目的先天性心脏病(CHD)是一种在全球范围内高发的慢性疾病,通常在出生时或出生前确诊。由于诊断以及早期药物和手术治疗的条件较好,患者的生存率达到90%,并且生存时间不断延长,面临着生活中的各种挑战。该研究探讨了不同人口统计学、临床和心理社会变量对CHD青少年和年轻成人患者的生活质量(QOL)感知、心理社会适应和精神障碍患病率的影响,评估CHD青少年和年轻成人患者的生活质量、心理社会适应和精神障碍发病率,确定哪些变量(人口统计学、临床和心理社会变量)对缓解压力、促进恢复起到积极作用,哪些变量起到不良作用。方法该研究入组了150名CHD患者(男87例,女63例),年龄12～26岁(平均年龄:17.45±3.373岁)。对参与者进行了关于社会支持、家庭教育方式、自我形象、人口统计信息和生理限制等方面的访谈。他们对标准化精神病学访谈(SADS-L)中的问题进行了回答,并填写了自我报告问卷,以进行生活质量(WHOQOL-BREF)和心理社会适应(YSR/ASR)的评估。结果所有参与者精神障碍的终身患病率为18.7%(女性25.4%,男性13.8%)。57.1%的参与者有过留级(平均1.53±0.804年)。与整体葡萄牙人相比,CHD患者的生活质量感知在社会关系、环境、身体和总体方面较好。而女性、学习成绩差、较少的社会支持、复杂性CHD、紫绀型CHD、中重度残留病灶、接受过手术和有生理限制患者生活质量感知较差。除了紫绀,所有这些变量也会引起较差的心理社会适应。结论女性CHD患者及学习成绩差且获得社会支持少的CHD患者的心理社会适应和生活质量较差。

Effects of Exercise Training on Endothelial Function,Arterial Structure,and physical Conditioning in patients with Systemic Autoimmune Myopathies:A Case Series Study

Introduction. Exercise training has been effective in improving endothelial function and decreasing arterial stiffness in several systemic autoimmune diseases. However, to date, no studies have assessed patients with systemic autoimmune myopathies (SAM). Methods. Five female patients with definite SAM (3 dermatomyositis and 2 polymyositis) who underwent a 12-week, twice-a-week, exercise training program were assessed prospectively. The following parameters were assessed: endothelial function measured by flow-mediated dilatation (FMD), carotid femoral by pulse wave velocity (pWV), disease status by International Myositis Assessment & Clinical Studies Group (IMACS) set score, the ventilatory anaerobic threefold (VAT), respiratory compensation point (RCp), maximum effort (ME), maximal oxygen uptake (VO2max) measured by ergospyrometer, and strength and muscle function. Results. Mean age of the patients and duration of disease were 43.9 and 10.5 years, respectively. The median FMD and pWV were 10.2% and 7.1 m/s. After exercise training, important FMD reduction was observed in 4 out of 5 patients, whereas the pWV parameters decreased in only 2 out of 5 patients. The exercise training was safe, without clinical intercurrences or disease relapses. Moreover, an increase in strength and functionality was observed. Concerning aerobic capacity, there was a decrease in the VAT and an increase in ME, without al-terations in the maximum oxygen consumption. Conclusions. In general, exercise training does not alter endothelial function and pWV values, but improves muscle strength and function and also, does not lead to disease reactivation (Clinical Trials (NCT03092167)).

This work aimed at investigating whether the liver glucose metabolism could have a role in any change of glucose homeostasis that might exist in the reduced-litter rat at the age of 60 days. Additionally, post-weaning caloric restriction and its hepatic effects were explored, as this intervention is reported as a useful measure against obesity and its related disturbances. The animals were raised in litters of nine （control, CG） or three pups during lactation. These small-litter pups were FG （fed freely） or subjected to 30% RG （caloric restriction） after weaning until 60 days of age. The increased adiposity induced by lactational overfeeding was not reversed by caloric restriction. Hepatocyte glucose metabolism and glucose tolerance test were not affected by litter size, but caloric restriction increased liver basal glucose release, diminished gluconeogenesis and retarded the glycemic decay during the insulin tolerance test. Liver glucose metabolism of young adult rats was not affected by lactational overfeeding. Up to this age, however, moderate caloric restriction had a potent influence that might compromise whole-body glucose homeostasis and prompt to insulin resistance.

In this manuscript the authors have studied gigantomastia in female workers. After this weight loss, they perform mammary reduction and, after a year, the patients were referred to the bariatric team. All patients stayed for one year at the Obesity Workshop to learn about the dangers and benefits of weight loss, breast reduction and the possibilities of a more healthy body and future repairing plastics. After all these procedures, they observe an adequate psychological preparation. The patients felt more comfortable and happy with the smaller breasts, operated by using the Resende technique.

Chlorination is an efficient and low-cost technique in disinfecting water for public water supply. However, during the process, the formation of undesirable by-products called halogen compounds, such as trihalome-thanes (THMs), occurs. Some of these compounds are proven to be carcinogenic to laboratory animals. The goal of this study was to investigate the occurrence of THMs in household reservoirs and the influence of cleaning time of these reservoirs on the formation of THMs. A total of 103 households were investigated in the city of Maringá, State of paraná, Brazil. The residences were classified according to the cleaning time of the reservoir into: up to one year, one to two years and more than two years. Samples were taken from the reservoirs and total trihalomethanes (THMt) were analyzed by gas chromatography (GC-MS), total organic carbon (TOC) by means of a Hach kit for low concentrations and free residual chlorine (CLres) by means of a kit for in loco determination. According to the results, the highest concentrations of THMt were observed in the reservoirs with cleaning time of more than 2 years. However, no amount has exceeded the maximum limit allowed by law. In general, the longer the time for cleaning the reservoir, the greater the degradation of water quality in THMt.

Background: The present work aims to characterize the profile of patients with stroke treat at a hospital located in the Region of the Mata of Minas Gerais, Brazil, considering the findings of the clinical vocal tract, kind of stroke, age and gender of such patients. Methodology: To obtain data, the clinical profile of 133 patients with a clinical or tomography diagnosis of stroke was analyzed, and the results were presented in percentage. For quantitative data average and analysis the tests were done with associations that held χ2 test, and for significance it was considered p Results: From the total of patients, 63 were women, accounting for 47.4% and the other 52.6% were males. Clinically, they were characterized with the highest percentage for ischemic stroke (89.4%) compared to the hemorrhagic type (10.6%). Most of them were referred for computed tomography (86.5%) and remained hospitalized for an average of 6.496 ± 7.372 days. Similar percentages were obtained in the analysis of the population in question, when considering if they had (54.1%) or not (49.6%) any damage in their speech, language skills or swallowing. There were different types of disabilities in patients with stroke. Men with an average age of 69.8 ± 13.9 presents mostly ischemic stroke, and the majority of patients with stroke had hemiplegia and abnormalities of the vocal tract, dysphasia, and aphasia. While older patients had an ischemic stroke and were presented with left hemiplegia, the younger ones suffered from hemorrhagic strokes that caused a disability characterized as right hemiplegic. Conclusion: Our results show important conclusions regarding the clinical evolution of the vocal tract of patients who suffered strokes during the period of the analysis, being useful for better comprehension of how the vocal tract from these patients evolved according to the kind of stroke, sex and age also allowing a contraposition with other future statistics periods available in literature.
