An experimental study of the response of a piezoceramic actuator set at the throat to a transonic diffuser is carried out by measuring wall static pressure fluctuations and by visualizing the flow field using schlieren technique. The visualized flow fields are captured with a digital still camera and a digital high speed video camera. The piezo ceramic actuator is attached at the throat of the diffuser and driven by sinusoidal amplified voltage signals. The diffuser used in this experiment is circular arc half nozzle with the height h* and width w of 3 mm and 25 mm, respectively. The blockage factor of the piezoceramic actuator to the diffuser throat is 9.2% assuring the effect of change in the throat area rather than the boundary layer disturbances. The piezoceramic actuator is driven at the frequency of 100 Hz, 200 Hz, and 300 Hz and its amplitude is about 1 mm. It is found that the wall static pressure fluctuations and the behavior of the shock wave clearly correspond to the vibration of the piezo ceramic actuator for all the frequency ranges whereas the averaged shock position remains almost unchanged. All the results mentioned above suggest that driving the piezo ceramic actuator at the diffuser throat can be one of the promising techniques to control unsteady transonic diffuser flow.

Background:We investigated potential disparities in health-related quality of life,particularly concerning urinary function,between patients with preserved and those with impaired sexual function after robot-assisted radical prostatectomy(RARP).Materials and methods:BetweenDecember 2012 and April 2020,704men underwent RARP in our *** study included 155 patients with a preoperative 5-item International Index of Erectile Function(IIEF-5)of≥12 points and an assessable IIEF-5 at 12 months ***-related quality of life was assessed using the 8-item Short-Form Health Survey and Expanded Prostate Cancer Index Composite(EPIC)preoperatively and at 3,6,and 12months postoperatively.A logistic regression analysis andWilcoxon rank sum tests were ***:Patients were grouped according to the median IIEF-5 score 12 months after surgery:those with preserved sexual function(n=71)and those with impaired sexual function(n=84).The mental component summary of the 8-item Short-Form Health Survey was better in the group with preserved sexual function at 6 months postoperatively than in the group with impaired sexual function(p<0.01).In the EPIC,the group with preserved sexual function performed better not only in the sexual domain but also in the urinary domain at all time points comparedwith the groupwith impaired sexual function(p<0.01).In the comparison of the urinary subdomains of the EPIC,there were no significant differences in urinary function or incontinence,but there were significant differences in urinary distress and irritative/obstructive scores(p<0.01).Conclusions:Patients with preserved postoperative sexual function after RARP showed better urinary function than those with impaired sexual ***,preserved sexual function is closely associated with urinary function.

Carpal Tunnel Syndrome: A Marker for Amyloidosis

Introduction: Amyloidosis are systemic conditions and carpal tunnel syndrome (CTS) precedes the principal systemic complications and can be used as an early marker. Our objective was to determine the frequency of amyloid deposition in idiopathic CTS and its systemic impact. Methods: We retrospectively evaluated patients with CTS between September 2019 to January 2020. Samples from the anterior carpal ligament were pathologically evaluated and amyloid deposition was confirmed by apple-green birefringence on polarized light using Congo red stain. When amyloid was detected we performed genetic testing for transthyretin variants (ATTRv), immunofixation electrophoresis in serum and urine for light chains and multidisciplinary evaluation. Results: Thirty consecutive patients were included, 19 women, 11 men, mean age 70 years old (range 42 - 89 years). We identified 3 patients (10%) with amyloid deposits (mean age: 78.6 years, 2 men, 1 woman). Genetic testing for ATTRv and light chains studies were negative. During follow-up: The first patient required aortic valve replacement. The second patient developed progressive cardiac failure with syncopal episodes, atrioventricular block and atrial fibrillation and required a pacemaker and anticoagulation. The third patient had unexplained chronic edemas. The cardiac evaluation in all 3 patients revealed left ventricular hypertrophy and myocardial uptake (Perugini Score > 2) in their nuclear bone scintigraphies with technetium pyrophosphate. Two patients were treated with tafamidis and one patient died due to refractory cardiac insufficiency. Discussion: Our findings underline the importance of investigating amyloidosis in idiopathic CTS. The identification of deposits allows early diagnosis of cardiac amyloidosis leading to timely intervention and treatment.

Bamboos are widely distributed in mainland Southeast Asia and have abundant biomass. They are characterized by prolonged vegetative growth and semelparity. Where bamboos are dominant, their synchronous flowering and death has a major impact on forest vegetation. Although the small-scale dynamics of this process have become clearer in recent years, the history, geographical scale and synchronicity of bamboo flowering over broad areas remains unknown. This study focused on the flowering history of six bamboo species, Bambusa tulda, Cephalostachyum virgatum, Dendrocalamus hamiltonii, Dendrocalamus membranaceus, Indosasa sinica and Oxytenanthera parvifolia, over 40 years across a broad area of northern Laos. We also examined the synchronicity of flowering in D. membranaceus. We visited 49 villages in northern Laos and surveyed knowledgeable inhabitants about bamboo flowering history. The timing, scale and synchronicity of gregarious flowering varied among species. D. hamiltonii and D. membranaceus showed higher flowering synchronicity than other species. All the species except I. sinica had both sporadic and gregarious flowering traits, and showed conspicuous variability in their flowering scale. The flowering bamboo population at two gregarious flowering sites for D. membranaceus was surveyed. While thisspecies had the highest synchronicity in this study, its synchronicity was lower than other species in previous studies worldwide. We found that the gregarious flowering of bamboos in northern Laos over the last 40 years showed lower synchronicity than bamboo flowering reported in other areas of the world. The historical dynamics and scale of bamboo flowering must be further clarified to understand the vegetation composition of this area.

Cardiovascular effects of hemoglobin response in patients receiving epoetin alfa and oral iron in heart failure with a preserved ejection fraction

Background Previous data from a recently conducted prospective, single blind randomized clinical trial among community dwelling older patients with heart failure with a preserved ejection fraction （HFPEF） and anemia randomized to treatment with epoetin alfa （erythro-poiesis-stimulating agents, ESA） vs. placebo did not demonstrate significant benefits of therapy regarding left ventricular （LV） structure, functional capacity, or quality of life （QOL）. However, several patients randomized to the treatment arm were non-responders with a subop-timal increase in hemoglobin. All patients in the trial also received oral ferrous gluconate, which could have contributed to increases in he-moglobin observed in those receiving placebo. Accordingly, we performed an analysis separating patients into responders vs. non-responders in order to determine if measured improvement in anemia would have any effect on clinical endpoints. Methods A total of 56 patients （age 77 ± 11 years, 68%female） were recruited who had anemia defined as a hemoglobin of≤12 g/dL （average, 10.4 ± 1 g/dL） with HFPEF defined as having NHANES-CHF （National Health And Nutrition Examination Survey：Congestive Heart Failure） criteria score of≥3 and an ejection fraction of＆gt;40%（average EF=63%±15%）. Patients were randomly allocated to receive either ESA and ferrous gluconate or ferrous gluconate only. In this analysis, a responder was defined as a patient with an increase of 1 g/dL in the first 4 weeks of the trial. Re-sults Nineteen subjects were classified as responders compared to 33 non-responders. While the average hemoglobin increased signifi-cantly at the end of 6 months for responders （1.8 ± 0.3 vs. 0.8 ± 0.2 g/dL, P = 0.004）, 50% of the subjects assigned to ESA were non-responders. Left ventricular function including ejection fraction （P=0.32） and end diastolic volume （P=0.59） was unchanged in res-ponders compared to non-responders.
