BACKGROUnD Metastatic pancreatic ductal adenocarcinoma(PDAC)is a lethal malignancy with dispiriting survival *** is a promising approach to many cancer types,but achieves poor outcomes in advanced PDAC due to its immunosuppressive tumor *** describe a case of metastatic PDAC effectively treated with *** SUMMARY We report the case of a 67-year-old woman with unresectable locally advanced PDAC,treated with gemcitabine plus nab-paclitaxel followed by radiotherapy plus *** nine months,pancreatic tumor progression was observed at the level of the hepatic hilum with the appearance of a new pulmonary nodule suggestive of a second primary,confirmed by left lung *** immunotherapy was then initiated with pembrolizumab,an immune checkpoint inhibitor targeting programmed cell death protein-1 that covers the two tumor *** patient showed a complete metabolic response that was maintained throughout the *** patient continues to be disease-free at 5.6 years since the start of *** These results suggest that the administration of pembrolizumab after chemoradiotherapy has a beneficial effect in patients with metastatic *** our knowledge,this is the first reported case of a patient with metastatic PDAC and metastatic lung cancer showing such a long-lasting complete response after pembrolizumab treatment without curative *** studies are required to determine biomarkers that identify PDAC patients most likely to benefit from this immunotherapy.

There is an increasing interest in restoring degraded forests,which occupy half of the forest *** the forms of restoration,passive restoration,which involves the elimination of degrading factors and the free evolution of natural dynamics by applying minimal or no management,is gaining *** dynamics is difficult to predict due to the influence of multiple interacting factors such as climatic and edaphic conditions,composition and abundance of species,and the successional character of these ***,we study the natural dynamics of a mixed forest located in central Spain,which maintained an open forest structure,due to intensive use,until grazing and cutting were banned in the *** most frequent woody species in this forest are Fagus sylvatica,Quercus petraea,Quercus pyrenaica,Ilex aquifolium,Sorbus aucuparia,Sorbus aria and Prunus avium,with contrasting shade and drought *** species are common in temperate European deciduous forest and are found here near their southern distribution limit,except for *** order to analyze forest dynamics and composition,three inventories were carried out in 1994,2005 and *** results show that,despite the Mediterranean influence,the natural dynamics of this forest has been mainly determined by different levels of shade *** the abandonment of grazing and cutting,*** expanded rapidly due to its lower shade tolerance,whereas after canopy closure and forest densification,shade-tolerant species gained ground,particularly ***,despite its lower drought and late-frost *** the current dynamics continue,*** will overtake the rest of the species,which will be relegated to sites with shallow soils and steep ***,all the multi-centennial beech trees,which are undergoing a rapid mortality and decline process,will disappear.

The objective of the present work is modeling the mobility habitus in the public transport system with low Co2 emission mechanics in the center of *** this sense,a documentary work was carried out with a review of sources indexed to national repositories,using the Delphi technique for the content analysis and the specification of the model.A study was conducted with a selection of 100 students to establish the reliability,linearity,sphericity,adequacy,and validity of an instrument from which a relationship of dependence between income and mobility habitus was *** limits of the search,selection,and processing of information are recognized,and it is recommended to extend the research to international repositories,as well as to employ a more sophisticated technique.

Melanin is the basis of coloration in many animals,and although it is often used in communication,thermoregulation,or camouflage,melanin has many other physiological *** example,in polluted habitats,melanin can have a detoxifying *** coloration would help to sequester in the skin the heavy metal contaminants from inside the body,which will be expelled to the exterior when the skin is ***,animals should have evolved more melanic colorations in more polluted habitats(“industrial melanism”hypothesis).We examined whether the fossorial amphisbaenian reptile,Trogonophis wiegmanni,is able to eliminate heavy metals,derived from soil pollution by seagull depositions,through sloughing its *** results suggest a covariation between levels of soil pollution by heavy metals and the concentration of heavy metals in the sloughed skins of *** suggests that amphisbaenians may expel heavy metals from their bodies when they slough the *** also tested whether amphisbaenians inhabiting soils with higher levels of heavy metal pollution had darker(melanin-dependent)body ***,contrary to predictions from the“industrial melanization”hypothesis,we found a negative relationship between soil pollution and proportions of melanic *** contradictory result could,however,be explained because heavy metals have endocrine disruption effects that increase physiological stress,and higher stress levels could result in decreased *** suggest that although amphisbaenians might have some detoxifying mechanism linked to melanin in the skin,this process might be negatively affected by stress and result ineffective under conditions of high soil pollution.

Immediate effect of high-intensity exercise on brain-derived neurotrophic factor in healthy young adults:A systematic review and meta-analysis

Background Although brain-derived neurotrophic factor(BDnF)has been identified as a molecular biomarker of the neurophysiological effects induced by exercise,the acute effects of high-intensity exercise(HIE)on BDnF levels are *** study aims to estimate the immediate effects of HIE on BDnF levels in healthy young *** A systematic search was conducted in the MEDLInE,Scopus,Cochrane CEnTRAL,and SPORTDiscuss databases up to December *** controlled trials(RCTs)and non-RCTs reporting pre-post changes in serum or plasma BDnF after an acute intervention of HIE compared to a control condition were *** effect sizes(p-ESs)and 95%confidence intervals(95%CIs)were calculated for RCTs using a random effects model with Stata/SE(Version 15.0;StataCorp.,College Station,TX,USA).The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were *** registration number:*** A total of 22 studies with 552 individuals(age range:20-31 years;59.1%male)were *** meta-analysis included 10 RCTs that reported valid outcome *** BDnF levels were observed when HIE interventions were compared with non-exercise(p-ES=0.55,95%CI:0.12-0.98;I^(2)=25.7%;n=4 studies)and light-intensity exercise(p-ES=0.78,95%CI:0.15-1.40;I^(2)=52.4%;n=3 studies)but not moderate-intensity exercise(p-ES=0.93,95%CI:-0.16 to 2.02;I^(2)=88.5%;n=4 studies)*** In comparison to non-exercise or light-intensity exercises,an immediate increase in BDnF levels may occur when young adults perform *** the benefits obtained maximizing circulating BDnF when performing HIE and its potential effects on brain health,our findings suggest that HIE could be recommended by clinicians as a useful exercise strategy to healthy adults.

In recent years,there has been a growing interest in exploring alternative treatments for bone *** tissue engineering has turned its attention to plant extracts containing osteogenic flavonoids as potential promoters of bone *** our study,we specifically investigated the extract of Petroselinum crispum,a plant known for its abundance of osteogenic flavonoids such as apigenin,quercetin,and *** objective was to compare the total flavonoid content(TFC)and their accumulation in different *** obtained hydrolyzed aqueous extracts from the leaves of parsley plants(grown for 12 weeks in the field),weekly embryogenic cultures,and suspension cell *** TFC,measured in mg quercetin equivalent per gram of dry weight(QE mg/g dw),was found to be 53.81±0.97 for 52 g dw of leaves(1 kg plant fw),31.10±1.52 for 420 g dw of embryogenic cultures,and 11.80±0.76 for 210 g dw of cell ***,only the leaves and embryogenic cultures showed significant accumulation of osteogenic flavonoids,with apigenin levels of 1.8±0.3 and 0.32±0.02 mg/g dw,and kaempferol levels of 1.0±0.18 and 0.2±0.01 mg/g dw,*** findings indicate that embryogenic cultures have the potential to prod.ce up to 40%more apigenin in 12 weeks compared to 1 kg of parsley plant,thanks to their superior biomass reprod.ctive *** results present a promising biotechnological approach for generating extracts enriched with bioactive compounds,which can be further utilized in in vitro osteogenic testing.

Out-of-hospital cardiac arrest(OHCA)is one of the three leading causes of death in industrialized countries.[1,2]Some studies have described the impact of the first COVID-19 pandemic wave in terms of the number of cases and OHCA survival rates in various regions,[3-5]but few have addressed the relationship between the successive phases and how they affected OHCA.[6,7]The 14-d cumulative incidence peaked at more than 990 cases,with these rates remaining above 200 for an eight-month period between 15 March 2020 and 15 March 2021 in Madrid,Spain.

Crop growth in greenhouses is basically determined by the climate variables in the environment and by the amounts of water and fertilizers supplied by *** management of these factors depends on the expertise of agricultural technicians and farmers,usually assisted by control systems installed within the *** this context,decision support features enable us to incorporate invaluable human experience so thatwe can take quick and effective decisions to ensure efficient crop *** work describes a real-time decision support system for greenhouse tomatoes that supports decisions at three stages–the supervision stage identifies climate sensor faults,the control stage maintains climate variables at setpoints,and the strategic stage identifies diseases affecting the crop and changes climate variables accordingly to minimize *** DSS was implemented by integrating a real-time rule-based tool into the control *** results show that the system increases climate control effectiveness,while providing support in preventing diseaseswhich are difficult to *** system was tested by simulating the appearance of the disease and observing the real *** main contribution has been to demonstrate that prod.ction rules,which aremature and well-known in the artificial intelligence domain,can act as a shared technology for the whole *** means that fault detection,temperature control and disease monitoring features are not dealt with in isolation.

Background:Knee examination guidelines in minors are intended to aid decisionmaking in the management of knee *** question:A Delphi study was conducted with a formal consensus process using a validated methodology with sufficient scientific evidence.A group consensus meeting was held to develop recommendations and practical guidelines for use in the assessment of instability injuries in *** findings:there is a lack of evidence to analyse anterior cruciate ligament injuries in children and their subsequent surgical management if *** guidelines and clinical assessment of the patient based on a thorough examination of the knee are performed and a guide to anterior cruciate ligament exploration in children is *** application:In the absence of a strong evidence base,these established guidelines are intended to assist in that decision-making process to help the clinician decide on the most optimal treatment with the aim of benefiting the patient as much as *** this expert consensus,surgical treatment is advised when the patient has a subjective sensation of instability accompanied by a pivot shift test++,and may include an anterior drawer test+and a Lachman test+.If these conditions are not present,the conservative approach should be chosen,as the anatomical and functional development of children,together with a physiotherapy programme,may improve the evolution of the injury.
