Combination approaches in hepatocellular carcinoma:how systemic treatment can benefit candidates to locoregional modalities

The management of hepatocellular carcinoma(hCC)is challenging because most patients have underlying cirrhosis,and the treatment provides,historically,a limited impact on the natural history of patients with advanced-stage ***,recurrence rates are high for those patients who receive local and locoregional modalities,such as surgical(resection and transplantation)or imageguided(ablation and intra-arterial)*** research has led to new concepts that are reshaping the current clinical *** advancements were achieved in the understanding of the hallmarks that drive *** has primed a successful incorporation of novel agents with different targets,such as anti-angiogenic drugs,targeted-therapies,and immune-checkpoint *** clinical trials have proven efficacy of systemic agents in advanced stage disease,there is no conclusive evidence to support their use in combination with loco-regional *** novel local modalities are being incorporated(e.g.,radioembolization,microwave ablation,and irreversible electroporation),emerging data indicate that locoregional treatments may induce tumor microenvironment changes,such as hyperexpression of growth factors,release of tumor antigens,infiltration of cytotoxic lymphocytes,and modulation of adaptative and innate immune *** trials that evaluated the use of antiangiogenic drugs in the adjuvant setting after ablation or chemoembolization fail to demonstrate a substantial *** efforts are directed to investigate the role of immunotherapy-based regimens in this *** present review aims to describe the current landscape of systemic and locoregional treatments for hCC,present evidence to support combination approaches,and address future perspectives.

Trial eligibility in advanced hepatocellular carcinoma: Does it support clinical practice in underrepresented subgroups?

Although hepatocellular carcinoma is considered a highly lethal malignancy,recent therapeutic advances have been achieved during the last 10 *** scenario resulted in an unprecedented improvement in survival for patients with advanced hepatocellular carcinoma,almost reaching 20-26 mo of overall survival after first-second line sequential *** advent of the combination of atezolizumab with bevacizumab showed,for the first time,superiority over sorafenib with improvement in overall ***,first and second-line trials were correctly based on the premise that a strict selection of patients enhances the power to capture the positive effect of treatment by excluding competing risks for mortality such as liver failure,decompensated cirrhosis or other underlying medical *** a result,the inclusion criteria used in clinical trials do not support the use of novel therapies in several real-world scenarios involving underrepresented subgroups,such as patients with unpreserved liver function,other comorbid conditions,a history of solid-organ transplantation,autoimmune disorders and those with a high risk of *** present text aims at discussing treatment strategies in these subgroups.

Critically ill cirrhotic patients have high in-hospital mortality and utilize significant health care resources as a consequence of the need for multiorgan *** this fact,their mortality has decreased in recent decades due to improved care of critically ill ***-on-chronic liver failure(AClF),sepsis and elevated hepatic scores are associated with increased mortality in this population,especially among those not eligible for liver *** score is superior to another in the prognostic assessment of these patients,and both liver-specific and intensive care unit-specific scores have satisfactory predictive *** sequential assessment of the scores,especially the Sequential Organ Failure Assessment(SOFA)and Chronic liver Failure Consortium(ClIF)-SOFA scores,may be useful as an auxiliary tool in the decision-making process regarding the benefits of maintaining supportive therapies in this population.A ClIF-AClF>70 at admission or at day 3 was associated with a poor prognosis,as well as SOFA score>19 at baseline or increasing SOFA score>*** studies addressing the prognostic assessment of these patients are necessary.

Automatic detection and evaluation of sugarcane planting rows in aerial images

Sugarcane planting is an important and growing activity in ***,several techniques have been developed over the years to maximize crop productivity and profit,amongst them,processing of sugarcane field *** this sense,this research aims to identify and analyze crop rows and measure their gaps from aerial images of sugarcane *** this,a small Remotely Piloted Aircraft captured the images,generating orthomosaics of the areas for ***,each orthomosaic is classified with the K-Nearest Neighbor algorithm to segment regions of *** row orientation is estimated using the RGB gradient *** operations and computational geometry models are then used to detect and map rows and gaps along the planting row *** evaluate the results,crop rows are mapped and compared to manually taken *** technique obtained an error smaller than 2%when compared to gap length in crop rows from an orthomosaic with the area of 8.05 ha(ha).The proposed approach can map the positioning of the automatically generated row segments appropriately onto manually created ***,our method also achieved similar results when confronted with a manual technique for differing growth stages(40 and 80 days after harvest)of the sugarcane *** proposed method presents a great potential to be adopted in sugarcane planting monitoring。

Stabilisation of estuarine sediments with an alkali-activated cement for deep soil mixing applications

In this work,an alternative alkali-activated cement(AAC)made of ladle slag precursor mixed with sodium hydroxide and sodium silicate has been developed to enhance the bearing capacity of estuarine soils in coastal conditions via deep soil mixing(DSM).The AAC was optimized to use a low reactivity precursor(ladle slag)and to deal with a contaminated high-water content natural sediment cured under *** material performance was analysed by comparison to a mixture made with Portland cement and cured in the same *** and unconfined compressive strength tests as well as seismic waves measurements after 3-,7-,14-and 28-d curing were performed to obtain a relationship between elastic stiffness and strength with curing time for both ***,the AAC mix demonstrated superior strength results,exhibiting almost double flexural and compressive strengths after 28 d compared to the Portland cement *** AAC mix also showed a higher rate of stiffness increase than the Portland cement mix,which has a higher initial stiffness at young ages but lower stiffness *** analysis confirmed that the proposed AAC could effectively immobilise any contaminants from soil or *** effect of curing under stress was analysed in triaxial compression tests and found to be insignificant,indicating that laboratory data obtained without stress curing can represent the material's behaviour in a DSM column,which will cure under the weight of the column.

Idiopathic sudden sensorineural hearing loss:Effectiveness of salvage treatment with low-dose intratympanic dexamethasone

Objectives:To evaluate hearing outcome of salvage treatment with intratympanic steroids(ITS)in idiopathic sudden sensorineural hearing loss(ISSNhl)refractory to initial systemic steroid(SS)*** and methods:A retrospective medical chart review was conducted on 54 consecutive patients with ISSNhl refractory to *** treatment with a low dose intratympanic dexamethasone(4 mg/ml)was offered after one week of primary *** were divided into two groups:25 patients accepted ITS(treatment group)and 29 patients did not undergo additional treatment(control group).A pure tone average(PTA)gain of at least 10 dB was considered hearing ***:hearing improvement rate was higher in ITS group compared to control group(40%vs.13.8%,p=0.035).A mean PTA improvement of 8.6±9.8 dB was observed in the ITS group and,whereas the control group had an average hearing gain of 0.7±2 dB(plysis revealed a significant hearing gain in ITS group at all tested frequencies compared to control group(plysis of the selected variables,identified intratympanic steroid treatment as the only independent prognostic factor for hearing improvement(OR=4.2,95%CI:1.1e15.7;p=0.04).Conclusion:Intratympanic low dose dexamethasone is effective in patients with incomplete hearing recovery after primary systemic steroid treatment.

人类许多领域的活动依赖于蜂窝通讯的可用性。然而,尽管对频谱的需求很高,但研究表明频谱的利用率很低。有限的频谱可用性和低效率的现在导致了对新拓扑和新技术的需求以提高对频谱的利用。认知网络这一新概念正是在这种背景下被提出的,这种技术允许更有效的使用频带带宽。无线应用近年来发展迅猛。所以,电磁波谱越来越少。由于这个原因,研究人员对认知无线电的研究越来越感兴趣。认知无线电是一种可以动态编程和配置的智能技术。认知无线电网络的一个非常重要的任务是频谱感知,感知是否有可用的频谱。在多用户合作的情况下,所有的认知用户都会对频谱进行感知,然后向中央节点报告,该节点作为一个融合中心来做出频谱决策。这被称为集中合作频谱感知。异构网络由覆盖范围大、容量大的宏蜂窝,以及发送功率小、覆盖范围小的小蜂窝组成。异构网络保障了网络性能和用户的高通信速率。异构网络中,每个用户的感知能力不同,这个特征是不可忽视的。考虑到认知无线电网络的动态性和自治性以及异构网络高容量的需求,在异构网络中引入认知无线电技术可实现系统容量的进一步提升。因此,异构认知无线电网络的概念被提出,被研究用于系统性能的进一步提高。本文,我们说明了根据用户的异构能力,在接收频谱条件信息后通过中央节点与用户通信的方式,分配不同的本地检测算法,可以实现更好的频谱感知性能。通过研究和仿真分析,证明了在认知网络中充分利用每一个次用户的异构检测能力,可以大大提高频谱感知性能。本文首先调研了异构网络和认知无线电的研究现状,重点总结和比较了异构认知网络合作频谱感知技术和传统的合作频谱感知技术。然后结合异构网络的特征,利用MATlAB对合作频谱感知技术进行了仿真分析和比较。最后,为了增强合作频谱感知算法的性能,我们提出了一个新的基于次用户异构检测能力的合作频谱感知机制。在此机制中,认知用户将异构检测能力的相关参数将报告发送到中央节点,该节点作为一个融合中心,依次分析了二次用户的报告,根据每个次用户根据其异构检测特性选择最合适的频谱感知算法,并最终得到频谱感知结果。认知无线电网络根据频谱感知结果重新配置参数,在允许通信的波段上进行通讯。仿真结果表明,由于利用了认知用户的异构感知能力,本文提出的新算法可以大幅提升合作频谱感知的性能。
