Extracellular vesicles or exosomes are membrane encapsulated biological nanometric particles secreted virtually by all types of cells throughout the animal *** carry a cargo of active molecules to proximal and distal cells of the body as mechanism of physiological communication,to maintain natural homeostasis as well as pathological *** carry a tremendous potential for liquid biopsy and therapeutic ***,there is a global demand for simple and robust exosome isolation methods amenable to point-of-care diagnosis and quality control of therapeutic exosome *** can be achieved by molecular profling of the exosomes for use with specifc sets of molecular-markers for diagnosis and quality *** biopsy is undoubtedly the most promising diagnosis process to advance“personalized medicine.”Currently,liquid biopsy is based on circulating cancer cells,cell free-DNA,or *** potentially provide promise for early-stage diagnostic possibility;in order to facilitate superior diagnosis and isolation of exosomes,a novel platform is developed to detect and capture them,based on localized surface plasmon resonance(LsPR)of gold nanoislands,through strong afnity between exosomes and peptide called Venceremin or *** modeling,based on the characteristics of the gold nanoislands and the bioentities involved in the sensing,is also developed to determine the detection capability of the platform,which is optimized experimentally at each *** results and modeling present a relationship between the plasmonic shif and the concentration of exosomes and,essentially,indicate possibilities for label-free early diagnosis.

Liver fibrosis is characterised by excessive deposition of extracellular matrix that interrupts normal liver functionality. It is a pathological stage in several untreated chronic liver diseases such as the iron overload syndrome hereditary haemochromatosis, viral hepatitis, alcoholic liver disease, non-alcoholic fatty liver disease, non-alcoholic steatohepatitis and diabetes. Interestingly, regardless of the aetiology, iron-loading is frequently observed in chronic liver diseases. Excess iron can feed the Fenton reaction to generate unquenchable amounts of free radicals that cause grave cellular and tissue damage and thereby contribute to fibrosis. Moreover, excess iron can induce fibrosis-promoting signals in the parenchymal and non-parenchymal cells, which accelerate disease progression and exacerbate liver pathology. Fibrosis regression is achievable following treatment, but if untreated or unsuccessful, it can progress to the irreversible cirrhotic stage leading to organ failure and hepatocellular carcinoma, where resection or transplantation remain the only curative options. Therefore,understanding the role of iron in liver fibrosis is extremely essential as it can help in formulating iron-related diagnostic, prognostic and treatment strategies. These can be implemented in isolation or in combination with the current approaches to prepone detection, and halt or decelerate fibrosis progression before it reaches the irreparable stage. Thus, this review narrates the role of iron in liver fibrosis. It examines the underlying mechanisms by which excess iron can facilitate fibrotic responses. It describes the role of iron in various clinical pathologies and lastly,highlights the significance and potential of iron-related proteins in the diagnosis and therapeutics of liver fibrosis.

surgical intervention for post-infarct ventricular septal defect (VsD) is a challenging procedure due to patients’ complex preoperative conditions. While percutaneous VsD closure can be considered as an alternative to surgical repair, complete closure of the defect remains difficult and is associated with various procedural complications. We report a rare case of a patient with postoperative residual shunts who experienced recurrent stroke episodes, requiring surgical intervention for repair. The patient, a 71-year-old female, developed acute anterior myocardial infarction and post-infarct VsD. Percutaneous closure with a 14-mm Amplatzer VsD occluder device was performed, yet the closure was incomplete. Following discharge, she developed multiple embolic stroke episodes, likely stemming from the residual VsD, which led to the surgical extraction of the device and VsD repair. Fibrous tissue was found to be solely attached to the core and right ventricle side of the device, whilst no fibrous tissue was observed on the side of the left ventricle. The patient has not experienced new neurological symptoms at an 18-month follow-up. Thus, it is paramount to keep in mind that an embolic stroke may occur in the setting of percutaneous post-infarct VsD closure. surgical repair of VsD with device removal should be considered as a treatment option to such a complex case.

Introduction: skin prick testing (sPT) is a common test to diagnose IgE sensitization and the rate of adverse reactions is very low. Case Presentation: 29-year-old woman with anaphylactic reaction to routine sPT with 11 allergens (aero and food). Discussion: Allergy profile included multiple food and inhalant allergies, polysensitization, prior episode of anaphylaxis to mushrooms, active eczema and active hay fever but not active asthma. Conclusion: Anaphylaxis to sPT, although a rare event, is unpredictable necessitating availability of appropriate personnel and adequate resuscitation facilities for all testing.

The combination of kidney paired donation(KPD) with desensitization represents a promising method of increasing the rate of living donor kidney transplantation(LDKT) in immunologically challenging patients. Patients who are difficult to match and desensitize due to strong donor specific antibody are may be transplanted by a combination of desensitization and KPD protocol with more immunologically favorable donor. We present our experience of combination of desensitization protocol with three-way KPD which contributed to successful LDKT in highly sensitized end stage renal disease patient. All recipients were discharged with normal and stable allograft function at 24 mo follow up. We believe that this is first report from India where three-way KPD exchange was performed with the combination of KPD and desensitization. The combination of desensitization protocol with KPD improves access and outcomes of LDKT.

We investigated the magnetocaloric effect in commercial Er_(2)O_(3) powders which presents almost no hysteresis losses at low *** a magnetic field change of 5 T,it displays large magnetic entropy change(-ΔsM)max of 15.02 j/(kg·K)and a refrigerant capacity(RC)of 311 j/K at Neel temperature TN=3.32 *** magnetic transition was found to be of a *** maximum values of adiabatic temperature change(ΔTad)max reach 0.70 K for a magnetic field change of 1 *** large value,of(-AsM)max as well as no hysteresis loss,makes Er_(2)O_(3) a promising material as a magnetic refrigerant at low temperature.
