Isolated intestinal neurofibromatous proliferations in the absence of associated systemic syndromes

Gastrointestinal tract involvement by neurofibromatous lesions is rare and occurs most frequently as one of the systemic manifestations of generalized neurofibromatosis type 1 （NF1）. In this setting, the lesions may manifest as focal scattered neurofibromas or as an extensive diffuse neural hyperplasia designated ganglioneuromatosis. Occasionally, such lesions may be the initial sign of NF1 in patients without any other clinical manifestations of the disease. Rarely, cases of isolated neurofibromatosis of the large bowel with no prior or subsequent evidence of generalized neurofibromatosis have been documented. We present the case of a 52 year-old female with abdominal pain and alternating bowel habits. Colonoscopic evaluation revealed multiple small polyps in the cecum and the presence of nodular mucosa in the colon and rectum. Pathologic evaluation of the biopsies from the cecum, descending colon, sigmoid colon, and rectum revealed tangled fascicles of spindle cells expanding the lamina propia leading to separation of the intestinal crypts. Immunohistochemical stains helped confirm the diagnosis of diffuse intestinal neurofibromatosis. A thorough clinical evaluation failed to reveal any stigmata of generalized neurofibromatosis. This case represents a rare presentation of isolated intestinal neurofibromatosis in a patient without classic systemic manifestations of generalized neurofibromatosis and highlights the need in such cases for close clinical follow-up to exclude neurofibromatosis type I or multiple endocrine neoplasia type Ⅱ.

In this work,we investigated leaching of lanthanide and yttrium(REY) from a Central Appalachian coal and its ashes obtained at 550-950℃ with the main purpose of understanding the impact of ashing temperature on REY leachability in water,ammonium sulfate,and hydrochloric *** is found that the coal contains a negligible amount of water-soluble REY,less than 1% ion-exchangeable REY,and about 28% of HCl-soluble *** leads to dramatic changes in REY leachability in both ammonium sulfate and hydrochloric acid solutions,which is believed to be related to transformation and redistribution of organically-associated REY in coal during the ashing *** temperature significantly affects REY leaching from coal ashes;higher ashing temperature results in lower REY leachability in both *** minerals may play a significant role in changing the leachability of REY after *** addition,the results also suggest that the organic matter in the coal is relatively enriched in heavy REY.

Osteoporosis in primary biliary cholangitis

Primary biliary cholangitis(PBC) is an autoimmune cholestatic liver disease with multiple debilitating complications. Osteoporosis is a common complication of PBC resulting in frequent fractures and leading to significant morbidity in this population, yet evidence for effective therapy is lacking. We sought to summarize our current understanding of the pathophysiology of osteoporosis in PBC, as well as current and emerging therapies in order to guide future research directions. A complete search with a comprehensive literature review was performed with studies from Pub Med, EMBASE, Web of Science, Cochrane database, and the Countway Library. Osteoporosis in PBC is driven primarily by decreased bone formation, which differs from the increased bone resorption seen in postmenopausal osteoporosis. Despite this fundamental difference, current treatment recommendations are based primarily on experience with postmenopausal osteoporosis. Trials specific to PBC-related osteoporosis are small and have not consistently demonstrated a benefit in this population. As it stands, prevention of osteoporosis in PBC relies on the mitigation of risk factors such as smoking and alcohol use, as well as encouraging a healthy diet and weight-bearing exercise. The primary medical intervention for the treatment of osteoporosis in PBC remains bisphosphonates though a benefit in terms of fracture reduction has never been shown. This review outlines what is known regarding the pathogenesis of bone disease in PBC and summarizes current and emerging therapies.

The efficacy and safety profile of albumin administration for patients with cirrhosis at high risk of hepatorenal syndrome is dose dependent

Background:Albumin is a critical component in the standard therapeutic approach to acute renal failure(ARF)and spontaneous bacterial peritonitis(SBP)in the setting of ***,data regarding the safety and minimumeffective dose are ***:We conducted a retrospective review of patients with decompensated cirrhosis who received albumin within the first 48 hours of hospitalization at Beth Israel Deaconess Medical Center between 2010 and *** included 90-day risk of death or transplantation(primary)and(secondary)complications of albumin infusion(length of stay(LOS)and need for critical care),all adjusted for comorbidity and severity of ***:We included 169 patients with ARF and 88 patients with *** optimal doses of albumin for a survival benefit were found to be 87.5 g and 100 g in the ARF and SBP cohorts,*** odds ratio(OR)for the 90-day risk of death or liver transplantation associated with the optimal loading dose was 0.36(95%CI:0.17-0.76,P=0.008)and 0.28(95%CI:0.07-0.97,P=0.04)for the ARF and SBP cohorts,*** effect persisted for patients with ARF who had neither hepatorenal syndrome(HRS)nor SBP(OR:0.13,95%CI:0.007-0.79,P=0.02).LOS(beta coefficient per log albumin dose:1.69;95%CI:0.14-3.24,P=0.03)and risk of critical care(OR/g albumin:1.03;95%CI:1.01-1.05,P=0.01)were also dose ***:Albumin has a dose-dependent effect on both survival and complications in patients with cirrhosis with ARF(HRS and otherwise)and/or SBP.

Effect of High-Intensity Intermittent Hypoxic Training on 3-on-3 Female Basketball Player’s Performance

Purpose To investigate the effects of 4 weeks high-intensity interval training in hypoxia on aerobic and anaerobic perfor-mance of 3-on-3 basketball *** In a randomised controlled trial,15 female basketballers completed eight 1-h high-intensity training sessions in either normobaric hypoxia(hypoxic group n=8,altitude 3052 m)or normoxia(normoxic group n=7,sea-level).Results After training,the hypoxic group increased their 1-min all-out shuttle run distance by 2.5%±2.3%(mean±95%CI,d=0.83,P=0.04),compared to the normoxic group 0.2%±2.3%(d=0.06,P=0.8),with the difference between groups being clinically worthwhile but not statistically significant(d=0.77,P=0.1).Distance covered in the Yo-Yo intermittent recovery test tended to increase in the hypoxic(32.5%±39.3%,d=1.0,P=0.1)but not normoxic group(0.3%±24.5%,d=0.08,P=0.9),with a non-significant change between groups(d=0.9,P=0.2).Compared to normoxia,the hypoxic group significantly increased subjective markers of stress(d=0.53,P=0.005),fatigue(d=0.43,P=0.005),and muscle soreness(d=0.46,P=0.01),which resulted in a lower perceived training performance in the hypoxic compared to the normoxic group(d=0.68,P=0.001).Conclusion High-intensity interval training under hypoxic conditions likely improved 1-min all-out shuttle run ability in female basketball 3-on-3 players but also increased subjective markers of stress and fatigue which must be taken into con-sideration when prescribing such training.

Early testing of insulin resistance: a tale of two lipid ratios in a group of 5th graders screened by the Coronary Artery Risk Detection in Appalachian Communities Project (CARDIAC Project)

Background In West Virginia (WV),47% of fifth-grade children are either overweight or *** is no clear consensus regarding the definition of insulin resistance in children,and directly measuring insulin on the population level is *** proposed measures examined further in this study include triglyceride (TRIG)/high-density lipoprotein cholesterol (HDL-C) ratio and TRIG/low-density lipoprotein (LDL-C) *** purpose of this study is to examine the relationship between TRIG/HDL-C ratio,TRIG/LDL-C ratio and insulin resistance in fifth-graders with acanthosis nigricans (AN).Methods Between 2007 and 2016,52,545 fifth-grade students in WV were assessed for *** glucose and insulin levels were collected only for a sub-group of students who were AN-positive and was used to determine insulin resistance using the Homeostatic Model for Insulin Resistance (HOMA-IR) *** analysis included t tests and logistic regression with receiver operating characteristic *** Of the students assessed for AN,4.5% (n =2360) tested *** prevalence of insulin resistance was 79% (n =814) among 1030 with AN and complete ***/HDL-C ratio and TRIG/LDL-C ratio were significantly associated with insulin resistance (TRIG/HDL-C:Est.=0.36,P < 0.0001,AUC =0.68;TRIG/LDL-C:Est.=0.87,P < 0.0001,AUC =0.69).Multivariate analysis showed that increased body mass index (Est.=0.05,P < 0.0001),gender (Est.=0.49,p < 0.0001) and TRIG/HDL-C ratio (Est.=0.21,P < 0.0001) were significantly associated with insulin *** TRIG/HDL-C is a better surrogate marker of insulin resistance in AN-positive children compared to TRIG/ LDL-C ratio;so,on a population-level,cholesterol rather than insulin may be obtained for preliminary testing of early insulin resistance in children.
