BACKGROUND Transcatheter aortic valve replacement(TAVR)is a revolutionary procedure for severe aortic *** coexistence of chronic kidney disease(CKD)and TAVR introduces a challenge that significantly impacts patient *** To define readmission rates,predictors,and causes after TAVR procedure in CKD stage 1-4 *** we used the national readmission database 2018 and 2020 to look into readmission rates,causes and predictors after TAVR procedure in patients with CKD stage *** Out of 24758 who underwent TAVR and had CKD,7892(32.4%)patients were readmitted within 90 days,and had higher adjusted odds of being females(adjusted odds ratio:1.17,95%CI:1.02-1.31,P=0.02)with longer length of hospital stay>6 days,and more comorbidities including but not limited to diabetes mellitus,anemia,and congestive heart failure(CHF).CONCLUSION Most common causes of readmission included CHF(18.0%),sepsis,and complete atrioventricular *** readmission predictors with very close followup is warranted to prevent such high rate of readmission.

The success of many enteric bacteria is hinged on the ability to tolerate environmental stress such as extreme acidity. The acid stress response （ASR） has been investigated in many enteric bacteria and has been shown to involve variable expression of a broad spectrum of genes involved in transcriptional regulation, metabolism, colonization and virulence; representing a linkage between acid tolerance and pathogenicity. Though the majority of ASR studies have been conducted in laboratory conditions and from the perspective of pathogenicity, the role of environmental reservoirs on acid adaptation has recently emerged as an important aspect of pathogenic microbial ecology. This mini-review profiles ASR in three opportunistic enteric pathogens and synthesizes recent work pertaining to the study of this dynamic response.

AIM： To identify the bacterial flora in conditions such as Barrett＇s esophagus and reflux esophagitis to determine if they are similar to normal esophageal flora. METHODS： Using broad-range 16S rDNA PCR, esophageal biopsies were examined from 24 patients [9 with normal esophageal mucosa, 12 with gastroesophageal reflux disease （GERD）, and 3 with Barrett＇s esophagus]. Two separate broad-range PCR reactions were performed for each patient, and the resulting products were cloned. In one patient with Barrett＇s esophagus, 99 PCR clones were analyzed. RESULTS： Two separate clones were recovered from each patient （total = 48）, representing 24 different species, with 14 species homologous to known bacteria, 5 homologous to unidentified bacteria, and 5 were not homologous （〈97% identity） to any known bacterial 16S rDNA sequences. Seventeen species were found in the reflux esophagitis patients, 5 in the Barrett＇s esophagus patients, and 10 in normal esophagus patients. Further analysis concentrating on a single biopsy from an individual with Barrett＇s esophagus revealed the presence of 21 distinct bacterial species. Members of four phyla were represented, including Bacteroidetes, Firmicutes, Proteobacteria, and Actinobacteria. Microscopic examination of each biopsy demonstrated bacteria in intimate association with the distal esophageal epithelium, suggesting that the presence of these bacteria is not transitory. CONCLUSION： These findings provide evidence for a complex, residential bacterial population in esophageal reflux-related disorders. while much of this biota is present in the normal esophagus, more detailed comparisons may help identify potential disease associations.

AIM To examine the effect of center size on survival differences between simultaneous liver kidney transplantation(SLKT) and liver transplantation alone(LTA) in SLKT-listed *** The United Network of Organ Sharing database was queried for patients ≥ 18 years of age listed for SLKT between February 2002 and December 2015. Posttransplant survival was evaluated using stratified Cox regression with interaction between transplant type(LTA vs SLKT) and center *** During the study period, 393 of 4580 patients(9%) listed for SLKT underwent a LTA. Overall mortality was higher among LTA recipients(180/393, 46%) than SLKT recipients(1107/4187, 26%). The Cox model predicted a significant survival disadvantage for patients receiving LTA vs SLKT [hazard ratio, hazard ratio(HR) = 2.85; 95%CI: 2.21, 3.66; P was modestly attenuated as center SLKT volume increased, with a 3% reduction(HR = 0.97; 95%CI: 0.95, 0.99; P = 0.010) for every 10 SLKs *** In conclusion, LTA is associated with increased mortality among patients listed for SLKT. This difference is modestly attenuated at more experienced centers and may explain inconsistencies between smaller-center and larger registry-wide studies comparing SLKT and LTA outcomes.

BACKGROUND： The 26S proteasome is at the heart of the ubiquitin-proteasome system, which is the key cellular pathway for the regulated degradation of proteins and enforcement of protein quality control. The 26S proteasome is an unusually large and complicated protease comprising a 28-subunit core particle （CP） capped by one or two 19-subunit regulatory particles （RP）. Multiple activities within the RP process incoming ubiquitinated substrates for eventual degradation by the barrel-shaped CE The large size and elaborate architecture of the proteasome have made it an exceptional model for understanding mechanistic themes in macromolecular assembly. OBjECTIVE： In the present work, we highlight the most recent mechanistic insights into proteasome assembly, with particular emphasis on intrinsic and extrinsic factors regulating proteasome biogenesis. we also describe new and exciting questions arising about how proteasome assembly is regulated and deregulated in normal and diseased cells. METHODS： A comprehensive literature search using the PubMed search engine was performed, and key findings yielding mechanistic insight into proteasome assembly were included in this review. RESULTS： Key recent studies have revealed that proteasome biogenesis is dependent upon intrinsic features of the subunits themselves as well as extrinsic factors, many of which function as dedicated chaperones. CONCLUSION： Cells rely on a diverse set of mechanistic strategies to ensure the rapid, efficient, and faithful assembly of proteasomes from their cognate subunits. Importantly, physiological as well as pathological changes to proteasome assembly are emerging as exciting paradigms to alter protein degradation in vivo.

Zoologists rely on mechanistic niche models of behavioral thermoregulation to understand how animals respond to climate *** models predict that species will need to disperse to higher altitudes to persist in a warmer ***,thermal stress and,thus,thermoregulatory behavior may depend on atmospheric oxygen as well as environmental *** hypoxia causes animals to prefer lower body temperatures,which could be interpreted as evidence that oxygen supply limits heat *** a constraint could prevent animals from successfully dispersing to high elevations during climate ***,an effect of oxygen supply on preferred body temperature has only been observed when oxygen concentrations fall far below levels experienced in *** see whether animals perceive greater thermal stress at an ecologically relevant level of hypoxia,we studied the thermoregulatory behavior of lizards(Sceloporus tristichus)exposed to oxygen concentrations of 13%and 21%(equivalent to PO2 at 4000 m and 0 m,respectively).In addition,we exposed lizards to 29%oxygen to see whether they would accept a higher body temperature at hyperoxia than at *** each oxygen level,we measured a behavioral response to heat stress known as the voluntary thermal maximum:the temperature at which a warming animal sought a cool *** concentration had no discernable effect on the voluntary thermal maximum,suggesting that lizards experience thermal stress similarly at all 3 levels of oxygen(13%,12%and 29%).Future research should focus on thermoregulatory behaviors under ecologically relevant levels of hypoxia.

Portal vein thrombosis is a common complica- tion in cirrhotic patients. when portal vein thrombectomy is not a suitable option, a large collateral vessel can be used for allograft venous inflow reconstruction. we describe an unusu- al case of successful portal revascularization using the right gastroepiploic vein. The patient underwent a cadaveric or- thotropic liver transplantation with end-to-end anastomosis of the portal vein to the right gastroepiploic vein. Six months after liver transplantation the patient is well with good liver function. The use of the right gastroepiploic vein for allograft venous reconstruction is feasible and safe, with a great advan- tage of avoiding the need of venous jump graft.

Electron cyclotron emission imaging（ECEI） can provide measurements of 2D electron temperature fluctuation with high temporal and spatial resolution in magnetic fusion plasma devices. Two ECEI systems located in different toroidal ports with 67.5 degree separation have been implemented on j-TEXT to study the 3D structure of magnetohydrodynamic（MHD） instabilities. Each system consists of 12（vertical） × 16（horizontal） = 192 channels and the image of the 2nd harmonic X-mode electron cyclotron emission can be captured continuously in the core plasma region. The field curvature adjustment lens concept is developed to control the imaging plane for receiving optics of the ECEI systems. Field curvature of the image can be controlled to match the emission layer. Consequently, a quasi-3D image of the MHD instability in the core of the plasma has been achieved.

Background: Physical activity(PA) and diet are 2 lifestyle factors that affect cardiometabolic risk. However, data on how a high-fat highcarbohydrate(HFHC) diet influences the effect of different intensities of PA on cardiometabolic health and cardiovascular function in a controlled setting are yet to be fully established. This study investigated the effect of sedentary behavior, light-intensity training(LIT), and high-intensity interval training(HIIT) on cardiometabolic markers and vascular and cardiac function in HFHC-fed adult ***: Twelve-week-old wistar rats were randomly allocated to 4 groups(12 rats/group): control(CTL), sedentary(SED), LIT, and *** indices, glucose and lipid control, inflammatory and oxidative stress markers, vascular reactivity, and cardiac electrophysiology of the experimental groups were examined after 12 weeks of HFHC-diet feeding and PA ***: The SED group had slower cardiac conduction(p = 0.0426) and greater thoracic aortic contractile responses(p with the CTL group. The LIT group showed improved cardiac conduction compared with the SED group(p = 0.0003), and the HIIT group showed decreased mesenteric artery contractile responses compared with all other groups and improved endothelium-dependent mesenteric artery relaxation compared with the LIT group(both p wer visceral(p = 0.0057 for LIT, p = 0.0120 for HIIT)and epididymal fat(p with the CTL ***: LIT induced positive adaptations on fat accumulation and cardiac conduction, and HIIT induced a positive effect on fat accumulation,mesenteric artery contraction, and endothelium-dependent relaxation. No other differences were observed between groups. These findings suggest that few positive health effects can be achieved through LIT and HIIT when consuming a chronic and sustained HFHC diet.
