Gut dysbiosis and small intestinal bacterial overgrowth as independent forms of gut microbiota disorders in cirrhosis

BACKGROUND Gut dysbiosis and small intestinal bacterial overgrowth(SIBO)are commonly observed in patients with cirrhosis.Despite the substantial number of articles describing the relations between disorders of gut microbiota and various manifestations of cirrhosis,dysbiosis and SIBO were always studied separately.AIM To study the relationship of gut dysbiosis and SIBO in cirrhosis.METHODS This observational study included 47 in-patients with cirrhosis.Stool microbiome was assessed using 16 S r RNA gene sequencing.SIBO was assessed using the lactulose hydrogen breath test.RESULTS SIBO was found in 24/47(51.1%)patients.Patients with SIBO had a higher abundance of Firmicutes(P=0.017)and Fusobacteria(P=0.011),and a lower abundance of Bacteroidetes(P=0.013)than patients without SIBO.This increase in the abundance of Firmicutes occurred mainly due to an increase in the abundance of bacteria from the genus Blautia(P=0.020)of the Lachnospiraceae family(P=0.047),while the abundance of other major families of this phylum[Ruminococcaceae(P=0.856),Peptostreptococcaceae(P=0.066),Clostridiaceae(P=0.463),Eubacteriaceae(P=0.463),Lactobacillaceae(P=0.413),and Veillonellaceae(P=0.632)]did not differ significantly between the patients with and without SIBO.Reduced level of Bacteroidetes in samples from patients with SIBO was a result of the decrease in bacterial numbers from all the major families of this phylum[Bacteroidaceae(P=0.014),Porphyromonadaceae(P=0.002),and Rikenellaceae(P=0.047)],with the exception of Prevotellaceae(P=0.941).There were no significant differences in the abundance of taxa that were the main biomarkers of cirrhosis-associated gut dysbiosis[Proteobacteria(P=0.790),Bacilli(P=0.573),Enterobacteriaceae(P=0.632),Streptococcaceae(P=0.170),Staphylococcaceae(P=0.450),and Enterococcaceae(P=0.873)]between patients with and without SIBO.CONCLUSION Despite the differences observed in the gut microbiome between patients with and without SIBO,gut dysbiosis and SIBO are most likely independent disorders of gut microbiota

Bipolar Interval-Valued Neutrosophic Optimization Model of Integrated Healthcare System

Bipolar Interval-valued neutrosophic set is another generalization of fuzzy set,neutrosophic set,bipolar fuzzy set and bipolar neutrosophic set and thus when applied to the optimization problem handles uncertain data more efficiently and flexibly.Current work is an effort to design a flexible optimization model in the backdrop of interval-valued bipolar neutrosophic sets.Bipolar interval-valued neutrosophic membership grades are picked so that they indicate the restriction of the plausible infringement of the inequalities given in the problem.To prove the adequacy and effectiveness of the method a unified system of sustainable medical healthcare supply chain model with an uncertain figure of product complaints is used.Time,quality and cost are considered as satisfaction level to choose best supplier for medicine procurement.The proposed model ensures 99%satisfaction for cost reduction,63%satisfaction for the quality of product and 64%satisfaction for total time taken in medicine supply chain.

CRISPR-based genetic control strategies for insect pests

Genetic control strategies such as the sterile insect technique have successfully fought insect pests worldwide. The CRISPR(clustered regularly interspaced short palindromic repeats) technology, together with high-quality genomic resources obtained in more and more species, greatly facilitates the development of novel genetic control insect strains that can be used in area-wide and species-specific pest control programs. Here, we review the research progress towards state-of-art CRISPR-based genetic control strategies, including gene drive, sex ratio distortion, CRISPRengineered genetic sexing strains, and precision-guided sterile insect technique. These strategies’ working mechanisms,potential resistance development mechanisms, and regulations are illustrated and discussed. In addition, recent developments such as stacked and conditional systems are introduced. We envision that the advances in genetic technology will continue to be one of the driving forces for developing the next generation of pest control strategies.

Multimodal Fuzzy Downstream Petroleum Supply Chain:A Novel Pentagonal Fuzzy Optimization

The petroleum industry has a complex,inflexible and challenging supply chain(SC)that impacts both the national economy as well as people’s daily lives with a range of services,including transportation,heating,electricity,lubricants,as well as chemicals and petrochemicals.In the petroleum industry,supply chain management presents several challenges,especially in the logistics sector,that are not found in other industries.In addition,logistical challenges contribute significantly to the cost of oil.Uncertainty regarding customer demand and supply significantly affects SC networks.Hence,SC flexibility can be maintained by addressing uncertainty.On the other hand,in the real world,decision-making challenges are often ambiguous or vague.In some cases,measurements are incorrect owing to measurement errors,instrument faults,etc.,which lead to a pentagonal fuzzy number(PFN)which is the extension of a fuzzy number.Therefore,it is necessary to develop quantitative models to optimize logistics operations and supply chain networks.This study proposed a linear programming model under an uncertain environment.The model minimizes the cost along the refineries,depots,multimode transport and demand nodes.Further developed pentagonal fuzzy optimization,an alternative approach is developed to solve the downstream supply chain using themixed-integer linear programming(MILP)model to obtain a feasible solution to the fuzzy transportation cost problem.In this model,the coefficient of the transportation costs and parameters is assumed to be a pentagonal fuzzy number.Furthermore,defuzzification is performed using an accuracy function.To validate the model and technique and feasibility solution,an illustrative example of the oil and gas SC is considered,providing improved results compared with existing techniques and demonstrating its ability to benefit petroleum companies is the objective of this study.

A pilot study of the relative number of circulating tumor cells and leukocytes containing actin-binding proteins in head and neck cancer patients

Circulating tumor cells(CTCs)play an important role in tumor metastases,which is positively correlated with an increased risk of death.Actin-binding proteins,including cofilin(CFL1),profilin 1(PFN1),and adenylate cyclase-associated protein 1(CAP1),are thought to be involved in tumor cell motility and metastasis,specifically in head and neck squamous cell carcinoma(HNSCC).However,currently,there are no published studies on CFL1,PFN1,and CAP1 in CTCs and leukocytes in HNSCC patients.We assessed serum levels of CFL1,PFN1,and CAP1 and the number of CTCs and leukocytes containing these proteins in blood from 31 HNSCC patients(T1-4N0-2M0).The analysis used flow cytometry and an enzyme-linked immunosorbent assay kit.We found that CAP1+CTCs and CAP1+leukocyte subpopulations were prevalent in these HNSCC patient samples,while the prevalence rates of CFL1+and PFN1+CTCs were relatively low.Patients with stage T2-4N1-2M0 had CFL1+and PFN1+CTCs with an elevated PFN1 serum level,compared with the T1-3N0M0 group.In summary,the PFN1 serum level and the relative number of PFN1+CD326+CTCs could be valuable prognostic markers for HNSCC metastases.The current study is the first to obtain data regarding the contents of actin-binding proteins(ABPs)in CTCs,and leukocytes in blood from HNSCC patients.This is also the first to assess the relationship between the number of CTCs subgroups and disease characteristics.

Latitudinal gradients in body size and sexual size dimorphism in fleas:males drive Bergmann’s pattern

We tested for the effects of latitude and geographic range size(GRS)on body size,leg length,and sexual size dimorphism(SSD)across 103 species ofﬂeas,taking into account phylogenetic between-species relationships.When the data on body size were combined for males and females,the positive correlation between body size and latitude,but not GRS,was revealed.When the analysis was restricted to one sex only,the effect of latitude appeared to be non-signiﬁcant for females,whereas male body size increased with an increase in latitude.Intraspeciﬁc body size variation was not associated with either the latitude or the latitudinal span of the geographic range,independently of which data subset was analyzed.No evidence of association between size-independent tibia length and latitude was found for either females,males,or both sexes combined.The degree of SSD decreased with a decrease in latitude but was not affected by GRS.We conclude that macroecological patterns might be manifested differently in males and females.This should be kept in mind while searching for these patterns.

Synchronous manifestation of colorectal cancer and intraductal papillary mucinous neoplasms

High rates of extrapancreatic malignancies,in particular colorectal cancer(CRC),have been detected in patients with intraductal papillary mucinous neoplasm(IPMN).So far,there is no distinct explanation in the literature for the development of secondary or synchronous malignancies in patients with IPMN.In the past few years,some data related to common genetic alterations in IPMN and other affiliated cancers have been published.This review elucidated the association between IPMN and CRC,shedding light on the most relevant genetic alterations that may explain the possible relationship between these entities.In keeping with our findings,we suggested that once the diagnosis of IPMN is made,special consideration of CRC should be undertaken.Presently,there are no specific guidelines regarding colorectal screening programs for patients with IPMN.We recommend that patients with IPMNs are at high-risk for CRC,and a more rigorous colorectal surveillance program should be implemented.

Rare earth elements(REE)and isotope composition(δ^(13)C andδ^(18)O)of manganese ores of Chiatura deposit(Georgia):features of ore formation and genesis

The rare earth elements(REE)geochemistry and the isotope(δ^(13)C,δ^(18)O)composition of manganese ores of the Chiatura(Georgia)deposit were studied.One of the major features of all types of manganese ores is negative cerium(Ce/Ce*_(PAAS))anomaly and the absence of europium(Eu/Eu*_(PAAS))anomaly.Oxide oolitic manganese ores were formed in oxic shallow marine environments.The content and distribution of REEs(in particular Ce and Eu)in these ores are connected mainly with ferrous oxides.The performed C-and O-isotope research in Mn-carbonates(oolitic and massive)has indicated that carbonate ores were formed by the participation of isotopic ally light CO_(2)which is a result of the oxidation of organic matter in the sediment strata by reducing environments of early diagenesis(and,partially,catagenesis)zone.Obtained negative cerium anomalies in the studied carbonate ores reflect the specific REE patterns in pore waters of sediments of earlier isdiagenesis zone of the Oligocene Chiatura's basin.The deficiency of cerium in this zone remains debatable and requires further study.Formation of manganese carbonates took place multistage by the input of incisional solutions of different chemistry into sea bottom waters and sediments.The absence of europium anomaly indicates about lack of hydrothermal solution input.

Postoperative complications of colectomy and J- pouch with ileostomy versus without ileostomy in children with inflammatory bowel diseases: a systematic review and meta- analysis

Background The efficacy of performing a restorative proctocolectomy and J-pouch ileoanal anastomosis without diverting ileostomy in children with inflammatory bowel disease has been a longstanding debate.A systematic review and meta-analysis is presented comparing the occurrence of postoperative complications in children who underwent either the pouch-anal anastomosis(IPAA)with ileostomy(diverted)versus the undiverted procedure.Methods Records were sourced from CINAHL,CENTRAL,EMBASE and MEDLINE databases.Studies followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and compared postoperative complications in pediatric patients diagnosed with inflammatory diseases aged less than 18 years who underwent J-pouch with ileostomy versus without ileostomy.The primary outcome was the occurrence of postoperative leaks,and the secondary outcomes were presence of postoperative small bowel obstruction(SBO),pouchitis,stricture and fistula complications.A random-effects meta-analysis was used.Results Twenty-three observational studies in the systematic review were included with 658 patients(83%diverted,17%undiverted).Pooled estimates showed no difference in occurrence of leaks in children who underwent J-pouch/IPAA with ileostomy versus without(odds ratio(OR)0.54,95%confidence interval(CI)0.17 to 1.64,I^(2)=16%).There was no difference in the occurrence of SBO,pouchitis or strictures in children who underwent J-pouch/IPAA with ileostomy versus without(SBO:OR 2.27,95%CI 0.52 to 9.92,I^(2)=0%,pouchitis:OR 1.76,95%CI 0.95 to 3.24,I^(2)=0%,strictures:OR 2.72,95%CI 0.44 to 16.69,I^(2)=66%).Conclusion The meta-analysis did not find differences in the occurrence of complications in pediatric patients who underwent the IPAA with ileostomy procedure versus without ileostomy.

Adaptation to a novel host and performance trade-off in host-generalist and host-specific insect ectoparasites

We investigated the performance trade-offs of fleas(Siphonaptera)while adapting to a novel host using two host generalists(Xenopsylla conformis and Xenopsylla ramesis)and one host specialist(Parapulex chephrenis)maintained on their principal hosts(Meriones crassus for Xenopsylla and Acomys cahirinus for P.chephrenis).We asked whether,over generations,(i)a host generalist may become a specialist by evolving the ability to exploit a novel host and losing the ability to exploit an original host and(ii)a host specialist can become a generalist by evolving the ability to exploit a novel host without losing the ability to exploit an original host.We established an experimental line of each species on a novel host(Acomys russatus for Xenopsylla and M.crassus for P.chephrenis)and maintained this line on this host during 23 generations.We compared reproductive performance of progenitors of each line and their descendants when they exploited either original or novel host in terms of egg number and size,hatching success,offspring production,and offspring size.We found changes in performance over generations in female offspring size only.Xenopsylla conformis demonstrated a tendency to become a host specialist(increased performance on the novel host with a concomitant decreased performance on the original host),whereas P.chephrenis demonstrated a tendency to become a host generalist(increased performance on the novel host without a concomitant decreased performance on the original host).We conclude that the probability of generalist to specialist transition,and vice versa,is context-dependent and varies between species.
