Voluminous Cystic Trigeminal Schwannoma: Case Report and Literature Review

Voluminous trigeminal schwannomas are rare, predominantly benign growth encapsulated tumors composed of Schwann cells with controversial surgical treatment. They account for 20% to 40% of all trigeminal schwannomas and share the same imaging findings of neurinomas elsewhere. Surgery remains the treatment of choice for most lesions as long as complete excision is feasible. A 35-year-old house wife complained of a 3-month history of progressive right visual field impairment associated with headaches. Neurological examination revealed a complete cavernous sinus syndrome. CT scanning showed a voluminous cystic mass of the right parasellar compartment extending to the optic nerve and the orbit. A concomitant mRI scan revealed more exquisite anatomical details of the lesion. The patient benefitted from a subsequent CT-angiography (not included) which excluded any vascular abnormalities associated with the tumor. Surgical excision of the tumor was warranted and the patient underwent surgery a few days after admission. A pterional extradural-intradural approach combined with incision of the lateral wall of the parasellar compartment via a standard frontotemporal craniotomy was performed and pathological examination was in favor of a cystic trigeminal schwannoma. Post-operative mRI showed a complete excision of the tumor. A 2-year follow-up yielded persistent right monocular blindness. Given this typical appearance of this lesion indicative of both cisternal and parasellar compartment extension, as well as the level of controversy surrounding surgical treatment of these particular tumors;we sought to report this rare case of a voluminous cystic trigeminal schwannoma and share our humble surgical experience of dealing with these lesions. The patient was informed that non-identifying information from the case would be submitted for publication, and she provided consent.

Objective:To characterize the semen of three genetic types of boars(local,improved and Large White)reared in ***:Semen of local,improved and Large White boars reared in Benin were collected using the gloved hand method and analyzed to determine volume,pH,concentration,mobility,motility,and *** effect of the genetic type of boar on semen characteristics was aslo ***:Duration of ejaculation and semen volume of Large White boar were significantly higher than those of local and improved boars(Pmen of improved boars had a higher motility score than that of Large White and local boars(Pmen of local boars was more concentrated in the spermatozoa than that of improved and Large White boars(Pmatozoa of improved boars with normal morphology(93.6%)was significantly higher than that of local(82.2%)and Large White boars(81.6%)(Pmatozoa with folded tails in the semen of Large White boars(9.2%)was significantly higher than that observed in improved(1.8%)and local(5.0%)boars(Pmatozoa with proximal cytoplasmic droplets in semen of improved boars(2.7%)was significantly lower than that in Large White(6.8%)and local(9.7%)boars(Pnd Large White boars(1.1%)showed more spermatozoa with distal cytoplasmic droplets in their semen compared to the improved boars(0.4%).Conclusions:The semen characteristics of pigs reared in Benin vary from one genetic type to *** genetic type has a strong *** Large White boar produces larger semen,the local boar produces more concentrated semen and the improved boar produces spermatozoa that are morphologically *** semen of these three genetic types can be used in artificial insemination but the improved boar's semen is more recommended.

Background: The Anonymous Screening and Information Centre (CADI) is the oldest screening centre of HIV from Burkina Faso. Since its opening, no analysis on the evolution of Human Immunodeficiency Virus (HIV) prevalence has been carried out. Objective: The study aimed to describe the dynamics of HIV infection when managing centres offering voluntary counselling and testing to client, such as the Screening and Counselling Centre (CADI), in Bobo Dioulasso, Burkina Faso. Patients and methods: A cross-sectional study was performed including people screened at the CADI from may 1996 to June 2014. Pearson khi2 test and Cuzick trend test were used with a 5% significance level. Results: From 7.1% of the 11,451 cases analyzed, the average age of clients screened was 30 ± 10.83 year-old. There was a female predominance (sex ratio = 0.8). The 16.4% (15.7 - 17.1) of the cases from the 11,451 clients included in our study were tested HIV positive. The global prevalence rate of HIV in the centre has significantly decreased from 46.2% in 1996 to 1.5% in 2014 (p Conclusion: The behaviour change could be a justification for the decline in HIV prevalence at the centre, hence the importance of sensitization campaigns.

Diagnostic accuracy of biochemical markers of fibrosis in black African patients with chronic hepatitis B

Contradictory results of the accuracy of bio-chemical markers to predict the stage of fibrosis in black African patients with chronic hepatitis B (CHB) were previously published. We con-ducted a prospective cohort study to determine the diagnostic accuracy of aspartate ami-notransferase to platelet ratio (APRI), aspartate aminotransferase to alanine aminotransferase ratio (AAR), platelet count, age-platelet (AP) in-dex, and FIB-4 index for the prediction of sig-nificant fibrosis or cirrhosis in 117 black African patients (median age: 38 years, males: 73 %) with CHB not previously treated. Among them, 45 had significant fibrosis and 18 had cirrhosis using the mETAVIR score system. Factors as-sociated either with significant fibrosis or cir-rhosis were determined in logistic multivariate analysis. Areas under receiver operating curve were assessed and compared for APRI, AAR, AP index, FIB-4 index and platelet count. Sensitivity, specificity, positive and negative predictive values were determined for each biochemical markers. multivariate analysis showed that as-partate aminotransferase (p 1.1) and FIB-4 index (cut-off > 2.1) ruled out significant fibrosis with high specific-ity of 84.7 % and 86.1 % respectively and nega-tive predictive values of 78.2 % and 72.9 % re-spectively. more accurately, APRI (cut-off > 0.63) or FIB-4 index (cut-off > 1.26) ruled out cirrhosis with high sensitivity of 94.4% and 88.9% and high negative predictive values of 98.1% and 96.3% respectively. In conclusion, APRI and FIB-4 index are simple readily available markers to exclude significant fibrosis or more accu-rately cirrhosis in black African patients with CHB.

Epidemiological and Histological Aspects of Women Genital Cancers in Cote d’Ivoire

Genital cancers in women play an important role in cancer pathology in the developing countries. Objectives: This study aimed at clarifying the epidemiological and histopathological aspects of woman genital cancers in C?te d’Ivoire. materials and methods: This was a retrospective and descriptive study conducted in the anatomical pathology laboratories of the university hospitals in Abidjan. The study was carried out over a period of twenty-four years (1984-2007). The variables studied were: frequency, age, histological type and prognosis. Results: Genital cancers in women accounted for 41.28% (n = 2491) of cancer in women and 21% of cancers. The average age of patients was 46.65 years (range 2 - 88 years). The cervix (82.85%) was the main location followed by the ovary (8.6%). Histologically, carcinomas (92.93%) were the most common types of sarcomas. Cervical cancers were diagnosed at an average age of 47.36 years (range 2 to 88 years). Carcinomas were the predominant histological type (92.88%) with 57.4% (n = 450) diagnosed at pT2N0m0 stage. The average age of patients bearing ovarian cancer was 39.13 years (range 8 to 82 years).Common malignant epithelial tumours were the most common histological type (57.48%) (n = 123) followed by non-Hodgkin lymphoma (14.95%). Conclusion: Genital cancers in women are common and poorly prognosed in Cote d’Ivoire. The adoption of a policy of routine screening is needed to improve the prognosis of these tumours.
