Utilization of RNA sequencing to investigate olfactory dysfunction in chronic rhinosinusitis without nasal polyps:A pilot study

Objectives:Prior research on olfactory dysfunction in chronic rhinosinusitis(CRS)has focused on patients with polyps and suggests that direct inflammation of the olfactory cleft mucosa plays a contributory role.The purpose of this study was to evaluate gene expression in superior turbinate mucosal specimens,comparing normosmic and dysosmic CRS patients without polyps(CRSsNP).Methods:Tissue samples were obtained from the superior turbinates of patients with CRSsNP at the time of endoscopic sinus surgery.Samples subsequently underwent RNA sequencing and functional analysis to investigate biological pathways associated with differentially expressed genes between dysosmic(n=7)and normosmic(n=4)patients.Results:Differential gene expression analysis comparing dysosmic and normosmic CRSsNP patients showed upregulation of 563 genes and downregulation of 327 genes.Using stringent criteria for multiple comparisons,one upregulated gene(Immediate Early Response 3[IER3])had an false discovery rate(FDR)correction adjustedP value considered statistically significant(P<0.001,fold change 2.69).Reactome functional analysis revealed eight biological pathways significantly different between dysosmic and normosmic patients(P<0.05,FDR correction)including IL-4 and IL-13 signaling,IL-10 signaling,and rhodopsin-like receptors.Conclusions:RNA sequencing of the superior turbinates in patients with CRSsNP can provide valuable information regarding biological pathways and genes involved in olfactory dysfunction.This study supports literature suggesting that Type 2 inflammation may play a role in olfactory dysfunction in at least some patients with CRSsNP.This study also prompts questions regarding the role of IL-10,rhodopsin-like receptors,andIER3 in the pathogenesis of olfactory dysfunction.

Comparison of trigeminal lateralization with differing stimulants

Trigeminal-specific stimulants have been shown to activate different receptors preferentially and this likely accounts for variation in sensory perception.It is unclear whether trigeminal sensitivity is similar across different transient receptor potential(TRP)receptors or if dysfunction of different receptors results in differing patient symptoms.Therefore,a prospective cohort study was conducted,consisting of trigeminal lateralization testing with three different stimulants(eucalyptol,isothiocyanate,acetic acid),olfaction testing with Sniffin'Sticks,and measurement of various patient‐reported outcome measures(PROMs).A total of 50 participants were enrolled across the olfactory spectrum.Mean TDI score was 27.1±8.3(range 7.0–39.5)with 38%normosmic and 62%dysosmic.Mean trigeminal lateralization scores out of 20 in the overall cohort were 16.18(2.78)for eucalyptol,14.94(3.49)for mustard oil,and 15.28(3.68)for vinegar.Eucalyptol showed a significant correlation with threshold scores of Sniffin'Sticks.A significant correlation was found between acetic acid and various PROMs.None of the lateralization scores of the trigeminal stimulants correlated to each other significantly and there was no correlation to age.The lack of correlation suggests that the measured sensitivity of one type of TRP receptor may not translate to similar sensitivity of the other receptors.Additional investigations with TRPV1 and TRPA1 agonists are needed to corroborate our findings.

Background:It is well known that stretch training can induce prolonged increases in joint range of motion(ROM).However,to date more information is needed regarding which training variables might have greater influence on improvements in flexibility.Thus,the purpose of this metaanalysis was to investigate the effects of stretch training on ROM in healthy participants by considering potential moderating variables,such as stretching technique,intensity,duration,frequency,and muscles stretched,as well as sex-specific,age-specific,and/or trained state-specific adaptations to stretch training.Methods:We searched through PubMed,Scopus,Web of Science,and SportDiscus to find eligible studies and,finally,assessed the results from 77 studies and 186 effect sizes by applying a random-effect meta-analysis.Moreover,by applying a mixed-effect model,we performed the respective subgroup analyses.To find potential relationships between stretch duration or age and effect sizes,we performed a meta-regression.Results:We found a significant overall effect,indicating that stretch training can increase ROM with a moderate effect compared to the controls(effect size=-1.002;Z=-12.074;95%confidence interval:-1.165 to-0.840;p<0.001;I^(2)=74.97).Subgroup analysis showed a significant difference between the stretching techniques(p=0.01)indicating that proprioceptive neuromuscular facilitation and static stretching produced greater ROM than did ballistic/dynamic stretching.Moreover,there was a significant effect between the sexes(p=0.04),indicating that females showed higher gains in ROM compared to males.However,further moderating analysis showed no significant relation or difference.Conclusion:When the goal is to maximize ROM in the long term,proprioceptive neuromuscular facilitation or static stretching,rather than ballistic/dynamic stretching,should be applied.Something to consider in future research as well as sports practice is that neither volume,intensity,nor frequency of stretching were found to play a significant role in ROM yields.

Recent work has shown that the vaginal microbiome exerts a strong impact on women's gynecological health.However,collection of vaginal specimens is invasive and requires previous clinical training or the involvement of a trained clinician.In contrast,urine sample collection is routine and noninvasive and does not require involvement of a clinician.We sought to compare the vaginal and urogenital microbiomes to assess the utility of voided urine samples as a proxy for the vaginal microbiome.Paired urogenital and vaginal samples were collected from pregnant women and characterized by 16S rRNA taxonomic profiling.We examined diversities and compositions of paired urogenital and vaginal microbiomes using five discrete strategies to explore the similarity between the vaginal and urogenital microbiomes.A strategy comparing the paired urogenital and vaginal microbiomes in which taxa were assigned using the STIRRUPS database and urine-specific taxa were removed showed no significant difference in diversity and composition between the paired urogenital and vaginal microbiomes.Moreover,the relative abundances of common vaginal taxa were linearly correlated with those in the paired urogenitalmicrobiomes.These similarities suggest that voided urine samples could represent a noninvasive protocol for accurate profiling of the vaginalmicrobiome with likely clinical applications.Finally,a machine learning model was established in which the voided urine microbiome was compared favorably to the vaginal microbiome in predicting bacterial vaginosis.

Variation in the plant-mediated methane transport and its importance for methane emission from intact wetland peat mesocosms

Aims and Methods Vascular plants are known to influence the production,transport and oxidation of methane in wetland soils,but these processes are not well understood.using plants grown in intact peat cores,we compared the influence upon methane emissions of 20 forb and graminoid species from European wetlands.We measured plant-mediated transport of methane(conduit or chimney effect)using a novel agar-sealing technique that prevented methane exchange from the bare soil to the atmosphere.Important Findings the plant-mediated transport(chimney effect)represented between 30%and almost 100%of the total methane flux,with graminoids exhibiting greater internal transport than forbs.In general,plants with less dense root tissues and a relatively larger root volume exhibited a larger chimney effect.most species(12 out of 20)signif-icantly reduced methane emissions compared to bare soil and only one species,Succisa pratensis,increased them.We suggest that characterising vegetation in terms of plant functional traits and plant processes offers an effective method for estimating methane emis-sions from wetlands.However,we found no correlation between the magnitude of the chimney effect and the overall influence of different plant species on methane emissions.besides introducing a useful tool to study plant-mediated transport,this work suggests that characterising vegetation in terms of functional traits could improve estimates of methane emissions from wetlands,which in turn could help in designing mitigation strategies.

A colchicine-induced autotetraploid grapevine exhibiting potentially valuable agronomic traits for grape production and breeding,including self-pruning,was identified.This study investigated DNA methylation variation and its role in gene expression during self-pruning in the autotetraploid grapevine.We used RNA-Seq to estimate differentially expressed genes between diploid and autotetraploid grapevine shoot tips.The genes showing increases in the autotetraploid were mainly related to stress response pathways,whereas those showing decreases in the autotetraploid were related to biological metabolism and biosynthesis.Whole-genome bisulfite sequencing was performed to produce single-base methylomes for the diploid and autotetraploid grapevines.Comparison between the methylomes revealed that they were conserved in CG and CHG contexts.In the autotetraploid grapevine,hypodifferentially methylated regions(DMRs)and hyper-DMRs in the gene body increased or decreased gene expression,respectively.Our results indicated that a hypo-DMR in the ACO1 gene body increased its expression and might promote self-pruning.This study reports that hypo-DMRs in the gene body increase gene expression in plants and reveals the mechanism underlying the changes in the modifications affecting gene expression during genome duplication.Overall,our results provide valuable information for understanding the relationships between DNA methylation,gene expression,and autotetraploid breeding in grape.

Aims The volume of soil beyond a plant’s roots from which that plant is able to acquire a particular nutrient depends upon the mobility of that nutrient in the soil.For this reason it has been hypothesized that the strength of competitive interactions between plants vary with soil nutrient mobility.We aimed to provide an experimental test of this hypothesis.Methods We devised two experimental systems to investigate specifically the effect of nutrient transport rates upon intraspecific competition.In the first,the exchange of rhizosphere water and dissolved nutrients between two connected pots,each containing one plant,was manipulated by alternately raising and lowering the pots.In the second experiment,the roots systems of two competing plants were separated by partitions of differing porosity,thereby varying the plants’access to water and nutrients in the other plant's rhizosphere.In this second experiment,we also applied varying amounts of nutrients to test whether higher nutrient input would reduce competition when competition for light is avoided,and applied different water levels to affect nutrient concentrations without changing nutrient supply.Important findings In both experiments,lower mobility reduced competitive effects on plant biomass and on relative growth rate(RGR),as hypothesized.In the second experiment,however,competition was more intense under high nutrient input,suggesting that low nutrient supply rates reduced the strength of the superior competitor.Competitive effects on RGR were only evident under the low water level,suggesting that under lower nutrient concentrations,competitive effects might be less pronounced.Taken together,our results provide the first direct experimental evidence that a reduction in nutrient mobility can reduce the intensity of competition between plants.

Telehealth Combined with Differentiated ART Delivery Improves ART Pick Up during COVID 19 at a Large HIV Treatment Facility in Trinidad and Tobago

Objective: To describe the implementation and outcomes of using telehealth in combination with a differentiated ART delivery model to improve HIV antiretroviral therapy pick up at a large treatment facility in Trinidad and Tobago during COVID-19. Design and Methods: Beginning in April 2020, a list of patients was generated daily prior to their scheduled ART appointments. Nurses, doctors and social workers conducted telephone consultations to first screen patients for COVID-19 symptoms, conducted brief medical and behavioural health screenings, and helped patients to identify barriers to ART retention. Patients were recommended for 1) fast track ART refill collection at facility, 2) community ART refills, and 3) ART pick-up through patient peers. The uptake and outcomes of telehealth and ART pick up were compared with the corresponding period in 2019. Data was analyzed using SPSS 21.0. Results: During the period April-june 2020, 1361 patients were identified for telephone consultations, 1084 (80%) were successfully contacted and 984 patients (88%) participated in phone session. The independent t test showed a significant increase in ART pick-up when compared to the corresponding period in 2019. 59% of patients collected via fast-track ART refill, 30% had community refills, and 11% pick-up medications through patient peers. Conclusion: Telehealth is an integral component of DSD as part of the COVID-19 response at the MRF. Understanding the medium and longer-term outcomes of Telehealth can provide additional insights on the scale up of telehealth as a component of DSD to improve ART outcomes for patients in the context of the developing countries of the Caribbean.
