Incidence of hepatocellular carcinoma in outpatients with cirrhosis in Brazil: A 10-year retrospective cohort study

AIM To determine the incidence of hepatocellular carcinoma(HCC) and the impact of HCC surveillance on early diagnosis and survival of cirrhotic outpatients. METHODS In this retrospective cohort study, cirrhotic outpatients undergoing HCC surveillance between March 2005 and March 2014 were analyzed. Exclusion criteria were HIV coinfection; previous organ transplantation; diagnosis of HCC at first consultation; missing data in the medical chart; and less than 1 year of follow-up. Surveillance was carried out every six months using ultrasound and serum alpha-fetoprotein determination. Ten-year cumulative incidence and survival were estimated through Kaplan-Meier analysis. RESULTS Four hundred and fifty-three patients were enrolled, of which 57.6% were male. Mean age was 55 years. Hepatitis C virus and heavy use of alcohol were the main etiologic agents of cirrhosis. HCC was diagnosed in 75 patients(16.6%), with an estimated cumulative incidence of 2.6% in the 1st year, 15.4% in the 5th year, and 28.8% in the 10 th year. Median survival was estimated at 17.6 mo in HCC patients compared to 234 mo in non-HCC patients(P < 0.001). Early-stage HCC was more often detected in patients who underwent surveillance every 6 mo or less(P = 0.05). However, survival was not different between patients with early stage vs non-early stage tumors [HR = 0.54(0.15-1.89), P = 0.33].CONCLUSION HCC is a frequent complication in patients with cirrhosis and adherence to surveillance programs favors early diagnosis.

The 18th Workshop on Antarctic Meteorology and Climate was held on 31 May through 2 June *** Antarctic Meteorological Research and Data Center hosted this hybrid workshop at the Pyle Center at the University of Wisconsin-Madison in Madison,WI,*** members of the Antarctic meteorological community gathered to present and discuss topics from scientific research to operational support within Antarctic meteorology and *** and early career professionals chaired various presentations and discussions while all attendees engaged to share results,build collaborative plans,and discuss further *** main workshop topics included updates on the current Automatic Weather Station(AWS)networks,challenges,and planning concerning Antarctic forecasting and operational services,Antarctic numerical modeling systems,observational applications and research,and Antarctic community and data *** six sessions,over 25 different presenters elaborated on their work in areas such as field season updates,atmospheric rivers,remote sensing,the Antarctic Mesoscale Prediction System(AMPS),and forecasting challenges due to data *** discussions resulted in several key outcomes and action items regarding fieldwork impacts,exploration of field techniques,observation coverage,and communication between operations and research *** workshops will provide opportunities for continued discussion on the evolution of the AWS network and operational needs while providing a venue to promote collaboration and cooperation for Antarctic meteorology and climate activities.

Influence of proton pump inhibitors in the development of spontaneous bacterial peritonitis

AIM To investigate whether the use of proton pump inhibitors(PPIs) increases the incidence of spontaneous bacterial peritonitis(SBP) in patients with cirrhosis and *** An historical cohort study was carried out in cirrhotic outpatients with ascites followed in a specialized clinic at a tertiary hospital in Southern Brazil. Patient charts were reviewed to collect information on the variables of interest as the use of PPIs. Primary outcome was defined as development of SBP during the study period. SBP was diagnosed based on ascitic fluid polymorphonuclear cell count ≥ 250 cells/mm3 without evidence of an intraabdominal, surgically treatable source of *** Of 738 cirrhotic patients, 582(58.2% male) were enrolled, with mean age of 53.6 ± 12 years. Hepatitis C virus infection(36.2%) and alcohol abuse(25.6%) were the main etiologies of cirrhosis. The presence of ascites was detected in 299(51.4%) patients during the development of the study. Nineteen patients with previous diagnosis of SBP undergoing secondary prophylaxis and 22 patients with insufficient PPI data were further excluded. Of 258 patients with ascites, 151 used PPIs, and 34 developed SBP(22.5%). Among 107 non-users of PPIs, 23 developed SBP(21.5%)(HR = 1.44, 95%CI: 0.85-2.47, P = 0.176). The median follow-up time of patients using PPI was 27 mo vs 32 mo for non-users. Univariate analysis of the risk factors associated with the development of SBP revealed a significant association of SPB with the severity of liver disease according to the Child-Turcotte-Pugh(CTP) score. Multivariate analysis confirmed that CTP score was the only independent variable influencing the occurrence of SBP. Survival at 60 mo(Kaplan-Meier analysis) was similar in users and non-users of PPI, independently of the presence of SBP(58.4% vs 62.7% respectively, P = 0.66). For patients with SBP, survival at 60 mo was 55.1%, vs 61.7% in patients without SBP(P = 0.34). CONCLUSION In conclusion, the rate of SBP was not

Review of allergic contact dermatitis: Scratching the surface

Contact dermatitis-including allergic contact dermatitis(ACD)-n and results in over four million lost work days per year in the United States alone. ACD is a classic example of a type IV delayed hypersensitivity reaction, and represents a significant burden on the health system, economy, and patient quality of life. Thorough history taking, clinical examination, histologic evaluation, and patch testing are keys to diagnosing contact dermatitis. Patch testing, especially with comprehensive and customized panels based on the patient's exposure history, is particularly useful in identifying potential allergens inthe case of allergic contact dermatitis. ACD management requires a combination of direct medical intervention, patient education, and appropriate environmental modification to prevent exposure to offending allergens in the home or workplace. Continuing advances in the study of ACD has led to an increased understanding of the disease processes, new methods for diagnosis, and improved management. This article reviews ACD-aiming to connect recent investigational data with the current clinical understanding of disease pathophysiology, diagnostic techniques, and management strategies.

Cowpea Production Challenges and Contribution to Livelihood in Sub-Saharan Region

Cowpea [Vigna unguiculata (L.) Walp.] is an important grain legume mainly grown in tropical and subtropical regions for vegetables, grains, and fodder. The crop is grown predominantly in the dry savannahs to the Sahel in the fringes of the Sahara Desert where the annual rainfall is around 300 mm or less. Cowpea provides shelter as a cover crop and improves soil fertility by fixing atmospheric nitrogen. Its grain is preferred for high levels of protein, energy, micro-and macronutrients. Cowpea belongs to the family Fabaceae and sub-family Faboidea, and it is a self-pollinating crop with low and narrow genetic diversity, making it susceptible to various environmental factors. Various research projects have been established on cowpea, and as a result, various cowpea germplasm is stored at Africa International Institute of Tropical Agriculture (IITA)—Nigeria, the USDA repository in Griffin, GA (USA), the University of California, Riverside, CA (USA), and at the National Bureau of Plant Genetic Resources (NBPGR) in New Delhi, India. Despite the available germplasms, cowpea productivity remains very low in many countries due to a wide array of abiotic and biotic stresses and socio-economic constraints. This review summarizes some aspects of cowpea, including origin and history, challenges, economic importance, and constraints to production, providing possible useful information for cowpea researchers, agronomists, and producers in SSA.
