Octogenarian patients with colorectal cancer: Characterizing an emerging clinical entity

AIM To characterize colorectal cancer(CRC) in octogenarians as compared with younger *** A single-center, retrospective cohort study which included patients diagnosed with CRC at the age of 80 years or older between 2008-2013. A control group included consecutive patients younger than 80 years diagnosed with CRC during the same period. Clinicopathological characteristics, treatment and outcome were compared between the groups. Fisher's exact test was used for dichotomous variables and χ2 was used for variables with more than two categories. Overall survival was assessed by Kaplan-Meier survival analysis, with the log-rank test. Cancer specific survival(CSS) and disease-free survival were assessed by the Cox proportional hazards model, with the Fine and Gray correction for non-cancer death as a competing *** The study included 350 patients, 175 patients in each group. Median follow-up was 40.2 mo(range 1.8-97.5). Several significant differences were noted. Octogenarians had a higher proportion of Ashkenazi ethnicity(64.8% vs 47.9%, P < 0.001), a higher rate of personal history of other malignancies(22.4% vs 13.7%, P = 0.035) and lower rates of family history of any cancer(36.6% vs 64.6%, P < 0.001) and family history of CRC(14.4% vs 27.3%, P = 0.006). CRC diagnosis by screening was less frequent in octogenarians(5.7% vs 20%, P < 0.001) and presentation with performance status(PS) of 0-1 was less common in octogenarians(71% vs 93.9%, P < 0.001). Octogenarians were more likely to have tumors located in the right colon(45.7% vs 34.3%, P = 0.029) and had a lower prevalence of well differentiated histology(10.4% vs 19.3%, P = 0.025). They received less treatment and treatment was less aggressive, both in patients with metastatic and non-metastatic disease, regardless of PS. Cisplatin-Induced Posterior Reversible Encephalopathy Syndrome—Brief Report and Review of the Literature

Cisplatin is one of the most broadly used chemotherapeutic agents. Several central nervous system toxicities have been attributed to this agent, among which is a rare clinicoradiological condition referred to as posterior reversible leukoen-cephalopathy syndrome (PRES) or reversible posterior leukoencephalopathy syndrome (RPLS). PRES is characterized by subacute onset of headache, visual disturbances, confusion, seizures and rarely, coma. This usually reversible syn-drome was associated in the past mainly with hypertension. Several reports have associated the syndrome with several immunosuppressive and chemotherapeutic agents suggesting a disruption of the blood brain barrier due to direct damage to the endothelium and vasogenic edema. This syndrome is readily diagnosed with Computed Tomography (CT) or Magnetic resonance imaging (MRI) studies and quick withdrawal of the causative agent in conjunction with supportive care including rapid hypertension control and reduction of elevated intracranial pressure when needed, usually reverse the neurological symptoms with minimal or even no sequela within several days. In this brief report and review of the literature we describe two cases of PRES in two female patients that occurred a few days after receiving Cisplatin-based chemotherapy. We discuss the clinical presentation, diagnosis and treatment of this syndrome by reviewing the literature. The escalating number of clinical reports of PRES merit further studies of the mechanism of toxicity, appropriate treatment and awareness of physicians to this life threatening

Extracellular DNA Plays an Important Structural Role in the Biofilm of the Plastic Degrading Actinomycete <i>Rhodo-coccus ruber</i>

Biofilms, the preferred bacterial mode of living and survival, are employed by most microorganisms—which tend to attach to surfaces—to gain physical support, increase nutrient utilization and availability, and augment their resistance against anti-bacterial agents. Rhodococcus ruber (C208) has been shown to form a dense biofilm on polyethylene surfaces while degrading them. Bacterial biofilms comprise bacterial cells embedded in self-secreted extracellular polymeric substances (EPS) whose main components are polysaccharides, proteins and nucleic acids. Revealing the roles of these components will enable further insight into biofilm development and, therefore, the EPS structure-function relationship. The current study focuses on contribution of extracellular DNA to biofilm formation and stability. This was approached by investigating the influence of nucleases on biofilm formation via degradation of their corresponding substrates within the biofilm of C208. RNase application to cultures of C208 decreased biofilm formation. Degradation of biofilm DNA by DNase reduced early-stage biofilm formation by 20% -25% but had no significant effect on established, mature biofilm. Likewise, the addition of DNA to cultures significantly enhanced early-stage biofilm formation by 50% -100%. RAPD-PCR analysis revealed different band patterns from intra-cellular DNA and extra-cellular DNA and also between the supernatant and biofilm fractions of extra-cellular DNA, indicating that perhaps only certain DNA molecules are utilized as part of the biofilm.

Coupling Agent Based Simulation with Dynamic Networks Analysis to Study the Emergence of Mutual Knowledge as a Percolation Phenomenon

The emergence of mutual knowledge is a major cognitive mechanism for the robustness of complex socio-technical systems. It has been extensively studied from an ethnomethodological point of view and empirically reproduced by multi-agent simulations. Whilst such simulations have been used to design real work settings the underlying theoretical grounding for the process is vague. The aim of this paper is to investigate whether the emergence of mutual knowledge(MK) in a group of colocated individuals can be explained as a percolation phenomenon. The followed methodology consists in coupling agent-based simulation with dynamic networks analysis to study information propagation phenomena: After using an agent-based simulation the authors generated and then analyzed its traces as networks where agents met and exchanged knowledge. Deep analysis of the resulting networks clearly shows that the emergence of MK is comparable to a percolation process. The authors specifically focus on how changes at the microscopic level in the proposed agent based simulator affect percolation and robustness. These results therefore provide theoretical basis for the analysis of social organizations.

Pegylated interferon alfa and ribavirin for children with chronic hepatitis C

AIM:To study current treatment options for pediatric hepatitis C infection and their associated success ***:We retrospectively reviewed charts of thirty children who had been treated with combination therapy of pegylated interferon alfa plus ribavirin for chronic hepatitis C *** had been treated with ribavirin(15 mg/kg per day) and either pegylated interferon alfa 2a(180 mg/m 2 once weekly) or pegylated interferon alfa 2b(1.5 mg/kg once weekly).Patients' follow-up included subjective assessment of complaints,physical examination including weight and height,as well as laboratory evaluations for viral load [before treatment,at 12 wk,and 6 mo following treatment completion,as determined by sustained viral response(SVR)],complete blood count,liver enzymes,alkaline phosphatase,bilirubin,renal function tests,and thyroid function *** patients not achieving a two log decrease in viral load at treatment week 12,treatment was discontinued and the patient was considered a treatment ***:Thirty children aged 3-18 years were included in the *** patients(11 males,9 females) received pegylated interferon alfa 2b and ten patients(6 males,4 females) received pegylated interferon alfa ***-three patients were infected with genotype 1,six patients were infected with genotype 3,and one patient was infected with genotype *** patients(67%) achieved *** success rates were 90% with pegylated interferon alfa 2a vs 55% with pegylated interferon alfa *** a trend was noted for improved outcomes in the group receiving pegylated interferon alfa 2a,there were no statistically significant outcome differences between the two treatment groups(P = 0.1).Treatment success was 56.5% for patients infected with genotype 1 virus,compared to 100% for patients infected with other genotypes(P = 0.064).There was no difference in treatment response between males and females.A cut-off age of twelve years was used to dichotomize
