Risk Stratification in Obstetrics: An Integrated Approach to Maternal Health

Introduction: Prenatal care is crucial for maternal and fetal health, highlighting the importance of obstetric risk stratification. This enables personalized care, avoiding unnecessary interventions and reducing costs. Conditions such as first trimester bleeding, spontaneous abortion, gestational trophoblastic disease, and ectopic pregnancy require specific approaches. Early identification of these complications is vital, especially in urgent and emergency obstetric cases, which demand immediate hospital attention. Objective: Comprehensive review of first trimester pathologies and gestational hypertension, focusing on obstetric risks and personalized prenatal management. Methods: Literature review on bleeding and gestational hypertension. Results: The study provides a detailed analysis of pathologies associated with first trimester uterine bleeding and hypertensive disorders, focusing on obstetric risk stratification. The main causes of maternal and fetal mortality are hypertensive disorders, hemorrhages, infections, childbirth complications, and abortion. Spontaneous abortion is common, with different classifications, ranging from threatened to missed abortion. Infected abortion is a severe complication. Brazil has restrictive abortion laws, but many unsafe abortions occur, resulting in high public health costs. Pharmacological treatment with misoprostol is a safe option. Other pathologies include gestational trophoblastic disease, ectopic pregnancy, and placenta previa. Preeclampsia and eclampsia are severe conditions requiring immediate treatment to avoid complications. Conclusion: Early identification and management of obstetric risk factors, such as uterine bleeding, are essential for positive maternal and fetal outcomes. A multidisciplinary approach is fundamental.

Background: We currently have international and national guidelines regarding the assessment and monitoring of clinical signs and humane endpoints in animals used in teaching and research, which make the performance of these activities mandatory for any experiment and professional working in this area. Assigning the severity of a research experiment is the result of an analysis of records of observations of the animal’s behavior, and clinical signs. The aim of this study was to describe the importance of carrying out a severity assessment associated with clinical and behavioral monitoring of rodents and rabbits during experimentation to maintain the welfare of these animals undergoing scientific research. Methods: The literature search was carried out using the following terms: “Monitoring”;“Humane endpoints”;“Animal welfare”, “Rodents”;“Rabbits”, and as connectors “and”;“or”, in the following databases: PubMed;LILACS/BIREME and SciELO. Results: A total of 987 articles were identified in the databases, and 20 of these studies were included in this review. Conclusions: Humane endpoint protocols and procedure severity tables are of the utmost importance, both from an ethical point and to refine the results of research conducted on laboratory animals. They should be drawn up jointly by the teams responsible for the project and the maintenance of the animals during the research period, and the data obtained should be published so that the scientific community can have access to it, helping to disseminate these practices, as well as helping to draw up new procedures. Monitoring and evaluating the welfare and clinical condition of animals undergoing scientific research procedures is the responsibility of the professors, researchers, veterinarians, and animal facility coordinators. Association between Infrared Thermography, Blood Count and Creatine Kinase in the Evaluation of the Welfare of Vaquejada Horses

The Quarter-Horses regularly participate in non-traditional sports that simulate activities on cattle ranches. In the vaquejada races, the horses run at about 30 km/h, reaching maximum heart rates of around 200 bpm, indicating the great physical effort during competitions and with that could impact welfare. To test the hypothesis that vaquejada horses can be evaluated for the quality of welfare through a combination of non-invasive and invasive methods, an experiment was developed that aimed to assess the quality of welfare through thermography and blood biomarkers before and after vaquejada races. Ten vaquejada horses, which were in regular competition, were submitted to the vaquejadas racing test were used. Thermography was performed in 14 regions of interest (head, neck and thorax) in the following phases: pre-race and +1, +4 and +24 hours of the races. Blood samples were collected in pre-race, immediately after and +1, +4 and +24 hours of the races. Results were submitted to ANOVA and Tukey’s test, with p set at 5%. Thermography showed that local temperatures before the race and +24 hours after the races were similar (p > 0.05), as well as between +1 and +4 hours after the races (p > 0.05), which showed higher surface temperatures than the first group (pre-race and +24 hours) (p 0.05) after the exercises. In conclusion, the vaquejada horses evaluated by infrared thermography and blood analyses maintained the quality of well-being, with the physiological elevation of several parameters after the races and recovery in less than 24 hours after the races. Thermographic images were also able to help in the analysis of the regions of interest that have contact with harnesses, demonstrating that the use of adequate and correct equipment does not compromise animal welfare.

In recent decades, the intensity of training and equestrian competitions has significantly increased, thus the assessment of the well-being of the equine athlete has become essential in all equestrian modalities. The aim of this study was to ascertain whether equine athletes submitted to a vaquejada simulation test (VqST), comprised of three races, presented changes in blood biomarkers related to stress and health status. Fourteen healthy Quarter Horses, used as pull horses in this equestrian modality, were evaluated. Ten animals were submitted to the VqST and the remaining four were used as a control group. Blood samples were collected pre-test (during fast), immediately after, and at 1, 4 and 24 hours of recovery. The assessed blood biomarkers included cortisol, interleukin (IL)-6, IL-1β, iron, urea, creatinine, and gamma-glutamyl transferase (GGT) concentrations and results were analyzed using One Way ANOVA (time) with the SigmaStat 13.0 software. No differences between sample times were detected in both groups (p > 0.05) and no differences were found between groups (p > 0.05). The results suggest that all horses were well conditioned for the level of effort imposed by the three vaquejada races. The adaptation to physical exercise may enable the regulation of the acute response to stress in the tissues involved in the exercises, with no differences being observed in stress and health biomarkers, such as IL-6, IL-1β and cortisol. In conclusion, well-conditioned vaquejada horses exhibit a balanced regulation of biological processes, which contributes an increased athletic longevity and better quality of athletic life.

Subacute Cutaneous Lupus Erythematosus in Older Man: Case Report

Systemic lupus erythematosus (SLE) is an autoimmune disease that affects mainly women, presents several clinical manifestations and has no known exact cause. The skin is one of the organs that can be affected, with subacute cutaneous lupus erythematosus (SCLE) being one of the classifications according to cutaneous lupus erythematosus lesions. SCLE is commonly found in young to middle age women and carries characteristics related to sun exposure. In this report, there is a rare case of a male and elderly patient that presents SCLE subgroup initially asymptomatic, with subsequent appearance of macular skin lesions and vitiligo appearance. This is an important case to alert health professionals that the disease can manifest itself and open its clinical profile even in patients of sex and unusual ages.

Objective:To evaluate antimicrobial and antioxidant properties of saline extract from Tithonia diversifolia leaves by phytochemical bioprospecting,and investigate its safety against animal ***:The saline extract was prepared,with NaCl(0.15 M),by constant stirring of the dried and pulverized leaves,followed by volume reduction by *** extract was phytochemical characterized using ultra-performance liquid chromatography,and total phenol and flavonoid analysis also was *** antioxidant capacity was determined through DPPH*radical,the antimicrobial property was evaluated against standard bacteria and fungi,and the viability assays were performed against mice ***:Fifteen compounds were identified belonging to two main classes terpenoids and *** extract showed 22.185 mg GAE/g of total phenolic compounds and 3.220 mg QE/g of ***,extract showed higher antioxidant ability similar to butylated hydroxytoluene a standard molecule[(3.042±0.019)mg AAE/g and(4.12±0.10)mg AAE/g to saline extract and butylated hydroxytoluene,respectively].The antimicrobial assays demonstrated that the extract had a significant antifungal potential against Candida species and could be used with safety against mice splenocytes,in concentrations lower than 50μg/mL,promoting higher proliferation in these ***:Saline extract from Tithonia diversifolia leaves presents potential antioxidant,antifungal properties and induces immunostimulation in mice splenocytes.

Introduction: Syphilis consists in a systemic infect contagious pathology with a chronic character. The etiological agent consists of an anaerobic spirochete bacterium, Treponema pallidum. Laboratory diagnosis can be made through a direct investigation of the etiologic agent or non-treponemal (VDRL) and treponemal (FTA-Abs) serological tests. False-negative results are rare. Objectives: The present report presents a rare case of a man with a secondary syphilis syndrome who was initially undiagnosed due to the prozone effect, since he presented VDRL in low titers, and was later confirmed with a new diluted serum sample and VDRL and FTA-Abs, which were positive. Conclusion: A close look should be given to patients who, although the clinical condition is compatible, complementary exams may not be, and clinical cases should be carefully evaluated so that the patient is not treated late.

Cutaneous Vasculitis with Renal Impairment in Octogenarian

Henoch-Schönlein purpura is a vasculitis of small vessels, characterized by tissue deposition of IgA, affecting predominantly the skin, intestine, joints and kidneys. It mainly affects the pediatric age group, especially from 2 to 11 years old. Most often, it is preceded by episode of upper airway infection from days to weeks. The most common clinical manifestations are purpuric cutaneous lesions located in the lower limbs and buttocks, abdominal pain in colic, nausea, vomiting, diarrhea, in addition to arthralgia/arthritis, usually with spontaneous resolution. Renal impairment usually manifests with hematuria and proteinuria, and renal function may also be impaired. The case described refers to a female patient, 86 years old, with a previous history of infection of the upper airways, followed by purpura in the lower limbs and renal impairment: serum creatinine 3.1 mg/dL (N 0.5 - 1.2 mg/dL);glomerular filtration rate 15 ml/min (N > 60 ml/min);urea 118 mg/dL (N önlein purpura in an atypical age group, with significant renal impairment, improvement only with conservative treatment, thus avoiding the use of immunosuppressants and the risks of immunosuppression and toxicity related to such drugs.

Grid-Connected PV Solar Energy Converter with Active and Reactive Power Control

This work proposes a 12 kW three-phase grid-connected single stage PWM DC-AC converter destined to process the energy provided by a photovoltaic array composed of 57 KC200GT PV modules with high power factor for any solar radiation. The PWM inverter modeling and the control strategy, using dq0 transformation, are proposed in order to also allow the system operation as an active power filter, capable to compensate harmonic components and react power generated by the non-linear loads connected to the mains grid. An input voltage clamping technique is proposed to impose the photovoltaic operation on the maximum power point. Simulation and experimental results are presented to validate the proposed methodology for grid connected photovoltaic generation system.
