BACKGROUND Patients with myasthenia gravis(MG)are at a higher risk of developing Takotsubo cardiomyopathy(TTC),particularly during a myasthenic *** crisis-associated TTC occurs predominantly in *** this case report,we present a man with metastasized prostate carcinoma who developed TTC after new-onset *** SUMMARY An 81-year-old man with non-insulin dependent diabetes mellitus and metastasized prostate carcinoma presented with *** primary assessment examination at the emergency department,there was evident blepharoptosis of his right *** electrocardiograms were suggestive of an acute anterior wall myocardial infarction,for which he underwent emergency coronary *** obstructive coronary artery disease was *** the coronary angiography,the patient developed respiratory failure and was admitted to the Intensive Care Unit for non-invasive respiratory *** following day,diagnostic neostigmine test revealed a myasthenic *** echocardiography revealed left ventricular apical ballooning with a typical appearance of *** the potentially reversible character of both MG and TTC,the patient and family requested an end of support in the Intensive Care Unit due to age and chronic malignancy with reduced quality of life in recent months after non-chemo-responding prostate *** patient died soon after treatment *** Elderly men should be carefully evaluated for TTC when new-onset MG is diagnosed.

AIM： To determine by brain functional magnetic resonance imaging （fMRI） whether cerebral processing of non-visceral stimuli is altered in irritable bowel syndrome （IBS） patients compared with healthy subjects. To circumvent spinal viscerosomatic convergence mechanisms, we used auditory stimulation, and to identify a possible influence of psychological factors the stimuli differed in their emotional quality. METHODS： In 8 IBS patients and 8 controls, fMRI measurements were performed using a block design of 4 auditory stimuli of different emotional quality （pleasant sounds of chimes, unpleasant peep （2000 Hz）, neutral words, and emotional words）. A gradient echo T2＊weighted sequence was used for the functional scans. Statistical maps were constructed using the general linear model. RESULTS： To emotional auditory stimuli, IBS patients relative to controls responded with stronger deactivations in a greater variety of emotional processing regions, while the response patterns, unlike in controls, did not differentiate between distressing or pleasant sounds. To neutral auditory stimuli, by contrast, only IBS patients responded with large significant activations. CONCLUSION： Altered cerebral response patterns to auditory stimuli in emotional stimulus-processing regions suggest that altered sensory processing in IBS may not be specific for visceral sensation, but might reflect generalized changes in emotional sensitivity and affectire reactivity, possibly associated with the psychological comorbidity often found in IBS patients.

Hirschsprung＇s disease （HD） is a disorder associated with congenital malformation of the enteric nervous system with segmental aganglionosis. Prevailing therapy includes a resection of the affected part of the bowel, However, patients often do not obtain complete functional improvement after surgical treatment. We present the case of a 25-year-old woman who had surgical treatment of lid in early childhood. After that procedure she had clinical features of constipation for years in the end, passing of stool once a week, requiring laxatives and enemas. We diagnosed an incomplete resection of the aganglionic bowel via rectal biopsy and resected the remaining aganglionic segment. Two months after surgery the patient＇s bowel function improved to a frequency of 1-4 stools per day. We conclude that regular follow-up is required to identify lid patients with persistent alterations of bowel function after surgery. In patients presenting with constipation, recognition of a remaining aganglionic segment or other alterations of the enteric nervous system should be aimed at in an early stage. 2005 The WJG Press and Elsevier Inc. The introduction of automatic intersection management (AIM) promises higher traffic flows and air quality by increasing the effective capacity of the intersection. Unfortunately, automatic control is only possible in a fully autonomous environment without legacy vehicles and vulnerable road users like cyclists and pedestrians. In this paper we introduce hybrid intersection management: a concept which makes it possible to gain the benefits from autonomous vehicles in the situation with mixed traffic. A new message protocol, the individual Space & Time message (iSPaT), is introduced to effectively send instructions to autonomous vehicles. The iSPaT message is also used to guide autonomous vehicles safely over the intersection in a “woven” manner. The development of hybrid intersection management can be started today, and shifts towards automatic control when the amount of autonomous vehicles increases.

Organochlorine pesticides (OCP) were monitored at the Weissfluhjoch site (Switzerland) and the top of Mt. Sonnblick (Austria) with a low volume active air sampler and semipermeable membrane devices (SPMD). The air sampling rates (Rair) of the SPMD for OCP were calculated. Statistical tests showed that there was no significant difference between Rair at the two different sampling sites. Quantitative structure-property relationship (QSPR) models of the Rair of the SPMD were developed for OCP using partial least square (PLS) regression. Quantum chemical descriptors computed by the semi-empirical PM6 method were used as predictor variables. The cumulative variance of the dependent variable explained by the PLS components and determined by cross-validation (Q2cum) was >0.818 for each optimal model. This indicates that the model has good predictive ability and robustness. The Rair of the SPMD for OCP is related to the total energy, the van der Waals area and the total dipole moment of the OCP molecules. The main factors governing Rair values of OCP are intermolecular interactions and the energy required for cavity-formation in dissolution of OCP into triolein of the SPMD. The linear correlation coefficient between predicted and experimental values were all >0.921.

Recent changes in precipitation regime in South-East Asia are a subject of ongoing discussion. In this article, for the first time, evidence of a precipitation regime shift during the mid-1970s in the Northern Hemispheric part of South-East Asia is demonstrated. The detection of regime shifts is made possible by using a new comprehensive dataset of daily precipitation records (South-East Asian Climate Assessment and Dataset) and applying a novel Bayesian approach for regime shift detection. After the detected regime shift event in the mid-1970s, significant changes in precipitation distribution occurred in the Northern Hemispheric regions—Indochina Peninsula and the Philippines. More specifically, dry days became up to 10% more frequent in some regions. However, no precipitation regime shift is detected in Southern Hemisphere regions—Java and Northern Australia, were the number of observed dry days increased gradually.

AIM： To evaluate the efficacy of transjugular intrahepatic portosystemic shunts （TIPSs） after liver transplantation （LT）. METHODS： Between November 1996 and December 2005, 10 patients with severe recurrent hepatitis C virus infection （n = 4）, ductopenic rejection （n = 5） or portal vein thrombosis （n = 1） were included in this analysis. Eleven TIPSs （one patient underwent two TIPS procedures） were placed for management of therapy-refractory ascites （n = 7）, hydrothorax （n = 2） or bleeding from colonic varices （n = 1）. The median time interval between LT and TIPS placement was 15 （4-158） mo. RESULTS： TIPS placement was successful in all patients. The mean portosystemic pressure gradient was reduced from 12.5 to 8.7 mmHg. Complete and partial remission could be achieved in 43% and 29% of patients with ascites. Both patients with hydrothorax did not respond to TIPS. No recurrent bleeding was seen in the patient with colonic varices. Nine of 10 patients died during the study period. Only one of two patients, who underwent retransplantation after the TIPS procedure, survived. The median survival period after TIPS placement was 3.3 （range 0.4-20） too. The majority of patients died from sepsis with multiorgan failure. CONCLUSION： Indications for TIPS and technical performance in LT patients correspond to those in non-transplanted patients. At least partial control of therapy-refractory ascites and variceal bleeding could be achieved in most patients. Nevertheless, survival rates were disappointing, most probably because of the advanced stages of liver disease at the time of TIPS placement and the high risk of sepsis as a consequence of immunosuppression.

The soil-borne fungal pathogen Verticillium Iongisporum causes vascular disease on Brassicaceae host plants such as oilseed rape. The fungus colonizes the root xylem and moves upwards to the foliage where disease symptoms become visible. Using Arabidopsis as a model for early gene induction, we performed root transcriptome analyses in re- sponse to hyphal growth immediately after spore germination and during penetration of the root cortex, respectively. Infected roots showed a rapid reprogramming of gene expression such as activation of transcription factors, stress-, and defense-related genes. Here, we focused on the highly coordinated gene induction resulting in the production of tryp- tophan-derived secondary metabolites. Previous studies in leaves showed that enzymes encoded by CYP81F2 and PEN2 （PENETRATION2） execute the formation of antifungal indole glucosinolate （IGS） metabolites. In Verticillium-infected roots, we found transcriptional activation of CYP81F2 and the PEN2 homolog PEL 1 （PEN2-LIKE1）, but no increase in antifungal IGS breakdown products. In contrast, indole-3-carboxylic acid （13CA） and the phytoalexin camalexin accumulated in infected roots but only camalexin inhibited Verticillium growth in vitro. Whereas genetic disruption of the individual metabolic pathways leading to either camalexin or CYP81F2-dependent IGS metabolites did not alter Verticillium-induced disease symptoms, a cyp79b2 cyp79b3 mutant impaired in both branches resulted in significantly enhanced susceptibility. Hence, our data provide an insight into root-specific early defenses and suggest tryptophan-derived metabolites as active anti- fungal compounds against a vascular pathogen.
