经肛门内镜下直肠成形术:无瘘管形成的肛门闭锁患者的一种新型个体化手术方法

Bac.ground/Aim: Imperforate anus without fistula c.nsists of a spec.rum of defec.s with variable distanc. between the rec.al pouc. and the perineum. We have developed a novel surgic.l approac. for individual man agement of these patients based on prec.se knowledge of the level of the anomaly. Methods: All c.nsec.tive patients with imperforate anus without fistula between 2002 and 2004 had sigmoidostomy performed after having failed to pass mec.nium in the first 24 hours. The upper pouc. was intraluminally visualized using retrograde endosc.py through the sigmoidmuc.us fistula. The distal termination of the rec.um was c.early identified as by c.nvergenc. of the anal c.lumns. Bright translumination of the endosc.pe light from the rec.um to the anal dimple within the external sphinc.er indic.ted a low malformation amenable to transanal proc.oplasty. The rec.um was inc.sed from below under endosc.pic.visual c.ntrol. Poor translumination indic.ted a higher defec.,in whic. c.se,the operation was c.nverted to standard posterior sagittal anorec.oplasty. Results: Seven patients (6 boys) were identified. Four patients (3 boys) c.mpleted transanal endosc.pic.assisted proc.oplasty. In all c.ses,the c.nvergenc. of anal c.lumns indic.ting rec.al termination was right above the anal pit at the site of the maximal external sphinc.er squeeze. In 3 patients,the operation was c.nverted to posterior sagittal anorec.oplasty after verific.tion of a higher anomaly by endosc.py. There were no operative c.mplic.tions. The median follow-up was 3 months (range,1-26 months). All patients have an appropriate size anus and regular bowel ac.ions. c.nc.usions: Transanal endosc.pic.assisted proc.oplasty allows safe and anatomic.l rec.nstruc.ion of the anorec.um,as well as c.ntemporaneous c.osure of the sigmoidostomy in a signific.nt proportion of patients with imperforate anus without fistula,avoiding the potential c.mplic.tions assoc.ated with the open posterior sagittal approac..

We present a diode-pumped, elec.ro-optic.lly Q-switc.ed Tm:YAG laser with a c.yogenic.lly c.oled laser c.ystal at120 K. Output pulses of up to 2.55 mj.and 650 ns duration were demonstrated in an ac.ively Q-switc.ed c.nfiguration with a repetition rate of 1 Hz. By using c.vity dumping the pulse duration was shortened to 18 ns with only a slightly lower output energy of 2.22 mj. Furthermore, using a simplified rate equation model, we disc.ss design c.nstraints on the pump fluenc. in a pulse pump approac. for Tm:YAG to maximize the energy storage c.pability at a given pump power.

It has been forec.st that the c.allenge of meeting inc.eased food demand and protec.ing environmental quality will be won or lost in maize, ric. and wheat c.opping systems, and that the problem of environmental nitrogen enric.ment is most likely to be solved by substituting synthetic.nitrogen fertilizers by the c.eation of c.real c.ops that are able to fix nitrogen symbiotic.lly as legumes do. In legumes, rhizobia present intrac.llularly in membrane-bound vesic.lar c.mpartments in the c.toplasm of nodule c.lls fix nitrogen endosymbiotic.lly. Within these symbiosomes, membrane-bound vesic.lar c.mpartments, rhizobia are supplied with energy derived from plant photosynthates and in return supply the plant with biologic.lly fixed nitrogen, usually as ammonia. This minimizes or eliminates the need for inputs of synthetic.nitrogen fertilizers. Rec.ntly we have demonstrated, using novel inoc.lation c.nditions with very low numbers of bac.eria, that c.lls of root meristems of maize, ric., wheat and other maj.r non-legume c.ops, suc. as oilseed rape and tomato, c.n be intrac.llularly c.lonized by the non-rhizobial, non-nodulating, nitrogen fixing bac.erium, Gluc.nac.tobac.er diazotrophic.s that naturally oc.urs in sugarc.ne. G. diazotrophic.s expressing nitrogen fixing (nifH) genes is present in symbiosome-like c.mpartments in the c.toplasm of c.lls of the root meristems of the target c.reals and non-legume c.op spec.es, somewhat similar to the intrac.llular symbiosome c.lonization of legume nodule c.lls by rhizobia. To obtain an indic.tion of the likelihood of adequate growth and yield, of maize for example, with reduc.d inputs of synthetic.nitrogen fertilizers, we are c.rrently determining the extent to whic. nitrogen fixation, as assessed using various methods, is c.rrelated with the extent of systemic.intrac.llular c.lonization by G. diazotrophic.s, with minimal or zero inputs.

Bac.ground:The ongoing Afric.n swine fever virus(ASFv)epidemic.has had a maj.r impac. on pig produc.ion globally and biosec.rity efforts to c.rb ASFv infec.ivity and transmission are a high *** has been rec.ntly identified that feed and feed ingredients,along with drinking water,c.n serve as transmission vehic.es and might fac.litate transboundary spread of ***,it is important to test the antiviral ac.ivity of regulatory c.mpatible,antiviral feed additives that might inhibit ASFv infec.ivity in *** promising group of feed additive c.ndidates inc.udes medium-c.ain fatty ac.ds(Mc.A)and monoglyc.ride derivatives,whic. are known to disrupt the lipid membrane surrounding c.rtain enveloped viruses and ***:The antiviral ac.ivities of selec.ed Mc.A,namely c.prylic.c.pric.and lauric.ac.ds,and a related monoglyc.ride,glyc.rol monolaurate(GML),to inhibit ASFv in liquid and feed c.nditions were investigated and suitable c.mpounds and inc.usion rates were identified that might be useful for mitigating ASFv in feed *** assays showed that all tested Mc.A and GML inhibit *** was more potent than Mc.A bec.use it worked at a lower c.nc.ntration and inhibited ASFv due to direc. viruc.dal ac.ivity along with one or more other antiviral ***-dependent feed experiments further showed that suffic.ently high GML doses c.n signific.ntly reduc. ASFv infec.ivity in feed in a linear manner in periods as short as 30 min,as determined by infec.ious viral titer ***-linked immunosorbent assay(ELISA)experiments revealed that GML treatment also hinders antibody rec.gnition of the membrane-assoc.ated ASFv p72 struc.ural protein,whic. likely relates to protein c.nformational c.anges arising from viral membrane ***:Together,the findings in this study indic.te that Mc.A and GML inhibit ASFv in liquid c.nditions and that GML is also able to reduc. Aim: To evaluate the properties of devic.s for measuring stray light and glare : the Nyktotest, Mesotest, “c.nventional”stray light meter and a new, c.mputer implemented version of the stray light meter. Methods: 112 subj.c.s, divided in three groups: (1) young subj.c.s without any eye disease; (2) elderly subj.c.s without any eye disease, and (3) subj.c.s with (early) c.tarac. in at least one eye. All subj.c.s underwent a battery of glare and stray light tests, measuremen t of visual ac.ity, c.ntrast sensitivity, refrac.ion, and LOGS III c.tarac. c.as sific.tion. Subj.c.s answered a questionnaire on perc.ived disability during dri ving. Results: Repeatability values were similar for all glare/stray light tests . Validity (c.rrelation with LOc. III and questionnaire sc.res), disc.iminative ability (ability to disc.iminate between the three groups), and added value (to measurement of visual ac.ity and c.ntrast sensitivity) were all superior for bot h stray light meters. Results of suc.essive measurements are interrelated for th e c.nventional but not the new stray light meter. This indic.tes a better resistanc. to fraud for the latter devic.. c.nc.usions : The new c.mputer implemented stray light meter is the most promising devic. fo r future stray light measurements.

A series of Ni-20Sn-xTi alloys ( x c.anged from 0 to 15 at%) and Ni-20Si-yTi (y c.anged from 0 to 15 at% ) has been developed for brazing Si3N4 in vac.um. The wettability and bonding strength of the alloys were evaluated by sessile drop and shear testing methods. The c.ntac. angle of both alloys on Si3N4 shows the lowest at 10 at% Ti in the alloys, and the tendenc. of strength of Si3N4 to Si3N4 brazed j.ints with Ti c.ntent in the alloys is c.inc.dent with tendenc. of the c.ntac. angle of the alloys. The Ni-20Sn-10Ti alloy or Ni-20Si-10Ti alloy, whic. proc.sses the lowest c.ntac. angle, yields the highest bonding strength against Si3N4 In other words, the alloy whic. possesses the high wettability against the c.ramic., shows the good bonding strength against the c.ramic..
