This research assessed the experience of stakeholders and the efficacy of integrating traditional medicine into the ghanaian health system using the Ashanti Region as the focal *** of an integrative healthcare delivery model including philosophies/values,structure,process and outcome were used to conduct a quality assessment of the integrated health system in *** element clearly showed that ghana is currently not running a coordinated health practice model,thus making it a tolerant,rather than an inclusive,health ***,the primary purpose of this research is to discuss the development of a new and appropriately customised model that could enhance the practice of integrated healthcare in *** model we present has flexibility and far-reaching applicability in other African countries because such countries share similar socio-cultural and economic *** such,governments and health practitioners could adapt this model to improve the practice of integrated healthcare in their specific *** administrators and health system researchers could also adapt the model to investigate or to monitor the progress and efficacy of integrated healthcare practices within their *** might help to understand the relationships between the integration of traditional medicine and health outcomes for a given population.

Absolute erythrocytosis,due to an increased production of red blood cells,becomes manifest with hemoglobin(Hb)levels above 165 or 160 g/L or with a hematocrit(HCT)above 51%and 48%in males and females,respectively.1 A minority of patients have polycythemia vera(PV);acquired secondary erythrocytosis frequently occurs due to appropriate or inappropriate erythropoietin(EPO)excess,while congenital secondary erythrocytosis may derive from genetic defects causing tissue *** erythrocytosis occurs in patients with genetic mutations of the oxygen-sensing pathway(VHL,PHD2,_HIF-1-alpha)or the erythropoietin receptor(EPOR)genes.2 Recently,HFE gene variants have been described in sporadic erythrocytoses.3 In a high proportion(about 70%)of patients with erythrocytosis,a specific etiology remains elusive despite extensive testing,and the diagnosis of idiopathic erythrocytosis(IE)is ruled out.

Background:One of the pathological hallmarks distinguishing Alzheimer’s disease from other dementias is the accumulation of amyloid beta(Aβ).Higher physical activity is associated with decreased dementia risk,and one potential path could be through Aβlevels *** aimed to explore the relationship between physical activity and Aβin middle-aged and older ***:A systematic search of PubMed,Web of Science,PsycINFO,Cochrane Central Register of Controlled Trials,and SPORTDiscus was performed from inception to April 28,*** were eligible if they included physical activity and Aβdata in adults aged 45 years or ***-level metaanalyses of intervention and observational studies were performed to examine the role of physical activity in modulating Aβ***:In total,37 articles were included(8 randomized controlled trials,3 non-randomized controlled trials,4 prospective longitudinal studies,and 22 cross-sectional studies).The overall effect size of physical activity interventions on changes in blood Aβwas medium(pooled standardized mean difference=-0.69,95%confidence interval(95%CI):-1.41 to 0.03;I^(2)=74.6%).However,these results were not statistically significant,and there were not enough studies to explore the effects of physical activity on cerebrospinal fluid(CSF)and brain Aβ.Data from observational studies were examined based on measurements of Aβin the brain using positron emission tomography scans,CSF,and *** physical activity was positively associated with Aβonly in the CSF(Estimate r=0.12;95%CI:0.05-0.18;I^(2)=38.00%).Conclusion:Physical activity might moderately reduce blood Aβin middle-aged and older ***,results were only near statistical significance and might be interpreted with caution given the methodological limitations observed in some of the included *** observational studies,higher levels of physical activity were positively associated with Aβonly in ***,further research is needed t

Memory deficit,which is often associated with aging and many psychiatric,neurological,and neurodegenerative diseases,has been a challenging issue for *** till now,all potential drug candidates have failed to produce satisfa ctory ***,in the search for a solution,we found that a treatment with the gene corresponding to the RgS14414protein in visual area V2,a brain area connected with brain circuits of the ventral stream and the medial temporal lobe,which is crucial for object recognition memory(ORM),can induce enhancement of *** this study,we demonstrated that the same treatment with RgS14414in visual area V2,which is relatively unaffected in neurodegenerative diseases such as Alzheimer s disease,produced longlasting enhancement of ORM in young animals and prevent ORM deficits in rodent models of aging and Alzheimer’s ***,we found that the prevention of memory deficits was mediated through the upregulation of neuronal arbo rization and spine density,as well as an increase in brain-derived neurotrophic factor(BDNF).A knockdown of BDNF gene in RgS14414-treated aging rats and Alzheimer s disease model mice caused complete loss in the upregulation of neuronal structural plasticity and in the prevention of ORM *** findings suggest that BDNF-mediated neuronal structural plasticity in area V2 is crucial in the prevention of memory deficits in RgS14414-treated rodent models of aging and Alzheimer’s ***,our findings of RgS14414gene-mediated activation of neuronal circuits in visual area V2 have therapeutic relevance in the treatment of memory deficits.

Pathology of pancreatic ductal adenocarcinoma:Facts,challenges and future developments

Despite major improvements concerning its diagnosis and treatment,pancreatic ductal adenocarcinoma(PDAC) remains an aggressive disease with an extremely poor prognosis. Pathology,as interface discipline between basic and clinical medicine,has substantially contributed to the recent developments and has laid the basis for further progress. The definition and classification of precursor lesions of PDAC and their molecular characterization is a fundamental step for the potential identification of biomarkers and the development of imaging methods for early detection. In addition,by integrating findings in humans with the knowledge acquired through the investigation of transgenic mouse models for PDAC,a new model for pancreatic carcinogenesis has been proposed and partially validated in individuals with genetic predisposition for PDAC. The introduction and validation of a standardized system for pathology reporting based on the axial slicing technique has shown that most pancreatic cancer resections are R1 resections and that this is due to inherent anatomical and biological properties of *** standardized assessment of prognostic relevant parameters represents the basis for the successful conduction of multicentric studies and for the interpretation of their ***,recent studies have shown that distinct molecular subtypes of PDAC exist and are associated with different prognosis and therapy *** prospective validation of these results and the integration of molecular analyses in a comprehensive pathology report in the context of individualised cancer therapy represent a major challenge for the future.

Retreived bacteria from Noctiluca miliaris (green) bloom of the northeastern Arabian Sea

In recent years, seasonal blooms of the dinoflagellate Noctiluca miliaris have appeared in the open-waters of the northern Arabian Sea （NAS）. This study provides the first characterization of bacteria from a seasonal bloom of green Noctiluca ofNAS （20°N-17°N and 64°E-70°E）, during the spring-inter-monsoon cruise ofSagar Sampada 253, in March 2007. Bacterial growth as assessed by most-probable number （MPN） and plate counts, revealed ＇variable-physiotypes＇ over a wide range of salinities （0%-25% w/v NaC1）, pH levels （5-8.5）, and organic nutrient strengths, in comparison to non-bloom waters. MPN indices of bacteria in surface waters of bloom stations ＊DWK and ＊PRB, corresponded to （3.08-4.41）×103 cells/mL at 3.5% NaC1 （w/v）, and （2.82-9.49）× 102 cells/mL at 25% （w/v） NaC1 in tryptone-yeast extract broth （TYE）. Plate counts were （1.12-4） × 106 CFU/mL at 0% （w/v） NaC1, （1.28-3.9） × 106 CFU/mL at 3.5% （w/v） NaC1, and （0.4- 7）×104 CFU/mL at 25% NaC1 （w/v） on TYE. One-tenth-strength Zobell＇s gave （0.6-3.74）× 105 CFU/mL at pH 5 to （3.58-7.5）× 105 CFU/mL at pH 8.5. These bacteria were identified to the genera Bacillus, Cellulomonas, Staphylococcus, Planococcus, Dietzia, Diergibacillus, Micrococcus, Sporosarcinae, Leucobacter, and Halomonas. The identity of three strains （gUFBSS253N2, gUFBSS253N30, and gUFBSS253N84） was confirmed through 16S rDNA sequence homology as Bacillus cohnii, Bacillusflexus, and Bacillus cereus. The -2-3-fold higher plate counts of culturable bacteria from the open-waters of the NAS indicate that these bacteria could critically determine the biogeochemical dynamics of the bloom and its milieu. The role of these bacteria in sustaining/terminating the bloom is under evaluation.

The onset of the movement of particles placed on a horizontal rough surface subject to a vertical sinusoidal vibration is investigated through tracking experiments,theoretical analysis,and numerical simulations. The frequency of vibration needed to move particles decays exponentially with the amplitude of the oscillatory *** behavior is explained through a simple mechanism in which a forced damped harmonic oscillator with a spring constant represents all the interactions between the particle and the *** numerical results compare well with experimental data,demonstrating that the forces included in the numerical calculations suitably account for the main particle response,even though the complexity of the surface is not fully taken into *** the way in which frequency varies with amplitude could be relevant to technological applications such as cleaning of material surfaces.

Primary liposarcoma of esophagus:A case report

Liposarcoma is the most common soft tissue sarcoma in adult life while esophageal liposarcoma is an extremely rare tumor. In the world literature, only 14 cases of esophageal liposarcomas have been described. We report a 72-year old male patient who was urgently admitted to our hospital for acute epigastric pain with a burning retrosternal sensation, persistent nausea, vomiting and dysphagia. Barium swallow, upper gastrointestinal （gI） endoscopy, esophageal manometry and CT scan, failed to accurately diagnose the lesion. After surgical resection of an esophageal polypoid tumor, the histological examination revealed a well-differentiated grade Ⅰ liposarcoma. Diagnostic and therapeutic tools were discussed and the results of literature were reviewed.

Update on surgical treatment of pancreatic neuroendocrine neoplasms

Pancreatic neuroendocrine neoplasms(PNENs) are rare and account for only 2%-4% of all pancreatic neoplasms. All PNENs are potential(neurendocrine tumors PNETs) or overt(neuroendocrine carcinomas PNECs) malignant,but a subset of PNETs is low-risk. Even in case of low-risk PNETs surgical resection is frequently required to treat hormone-related symptoms and to obtain an appropriate pathological diagnosis. Low-risk PNETs in the body and the tail are ideal for minimallyinvasive approaches which should be tailored to the individual patient. generally,surgeons must aim for parenchyma sparing in these cases. In high-risk and malignant PNENs,indications for tumor resection are much wider than for pancreatic adenocarcinoma,in many cases due to the relatively benign tumor biology. Thus,patients with locally advanced and metastatic PNETs may benefit from extensive resection. In experienced hands,even multi-organ resections are accomplished with acceptable perioperative morbidity and mortality rates and are associated with excellent long term survival. However,poorly differentiated neoplasms with high proliferation rates are associated with a dismal prognosis and may frequently only be treated with chemotherapy. The evidence on surgical treatment of PNENs stems from reviews of mostly singlecenter series and some analyses of nation-wide tumor registries. No randomized trial has been performed to compare surgical and non-surgical therapies in potentially resectable PNEN. Though such a trial would principally be desirable,ethical considerations and the heterogeneity of PNENs preclude realization of such a study. In the current review,we summarize recent advances in the surgical treatment of PNENs.
