Outcomes following minimally invasive dorsal cheilectomy for hallux rigidus:A systematic review

BACKGROUND Cheilectomy of the 1^(st)metatarsophalangeal joint(MTPJ)is one of the most common procedures for the management of hallux rigidus.However,there is no consensus regarding outcomes following minimally invasive dorsal cheilectomy(MIDC)for the management of hallux rigidus.AIM To evaluate outcomes following MIDC for the management of hallux rigidus.METHODS During November 2023,the PubMed,EMBASE and Cochrane Library databases were systematically reviewed to identify clinical studies examining outcomes following MIDC for the management of hallux rigidus.RESULTS Six studies were included.In total,348 patients(370 feet)underwent MIDC for hallux rigidus at a weighted mean follow-up of 37.9±16.5 months.The distribution of patients by Coughlin and Shurna's classification was recorded in 4 studies as follows:Ⅰ(58 patients,27.1%),Ⅱ(112 patients,52.3%),Ⅲ(44 patients,20.6%).Three studies performed an additional 1^(st)MTPJ arthroscopy and debridement following MIDC.Retained intra-articular bone debris was observed in 100%of patients in 1 study.The weighted mean American orthopedic foot and ankle society score improved from a preoperative score of 68.9±3.2 to a postoperative score of 87.1.The complication rate was 8.4%,the most common of which was persistent joint pain and stiffness.Thirty-two failures(8.7%)were observed.Thirty-three secondary procedures(8.9%)were performed at a weighted mean time of 8.6±3.2 months following the index procedure.CONCLUSION This systematic review demonstrated improvements in subjective clinical outcomes together with a moderate complication rate following MIDC for the management of hallux rigidus at short-term follow-up.A moderate reoperation rate at short-term follow-up was recorded.The marked heterogeneity between included studies and paucity of high quality comparative studies limits the generation of any robust conclusions.

古人类居住环境和农业发展与当地的地貌环境与气候环境密切相关.然而,不同地貌环境和气候背景下的史前农业发展时空模式依然不清楚.文章通过对山西省滹沱河流域34个新石器晚期-青铜时代的遗址进行植物考古研究和碳十四年代分析,同时对比山西省涑水河流域及河南省地区已发表的植物考古结果,探讨了中国中北部地区不同地貌和气候环境下农业发展模式的异同.结果显示,虽然小麦和大麦在4000a BP前后已传入滹沱河流域,但先民在庙底沟文化(6500~5700cal.a BP)至商周时期(3600~2256cal.a BP)的农业模式一直是以种植粟黍为主的北方旱作农业.滹沱河流域、涑水河流域以及河南省一些地区的气候和浅山丘陵地貌条件仅能支持先民在7000~3000a BP期间发展以种植粟黍为主的北方旱作农业.然而,河南省一些其他地区的气候和冲积平原地貌条件却能够促使先民在7000~4000a BP期间发展以种植粟、黍、水稻和大豆为主的混合农业,在4000~3000a BP期间小麦作物也成为其中之一.三个地区先民不同的农业技术创新和对外来文化的接纳程度可能也是三个地区形成不同农业发展模式的原因.7000~3000a BP期间人口爆炸式增长可能是三个地区先民主要种植高产的粟作物而不是低产的黍作物的原因.然而,滹沱河流域炭化黍出土比例较高(相对涑水河流域和河南省地区)反映了其良好的抗旱性,代表了河谷地貌的一种农业适应模式.

Spatiotemporal variation of agricultural patterns in different geomorphologic and climatic environments in the eastern Loess Plateau,north-central China during the late Neolithic and Bronze Ages

Human settlement and agricultural development are closely linked to local geomorphological and climatic environments.However,the variation in agricultural systems in different environmental and prehistoric contexts remains unknown.We report new archaeobotanical and radiocarbon dates from 34 Neolithic and Bronze Age sites in the Hutuo River Valley(HTRV)in north-central China and compare them with updated archaeobotanical studies in the Sushui River Valley(SSRV)and Henan Province(HNP),to explore the similarities and differences of agricultural patterns under different geomorphologic and climatic environments in north-central China.Our results reveal that humans consistently cultivated foxtail and broomcorn millet in the HTRV from the Miaodigou(6000–5700 cal.a BP)to the Shang-Zhou(3600–2256 cal.a BP)period,despite the introduction of wheat and barley into the area around 4000 a BP.Climate conditions and hilly landforms in the HTRV,SSRV and some parts of the HNP led people to develop foxtail and broomcorn millet dry land farming practices between 7000 and 3000 a BP.Alternatively,in other areas of the HNP,the climate conditions and alluvial plains enabled people to develop a mixed agriculture of millets,soybeans,and rice from 7000 to 4000 a BP,with the addition of wheat between 4000 and 3000 a BP.Farmers’different agricultural technologies and interactions with foreign cultures may have also influenced the formation of different agricultural patterns in the three regions between 7000 and 3000 a BP.Population growth during 7000–3000 a BP can explain the overall propensity of the higher-yield foxtail millet rather than broomcorn millet as the main cultivated crop.In the HTRV,however,higher proportions of broomcorn millet(as compared to the SSRV and HNP),may reflect the greater drought and heat tolerance of broomcorn millet,representing an adaptive agricultural strategy in the river valley.

Alignment of the hindfoot following total knee arthroplasty: A systematic review

BACKGROUND There appears to be a close relationship between deformities at the knee joint and at the hindfoot in patients with knee osteoarthritis(OA).Despite this intrinsic link,there is a dearth of studies investigating alterations in hindfoot alignment following total knee arthroplasty(TKA)in patients with knee OA.AIM To evaluate changes in alignment of the hindfoot following TKA,foot and ankle clinical outcomes in terms of subjective clinical scoring tools following surgical intervention,and to analyse the level of evidence(LOE)and quality of evidence(QOE)of the included studies.METHODS MEDLINE,EMBASE and Cochrane Library databases were systematically reviewed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.Studies reporting changes in the postoperative alignment of the hindfoot following TKA were included.The level and QOE were recorded and assessed.RESULTS Eleven studies with a total of 1142 patients(1358 knees)met the inclusion/exclusion criteria.Six studies were of LOE II and 5 studies were of LOE III.Patients with preoperative varus knee deformity and valgus hindfoot deformity demonstrated improvement in hindfoot alignment post TKA.Patients with preoperative varus knee deformity and varus hindfoot deformity demonstrated no improvement in hindfoot alignment following TKA.Twelve different radiographic parameters were used to measure the alignment of the hindfoot across the included studies,with the tibio-calcaneal angle most frequently utilised(27.3%).CONCLUSION This systematic review demonstrated that the hindfoot may display compensatory changes in alignment following TKA in patients with knee OA.However,the marked heterogeneity between the included studies and poor QOE limits any meaningful cross sectional comparisons between studies.Further,well designed studies are necessary to determine the changes and outcomes of hindfoot alignment following TKA.

Indicators of global sustainable sourcing as a set covering problem:an integrated approach to sustainability

Sustainability describes a broad set of themes centered on current human uses of the planet’s resources.The multiple uses and users of the term have led to a proliferation of salient issues and associated indicators.We present a new method to systematically link these issues and indicators under two conceptual frameworks of sustainability in order to enable quantitative analyses.We demonstrate this approach with a specific use case focused on the global sourcing of agricultural products.We use the optimization software Marxan in a novel way to develop minimum sets of indicators that provide maximum coverage of sustainability issues.Minimum covering sets were identified and accumulation curves were developed to measure the contribution of each indicator in each set to overall issues coverage.While greater detail in the assessment of each indicator would likely provide more effective sets of indicators,those that were generated provide optimism that this approach can bring better focus to sustainability assessments.

Rationale and Objectives: Cystic lung disease may be accurately diagnosed by imaging interpretation of specialist radiologists, without other information. We hypothesized that with minimal training non-specialists could perform similarly to specialist physicians in the diagnosis of cystic lung disease. Methods: 72 cystic lung disease cases and 25 cystic lung disease mimics were obtained from three sources: 1) a prospective acquired diffuse lung disease registry, 2) a retrospective search of medical records and 3) teaching files. Cases were anonymized, randomized and interpreted by 7 diffuse lung disease specialists and 15 non-specialist radiologists and pulmonologists. Clinical information other than age and sex was not provided. Prior to interpretation, non-specialists viewed a short PDF training document explaining cystic lung disease interpretation. Results: Correct first choice diagnosis of 85%-88% may be achieved by high-performing specialist readers and 71%-80% by non-specialists and lower-performing specialists, with mean accuracies in the diagnosis of LAM (91%, p Conclusion: With specific but limited training, non-specialist physicians can diagnose cystic lung diseases from CT appearance alone with similar accuracy to specialists, correctly identifying approximately 75% of cases.
