Management of recurrent rectal cancer:A population based study in greater Amsterdam

AIM： To analyze, retrospectively in a populationbased study, the management and survival of patients with recurrent rectal cancer initially treated with a macroscopically radical resection obtained with total mesorectal excision （TME）. METHODS： All rectal carcinomas diagnosed during 1998 to 2000 and initially treated with a macroscopically radical resection （632 patients） were selected from the Amsterdam Cancer Registry. For patients with recurrent disease, information on treatment of the recurrence was collected from the medical records. RESULTS： Local recurrence with or without clinically apparent distant dissemination occurred in 62 patients （10%）. Thirty-two patients had an isolated local recurrence. Ten of these 32 patients （31%） underwent radical re-resection and experienced the highest survival （three quarters survived for at least 3 years）. Eight patients （25%） underwent non-radical surgery （median survival 24 rno）, seven patients （22%） were treated with radio- and/or chemotherapy without surgery （median survival 15 mo） and seven patients （22%） only received best supportive care （median survival 5 too）. Distant dissemination occurred in 124 patients （20%） of whom 30 patients also had a local recurrence. The majority （54%） of these patients were treated with radio- and/or chemotherapy without surgery （median survival 15 mo）. Twenty-seven percent of these patients only received best supportive care （median survival 6 mo）, while 16% underwent surgery for their recurrence. Survival was best in the latter group （median survival 32 mo）. CONCLUSION： Although treatment options and survival are limited in case of recurrent rectal cancer after radical local resection obtained with TME, patients can benefit from additional treatment, especially if a radical resection is feasible.

Factors determining delay in relaparotomy for anastomotic leakage after colorectal resection

AIM： To analyze the time interval （‘delay＇） between the first occurrence of clinical parameters associated with anastomotic leakage alter colorectal resection and subsequent relaparotomy. METHODS： In 36 out of 289 consecutive patients with colorectal anastomosis, leakage was confirmed at relaparotomy. The medical records of these patients were retrospectively analysed and type and time of appearance of clinical parameters suggestive of anastomotic leakage were recorded. These parameters included heart rate, body temperature, local or generalized peritoneal reaction, leucocytosis, ileus and delayed gastric emptying. Factors influencing delay of relaparotomy and consequences of delayed recognition and treatment were determined. RESULTS： First documentation of at least one of the predefined parameters for anastomotic leakage was alter a median interval of 4 ± 1.7 d alter the operation. The median number of days between first parameter（s） associated with leakage and relaparotomy was 3.5 ± 5.7 d. The time interval between the first signs of leakage and relaparotomy was significantly longer when a weekend was included （4.2 d vs 2.4 d, P = 0.021） or radiological evaluation proved to be false-negative （8.1 d vs 3.5 d, P = 0.007）. No significant association between delay and number of additional relaparotomies, hospital stay or mortality could be demonstrated.CONCLUSION： An intervening weekend and negative diagnostic imaging reports may contribute to a delay in diagnosis and relaparotomy for anastomotic leakage. That delay was more than two days in two-thirds of the patients.

Myotonic dystrophy-related Cdc42-binding kinase alpha(MRCKα)is an integral component of signaling pathways controlling vital cellular processes,including cytoskeletal reorganization,cell proliferation and cell *** this study,we investigated the physiological role of MRCKα in milk protein and fat production in dairy cows,which requires a dynamic and strict organization of the cytoskeletal network in bovine mammary epithelial cells(bMEC).Within a selection of 9 Holstein cows,we found that both mRNA and protein expression of MRCKα in the mammary gland were upregulated during lactation and correlated positively(r>0.89)with the mRNA and protein levels of *** positive correlations(r>0.79)were found in a primary culture of bMEC stimulated with prolactin for 24 *** these cells,silencing of MRCKα decreased basal b-casein,sterol-regulatory element binding protein(SREbP)-1 and cyclin D1 protein level,phosphorylation of mTOR,triglyceride secretion,cell number and viabilitydwhile overexpression of MRCKα displayed the reversed ***,silencing of MRCKα completely prevented the stimulatory action of prolactin on the same *** data demonstrate that MRCKα is a critical mediator of prolactin-induced lactogenesis via stimulation of the mTOR/SREbP1/cyclin D1 signaling pathway.

Investigation of operational parameters for an industrial CFb combustor of coal,biomass and sludge

The combustion of coal and/or biomass （sludge, wood waste, RDF, etc.） in a circulating fluidized bed has been a commercial topper for over 20 years, and references to principles and applications are numerous and widespread although few data are presented concerning the operation of large scale CFb-units. The authors studied the CFb-combustion at UPM-Kymmene （Ayr）, a major paper mill relying for its steam production upon the combustion of coal （80-85 %）, wood bark （5-10%） and wastewater treatment sludge （5-10%）. The maximum capacity of the CFb is 58 MWth. A complete diagnostic of the operation was made, and additional tests were performed to assess the operating mode. The plant schematics, relevant dimensions and process data are given. To assess the operation of the UPM-CFb, it is important to review essential design parameters and principles of CFb combustors, which will be discussed in detail to include required data, heat balance and flowrates, operating versus transport velocity, kinetics and conversion （including the possible effect of the bouduard reaction if carbon is present）. Since the residence time in the riser and the cyclone efficiency determine the burnout of circulating fuel-particles, the UPM-CFb was subjected to a stimulus response technique using nickel oxide as tracer. Results illustrate the efficiency of the cyclone separation and the number of recycle loops for particles of a given size. Results will also be used to assess the cyclone operation and efficiency and to comment upon expected and measured carbon conversion.

background: Characterising the regulation of milk component synthesis in response to macronutrient supply is critical for understanding the implications of nutritional interventions on milk production. Gene expression in mammary gland secretory cells was measured using RNA isolated from milk fat globules from 6 Holstein-Friesian cows receiving 5-d abomasal infusions of saline, essential amino acids(AA), or glucose(GG) or palm olein(LG)without(LAA) or with(HAA) essential AA, according to a 6 × 6 Latin square design. RNA was isolated from milk fat samples collected on d 5 of infusion and subjected to real-time quantitative PCR. We hypothesised that m RNA expression of genes involved in de novo milk fatty acid(FA) synthesis would be differently affected by GG and LG,and that expression of genes regulating transfer of tricarboxylic acid cycle intermediates would increase at the HAA level. We also hypothesised that the HAA level would affect genes regulating endoplasmic reticulum(ER)homeostasis but would not affect genes related to the mechanistic target of rapamycin complex 1(m TORC1) or the integrated stress response(ISR) ***: Infusion of GG did not affect de novo milk FA yield but decreased expression of FA synthase(FASN).Infusion of LG decreased de novo FA yield and tended to decrease expression of acetyl-Co A carboxylase 1(ACC1).The HAA level increased both de novo FA yield and expression of ACC1, and tended to decrease expression of mitochondrial phosphoenolpyruvate carboxykinase(PCK2). m RNA expression of m TORC1 signaling participants was not affected by GG, LG, or AA level. The vitality of European forests continues to decline due to new pests and diseases,climate-change related disturbances and high loads of atmospheric nitrogen *** soil health is a major factor underpinning the low vitality of West-European *** tree species with soil ameliorative traits is proposed as an avenue to counteract soil acidification and improve overall forest *** we evaluate the impact of black cherry(Prunus serotina Ehrh.),a known rich litter species,on the vitality of neighboring pedunculate oak(Quercus robur L.)in ten mixed forests on sand in Germany,belgium and the *** found that black cherry admixture increases foliar N and P to a surplus whereas it causes deficiencies in foliar Mg,thereby resulting in an overall negative effect on oak foliar nutrient ***,defoliation of oak leaves by herbivory decreases with the proximity of black *** structural equation modelling(SEM),we tested the hypothesized‘improved soil health’*** analyses showed that black cherry admixture leads to lower accumulation in the humus layer,resulting in higher soil base saturation which has a positive effect on foliar Ca yet a negative effect on total ***,the SEM illustrated that herbivory of oak leaves decreases when black cherry is admixed,both via dilution and improved soil *** effects of black cherry on oak vitality via“improved soil health”in our SEM are however small in comparison to direct ***,our study showed that the combined positive and negative impacts of black cherry on oak vitality are limited,which tempers the potential benefits of using the rich litter species to counteract oak decline via improved soil health–yet,the concern of black cherry as an invasive alien species negatively affecting the vitality of mature pedunculate oak trees may also be exaggerated.
