Evaluation of the long-term safety and effectiveness of tadalafil once daily in Chinese men with erectile dysfunction： interim results of a multicenter, randomized, open-label trial

Once-daily tadalafil administration has been well established; however, studies about tadalafil once-daily treatment in the Chinese population are lacking. In this phase 4, postmarketing study, we ascertained the long-term safety and effectiveness of tadalafil 2.5 mg and 5.0 mg once daily in Chinese men with erectile dysfunction （n = 635）. The primary endpoint of the study was safety at 1. months as assessed by the proportion of patients experiencing at least one treatment-emergent adverse event （serious or nonserious）. The secondary endpoints included safety and effectiveness, measured by the International Index of Erectile Function-Erectile Function （IIEF-EF） domain scores. Similar adverse events to the known safety profile of tadalafil, such as nasopharyngitis, upper respiratory tract infection, headache, and dizziness, were detected. No new cardiovascular safety concerns were observed. After 3 months of treatment, significant increases in IIEF-EF domain scores were detected for both 2.5-mg （least squares [LS] mean change： 6.3; 95% confidence interval [CI]： 5.4-7.1. P 〈 0.001. and 5.0-mg （LS mean change： 7.4; 95% CI： 6.8-7.9; P 〈 0.001. tadalafil doses, and significance was maintained up to 1. months. In addition, approximately 40% of patients regained normal erectile function （IIEF-EF 〉26） following 1.year of tadalafil once-daily treatment. The findings in this study provide evidence for the extended effectiveness and tolerability of tadalafil, demonstrating no new safety concerns, in a Chinese population and make once-daily tadalafil administration a viable option for improving sexual performance and satisfaction in Chinese men with erectile dysfunction.

Background： Urine output （UO） is an essential criterion of the Kidney Disease Improving Global Outcomes （KD1.O） definition and classification system tbr acute kidney injury （AKI）, of which the diagnostic value has not been extensively studied. We aimed to determine whether AKI based on KDIGO UO criteria （KDtGOLro） could improve the diagnostic and prognostic accuracy, compared with KDIGO serum creatinine criteria （KDIGOscr）.Methods： We conducted a secondary analysis of the database of a previous study conducted by china Critical Care Clinical Trial Group （CCCCTG）, which was a 2-month prospective cohort study （July 1.2009 to August 31.2009） involving 3063 patients in 22 tertiary Intensive Care Units in Mainland of china. AKI was diagnosed and classified separately based on KDIGOt,o and KDlGOsc,. Hospital mortality of patients with more severe AKI classification based on KDIGOvo was compared with other patients by univariate and multivariate regression analyses. Results： The prevalence of AKl increased from 52.4% based on KDIGOscr to 55.4% based on KD1.Osc~ combined with KDIGOuo. KDIGOv~~ also restllted in an upgrade of AKI classification in 7.3% of patients, representing those with more severe AK1.classification based on KDIGOvo. Compared with non-AKI patients or those with maximum AKI classification by KDIGOscr, those with maximum AKI classification by KDIGOuo had a significantly higher hospital mortality of 58.4% （odds ratio [OR]： 7.580, 95% confidence interval [CI]： 4.1.1.1..873, P 〈 0.001.. In a multivariate logistic regression analysis, AKI based on KDIGOuo （OR： 2.891. 95% CI： 1.964-4.254, P 〈 0.001., but not based on KDIGOscr （OR： 1.322, 95% CI： 0.902-1.939, P = 0.1.2）, was an independent risk factor for hospital mortality. Conclusion： UO was a criterion with additional value beyond creatinine criterion for AKI diagnosis and classification, which can help identify a group of patients with high risk of death.

In the present study,we aimed to explore the protective effect of spironolactone on cardiac function in patients undergoing coronary stent implantation by determining the serum levels of GATA4 and hypoxia-inducible factor-1.(HIF-1.)proteins before and after the coronary stent implantation.A total of 1.4 patients undergoing coronary stent implantation in our hospital from March 201. to March 2020 were retrospectively selected using the propensity score matching(PSM)*** the 1.4 patients,67 patients taking spironolactone were used as a test group,and the other 67 patients without taking spironolactone were used as a control *** all patients,the levels of serum GATA4,HIF-1.,and troponin I proteins were determined before as well as 24 h and 6 months after the coronary stent *** ventricular ejection fraction(LVEF)and left ventricular systolic global longitudinal strain(GLS)were determined before and 6 months after the coronary stent *** were no significant differences in the HIF-1.level between the two groups before and 6 months after the operation,while the HIF-1.level was significantly lower in the test group compared with the control group at 24 h after the operation(P1..There were no significant differences in the GATA4 protein level between the two groups before and 24 h after the operation,while the GATA4 protein level was significantly lower in the test group compared with the control group at 6 months after the operation(P1..There was no significant difference in LVEF between the two groups before and 6 months after the *** was significantly improved at 6 months after the operation compared with that before the operation in both groups,while GLS was significantly better in the test group compared with the control group at 6 months after the operation(P1..Collectively,spironolactone could protect cardiac function probably by improving myocardial hypoxia and inhibiting myocardial remodeling.

目的:评价在广泛使用条件下黄莪胶囊治疗气虚血瘀、湿热阻滞型良性前列腺增生(BPH)的有效性。方法:605例BPH受试者口服黄莪胶囊治疗,3次/d,4粒/次,疗程为42 d。观察受试者治疗前后国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、平均尿流率(Qave)、生活质量(QOL)评分、前列腺体积变化。结果:共503例受试者完成了整个临床试验,治疗后受试者IPSS下降了(7. 54±4. 61.分,Qmax增加了(2. 05±5. 85) ml/s、Qave增加了(1. 04±3. 1.) ml/s、QOL评分下降了(1. 69±1. 00)分、前列腺体积减小了(2. 80±1.. 02) ml,变化均有统计学意义(P <0. 05)。结论:黄莪胶囊治疗气虚血瘀、湿热阻滞型BPH标本兼顾,有良好的疗效。

AIM:To identify and assess mutations in the K-ras and BRAF genes in a cohort of Chinese patients with colorectal cancer (CRC) for their association with various clinicopathological parameters and ***:Genomic DNA was isolated from frozen *** analysis was conducted to detect mutations in the K-ras (codons 1.,1.,and 61. and BRAF genes (codon 600).Statistical analysis was carried out using SPSS-1..0 ***:Among the 1.8 colorectal cancer patients,we detected 41.(34.7%) mutations in the K-ras *** frequencies at codon 1. and codon 1. were 23.7% (28/1.8) and 1..2% (1./1.8),*** one patient harbored a point mutation at codon 61.(0.8%,1.1.8).Gender was the only factor that showed an obvious relationship with K-ras gene mutation (female 44.7% vs male 28.2%,P=0.037).Other clinicopathological features,such as age,location of the tumor,tumor differentiation,Tumor,Node and Metastases classification,and the Union for International Cancer Control staging,showed no positive relationship with K-ras gene *** significant correlation was observed between the presence of K-ras mutations (codons 1.,1.,and 61. and the survival of the *** mutations were rare,and only two patients (1.7%) harbored a detectable mutation at codon ***:K-ras gene mutation is a common event in our 1.8 Chinese CRC patients,with an obvious relationship with ***,it seems not to be an independent prognostic factor in CRC *** BRAF gene is rarely mutated in Chinese CRC patients.
