the area they ***,these forests grow on steep slopes that lead to dramatic habitat gradients across short *** extreme habitat gradients make the Andean forest ecosystem an excellent natural laboratory for understanding the effect of elevation on forest community diversity,structure and *** established 31 plots(50 m×5 m)which are divided between two elevational transects in the cloud forest of the Siempre Verde Reserve in the western foothills of the Andes Mountains of northern *** trees and tree ferns with a diameter at breast height(dbh)≥5 cm were measured and *** examined changes in community composition,structure,and diversity along and between the elevational transects and three elevational zones:low(2437–2700 m),middle(2756–3052 m),and high(3163–3334 m).We found four main trends associated with the elevational gradients at this site:(1)community composition differed between the two transects and among the three elevational zones according to N-MDS,ANOSIM,and percentage of shared species,with some species having limited distributions,(2)metrics of community structure showed opposite relationships with elevation,depending on the transect,with the only significant relationship(negative)found between basal area and elevation in the open trail transect,(3)alpha diversity,in general,peaked at mid-elevations,and(4)beta diversity consistently increased with distance between plots along *** complexity of changes in community composition,structure,and alpha diversity along elevation may be related to the heterogeneity of the environment on a local scale,such as topography,soil composition,and even human impact,or to dispersal limitation and should be investigated *** changes in community composition and the relatively high beta diversity found at this site exemplify the biological complexity of mon

An opportunity for improvement with a medication reconciliation programme

Medication reconciliation is priority in safety. Between 54% and 67% of hospitalized patients have at least one unintended discrepancies (UDs) between pharmacological treatment of a hospital inpatient and treatment that they were previously taking at home. Quasi-experimental pre-post study performed. The objective is to assess the impact of an intervention aimed at decreasing UDs between medication prescribed on admission and patient’s regular treatment. Patients who were hospitalised for more than 24 hours and were undergoing treatment prior to admission which involved taking three or more medicines were included. The number of patients included was 331 and 3781 medicines were reconciled. The incidence of UDs decreased significantly from 7.24% to 4.18%. Omission was the most common UD, with a significant decrease from 5.8% to 3.4%. Respect to clinical impact, we observed that error type C (error reached patient without causing harm) predominates, how- ever, experienced a statistically significant decrease from 5.3% to 2.4%.

Based on the information that was collected on the County Health Rankings website [1], one of the top health concerns for Maricopa County was sexually transmitted infections or STIs. A big concern with STIs is this is not a common subject explored in educational settings or addressed socially. There are many organizations, including the CDC, that have access to materials, there is limited distribution of the educational content. Therefore, the public is not well informed and lacks the information to make educated decisions. The areas with insufficiencies are knowledge of the disease processes, prevention, and treatment.

Dynamic culture improves MSC adhesion on freeze-dried bone as a scaffold for bone engineering

AIM: To investigate the interaction between mesenchymal stem cells (MSCs) and bone grafts using two different cultivation methods: static and dynamic. METHODS: MSCs were isolated from rat bone marrow. MSC culture was analyzed according to the morphology, cell differentiation potential, and surface molecular markers. Before cell culture, freeze-dried bone (FDB) was maintained in culture for 3 d in order to verify culture medium pH. MSCs were co-cultured with FDB using two different cultivation methods: static co-culture (two-dimensional) and dynamic co-culture (three-dimensional). After 24 h of cultivation by dynamic or static methods, histological analysis of Cell adhesion on FDB was performed. Cell viability was assessed by the Trypan Blue exclusion method on days 0, 3 and 6 after dynamic or static culture. Adherent cells were detached from FDB surface, stained with Trypan Blue, and quantified to determine whether the cells remained on the graft surface in prolonged non-dynamic culture. Statistical analyses were performed with SPSS and a P 0.05). In histological analysis, there was a significant difference in the amount of adhered cells on FDB between the two cultivation methods (P < 0.05). The MSCs in the dynamic co-culture method demonstrated greater adhesion on the bone surface than in static co-culture method. On day 0, the cell viability in the dynamic system was significantly higher than in the static system (P < 0.05). There was a statistical difference in cell viability between days 0, 3 and 6 after dynamic culture (P < 0.05). In static culture, cell viability on day 6

In recent decades,life expectancy has been increasing *** this scenario,health interventions are ne-cessary to improve prognosis and quality of life of elderly with cardiovascular risk factors and cardiovascular ***,the number of elderly patients included in clinical trials is low,thus current clinical practice guidelines do not include specific *** document aims to review prevention recommendations focused in patients≥75 years with high or very high cardiovascular risk,regarding objectives,medical treatment options and also including physical exercise and their inclusion in cardiac rehabilitation ***,we will show why geriatric syndromes such as frailty,dependence,cognitive impair-ment,and nutritional status,as well as comorbidities,ought to be considered in this population regarding their important pro-gnostic impact.

The endemic silverside fish C. humboldtianum is of great ichtyologycal, economical and cultural relevance in central Mexico and it has been suggested that it is among a group of other “peces blancos”, the most ancestral species. Here we characterized a set of 26 microsatellite loci from the species in order to further assess population and phylogeographic issues that aid in evaluating their highly impacted populations. From 58 primer pairs tested on specimens from Villa Victoria dam (Rio Balsas Basin), 26 loci positively amplified on the species and cross-primed on specimens of the closely related and threatened Poblana alchichica, C. promelas and C. riojai. Loci resolved medium and high genetic variation (mean NA = 14.3, SD = 9.38;mean HO & HE = 0.47, SD = 0.32 and 0.58, SD = 0.32, respectively) and it is expected that these polymorphic loci are also useful in studing threatened atherinopsid species.

Clinical and prognostic implications of atrial fibrillation in patients undergoing transcatheter aortic valve implantation

AIM: To study a cohort of consecutive patients under-going transcatheter aortic valve implantation (TAVI) and compare the outcomes of atrial fibrillation (AF) patients vs patients in sinus rhythm (SR). METHODS: All consecutive patients undergoing TAVI in our hospital were included. The AF group comprised patients in AF at the time of TAVI or with history of AF, and were compared with the SR group. Procedural, echocardiographic and follow-up variables were compared. Likewise, the CHA 2 DS 2-VASC stroke risk score and HAS-BLED bleeding risk score and antithrombotic treatment at discharge in AF patients were compared with that in SR patients. RESULTS: From a total of 34 patients undergoing TAVI, 17 (50%) were allocated to the AF group, of whom 15 (88%) were under chronic oral anticoagulation. Patients in the AF group were similar to those in the SR group except for a trend (P = 0.07) for a higher logistic EuroSCORE (28% vs 19%), and a higher prevalence of hypertension (82% vs 53%) and chronic renal failure (17% vs 0%). Risk of both stroke and bleeding was high in the AF group (mean CHA 2 DS 2-VASC 4.3, mean HAS-BLED 2.9). In the AF group, treatment at discharge included chronic oral anticoagulation in all except one case, and in association with an antiplatelet drug in 57% of patients. During a mean follow-up of 11 mo (maximum 32), there were only two strokes, none of them during the peri-procedural period: one in the AF group at 30 mo and one in the SR group at 3 mo. There were no statistical differences in procedural success, and clinical outcome (survival at 1 year 81% vs 74% in AF and SR groups, respectively, P = NS). CONCLUSION: Patients in AF undergoing TAVI show a trend to a higher surgical risk. However, in our cohort, patients in AF did not have a higher stroke rate compared to the SR group, and the prognosis was similar in both groups.

Factors Associated to the Presence of Pneumothorax in Cystic Fibrosis Patients in the City of Madrid

Background: To identify risk factors associated with pneumothorax and to determine the prognosis of cystic fibrosis patients following an episode of pneumothorax in the city of Madrid. Methods: Records of 17 patients (10 males;age 24.4 ± 17.5 years) and 32 controls, and a total of 44 pneumothorax episodes were studied. We have analyzed the characteristics of the pneumothorax, the microbiology, the lung function tests (LFT) and the prognosis of patients. Two controls with cystic fibrosis and without pneumothorax matched for sex and age were selected. Results: Eight male and three female patients with pneumothorax were older than 18 years. The mean age of the first pneumothorax episode was 18.3 years (±9.6). The group with pneumothorax had a mean body mass index of 19.2(±2.42 kg/m2) and in the control group it was 26.5 (±1.98 kg/m2). Pseudomonas aeruginosa was present in fourteen patients (82%) with pneumothorax and in eleven patients (34.4%) in the control group (p = 0.002). Pneumothorax predominantly occurred in the coldest seasons. There was a significant drop in both forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) after the pneumothorax. In the same way, FEV1 and FVC were greater in the control group. Six patients (35.4%) with pneumothorax and two patients in the control group have died (p < 0.05). Conclusions: Patients with pneumothorax are more likely to have P. aeruginosa colonization. LFT drop after an episode of pneumohorax. Patients with pneumothorax have worse LFT than patients without pneumothorax. Mortality is greater in patients with pneumothorax.

Objective:Long-term survivors(LS)of non-small cell lung cancer(NSCLC)without driver alterations,displaying an overall survival(OS)of more than 3 years,comprise around 10%of cases in several series treated with *** are classical prognosis factors for these cases[stage,Eastern Cooperative Oncology Group(ECOG),etc.],but more data are required in the *** this multi-center study,we focused on LS of advanced NSCLC with OS above 36 months to perform a clinical-pathological and molecular ***:In the first step,we conducted a clinical-pathological characterization of the ***,we carried out a genetic analysis by comparing LS to a sample of short-term survivors(SS)(with an OS less than 9 months).We initially used whole-genome RNA-seq to identify differentiating profiles of LS and SS,and later confirmed these with reverse transcription-polymerase chain reaction(RT-PCR)for the rest of the ***:A total of 94 patients were included,who were mainly men,former smokers,having adenocarcinoma(AC)-type NSCLC with an ECOG of *** obtained an initial differential transcriptome expression,displaying 5 over-and 33 under-expressed genes involved in different pathways:namely,the secretin receptor,surfactant protein,trefoil factor 1(T FF1),serpin,Ca-channels,and Tolllike receptor(TLRs)***,RT-PCR analysis of 40(20 LS/20 SS)samples confirmed that four genes(surfactant proteins and SFTP)were significantly down-regulated in SS compared to LS by using an analysis of covariance(ANCOVA)model:SFTPA1(P=0.023),SFTPA2(P=0.027),SFTPB{P=0.02),and SFT PC(P=0.047).Conclusions:We present a sequential genetic analysis of a sample of NSCLCLS with no driver alterations,obtaining a differential RNA-seq/RT-PCR profile showing an abnormal expression of SF genes.
