Prevention and management of hepatitis B virus reactivation in patients with hematological malignancies treated with anticancer therapy

Hepatitis due to hepatitis B virus(HBV) reactivation can be severe and potentially fatal, but is preventable. HBV reactivation is most commonly reported in patients receiving cancer chemotherapy, especially rituximabcontaining therapy for hematological malignancies and those receiving stem cell transplantation. All patients with hematological malignancies receiving anticancer therapy should be screened for active or resolved HBV infection by blood tests for hepatitis B surface antigen(HBs Ag) and antibody to hepatitis B core antigen(antiHBc). Patients found to be positive for HBs Ag should be given prophylactic antiviral therapy to prevent HBV reactivation. For patients with resolved HBV infection, no standard strategy has yet been established to prevent HBV reactivation. There are usually two options. One is pre-emptive therapy guided by serial HBV DNA monitoring, whereby antiviral therapy is given as soon as HBV DNA becomes detectable. However, there is little evidence regarding the optimal interval and period of monitoring. An alternative approach is prophylactic antiviral therapy, especially for patients receiving highrisk therapy such as rituximab, newer generation of anti-CD20 monoclonal antibody, obinutuzumab or hematopoietic stem cell transplantation. This strategy may effectively prevent HBV reactivation and avoid the inconvenience of repeated HBV DNA monitoring. Entecavir or tenofovir are preferred over lamivudine as prophylactic therapy. Although there is no well-defined guideline on the optimal duration of prophylactic therapy, there is growing evidence to recommend continuing prophylactic antiviral therapy for at least 12 mo after cessation of chemotherapy, and even longer for those who receive rituximab or who had high serum HBV DNA levels before the start of immunosuppressive therapy. In this paper, we discuss how a manufacturer interacts with his multiple competing retailers to optimize their individual net profits through coordinating pricing and inventory decisions in a two-echelon supply chain. The manufacturer purchases raw materials outside to produce substitutable products to different retailers. The manufacturer determines his optimal wholesale prices for different retailers, setup time interval for the products and replenishment cycle for raw materials to maximize his profit. For each retailer, he considers optimal retail prices and replenishment policies to maximize his profit. The problem is modeled as a dynamic non-cooperative game in which all the competing retailers formulate a subgame and as a sector play the whole game with the manufacturer. Analytical method and solution algorithm are developed to determine the Nash equilibrium of this game. A numerical study is conducted to examine the influence of market and production related parameters on decisions and profits of the supply chain and its constituent members.

＜正＞ Scheduling is one of the most difficult issues in the planning and operations of the aircraft services industry. In this paper, the various scheduling problems in ground support operation of an aircraft maintenance service company are addressed. The authors developed a set of vehicle routings to cover each schedule flights;the objectives pursued are the maximization of vehicle and manpower utilization and minimization of operation time. To obtain the goals, an integer-programming model with genetic algorithm is formulated. It is found that the company can produce an effective and efficient schedules to deploy the manpower and equipment resources. Simulation is used to verify the method and a MATLAB program is used to code the genetic algorithm. This model is further illustrated by a case study in Hong Kong and the benefit elaborated. Finally, a conclusion is made to summarize the experience of this project and provide further improvement.

Background Proper parent-child interaction is crucial for child development, but an assessment tool in Chinese is currently lacking. This study aimed to develop and validate a parent-reported parent-child interaction scale for Chinese preschool children. Methods The Chinese parent-child interaction scale (CPCIS) was designed by an expert panel based on the literature and clinical observations in the Chinese context. The initial CPCIS had 14 parent-child interactive activity items. Psychometric properties of the CPCIS were examined using the Rasch model and confirmatory factor analysis (CFA). Convergent validity was investigated by the associations between CPCIS and family income, maternal education level, and children's school readiness. Results The study recruited 567 Chinese parent-child pairs from diverse socioeconomic backgrounds, who completed the CPCIS. Six out of the 14 items in the initial CPCIS were dropped due to suboptimal fi t values. The refined 8-item CPCIS was shown to be valid and reliable by Rasch models and CFA. The person separation reliability and Cronbach's α of the CPCIS were 0.81 and 0.82, respectively. The CPCIS scores were positively associated with family's socioeconomic status (η2 = 0.05,P < 0.001), maternal education level (η2 = 0.08,P < 0.001), and children's school readiness (η2 = 0.01,P < 0.01). Conclusion CPCIS is an easily administered, valid, and reliable tool for the assessment of parent-child interactions in Chinese families.

This study shows a novel three-dimensional （3D） parametric body model development using cross-section control and control algorithm retrieved from anthropometric survey. The 3D parametric body model was shaped into the most common body shape of the young Hong Kong female and be able to automatically change its critical body shape according to the user＇s critical body dimension inputs. The control algorithm controls the profile of the parametric model is retrieved from an anthropometric survey using 3D scanner to study the profile change of body and the relation between some critical body dimensions. Compared to the traditional anthropometric surveys, the 3D body scanner provides more accurate body dimension and information as well as new body shape measurements.
