Lupus enteritis as the only active manifestation of systemic lupus erythematosus: A case report

BACKGrOUND Lupus enteritis is a rare manifestation of systemic lupus erythematosus (SLE). Diagnosis of this condition is difficult, especially in the absence of other symptoms related to active SLE. We present the case of a 25-year-old female with lupus enteritis as the sole initial manifestation of active SLE. CASE SUMMArY A 25-year-old African American female presented to the Emergency Department complaining of diffuse abdominal pain, diarrhea, nausea, and vomiting for 2 days. Her past medical history was significant for seasonal allergies and family history was pertinent for discoid lupus in her father and SLE in a cousin. The patient’s vital signs on presentation were normal. Her physical exam was remarkable for significant lower abdominal tenderness without guarding or rigidity. A computed tomography of the abdomen and pelvis revealed marked circumferential wall thickening and edema of the proximal and mid small bowel predominantly involving the submucosa. Our main differential diagnoses were intestinal angioedema and mesenteric vein thrombosis. However, mesenteric vessels were patent, and laboratory testing for hereditary angioedema showed a normal C1 Esterase Inhibitor level and low C3 and C4 levels. Infectious work-up was negative. Autoimmune tests showed elevated anti-nuclear antibodies (ANA)(13.6), anti-Smith antibody, and anti-ribonucleoprotein (anti-rNP) antibody. The patient was diagnosed with SLE enteritis. She was maintained on bowel rest, given intravenous hydration, and started on methylprednisolone 60 mg IV daily. She had significant improvement in her abdominal pain, diarrhea, and emesis after 2 days of treatment. Steroids were tapered and maintained on Hydroxychloroquine with no relapses one year after *** This case of lupus enteritis represents a rare manifestation of SLE. Diagnosis requires clinical suspicion, characteristic imaging and laboratory tests. Endoscopic appearance and biopsies usually yield non-specific findings.

This paper presents preliminary results of three-dimensional thermomechanical finite-element models of a parameter study to compute the current temperature and stress distribution in the subduction zone of the central Andes （16°S-26°S） up to a depth of 400 km, the bottom of the asthenosphere. For this purpose a simulation running over c. 50,000 years will be realized based on the geometry of a generic subduction zone and an elasto-viscoplastic Drucker-Prager rheology. The kinematic and thermal boundary conditions as well as the rheological parameters represent the current state of the study area. In future works the model will be refined using a systematic study of physical parameters in order to estimate the influence of the main parameters （e.g. viscosity, fault friction, velocity, shear heating） on the results of the reference model presented here. The reference model is kept as simple as possible to be able to estimate the influence of the parameters in future studies in the best possible way, whilst minimizing comnutational time.

Second harmonic generation(SHG)is forbidden for materials with inversion symmetry,such as bulk *** can be broken by morphological or dielectric discontinuities,yet SHG from a smooth continuous metallic surface is *** non-linear microscopy,we experimentally demonstrate enhanced SHG within an area of smooth silver film surrounded by ***-assisted SHG is locally enhanced by more than one order of magnitude compared to a neighboring silver surface *** optical measurements and cathodoluminescence(CL)imaging substantiate these *** suggest that plasmonic modes launched from the edges of the nanocavities propagate onto the smooth silver film and annihilate,locally generating *** addition,we show that these hotspots can be dynamically controlled in intensity and location by altering the polarization of the incoming *** results show that switchable nonlinear hotspots can be generated on smooth metallic films,with important applications in photocatalysis,single-molecule spectroscopy and non-linear surface imaging.

The objective of this paper is to develop a neural network-based residual generator to detect the fault in the actuators for a specific communication satellite in its attitude control system （ACS）. First, a dynamic multilayer perceptron network with dynamic neurons is used, these neurons correspond to a second order linear Infinite Impulse response （IIr） filter and a nonlinear activation function with adjustable parameters. Second, the parameters from the network are adjusted to minimize a performance index specified by the output estimated error, with the given input-output data collected from the specific ACS. Then, the proposed dynamic neural network is trained and applied for detecting the faults injected to the wheel, which is the main actuator in the normal mode for the communication satellite. Then the performance and capabilities of the proposed network were tested and compared with a conventional model-based observer residual, showing the differences between these two methods, and indicating the benefit of the proposed algorithm to know the real status of the momentum wheel. Finally, the application of the methods in a satellite ground station is discussed.

USA nuclear waste management program has stalled due to its continued insistence upon using Yucca Mountain for its disposal of DOE （Department of Energy） and commercial power plants NSF （nuclear spent fuel）. Increased flexibility and significant changes in DOE program are proposed. They include a private waste company to manage it; the construction and operation of ICS （interim centralized storage） facilities for NSF; the search for an AGr （alternate geological repository）; the use of a different methodology involving key local and state participants; a new ＂as safe as practical＂ strategy with defined benefits to the involved locations and states. Assured removal of NSF from ICS and providing limited Price Anderson indemnity for the program will enhance its acceptance. Minimum politics, regular information meetings, compromises, good cost projections and meeting schedules will be necessary to increase the chances of the proposed nuclear waste management program.

Aim: Over the last 15 years, hyaluronic acid (HA) fillers have become the most popular injectable biomaterial for soft tissue correction. With the increasing number of available HA fillers and the multiplication of facial treatments all over the world, there has been a need from physicians to better understand the HA fillers science. There is especially a growing interest in the science-based evaluation of rheological characteristics which represents an essential tool to guide physicians in the selection of the most appropriate HA fillers, administration techniques and depths of injection for their clinical applications. methods: Four key rheological parameters (viscosity η, elasticity G', normal force FN and elasticity E') are measured and discussed on five HA fillers. results: These four key rheological parameters are demonstrated to play a pivotal role, in combination with the cohesivity, for better predicting the clinical behavior of HA fillers at different stages of their lifetime. Conclusion: This article discusses the importance of four key rheological parameters during the main steps of the clinical HA fillers' lifetime, from the product injection to the loss of clinical effects. A better knowledge of these HA fillers' rheological parameters can help the physicians to optimize their aesthetic outcomes, safety and patient satisfaction.
