Background: Following coronary artery bypass grafting (CABG), delirium emerges as a prevalent complication. This study aimed to assess the correlation between elevated serum cortisol levels and the occurrence of postoperative delirium subsequent to off-pump CABG. Methods: Conducted in the Department of Cardiac Surgery at BSMMU from October 2020 to September 2022, this comparative cross-sectional study included a total of 44 participants. Subjects, meeting specific criteria, were purposefully assigned to two groups based on off-pump CABG. Group A (n = 22) consisted of patients with normal serum cortisol levels, while Group B (n = 22) comprised individuals with high serum cortisol levels on the first postoperative day. Delirium onset was assessed at the bedside in the ICU on the 1st, 2nd, 3rd, 4th, and 5th postoperative days using standard tools, namely the Richmond Agitation Sedation score (RASS) and The Confusion Assessment Method (CAM-ICU). Data were collected based on the presence or absence of delirium. Statistical analysis utilized SPSS version 26.0, employing an independent Student’s t-test for continuous data and chi-square and Fischer’s exact test for categorical data. A p-value ≤ 0.05 was considered statistically significant. Results: Group-A had a mean age of 54.50 ± 17.97, and Group-B had a mean age of 55.22 ± 15.45, both with a male predominance (81.81% and 86.36% respectively). The mean serum cortisol level was significantly higher in Group B (829.71 ± vs. 389.98 ± 68.77). Postoperative delirium occurred in 27.3% of Group B patients, statistically significant compared to the 4.5% in Group A. However, patients in Group B who developed delirium experienced significantly longer postoperative ICU and hospital stays (79.29 ± 12.27 vs. 11.44 ± 2.85, p ≤ 0.05). 开发高效窄带发射的有机发光材料是实现有机发光二极管高色纯度、高分辨的关键.针对有机发光分子的发射光谱谱较宽的问题,基于芘基发光材料体系,提出了利用空间立体效应窄化发光分子的半峰宽的分子设计策略.利用空间位阻效应,通过在芘的2-位引入大位阻的取代基团,降低了3-位取代基团三苯胺的分子转动,设计合成的芘基发光分子5具有相对高的固态量子产率(>0.31)和固态下较窄的半峰宽(相较于溶液态和没有立体效应存在下).特别的,制备的化合物5c在固态下显示出最大的发射峰位于484 nm,量子产率较高(0.38)和发射半峰宽较窄(49 nm).

Preparation of Durable Antibacterial Polyester Fabrics by Grafting with the Natural Quaternary Ammonium Compound Betaine

Recently, the textile industry has increasingly advocated for natural resource-based healthcare textiles. This research presents a facile and eco-friendly approach to developing durable antibacterial polyester fabrics. Polyester fabric was first subjected to an alkaline hydrolysis to impart hydroxyl groups on the fiber surface. A natural antibacterial agent, betaine, was then covalently bonded to the hydrolyzed polyester fiber surface through esterification. XPS, Raman, SEM, and Wicking measurements were carried out to verify the esterification reaction. Antibacterial tests confirmed that betaine treatment grafted polyester fabrics revealed a remarkable antibacterial effect with inhibition rates > 99.9% against both E. coli and S. aureus and still remained inhibition rates of up to 91.5% against both bacteria after home washing for 20 cycles. Moreover, the modification significantly increased the capillary effect of polyester fabric but did not cause apparent adverse effects on the fabric’s hand or tensile strength. Overall, this grafting strategy for durable, antibacterial polyester fabric represents a significant practicality in the textile industry.

Is the Sundarbans of Bangladesh in a State of Pollution?

The Sundarbans is the world’s most extensive natural mangrove forest and home to various natural resources. The population in the vicinity has increased, causing more dependency on the resources of the Sundarbans. The increasing industrialization, urbanization, aquaculture, intensive agricultural practices, seaports, tourism facilities, and so on in the peripheral areas of the Sundarbans have made significant changes in the surrounding and upstream land uses of the Sundarbans. This situation may have detrimental influences on the ecosystem components of the Sundarbans. Therefore, it is highly demanded to prepare a piece of baseline information or database of different sources of pollution and their present status in the various components of the Sundarbans. This effort helps to identify issues and concerns, determine the key elements of the ecosystem to monitor the level or overall quality of the Sundarbans ecosystem. The present study systematically collects the potential sources of pollution, types, and current levels in the ecosystem components of the Sundarbans using academic databases, libraries, and online resources. Discharge of industrial waste into water, soil and air, heavy metal pollution, use of agrochemicals, oil (refined and crude) pollution, plastic materials from urban areas, and tourism are the major issues and concerns related to the sustainability of the Sundarbans ecosystem. The air quality of the Sundarbans is in good condition with 0 - 50 AQI of Bangladesh. While BOD, COD, TDS, TSS varied from 2.0 to 3.8 mg/L, 21.6 to 416 mg/L, 146.9 to 24,100 mg/L and 54 to 155 mg/L, respectively. Soil EC, organic carbon, total nitrogen, and total phosphorus ranged from 3.01 - 5.82 mS/cm, 1.41% - 2.69%, 0.51 - 1.05 mg/g, and 0.32 - 0.51 mg/g respectively. The air, water and soil quality parameters varied with the sites and seasons and not much at the state of contamination. Background: Off-pump coronary artery bypass grafting (OPCAB) is considered a safer alternative to on-pump surgery, especially in patients with left ventricular dysfunction (LVD). Objectives: This study assessed short-term outcomes and functional improvements in LVD patients post-OPCAB. Methods: The study included 200 coronary artery disease patients who underwent isolated off-pump coronary artery bypass grafting (OPCAB) at the National Heart Foundation Hospital and Research Institute between January 2019 and June 2020. Patients were categorized into Group 1, with a left ventricular ejection fraction (LVEF) of 30% - 39%, and Group 2, with an LVEF of 40% or higher. Echocardiographic assessments of left ventricular dimensions and ejection fraction were performed preoperatively, at discharge, and one month postoperatively. Results: In Group 1, preoperative left ventricular internal dimensions during diastole (LVIDd) and systole (LVIDs) were 53.48 ± 4.40 mm and 44.23 ± 3.93 mm, respectively, with a left ventricular ejection fraction (LVEF) of 35.28% ± 2.26%. At discharge, these values improved to 51.58 ± 4.04 mm (LVIDd), 41.23 ± 5.30 mm (LVIDs), and 39.25% ± 3.75% (LVEF). One month postoperatively, further improvements were observed: 46.29 ± 3.76 mm (LVIDd), 37.45 ± 3.68 mm (LVIDs), and 43.22% ± 4.67% (LVEF). Group 2 showed similar positive outcomes, with preoperative values of 47.09 ± 5.06 mm (LVIDd), 35.11 ± 5.25 mm (LVIDs), and 50.13% ± 7.25% (LVEF), improving to 42.37 ± 4.18 mm (LVIDd), 31.05 ± 4.19 mm (LVIDs), and 55.33% ± 7.05% (LVEF) at one month postoperatively. Both groups demonstrated significant improvements in left ventricular function and NYHA class, with most patients moving from class III/IV to I/II. Complications were minimal, and no mortality was observed. Conclusion: OPCAB is safe and effective for patients with LVEF 30% - 39% and LVEF ≥ 40%, providing significant short-term functional improvements without increased risk.

Impact of Action Observation Therapy along with Usual Physiotherapy Intervention of Individual with Alzheimer’s Disease

Alzheimer’s disease (AD) is a neurodegenerative disorder characterized by cognitive impairments in the initial stage, which lead to severe cognitive dysfunction in the later stage. Action observation therapy (AOT) is a multisensory cognitive rehabilitation technique where the patient initially observes the actions and then tries to perform. The study aimed to examine the impact of AOT along with usual physiotherapy interventions to reduce depression, improve cognition and balance of a patient with AD. A 67 years old patient with AD was selected for this study because the patient has been suffering from depression, dementia, and physical dysfunction along with some other health conditions like diabetes and hypertension. Before starting intervention, a baseline assessment was done through the Beck Depression Inventory (BDI) tool, the Mini-Cog Scale, and the Berg Balance Scale (BBS). The patient received 12 sessions of AOT along with usual physiotherapy interventions thrice a week for four weeks, which included 45 minutes of each session. After four weeks of intervention, the patient demonstrated significant improvement in depression, cognition, and balance, whereas the BDI score declined from moderate 21/63 to mild 15/63 level of depression. The Mini-Cog score improved from 2/5 to 4/5, and the BBS score increased from 18/56 to 37/56. It is concluded that AOT along with usual physiotherapy intervention helps to reduce depression, improve cognition and balance of people with AD.

Optimizing Resource Management for IoT Devices in Constrained Environments

Embedded computing device implementation on the Internet of Things (IoT) requires careful assessment of their intrinsic resource limitations. These constraints are not limited to memory and processing capabilities but extend to the network interfaces, particularly due to the low-power radio standards that these devices typically employ. The IPv6 protocol is shown to be a strong option for guaranteeing interoperability in the IoT, mostly because of its large address space, the range of current IP-based protocols, and its intrinsic versatility. Considering these benefits, we investigate if current IP-based network management protocols can be implemented on devices with limited resources. We investigate the resource needs in particular for implementing Network Configuration Protocol (NETCONF) and Simple Network Management Protocol (SNMP) on an 8-bit AVR-based device. Our investigation reveals the specific memory and processing demands of these protocols, providing valuable insights into their practicality and efficiency in constrained IoT environments. This study underscores the potential and challenges of leveraging IPv6-based network management protocols to enhance the functionality and interoperability of IoT devices while operating within stringent resource limitations.
