Hexachlorobutadiene(HCBD)is a halogenated hydrocarbon that is primarily produced as an unintentional byproduct in the manufacture of chlorinated *** between HCBD and other persistent organic pollutants(POPs)led to its listing in 2015 for global regulation under the Stockholm Convention on ***'s toxicity and propensity for long-range transport means there is special concern for its potential impacts on Arctic *** present review comprehensively summarizes all available information of the occurrence of HCBD in the Arctic environment,including its atmospheric,terrestrial,freshwater and marine ecosystems and ***,reports of HCBD in Arctic environmental media are *** has been measured in Arctic air collected from monitoring stations in Finland and Canada,yet there is a dearth of data for other abiotic matrices(***,sediments,glacier ice,freshwaters and seawater).Low HCBD concentrations have been measured in Arctic terrestrial and marine biota,which is consistent with laboratory studies that indicate that HCBD has the potential to bioaccumulate,but not to *** data for Arctic biota suggest that terrestrial birds and mammals and seabirds,have comparatively higher HCBD concentrations than fish and marine mammals,warranting additional *** spatial and temporal trends in HCBD concentrations in the Arctic are currently limited,future monitoring of HCBD in the Arctic will be important for assessing the impact of global regulations newly-imposed by the Stockholm Convention on POPs.

Role of perinatal long-chain omega-3 fatty acids in cortical circuit maturation:Mechanisms and implications for psychopathology

Accumulating translational evidence suggests that the long-chain omega-3 fatty acid docosahexaenoic acid(DHA) plays a role in the maturation and stability of cortical circuits that are impaired in different recurrent psychiatric disorders. Specifically, rodent and cell culture studies find that DHA preferentially accumulates in synaptic and growth cone membranes and promotes neurite outgrowth, dendritic spine stability, and synaptogenesis. Additional evidence suggests that DHA may play a role in microglia-mediated synaptic pruning, as well as myelin development and resilience. In nonhuman primates n-3 fatty acid insufficiency during perinatal development leads to widespread deficits in functional connectivity in adult frontal cortical networks compared to primates raised on DHA-fortified diet. Preterm delivery in non-human primates and humans is associated with early deficits in cortical DHA accrual. Human preterm birth is associated with longstanding deficits in myelin integrity and cortical circuit connectivity and increased risk for attention deficit/hyperactivity disorder(ADHD), mood, and psychotic disorders. In general, ADHD and mood and psychotic disorders initially emerge during rapid periods of cortical circuit maturation and are characterized by DHA deficits, myelin pathology, and impaired cortical circuit connectivity. Together these associations suggest that early and uncorrected deficits in fetal brain DHA accrual may represent a modifiable risk factor for cortical circuit maturation deficits in psychiatric disorders, and could therefore have significant implications for informing early intervention and prevention strategies.

A rare case of severe third degree friction burns and large Morel-Lavallee lesion of the abdominal wall

Background: Morel-Lavallee lesions (MLLs) are rare internal degloving injuries typically caused by blunt traumatic injuries and most commonly occur around the hips and in association with pelvic or acetabular fractures. MLL is often overlooked in the setting of poly-trauma;therefore, clinicians must maintain a high degree of suspicion and be familiar with the management of such injuries, especially in obese poly-trauma patients. Case presentation: We present a 30-year-old female pedestrian struck by a motor vehicle who sustained multiple long bone fractures, a mesenteric hematoma, and full-thickness abdominal skin friction burn which masked a significant underlying abdominal MLL. The internal degloving caused significant devascularization of the overlying soft tissue and skin which required surgical drainage of hematoma, abdominal wall reconstruction with tangential excision, allografting, negative pressure wound therapy, and ultimately autografting. Conclusion: MLL is a rare, often overlooked, internal degloving injury. Surgeons must maintain a high index of suspicion when dealing with third degree friction burns as they may mask underlying injuries such as MLL, and a delay in diagnosis can lead to increased morbidity.

Aim:Quadriceps strength and knee extension,the most important factors limiting the ability to rise from a chair,are crucial for walking at an appropriate speed,ascending and descending stairs,and performing activities such as running,dancing,and *** of the anterior compartment of the thigh,including all four quadriceps muscles,for the treatment of a sarcoma is uncommon;however,when necessary,it is very debilitating and adversely affects a patient's quality of life without functional ***,there are a limited number of complex and difficult reconstructions to restore quadriceps function that have been described with variable *** describe a simple technique that employs a single gracilis functional muscle transfer to replace essential quadriceps ***:This is a case series describing the use of either a free or pedicled single gracilis muscle to restore quadriceps function following sarcoma ***:Four patients underwent an anterior compartment sarcoma resection that resulted in a large segmental defect and/or denervation of all four quadriceps muscles such that no quadriceps function would remain without *** four patients underwent a functional reconstruction using a single *** of the living patients achieved British Medical Research Counsel Grade 4 strength,can achieve full knee extension,are able to navigate stairs,and are able to ambulate without a *** fourth patient unfortunately was deceased in under three months following his tumor ***:Despite its small size in comparison to the quadriceps muscles,with physiotherapy and training,the gracilis muscle demonstrates the capacity to hypertrophy and replace quadriceps function following limb salvage surgery.

Since the ban of polybrominated diphenyl ethers(PBDEs)and hexabromocyclododecane(HBCDD),other flame retardants may be increasingly ***-one current-use halogenated(HFRs)and 24 organophosphorous flame retardants(PFRs)have been sought in Arctic ecosystems so *** measurements provide evidence of long-range atmospheric transport for the majority of these compounds,with much higher concentrations for PFRs than for *** HFRs,***(2-ethylhexyl)-tetrabromophthalate(BEH-TEBP),2-ethylhexyl-2,3,4,5-tetrabromobenzoate(EH-TBB)and hexabromobenzene(HBBz),had air concentrations comparable to those of PBDEs in some *** data for seawater and ice indicate dry deposition of HFRs,while net volatilization from seawater was observed for some *** in the marine environment indicate a wide presence of HFRs in marine biota,but generally at low levels,*** lower than those of *** exist,namely 2,4,6-tribromophenyl 2,3-dibromopropyl ether(TBP-DBPE)and decabromodiphenyl ethane(DBDPE),which were found in concentrations comparable to PBDEs in some *** same was the case for 2,4,6-tribromophenyl allyl ether(TBP-AE)in a study from the terrestrial *** generally had low concentrations in biota,probably due to metabolic transformation of PFR triesters,as suggested by in vitro *** PFR concentrations occurred in some individuals,generally indicating a larger variability of PFRs in biota than found for *** commercially important tetrabromobisphenol A(TBBPA)was only detected sporadically,and only in abiotic matrices.

Knowledge of radiation exposure associated with common trauma imaging modalities among orthopaedic surgeons,emergency medicine physicians,and general surgeons in the United States

BACKGROUND:Few contemporary studies have assessed physicians’knowledge of radiation exposure associated with common imaging studies,especially in trauma *** purpose of this study was to assess the knowledge of physicians involved in caring for trauma patients regarding the effective radiation doses of musculoskeletal(MSK)imaging studies routinely utilized in the trauma ***:An electronic survey was distributed to United States orthopaedic surgery,general surgery,and emergency medicine(EM)residency *** were asked to estimate the radiation dose for common imaging modalities of the pelvis,lumbar spine,and lower extremity,in terms of chest X-ray(CXR)*** estimates were compared to the true effective radiation ***,participants were asked to report the frequency of discussing radiation risk with ***:A total of 218 physicians completed the survey;102(46.8%)were EM physicians,88(40.4%)wereorthopaedicsurgeons,and28(12.8%)*** underestimated the effective radiation doses of nearly all imaging modalities,most notably for pelvic computed tomaography(CT)(median 50 CXR estimation vs.162 CXR actual)and lumbar CT(median 50 CXR estimation vs.638 CXR actual).There was no difference between physician specialties regarding estimation accuracy(P=0.133).Physicians who regularly discussed radiation risks with patients more accurately estimated radiation exposure(P=0.007).CONCLUSION:The knowledge among orthopaedic and general surgeons and EM physicians regarding the radiation exposure associated with common MSK trauma imaging is *** investigation with larger scale studies is warranted,and additional education in this area may improve care.

The role of serial physical examinations in the management of angioedema involving the head and neck:A prospective observational study

Objective:To elucidate the progression of angioedema of the head and neck with routine management and to assess the utility of serial physical exams and fiberoptic laryngos-copy in its management. Methods:This study was a prospective observational research. From 2013 to 2014, a prospec-tive observational study was conducted at a tertiary referral center. Forty patient were ap-proached, 7 refused, 33 (18e90 years old) were enrolled. Patients presented with angioedema involving the head and neck over a 12 month period were asked to participate in the study. Physical examination and fiberoptic laryngoscopy were performed at presentation and then repeated at least 1 h later. Results:Thirty-three patients with head and neck angioedema from any cause were enrolled (mean age 58, range 23e89 years). The upper lip was the most commonly involved site (58%). On reevaluation, 82% of patients reported subjective improvement in symptoms. Theassociation between subjective improvement and the physical exam, including fiberoptic laryngoscopy findings, was statistically significant (P Conclusion:In stable patients with angioedema of any head and neck subsite, self-reported symptoms are associated with clinical stability or improvement as assessed by physical signs and fiberoptic laryngoscopy. Patients’ symptoms may be an appropriate surrogate to monitor clinical status without the need for routine serial physical examinations or fiberoptic laryngos-copy, though further study is needed. Disparities in employment outcomes among critical care graduates with different training backgrounds

Background:We sought to characterize the impact of critical care training pathways on subsequent employment ***:A survey assessing the postfellowship work environment and barriers to employment in a preferred setting was electronically distributed on September 16,2019,to program directors and coordinators at all US adult critical care fellowships with instructions to for-ward it to the prior year’s ***:Data collection was interrupted by the coronavirus disease 2019 ***-nine participants were included,reflecting a low rate of *** worked in urban areas(76.6%)at academic hospitals(81.7%).Graduates of internal medicine critical care(IMCC)were more likely than pulmonary critical care(PCC)to report their fellowship as a barrier to employment(50%vs 0%;P<0.05).Emergency medicine graduates were more likely than internal medicine to report their residency as a barrier to employment(83.3%vs 4.6%;P<0.05).Inability to find a desired position at an academic center(50%vs 5.9%;P<0.05)and in the preferred metropolitan setting(37.5%vs 0%;P<0.05)were more common among IMCC than ***:In this survey of critical care graduates,residency in emergency medicine and fellowship in IMCC were associated with in-creased barriers to finding desired employment.
