Use of donepezil for neurocognitive recovery after brain injury in adult and pediatric populations:a scoping review

There are few pharmacologic options for the treatment of cognitive deficits associated with traumatic brain injury in pediatric *** inhibitors such as donepezil have been evaluated in adult patients after traumatic brain injury,but relatively less is known about the effect in pediatric *** goal of this review is to identify knowledge gaps in the efficacy and safety of acetylcholinesterase inhibito rs as a potential a djuvant treatment fo r neurocognitive decline in pediatric patients with traumatic brain *** queried PubMed to identify literature published from database inception thro ugh june 2023 desc ribing the use of donepezil in young adult traumatic brain injury and pediatric patients with predefined *** on preselected search criteria,340 unique papers we re selected for title and abstra ct ***-two reco rds were reviewed in full after eliminating preclinical studies and pape rs outside the scope of the *** adult traumatic brain injury,we review results from 14 papers detailing 227 subjects where evidence suggests donepezil is well tole rated and shows both objective and patient-reported efficacy for reducing cognitive *** children,3 pape rs report on 5 children recovering from traumatic brain injury,showing limited *** additional 15 pediatric studies conducted in populations at risk for cognitive dysfunction provide a broader look at safety and efficacy in 210 patients in the pediatric age *** its promise for efficacy in adults with traumatic brain injury and tole rability in pediatric patients,we believe further study of donepezil for children and adolescents with traumatic brain injury is warranted.

Two-stage corrective operation for the treatment of pes cavovarus in patients with spina bifida

BACKGROUND Pes cavovarus has an estimated incidence of 8%-17%in patients with spina bifida(SB).The majority of the current literature on surgical treatment of cavovarus feet in children and adolescents includes a variety of *** are currently no case series describing a treatment algorithm for deformity correction in this specific patient *** authors of this study present the results of a retrospective case series performed to assess the radiographic outcomes of two-stage corrective surgery in patients with *** Retrospective chart review was performed on patients with SB with a diagnosis of pes cavovarus at a freestanding children’s hospital who underwent surgical correction of the *** were excluded for lack of two-stage corrective operation,nonambulatory status,lack of at least six months follow-up,and age>18 years at the time of *** resulted in a cohort of 19 *** analysis was performed on 11 feet that had a complete series of preoperative and postoperative weightbearing *** and postoperative radiographic outcome measurements were compared using a two-sample *** Significant changes between the preoperative and postoperative measurements were seen in Meary’s angle,the anteroposterior talo-first metatarsal(AP TMT1)angle,and the talonavicular *** values of Meary’s angle were 17.9±13.1 preoperatively and 4.7±10.3 postoperatively(P=0.016).Mean AP TMT1 angle was 20.6±15.1 preoperatively and 9.3±5.5 postoperatively(P=0.011).Mean talonavicular coverage values were-10.3±9.6 preoperatively and-3.8±10.1 postoperatively(P=0.025).CONCLUSION The two-stage corrective procedure demonstrated efficacy in correcting cavovarus deformity in patients with *** should strongly consider employing the staged surgical algorithm presented in this manuscript for management of these patients.

Vaccination against Coronavirus disease-19(COVID-19)was pivotal to limit spread,morbidity and *** aim is to find out whether vaccines against COVID-19 lead to an immunological response stimulating the production of de novo donor specific antibodies(DSAs)or increase in mean fluorescence intensity(MFI)of pre-existing DSAs in kidney transplant recipients(KTRs).This study involved a detailed literature search through December 2nd,2023 using PubMed as the primary *** search strategy incorporated a combination of relevant Medical Subject Headings terms and keywords:"COVID-19","SARS-CoV-2 Vaccination","Kidney,Renal Transplant",and"Donor specific antibodies".The results from related studies were collated and analyzed.A total of 6 studies were identified,encompassing 460 KTRs vaccinated against *** responses were detected in 8 KTRs of which 5 had increased MFIs,1 had de novo DSA,and 2 were categorized as either having de novo DSA or increased *** were 48 KTRs with pre-existing DSAs prior to vaccination,but one study(Massa et al)did not report whether pre-existing DSAs were associated with post vaccination *** the remaining 5 studies,35 KTRs with pre-existing DSAs were identified of which 7 KTRs(20%)developed de novo DSAs or increased ***,no immunological response was detected in 452(98.3%)*** study affirms prior reports that COVID-19 vaccination is safe for KTRs,especially if there are no pre-existing ***,if KTRs have pre-existing DSAs,then an increased immunological risk may be *** findings need to be taken cautiously as they are based on a limited number of patients so further studies are still needed for confirmation.

Modelling internal air systems ingas turbine engines

Rotating-disc systems can be used to model,experimentally and computationally,the flow and heat transfer that occur inside the internal cooling-air systems of gas turbine *** rotating-disc systems have been used successfully to simplify and understand some of the complex flows that occur in internal-air systems,and designers have used this insight to improve the cooling effectiveness,thereby increasing the engine efficiency and reducing the *** this review paper,three important cases are considered:hot-gas ingress;the pre-swirl system;and buoyancy-induced ***,or ingestion,occurs when hot gas from the mainstream gas path is ingested into the wheel-space between the turbine disc and its adjacent *** seals are fitted at the periphery of the system,and sealing flow is used to reduce or prevent ***,too much sealing air reduces the engine efficiency,and too little can cause serious overheating,resulting in damage to the turbine rim and blade *** the flow is three-dimensional and unsteady,there are encouraging signs that simple ‘orifice models’ could be used to estimate the amount of ingress into the *** a pre-swirl system,the cooling air for the gas-turbine blades is swirled by stationary nozzles,and the air is delivered to the blades via receiver holes in the rotating turbine *** the air reduces its temperature relative to the rotating blades,and the designer needs to calculate the air temperature and pressure drop in the *** designer also needs to calculate the effect of this swirling flow on the heat transfer from the turbine disc to the air,as this has a significant effect on the temperature distribution and stresses in the *** experimental and computational studies have given a better understanding of the flow and heat transfer in these ***-induced flow occurs in the cavity between two co-rotating compressor discs when the temperature of the discs is higher than

The sperm maturation and storage functions of the epididymis are importantdeterminants of ejaculate quality, and perhaps provide an avenue to male contraception. In the last 50 years, the creation of epididymal fertility profiles in laboratory animals was followed by recognition of new sperm maturation-related parameters （organization of the acrosome, of the sperm plasmalemma, and -S-S- -based structural change） which made it possible to confirm that a similar pattern of sperm maturation obtains in man. The novel sperm storage function of the cauda epididymidis in therian mammals is regulated by androgen, usually in conjunction with the low temperature of the *** temperature-dependence of the scrotal cauda is reflected in the secretory and ion transport functions of the epithelium, in its duct dimensions and so in sperm storage capacity. Moreover, a variety of indirect evidence suggests that an elevated temperature of the cauda created by clothing may be compromising its function in man. ＂l-he pattern of change in the sperm plasmalemma involving sterols, and also glycosylphosphatidylinositol-linked macromolecules as spermatozoa enter the cauda region, may underlie the need for their capacitation subsequentlyin the female tract. Further, in a variety of taxa the anatomy of the scrotum, together with the U-shaped configuration of the epididymis/vas deferens, suggests that the cauda＇s storage function may also underlie the evolution of the scrotum. Finally, despite the still relative paucity of comparative evidence, we can consider now why the epididymis has come to be organized as it is.

Plant viruses in the families Luteoviridae and Geminiviridae are phloem restricted and are transmitted in a persistent,circulative manner by homopteran *** fluorescence 2-D difference gel electrophoresis to compare the proteomes of F2 genotypes of Schizaphis graminum segregating for virus transmission ability,we recently discovered a panel of protein biomarkers that predict vector *** we used aphid and whitefly nucleotide and expressed sequence tag database mining to test whether these biomarkers are conserved in other homopteran *** gene homologs that shared a high degree of predicted amino acid identity were discovered in two other aphid species and in the whitefly Bemisia *** reaction monitoring mass spectrometry was used to validate the expression of these biomarkers proteins in multiple aphid vector *** conservation of these proteins in multiple insect taxa that transmit plant viruses along the circulative transmission pathway creates the opportunity to use these biomarkers to rapidly identify insect populations that are the most efficient vectors and allow them to be targeted for control prior to the spread of virus within a crop.

This review highlights the recent evolution of the imaging,medical management,surgical options and endovascular therapies for symptomatic intracranial atherosclerotic disease(ICAD).Recent imaging developments including optical coherence tomography and other modalities to assess the intracranial arteries for symptomatic ICAD are reviewed,not only to diagnose ICAD but to determine if ICAD plaques have any high-risk features for *** future developments in the treatment of ICAD are discussed,including the development of trackable drug-coated balloons for the cerebral circulation to treat primary or restenotic arteries,new iterations of self-expanding intracranial stents with easier delivery systems,and the re-examination of indirect surgical bypass techniques for *** addition to these important technological developments,however,is the evolving evidence regarding the best treatment window for these techniques and additional factors in medical management which can improve patient outcomes in this devastating pathology.

Dryland agriculture and rangeland restoration priorities in Afghanistan

Afghanistan is threatened by rangeland degradation.A quantitative visual analysis of Google Earth Imagery was used to systematically locate,characterize and quantify the current extent of rangelands in Afghanistan degraded as a consequence of dryland *** data were used in conjunction with dryland agriculture locations to establish a climate envelope comprised by temperature and mean annual precipitation to create a geographical mask known to contain dryland *** this mask we created a grid of 100 km2 cells that we analyzed individually to access dryland agriculture *** limits to sustainable dryland agriculture and areas of high restoration priority were also assessed as was the distribution of rain-fed agriculture with respect to the location of traditional migration routes for extensive livestock *** extents of agriculture in Afghanistan,at both upper and lower elevations,correlated most closely with mean annual temperature（MAT） at the upper elevation limits,and with mean annual precipitation（MAP） at the lower elevation *** total,dryland agriculture comprised 38,980 km2 of former native *** was highest in the northwestern,northern and northeastern provinces of Herat,Badghis,Faryab,jawzjan,Sar-e-Pul,Samangan,Balkh,Baghlan,Kunduz,Takhar and Badakhshan,with the highest percentage of conversion occurring in *** MAP value of 〈400 mm is perceived by farmers as the current climatic limit to sustainable dryland agriculture across the northern regions of the *** this MAP value,approximately 27,677 km2 of converted rangeland met the need for restoration *** projections indicate that Afghanistan will become warmer and drier in the coming *** consequence of this trend is that the MAP threshold of 〈400 mm to sustainable dryland agriculture will become obsolete in the coming *** of currently converted rangelands is needed to restore critical grazing area
