Differential treatment and early outcome in the interventional endoscopic management of pancreatic pseudocysts in 27 patients

AIM： Pancreatic pseudocysts （PPC） as a complication of pancreatitis are approached only in the case of abdominal pain, infection, bleeding, and compression onto the gastrointestinal tract or biliary tree. METHODS： From 02/01/2002 to 05/31/2004, all con- secutive patients with symptomatic PPC who underwent an interventional endoscopic approach were evaluated in this pilot case-series study： Group （Gr.） Ⅰ-Primary percutaneous （external）, ultrasound-guided drainage. Gr. Ⅱ- Primary EUS-guided cystogastrostomy. Gr. Ⅲ-EUS-guided cystogastrostomy including intracystic necrosectomy. RESULTS：（=＂follow up＂： n = 27）： Gr. Ⅰ （n = 9; 33.3%）： No complaints （n = 3）; change of an external into an internal drainage （n = 4）; complications：（a） bleeding （n = 1） followed by 3 d at ICU, discharge after 40 d; （b） septic shock （n = 1） followed by ICU and several laparotomies for programmed lavage and necrosectomy, death after 74 d. Gr. Ⅱ （n = 13; 48.1%）： No complaints （n = 11）; external drainage （n = 2）; complications/problems out of the 13 cases： 2nd separate pseudocyst （n = 1） with external drainage （since no communication with primary internal drainage）; infection of the residual cyst （n = 1）＋ following external drainage; spontaneous PPC perforation （n = 1）＋ following closure of the opening of the cystogastrostomy using clips and subsequently ICU for 2 d. Gr. Ⅲ （n = 5; 18.5%）： No complaints in all patients, in average two endoscopic procedures required （range, 2-6）. CONCLUSION： Interventional endoscopic management of pancreatic pseudocysts is a reasonable alternative treatment option with low invasiveness compared to surgery and an acceptable outcome with regard to the complication rate （11.1%） and mortality （3.7%）, as shown by these initial study results.

Cytomegalovirus(CMV)is distinct among members of the Herpesviridae family for having the largest dsDNA genome(230 kb).Packaging of large dsDNA genome is known to give rise to a highly pressurized viral capsid,but molecular interactions conducive to the formation of CMV capsid resistant to pressurization have not been ***,we report a cryo electron microscopy(cryoEM)structure of the murine cytomegalovirus(MCMV)capsid at a 9.1Åresolution and describe the molecular interactions among the~3000 protein molecules in the MCMV capsid at the secondary structure *** structural elements are resolved to provide landmarks for correlating with results from sequence-based prediction and for structure-based homology *** major capsid protein(MCP)upper domain(MCPud)containsα-helices andβ-sheets conserved with those in MCPud of herpes simplex virus type 1(HSV-1),with the largest differences identifi ed as a“saddle loop”region,located at the tip of MCPud and involved in interaction with the smallest capsid protein(SCP).Interactions among the bacteriophage HK97-like fl oor domain of MCP,the middle domain of MCP,the hook and clamp domains of the triplex proteins(hoop and clamp domains of TRI-1 and clamp domain of TRI-2)contribute to the formation of a mature *** results offer a framework for understanding how cytomegalovirus uses various secondary structural elements of its capsid proteins to build a robust capsid for packaging its large dsDNA genome inside and for attach-ing unique functional tegument proteins outside.

Productive Performance of Double-Muscled Piemontese Culled Cows in Finishing Period

The aims of the study were to evaluate productive performances and environmental impact of double-muscled Piemontese culled cows. In two trials, 39 Piemontese culled cows aged from 46 to 176 mon were fattened using diets commonly applied in farm conditions. The animals were weighed at the beginning and end of the trials. Concentrate and hay intake were recorded daily. After slaughtering, meat, fat and bone yields were recorded and dressing percentage was calculated. The backward linear regression model was employed for statistical analysis. Results showed average body weight （ABW） of cows as （556.97± 55.99） kg and relatively low dry matter intake （DMI） [（9.17± 0.35） kg d^-1] during the fattening period. The ratio between DMI and ABW, i.e., ratio between intake and weight （RIW）, （1.66±0.18）% was negatively influenced by initial body weight （IBW） and duration of finishing period （D）, but age had no effect on RIW. Average daily gain （ADG） was relatively low [（0.55 ±0.21） kg d^-1, and was negatively correlated with IBW and D, which implied that lower IBW and shorter D could have higher ADG and vice versa. The dressing percentage （DP） was （59.80± 1.83）% and the carcass red meat yield （MY） was （75.17±3.10）%. Nevertheless, N efficiency was very low [（4.35 ± 1.76）%], which must be considered in relation to environmental protection so both animal requirements and a well balanced diet need more attention. In conclusion when Piemontese cows will be culled and fattened, IBW and D should be considered carefully in order to get a reasonable ADG and N efficiency to guarantee productive benefits and environmental protection at the same time.

Along with the development of new materials, advanced medical imaging and surgical techniques, osseointegrated dental implants are considered a successful and constantly evolving treatment modality for the replacement of missing teeth in patients with complete or partial edentulism. The importance of restoring the peripheral neural feedback pathway and thus repairing the lack of periodontal rnechanoreceptors after tooth extraction has been highlighted in the literature. Nevertheless, regenerating the nerve fibers and reconstructing the neural feedback pathways around osseointegrated implants remain a challenge. Recent studies have provided evidence that platelet-rich plasma （PRP） therapy is a promising treatment for musculoskeletal injuries. Because of its high biological safety, convenience and usability, PRP therapy has gradually gained popularity in the clinical field Although much remains to be learned, the growth factors from PRP might play key roles in peripheral nerve repair mechanisms. This review presents known growth factors contributing to the biological efficacy of PRP and illustrates basic and （pre-）clinical evidence regarding the use of PRP and its relevant products in peripheral nerve regeneration. In addition, the potential of local application of PRP for structural and functional recovery of iniured peripheral nerves around dental implants is discussed.

Polyunsaturated fatty acids(PUFAs),particularly the omega-3 PUFAs,are thought to be involved in neuronal processes,to play a role in neurodevelopmental disorders and to be important for the integrity of central nervous system *** present study investigated the effects of nutritional omega-3 PUFAs on attentional functions and impulsive behavior in Wistar *** this purpose,female Wistar rats were fed an omega-3 deficient diet over several generations,and the dams of the seventh generation were randomly assigned to two diet groups and fed an omega-3 deficient or an omega-3 sufficient *** addition,a group of previously untreated dams was fed an omega-3 sufficient *** male offspring of these three diet groups were tested using an established paradigm for the assessment of attention and impulsive behavior,i.e.a modified version of the five-choice-serial-reaction-time task(5CSRTT).The present data show that the deficiency of omega-3 PUFAs over generations led to substantial changes in attentional processes and impulsive *** impairments associated with an omega-3 deficiency were partly corrected by treatment with the omega-3 sufficient diet in the last generation of the omega-3 deficient group which showed substantial improvements in attention *** there were no significant effects of dietary modifications on psychomotor activity levels,there was some evidence for changes in impulsive *** conclusion,transgenerational dietary changes in the availability of omega-3 PUFAs led to changes in attentional processes and impulsive behavior in rats,supporting the hypothesis that omega-3 PUFAs play a role in cognitive and behavioral *** present findings offer a promising approach in the investigation of the role of omega-3 PUFAs in a variety of cognitive and behavioral domains.

AIM： To evaluate the efficacy of transjugular intrahepatic portosystemic shunts （TIPSs） after liver transplantation （LT）. METHODS： Between November 1996 and December 2005, 10 patients with severe recurrent hepatitis C virus infection （n = 4）, ductopenic rejection （n = 5） or portal vein thrombosis （n = 1） were included in this analysis. Eleven TIPSs （one patient underwent two TIPS procedures） were placed for management of therapy-refractory ascites （n = 7）, hydrothorax （n = 2） or bleeding from colonic varices （n = 1）. The median time interval between LT and TIPS placement was 15 （4-158） mo. RESULTS： TIPS placement was successful in all patients. The mean portosystemic pressure gradient was reduced from 12.5 to 8.7 mmHg. Complete and partial remission could be achieved in 43% and 29% of patients with ascites. Both patients with hydrothorax did not respond to TIPS. No recurrent bleeding was seen in the patient with colonic varices. Nine of 10 patients died during the study period. Only one of two patients, who underwent retransplantation after the TIPS procedure, survived. The median survival period after TIPS placement was 3.3 （range 0.4-20） too. The majority of patients died from sepsis with multiorgan failure. CONCLUSION： Indications for TIPS and technical performance in LT patients correspond to those in non-transplanted patients. At least partial control of therapy-refractory ascites and variceal bleeding could be achieved in most patients. Nevertheless, survival rates were disappointing, most probably because of the advanced stages of liver disease at the time of TIPS placement and the high risk of sepsis as a consequence of immunosuppression.

Weather events put human lives at risk mostly when people might occupy areas susceptible to natural *** Professional Weather Stations(PWS)in vulnerable areas is key for monitoring weather with reliable ***,such professional instrumentation is notably expensive while remote sensing from a number of stations is *** imposes challenges on the large-scale weather station deployment for broad monitoring from large observation networks such as in Cemaden—The Brazilian National Center for Monitoring and Early Warning of Natural *** this context,in this paper,we propose a Low-Cost Automatic Weather Station(LCAWS)system developed from Commercial Off-The-Shelf(COTS)and open-source Internet of Things(IoT)technologies,which provides measurements as reliable as a reference PWS for natural disaster *** being automatic,LCAWS is a stand-alone photovoltaic system connected wirelessly to the Internet in order to provide real-time reliable end-to-end weather *** achieve data reliability,we propose an intelligent sensor calibration method to correct *** a 30-day uninterrupted observation with sampling in minute resolution,we show that the calibrated LCAWS sensors have no statistically significant differences from the PWS *** such,LCAWS has opened opportunities for reducing maintenance costs in Cemaden's observational network.

Based on the analysis of carrier dynamics in quantum dots （QDs）, the numerical model of InAs/GaAs QD laser is developed by means of complete rate equations. The model includes four energy levels and among them three energy levels join in lasing. A simulation is conducted by MATLAB according to the rate equation model we obtain. The simulation results of PI characteristic, gain characteristic and intensity modulation response are reasonable. Also, the relations between the left facet reflectivity of laser cavity and threshold current as well as modulation bandwidth are studied. It is indicated that the left facet reflectivity increasing can result in reduced threshold current and improved mo6ulation bandwidth, which is in accordance with experimental results. The internal mechanism of QD lasers is fully described with the rate equation model, which is helpful for QD lasers research.
