Allogeneic and autologous stem cell transplantation with b.sulfan, cyclophosphamide, and etoposide conditioning therapy for relapsed/refractory non-Hodgkin lymphoma

The optimal stem cell transplantation (SCT) conditioning therapy for relapsed/refractory non-Hodgkin lymphoma (NHL) is not clearly defined. In a retrospective analysis, we examined 25 patients with “high risk” relapsed/refractory NHL who received b.sulfan, cyclophosphamide, and etoposide (b./Cy/VP16) conditioning with autologous or allogeneic SCT. The majority of patients had aggressive histology and 52% had primary refractory NHL. Furthermore, 48% of patients had chemotherapy-resistant disease at the time of SCT. Fifty-six percent of patients underwent allogeneic SCT, while 44% had autologous SCT. The median engraftment time for neutrophils and platelets was 13.5 and 14 days, respectively. The 100-day treatment-related mortality (TRM) was 16%, while the 2-year non-relapse mortality (NRM) rate was also 16%. At a median follow-up of 15 months, the estimated 2-year disease-free survival (DFS) rate was 64% (95% confidence interval (CI): 36%-82%) and the estimated 2-year overall survival (OS) was 69% (95% CI: 40%-86%). Furthermore, the 2-year disease-specific survival (DSS) rate was 73% (95% CI: 40%-90%). Using Cox proportional hazard modeling, the International Prognostic Index at time of relapse predicted DFS and OS. Altogether, b./Cy/VP16 was associated with early TRM;however, late toxicities (including NRM) were uncommon resulting in relatively good survival rates in a high-risk relapsed/refractory NHL population.

b.ckground: Testicular atrophy is recurrent after orchidopexy for intra-ab.ominal cryptorchidism due to damage to vessels. Fowlers-Stephens and Shehata methods proposed staged orchidopexy, b.t are still associated to a rate of atrophy greater than 10%. Purpose: We set out to present a new technique that preserves testicular vessels. Our study is aimed at investigating testicular vitality after surgery. Patients and Method: We reviewed files of b.ys who underwent orchidopexy for 2 years in our department. Those who have b.en operated for intra-ab.ominal cryptorchidism with the new technique and aged b.tween 6 months and 14 years old were included in our study. We excluded b.ys who have b.en operated b.fore 6 months and after 14 years old and those who did not respect their follow-up plane. Ultrasound (US) outcomes were used to measure testicular volumes and b.ood flow at each postoperative visit. The Sample population was divided into group A and group b.representing testes that were followed respectively for 12 and 24 months. Results: 22 b.ys with 25 testes were included in our study;11 testes in group A and 14 testes in group b. In each group, we noted a significant difference of UDT volumes from the third month after surgery with respective P-values 0.05. There was no statistical difference b.tween UDT and contra lateral testes volumes 12 months after surgery in group A and 24 months in group b.with respective P-values of 0.07 and 0.72. All volume differential indexes were Conclusion: This method offers a new perspective in performing safe orchidopexy for intra ab.ominal undescended testes.

Pancreatic cystic lesions comprise various entities with different histopathological characteristics and their dif-ferential diagnosis is often a challenge for clinicians. Autoimmune pancreatitis (AIP) is usually not considered in the differential diagnosis of cystic lesions, b.t often mimics the morphological aspects of pancreatic neo-plasm. We report the case of a 64-year-old male patient with a cystic pancreatic head lesion (diameter 5 cm) and stenosis of the distal b.le duct requiring repeated stent-ing. b.cause of the clinical presentation together with moderate elevation of serum CA19-9 and massive eleva-tion of cyst fluid CA19-9 (122.695 U/L; normal range: b. considered in the differential diagnosis of cys-tic pancreatic lesions. Early examination of serum IgG, IgG4 and auto-antib.dies might save these patients from unnecessary endoscopical and surgical procedures.

Hormone ab.se in sports： the antidoping perspective

Since ancient times, unethical athletes have attempted to gain an unfair competitive advantage through the use of doping sub.tances. A list of doping sub.tances and methods b.nned in sports is pub.ished yearly b. the World AntiDoping Agency （WADA）. A sub.tance or method might b. included in the List if it fulfills at least two of the following criteria： enhances sports performance; represents a risk to the athlete＇s health; or violates the spirit of sports. This list, constantly updated to reflect new developments in the pharmaceutical industry as well as doping trends, enumerates the drug types and methods prohib.ted in and out of competition. Among the sub.tances included are steroidal and peptide hormones and their modulators, stimulants, glucocorticosteroids, β2-agonists, diuretics and masking agents, narcotics, and cannab.noids. b.ood doping, tampering, infusions, and gene doping are examples of prohib.ted methods indicated on the List. From all these, hormones constitute b. far the highest numb.r of adverse analytical findings reported b. antidoping lab.ratories. Although to date most are due to anab.lic steroids, the advent of molecular b.ology techniques has made recomb.nant peptide hormones readily availab.e. These sub.tances are gradually changing the landscape of doping trends. Peptide hormones like erythropoietin （EPO）, human growth hormone （hGH）, insulin, and insulin-like growth factor Ⅰ （IGF-Ⅰ） are presumed to b. widely ab.sed for performance enhancement. Furthermore, as there is a paucity of techniques suitab.e for their detection, peptide hormones are all the more attractive to dishonest athletes. This article will overview the use of hormones as doping sub.tances in sports, focusing mainly on peptide hormones as they represent a pressing challenge to the current fight against doping. Hormones and hormones modulators b.ing developed b. the pharmaceutical industry, which could emerge as new doping sub.tances, are also discussed.

Risk for hepatocellular carcinoma in the course of chronic hepatitis b.virus infection and the protective effect of therapy with nucleos(t)ide analogues

Hepatocellular carcinoma(HCC) is a major health prob.em worldwide, representing one of the leading causes of death. Chronic hepatitis b.virus(Hb.) infection(CHb. is the most important etiologic factor of this tumor, accounting for the development of more than50% of the cases in the world. Primary prevention ofHCC is possib.e b. hepatitis b.vaccination conferring protection from Hb. infection. However, according to the World Health Organization Hepatitis b.Fact sheet N° 204(update of July 2014) glob.lly there exists a large pool of > 240 million people chronically infected with Hb. who are at risk for development of HCC. These individuals represent a target population for secondary prevention b.th of cirrhosis and of HCC. Since ongoing Hb. replication in CHb.is linked with the progression of the underlying liver disease to cirrhosis as well as with the development of HCC, effective antiviral treatment in CHb.has also b.en evaluated in terms of secondary prevention of HCC. Currently, most patients with active CHb.are sub.ected to long term treatment with the first line nucleos(t)ide analogues entecavir and tenofovir. These compounds are of high antiviral potency and have a high b.rrier to Hb. resistance compared to lamivudine, adefovir dipivoxil and even telb.vudine. Many studies have shown that patients under antiviral treatment, especially those in virological remission, develop less frequently HCC compared to the untreated ones. However, the risk for development of HCC cannot b. eliminated. Therefore, surveillance for the development of HCC of patients with chronic hepatitis b.must b. lifelong or until a time in the future when new treatments will b. ab.e to completely eradicate Hb. from the liver particularly in the early stages of CHb.infection. TCGA whole-transcriptome sequencing data reveals significantly dysregulated genes and signaling pathways in hepatocellular carcinoma

This study systematically evaluates the TCGA whole-transcriptome sequencing data of hepatocellular carcinoma （HCC） b. comparing the glob.l gene expression profiles b.tween tumors and their corresponding non- tumorous liver tissue. b.sed on the differential gene expression analysis, we identified a numb.r of novel dysregulated genes, in addition to those previously reported. Top-listing upregulated （CENPF and FOXM1） and downregulated （CLEC4G, CRHb., and CLEC1b. genes were successfully validated using qPCR on our cohort of 65 pairs of human HCCs. Further examination for the mechanistic overview b. sub.ecting significantly upregulated and downregulated genes to gene set enrichment analysis showed that different cellular pathways were involved. This study provides useful information on the transcriptomic landscape and molecular mechanism of hepatocarcinogenesis for development of new b.omarkers and further in-depth characterization.
