The aim of the present study was to investigate the effect of caffeine on vertical jumping height in rested condition and after a heavy strength training *** well-trained young males with experience in jump and strength training were included in this double-blinded,randomised study with cross-over ***(3 mg/kg body weight)or placebo were ingested 45 min prior to the jump *** was performed on a force platform and vertical jumping height was *** a standardized warm up,participants performed jumping series consisting of three maximal jumps with 30 s rest between jumps followed by five maximal jumps with 7 s rest between *** participants performed a heavy strength training of the leg muscles(leg press:3×15 reps)and the jumping series was repeated immediately after(30 s),and after 5 min and 15 min *** increased the maximal vertical counter movement jump height(P≤0.05)and mean value of the 5-jump sequence prior to the strength *** increased jump height by 2.2 cm±0.5 cm at the first *** lactate after the strength training increased to 6.97±1.20 and 7.77±0.54 mmol/L in PLA and CAF,respectively(P=0.19).The jump height was reduced by 8 cm after the strength *** were no differences in jump height after ingestion of caffeine or placebo immediately after the strength training session or in the recovery period,but blood lactate in the recovery period was higher in CAF compared to PLA(ANOVA;Pjump height in the resting ***,after a maximal effort strength training session the positive effect of caffeine was no longer significant.

Background: Recurrent transitional cell bladder cancer (TCBC) can metastasize to the GI tract albeit uncommonly. This is the first report of the EUS appearan ce of metastatic TCBC to the GI tract. In addition to describing the EUS feature s of recurrent metastatic TCBC, this study determined the number of patients ref erred for evaluation of a primary GI luminal cancer in which EUS instead establi shed the diagnosis of metastatic recurrent TCBC. Methods: Patients referred from july 2000 through April 2004 for EUS evaluation of a suspected primary GI lumin al cancer were retrospectively reviewed. For patients with an established diagno sis of recurrent metastatic TCBC, EUS images were retrospectively reviewed to id entify characteristic features. Results: Of 2216 patients undergoing EUS to eval uate a suspected primary GI luminal cancer, 3 men (0.14% : 95% confidence int erval [0.02% , 0.29% ]) (mean age 67 years, range 54- 74 years) were found in stead to have recurrent metastatic TCBC involving the duodenum (n = 1) or rectum (n = 2). The patients presented a mean of 32 months after diagnosis of the prim ary TCBC with change in bowel habit (n = 1) and symptoms of bowel obstruction (n = 2). In each patient, initial endoscopy revealed circumferential luminal steno sis and mucosal erythema, but mucosal biopsy specimens revealed normal tissue. E US demonstrated hypoechoic, symmetric, circumferential wall thickening, loss of deep wall layers, and pseudopodia- like extensions into the peri- intestinal t issues. In the two patients with rectal involvement, no evidence of direct infil tration from the bladder bed was seen. EUS- guided FNA was diagnostic of metast atic TCBC in all patients. Conclusions: Although most cases of hypoechoic bowel - wall thickening and stenosis are from primary GI neoplasia, recurrent TCBC sh ould be considered in patients with a history of this tumor. Vascular lesions are infrequently recognized as manifestations of neurofibromatosis 1 (NF1), but they can produce serious complications and contribute to mortality at younger ages. Here we report the case of a young female patient with NF1 who suffered from a fatal middle cerebral artery (MCA) infarct. In addition to the MCA infarct, head MRI also disclosed old infarcts at bilateral cerebella and multiple intracranial arterial steno- occlusions. Stroke related to NF1 cerebral vasculopathy was highly suggested after other diseases were excluded. Additionally, genetic analysis of the patient identified a novel mutation at the splicing donor site, c. 2409 + 2T >G that resulted in a splicing aberration.

Background and Purpose:Carotid and Vertebral Artery Transluminal Angioplasty Study (CAVATAS) patients with carotid stenosis were randomized between endovascu lar treatment and endarterectomy. The rates of residual severe stenosis and rest enosis and their contribution to recurrent symptoms was ***:Endovas cular patients:were treated by balloon angioplasty alone (88%) or stenting (22 %). Patches were used in 63%of endarterectomy patients. Carotid stenosis was c ategorizedas mild (0%to 49%), moderate (50%to 69%), severe (70%to 99%), or occluded, using standardized Doppler ultrasound criteria at the examination clo sest to 1 month (n=283) and 1 year (n=347) after treatment. Recurrent cerebrovas cular symptoms during follow-up were analyzed. Results:More patients had ≥70 %stenosis of the ipsilateral carotid artery 1 year after endovascular treatment than after endarterectomy (18.5%versus 5.2%, P=0.0001). Residual severe steno sis was present in 6.5%of patients at 1 month after endovascular *** een 1 month and 1 year, restenosis to ≥70%stenosis occurred in 10.5%of the en dovascular group. After endarterectomy,1.7%had residual severe stenosis at 1 mo nth, and 2.5%developed severe restenosis. The results were significantly better after stenting compared with angioplasty alone at 1 month(Pstenosis(5/32 [15.6%]) compared with lesser degrees of stenosis at 1 year (11/141 [7.8%], P=0.02), but most were transient ischemic attacks and n one were disabling or fatal strokes. There were no recurrent symptoms in the 9 e ndarterectomy patients with≥70%stenosis at 1 year. Conclusions:Carotid stenos is 1 year after endovascular treatment is partly explained by poor initial anato mical results and partly by restenosis. The majority of patients were treated by angioplasty without stenting. Further randomized studies are required to determ ine whether new
