Endoscopic retrograde cholangiopancreatography in pancreatic and biliary tract disease in Korean children

AIM:To assess the indications,findings,therapeutic procedures,safety,and complications of endoscopic retrograde cholangiopancreatography(ERCP) performed in Korean ***:The demographic characteristics,indications for ERCP,findings,therapeutic procedures,and complications of 122 pediatric patients who underwent 245 ERCPs in the Asan Medical Center between June 1994 and March 2008 were ***:The mean age of the 122 patients was 8.0 ± 4.2 *** were biliary pathology in 78(64.0%),pancreatic pathology in 43(35.2%),and chronic abdominal pain in *** indications included choledochal cysts in 40,choledocholithiasis in 24,suspected sclerosing cholangitis in 8,trauma in 2,and other conditions in *** indications includedacute pancreatitis in 7,acute recurrent pancreatitis in 11,chronic pancreatitis in 20,trauma in 3,and pancreatic mass in *** the 245 ERCPs,success rate was 98.4% and 190(77.6%) were for therapeutic purposes,including endoscopic nasal drainage(51.8%),biliary sphincterotomy(38.0%),pancreatic sphincterotomy(23.3%),stent insertion(15.1%),stone extraction(18.8%),and balloon dilatation(11.0%).Complications were postERCP pancreatitis in 16(6.5%),ileus in 23(9.4%),hemorrhage in 2(0.8%),perforation in 2(0.8%),sepsis in 1(0.4%),and impacted basket in 1(0.4%).There were no procedure-related deaths,and most complications improved under supportive ***:This study showed that there is a high incidence of choledochal cyst and diagnostic and therapeutic ERCP for the management of various biliary and pancreatic diseases was safe and effective in Korean children.

Changes in Small Intestinal Motility and Related Hormones by Acupuncture Stimulation at Zusanli(ST 36) in Mice

Objective： To clarify the effects of acupuncture stimulation at Zusanli（ST 36） on the hormonal changes. Methods： Eight-week-old male C57BL/6 mice received acupuncture stimulation at acupoint ST 36 or Quchi（LI 11） once a day for 3 or 5 days in the acupuncture-stimulated groups, but not received in the normal group（n=6 in each group）. On day 3 or 5, animals were given 0.1 m L of charcoal orally with a bulbed steel needle, 30 min after the last acupuncture stimulation. Ten minutes later, mice were anesthetized, and the intestinal transit and the concentrations of vasoactive intestinal peptide（VIP）, motilin, ghrelin and gastrin in the serum were measured. Results： Compared to no acupuncture stimulation, acupuncture stimulation at ST 36 for 5 days increased the intestinal transit and down-regulated the concentration of VIP and up-regulated the concentrations of motilin, ghrelin and gastrin（P〈0.05 or 0.01）, whereas acupuncture stimulation at LI 11 did not change them significantly（P〉0.05）. Conclusion： Acupuncture stimulation at ST 36 for 5 days enhances the small intestinal motility and regulates the secretion of hormones related to small intestinal motility.

Flux Vector Splitting Schemes for Water Hammer Flows in Pumping Supply Systems with Air Vessels

To solve water hammer problems in pipeline systems,many numerical simulation approaches have been developed. This paper improves a flux vector splitting( FVS) scheme whose grid is the same as the fixedgrid MOC scheme. The proposed FVS scheme is used to analyze water hammer problems caused by a pump abrupt shutdown in a pumping system with an air vessel. This paper also proposes a pump-valve-vessel model combining a pump-valve model with an air vessel model. The results show that the data obtained by the FVS scheme are similar to the ones obtained by the fixed-grid method of characteristics( MOC). And the results using the pump-valve-vessel model are almost the same as the ones using both the pump-valve model and the air vessel model. Therefore,it is effective that the proposed FVS scheme is used to solve water hammer problems and the pump-valve-vessel model replaces both the pump-valve model and the air vessel model to simulate water hammer flows in the pumping system with the air vessel.

Computing the User Experience via Big Data Analysis:A Case of Uber Services

As of 2020,the issue of user satisfaction has generated a significant amount of ***,we employ a big data approach for exploring user satisfaction among Uber *** develop a research model of user satisfaction by expanding the list of user experience(UX)elements(i.e.,pragmatic,expectation confirmation,hedonic,and burden)by including more elements,namely:risk,cost,promotion,anxiety,sadness,and ***,we collect 125,768 comments from online reviews of Uber services and perform a sentiment analysis to extract the UX *** results of a regression analysis reveal the following:hedonic,promotion,and pragmatic significantly and positively affect user satisfaction,while burden,cost,and risk have a substantial negative ***,the influence of expectation confirmation on user satisfaction is not ***,sadness,anxiety,and anger are positively related to the perceived risk of *** with sadness and anxiety,anger has a more important role in increasing the perceived burden of *** on these findings,we also provide some theoretical implications for future UX literature and some core suggestions related to establishing strategies for Uber and similar *** proposed big data approach may be utilized in other UX studies in the future.

Complications and management of forgotten long-term biliary stents

AIMTo evaluate complications and management outcomes of retained long-term plastic biliary *** plastic biliary stent placement was performed in 802 patients at Yeungnam University Hospital between January 2000 and December 2014. Follow-up loss with a subsequently forgotten stent for more than 12 mo occurred in 38 patients. We retrospectively examined the cause of biliary stent insertion, status of stents, complications associated with biliary stents and management outcomes of long-term plastic biliary stents. Continuous variables were analyzed using the t test. Observed frequencies in subsets of the study population were compared using Fisher’s exact test and χ2 tests. Statistical significance was defined as P < 0.05 (two-tailed).RESULTSMean age of patients was 73.7 ± 12 years and male-to-female ratio was 2.2:1. Indications of plastic biliary stent insertion were bile duct stones (63.2%, 24/38) and benign bile duct stricture (52.6%, 20/38). Mean duration of retained plastic stent was 22.6 ± 12.2 mo, and in 10 cases (26.3%), stents were retained for more than 24 mo. Common bile duct (CBD) stones or sludge were found in most cases (92.1%, 35/38). The most common complication was acute cholangitis (94.7%, 36/38). Stent removal by endoscopic approach was successfully performed in 92.1% (35/38) of the cases. In 3 cases, an additional plastic stent was inserted alongside the previous stent due to failure of the stent removal. Endoscopic removal of bile duct stones was successful in 73.7% (28/38) of the cases. When patients were divided into two groups by duration of stent placement (12 to 24 mo vs over 24 mo), there were no differences in the development of cholangitis, presence of biliary stones, and success rate of endoscopic removal of stones and biliary *** most common complication of retained long-term plastic biliary stents was acute cholangitis associated with CBD stones. Velocity Profiles between Two Baffles in a Shell and Tube Heat Exchanger

Heat exchangers are extensively utilized for waste heat recovery,oil refining,chemical processing,and steam *** this study,velocity profiles are measured using a 3D particle image velocimetry(PIV)system between two baffles in a shell and tube heat exchanger for parallel and counter *** PIV and computational fluid dynamics results show the occurrence of some strong vectors near the *** vectors are assumed due to the clearance between the inner tubes and the front ***,the major parts of the vectors are moved out through the bottom opening of the rear baffle,and other vectors produce a large circle between the two *** simulations are conducted to investigate the effects of the baffle on the heat exchanger using the Fluent *** k-εturbulence model is employed to calculate the flows along the heat exchanger.
