D-Dimer: Predictor of Postpartum Hemorrhage among Pre-Eclampsia at Kilimanjaro Christian Medical Centre

Background: Postpartum hemorrhage (PPH) is the major contributor to maternal mortality and morbidity worldwide as well as in Tanzania. studies have shown Pre-eclampsia as a risk indicator for Postpartum hemorrhage and D-dimer tends to rise in women with pre-eclampsia. Few studies that have shown the association between D-dimer and PPH have been controversial and differ according to ethnicity and lifestyle. Hence there is no suitable reference interval for D-dimer in predicting Postpartum hemorrhage among women with pre-eclampsia. Broad Objective: This study aimed to assess the association, sensitivity, and specificity of D-dimer as a laboratory predictor of postpartum hemorrhage among women with pre-eclampsia at KCMC hospital. Methodology: This was a hospital-based analytical cross-sectional study conducted at KCMC Hospital in Northern Tanzania from september 2022 to March 2023. A total of 195 women with pre-eclampsia were included in this study. Plasma D-dimer levels were taken from women with pre-eclampsia pre-delivery. Haematocrit was compared before and after delivery, and a fall of 10% was considered as Postpartum hemorrhage together with clinical assessment of the patient. Participants were divided among those who had severe features and those who did not have severe features and were further categorized into those who had PPH and those who did not have PPH. Logistic regression was used to determine the association between D-dimer and PPH adjusting for other factors. The Receiver Operating Curve (ROC) was used to evaluate the predictive value. Results: Higher median D-dimer levels were seen among women who had PPH compared to those who had no PPH. D-dimer was seen to be associated with PPH, thus for every unit increase of µg/ml of D-dimer among women who had pre-eclampsia without severe features there was a 14% significant increase in the odds of having postpartum hemorrhage and a 45% significant increase of having postpartum hemorrhage among those who had pre

Interaction between diet and genetics in patients with inflammatory bowel disease

In this editorial,we comment on the article by Marangoni et al,published in the recent issue of the World journal of Gastroenterology 2023;29:5618-5629,about“Diet as an epigenetic factor in inflammatory bowel disease”.The authors emphasized the role of diet,especially the interaction with genetics,in promoting the inflam-matory process in inflammatory bowel disease(IBD)patients,focusing on DNA methylation,histone modifications,and the influence of *** this editorial,we explore the interaction between genetics,gut microbiota,and diet,in an only ***,we provided dietary recommendations for patients with *** Western diet,characterized by a low fiber content and deficiency the micronutrients,impacts short-chain fatty acids production and may be related to the pathogenesis of *** the other hand,the consumption of the Mediter-ranean diet and dietary fibers are associated with reduced risk of IBD flares,particularly in Crohn’s disease(CD)*** to the dietary guidance from the International Organization for the study of Inflammatory Bowel Diseases(IOIBD),the regular consumption of fruits and vegetables while reducing the consumption of saturated,trans,dairy fat,additives,processed foods rich in maltodextrins,and artificial sweeteners containing sucralose or saccharine is recommended to CD *** patients with ulcerative colitis,the IOIBD recommends the increased intake of natural sources of omega-3 fatty acids and follows the same restrictive recommendations aimed at CD patients,with the possible inclusion of red *** conclusion,IBD is a complex and hetero-geneous disease,and future studies are needed to elucidate the influence of epigenetics on diet and microbiota in IBD patients.

Two-stage corrective operation for the treatment of pes cavovarus in patients with spina bifida

BACKGROUND Pes cavovarus has an estimated incidence of 8%-17%in patients with spina bifida(sB).The majority of the current literature on surgical treatment of cavovarus feet in children and adolescents includes a variety of *** are currently no case series describing a treatment algorithm for deformity correction in this specific patient *** authors of this study present the results of a retrospective case series performed to assess the radiographic outcomes of two-stage corrective surgery in patients with *** Retrospective chart review was performed on patients with sB with a diagnosis of pes cavovarus at a freestanding children’s hospital who underwent surgical correction of the *** were excluded for lack of two-stage corrective operation,nonambulatory status,lack of at least six months follow-up,and age>18 years at the time of *** resulted in a cohort of 19 *** analysis was performed on 11 feet that had a complete series of preoperative and postoperative weightbearing *** and postoperative radiographic outcome measurements were compared using a two-sample *** significant changes between the preoperative and postoperative measurements were seen in Meary’s angle,the anteroposterior talo-first metatarsal(AP TMT1)angle,and the talonavicular *** values of Meary’s angle were 17.9±13.1 preoperatively and 4.7±10.3 postoperatively(P=0.016).Mean AP TMT1 angle was 20.6±15.1 preoperatively and 9.3±5.5 postoperatively(P=0.011).Mean talonavicular coverage values were-10.3±9.6 preoperatively and-3.8±10.1 postoperatively(P=0.025).CONCLUsION The two-stage corrective procedure demonstrated efficacy in correcting cavovarus deformity in patients with *** should strongly consider employing the staged surgical algorithm presented in this manuscript for management of these patients.

Development and characterization of 3D-printed electroconductive pHEMA-co-MAA NP-laden hydrogels for tissue engineering

Tissue engineering(TE)continues to be widely explored as a potential solution to meet critical clinical needs for diseased tissue replacement and tissue *** this study,we developed a poly(2-hydroxyethyl methacrylate-co-methacrylic acid)(pHEMA-co-MAA)based hydrogel loaded with newly synthesized conductive poly(3,4-ethylene-dioxythiophene)(PEDOT)and polypyrrole(PPy)nanoparticles(NPs),and subsequently processed these hydrogels into tissue engineered constructs via three-dimensional(3D)*** presence of the NPs was critical as they altered the rheological properties during ***,all samples exhibited suitable shear thinning properties,allowing for the development of an optimized processing window for 3D *** were 3D printed into pre-determined disk-shaped configurations of 2 and 10 mm in height and diameter,*** observed that the NPs disrupted the gel crosslinking efficiencies,leading to shorter degradation times and compressive mechanical properties ranging between 450 and 550 *** conductivity of the printed hydrogels increased along with the NP concentration to(5.10±0.37)×10^(−7)s/*** vitro studies with cortical astrocyte cell cultures demonstrated that exposure to the pHEMA-co-MAA NP hydrogels yielded high cellular viability and proliferation ***,hydrogel antimicrobial studies with staphylococcus epidermidis bacteria revealed that the developed hydrogels affected bacterial *** together,these materials show promise for various TE strategies.

An Assessment of saltwater Intrusion in Coastal Regions of Lagos, Nigeria

This paper explains various factors that contribute to saltwater intrusion, including overexploitation of freshwater resources and climate change as well as the different techniques essential for effective saltwater intrusion management. The impact of saltwater intrusion along coastal regions and its impact on the environment, hydrogeology and groundwater contamination. It suggests potential solutions to mitigate the impact of saltwater intrusion, including effective water management and techniques for managing sWI. The application of A.I (assessment index) serves as a guideline to correctly identify wells with sWI ranging from no intrusion, slight intrusion and strong intrusion. The challenges of saltwater intrusion in Lagos and the salinization of wells were investigated using the hydro-chemical parameters. The study identifies four wells (“AA”, “CMs”, “OBA” and “VIL”) as having high electric conductivities, indicating saline water intrusion, while other wells (“EBM”, “IKj, and “IKO”) with lower electric conductivities, indicate little or no salt-water intrusion, and “Aj” well shows slight intrusion. The elevation of the wells also played a vital role in the sWI across coastal regions of Lagos. The study recommends continuous monitoring of coastal wells to help sustain and reduce saline intrusion. The findings of the study are important for policymakers, researchers, and practitioners who are interested in addressing the challenges of saltwater intrusion along coastal regions. We assessed the sWI across the eight (8) wells using the Assessment Index to identify wells with sWI. Wells in “CMs” and “VIL” has strong intrusions. A proposed classification system based on specific ion ratios categorizes water quality from good (+) to highly (-) contaminated (refer to Table 4). These findings underscore the need for attention and effective management strategies to address groundwater unsuitability for various purposes.

We have recently proposed a new technique of plasma tailoring by laser-driven hydrodynamic shockwaves generated on both sides of a gas jet[Marquès et al.,*** 28,023103(2021)].In a continuation of this numerical work,we study experimentally the influence of the tailoring on proton acceleration driven by a high-intensity picosecond laser in three cases:without tailoring,by tailoring only the entrance side of the picosecond laser,and by tailoring both sides of the gas *** tailoring,the acceleration is transverse to the laser axis,with a low-energy exponential spectrum,produced by Coulomb *** the front side of the gas jet is tailored,a forward acceleration appears,which is significantly enhanced when both the front and back sides of the plasma are *** forward acceleration produces higher-energy protons,with a peaked spectrum,and is in good agreement with the mechanism of collisionless shock acceleration(CsA).The spatiotemporal evolution of the plasma profile is characterized by optical shadowgraphy of a probe *** refraction and absorption of this beam are simulated by post-processing 3D hydrodynamic simulations of the plasma *** with the experimental results allows estimation of the thickness and near-critical density of the plasma slab produced by tailoring both sides of the gas *** parameters are in good agreement with those required for CsA.
