This work discusses issues related to the impact of urbanization on the microphysical processes of precipitating systems associated with synoptic, mesoscale, and local scale systems. Among the issues addressed is the impact of urban heat island (UHI) in Sãpan>pan>pan>o paulo city center and urban densification (UD) in thepan> Metropolitan Area of Sãpan>pan>pan>o paulo (MASp) on the microphysical, dynamic, andpan>pan style="font-family:Verdana;"> thermodynamic properties and distribution of precipitation and heavier rainfall from sea-breeze (SB) e cold-front (CF) combined during their space-time evolutions. For this purpose, it used four components: classification of hydrometeors with fuzzy logic, calculation of the raindrop diameters, an estimate of liquid water mass and ice mass from polarimetric-variables measured with dual-pola- rization X-band meteorolog

A Dam Safety program aims to reduce the risks to human life, property, and the environment from dam related hazards. In Brazil, despite of the contemporary law about dam safety, there is still no cadastral information nor the classification of risk and associated potential damage to all dams. Besides that, the recent disaster caused by the failure of the Fund?o dam, located in Mariana city, Minas Gerais State, is an issue that aggravates the urgency of preventive measures and plans for disaster action. The present study proposes to classify the Juturnaíba dam, which maintains the largest reservoir for water supply in the state of Rio de Janeiro, Southeast region of Brazil. It was sought to analyze the risks and potential damage associated with the dam, in accordance with two classifications: one from the Brazilian Dams Committee and the other from the National Water Resources Council. It was possible to conclude that the Juturnaíba dam potentially presents high risk and associated high rates of damage, both regarding losses of human lives and regarding environmental and socioeconomic impacts. This is mainly due to the poor state of conservation that it currently presents. It is recommended that future studies should assess the slope stability of the earth dam, and that repair work should be implemented on the degraded concrete structures, with recuperation or installation of instruments that would enable monitoring of possible movement of the earth dam.

AIM: To determine serum vitamin D levels and colonic vitamin D receptor(VDR) expression in inflammatory bowel disease(IBD) and non-IBD patients and correlate these with ***: puerto Rican IBD(n = 10) and non-IBD(n = 10) patients ≥ 21 years old scheduled for colonoscopy were recruited. Each patient completed a questionnaire and provided a serum sample and a colonic biopsy of normal-appearing mucosa. For IBD patients, an additional biopsy was collected from visually diseased mucosa. Serum vitamin D levels were measured by ultra-performance liquid chromatography and mass spectrometry. Hematoxylin and eosin stained tissue sections from colonic biopsies were classified histologically as normal or colitis(active/inactive), and scored for the degree of inflammation present(0-3, inactive/absent to severe). Tissue sections from colonic biopsies were also stained by immunohistochemistry for VDR, for which representative diagnostic areas were photographed and scored for staining intensity using a 4-point ***: The IBD cohort was significantly younger(40.40 ± 5.27, p prevalence of vitamin D deficiency(40% vs 20%, respectively) and insufficiency(70% vs 50%, respectively). Histologic inflammation was significantly higher in visually diseased mucosa from IBD patients(1.95 ± 0.25) than in normalappearing mucosa from control patients(0.25 ± 0.08, p patients(0.65 ± 0.36, p pression in visually diseased colonic tissue from IBD patients(r =-0.44, p patients with Crohn's disease(r =-0.69, p ppearing colonic tissue from control patients or IBD patients. Contralateral axillary metastasis (CAM) is a rare entity normally treated as a systemic disease. Recent publications have proposed aggressive surgical treatment with benefits to the patients in terms of survival. We present a case of a 74-year-old patient with a history of unilateral breast cancer, recurrence on the ipsilateral breast and then development of a CAM. The patient was treated with aggressive surgical treatment, but she developed an early recurrence of the disease with distant metastasis. There is limited evidence of the correct management of CAM, although proposed to treat it as a loco regional disease, individualized and multidisciplinary management is the best option for these patients.

Introduction: post-hospital syndrome (pHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of pHS. Objective: The objective is to estimate the prevalence of pHS that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. Material and Methods: It is a descriptive observational study of patients with hyponatremia who were discharged from 1 September 2010 to 2 February 2020 at the Internal Medicine Service of the Hospital University of San Juan (Alicante, Spain). Results: Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [SD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. Conclusion: Hyponatremia may be the cause of pHS, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on pHS.

Oxygen Therapy in Critical Care: A Double Edged Sword

The oxygen therapy is a universal treatment in the hospital setting, especially in the critical care units. The purpose of this therapy is to avoid hypoxemia and to ensure an adequate supply of oxygen to the tissues. But often we overlook the potential adverse effects of oxygen therapy. Oxygen produces lung damage and induces apoptosis and cell death creating an imbalance between the production of reactive species of oxygen and the antioxidant mechanisms. The oxygen therapy inhibits systemic adaptive changes induced by hypoxia, disrupting compensatory mechanisms and causing deleterious effects. We are faced with a challenge in order to treat patients with respiratory failure, counterbalancing hypoxia with hyperoxia-induced damage and introducing therapy lines that are innovative but not risk-free as permissive hypoxemia. Currently many questions remain unresolved and there are not enough clinical studies that validate the therapeutic optimal oxygenation ranges. These ranges may differ depending on each patient and the underlying disease.

Objective: To describe the radiological, histological and immunohistochemical findings in a case of progressive Multifocal Leukoencephalopathy (pML) affecting the cerebellar peduncles in a patient with chronic lymphocytic leukemia. patient and Methods: Magnetic Resonance Imaging (MRI), histological picture (H.E., Kluver-Barrera) and immunohistochemical picture (GFAp, neurofilaments, CD68, JC virus) were obtained. Results: 1) Magnetic resonance imaging: Asymmetric and progressive lesions on middle cerebellar peduncles, that were hyperintense in T2/FLAIR, extended towards the pons, had no mass effect and were unmodified after intravenous contrast. 2) Histology: Marked reactive gliosis with cytopathic changes suggesting viral infection, plus demyelination areas with axonal preservation. 3) Immunohistochemistry: Marked positivity for viral (polyoma and JC virus) markers in glial cells showing cytopathic changes. Conclusions: The importance of histological and immunohistochemical diagnosis in everyday assistance;of the collaboration between clinicians, radiologists and pathologists;and the validity of postmortem studies as a key element for research and clinical quality assessment must be stressed.
