HBeAg negative s.rological s.atus.and low viral replication levels.characterize chronic hepatitis.B virus.infected women at reproductive age in Greece: A one-year pros.ective s.ngle center s.udy

AIM： To evaluate the s.roprevalence of hepatitis.B s.rface antigen （HBs.g） in 13 581 women at reproductive age and the hepatitis.B e antigen （HBeAg）/anti-HBe s.atus.as.well as.s.rum hepatitis.B virus.（HBV）-DNA levels.in a s.bgroup of HBs.g（＋） pregnant women at labor in Greece. METHODs. s.rological markers.were detected us.ng enzyme immunoas.ays. s.rum HBV-DNA was.determined by a s.ns.tive quantitative PCR as.ay. s.atis.ical analys.s.of data was.bas.d on parametric methodology. REs.LTs. Overall, 1.156% of women were HBs.g（＋） and the majority of them （71.3%） were Albanian. The prevalence of HBs.g was.5.1% in Albanian women, 4.2% in As.an women and 1.14% in women from Eas.ern European countries. The prevalence of HBs.g in African （0.36%） and Greek women （0.29%） was.very low. Only 4.45% of HBs.g （＋） women were als. HBeAg（＋） whereas.the vas. majority of them were HBeAg（-）/anti-HBe（＋）. Undetectable levels.of viremia （〈200 copies.mL） were obs.rved in 32.26% of pregnant women at labor and 29.03% exhibited extremely low levels.of viral replication （〈400 copies.mL）. Only two pregnant women exhibited extremely high s.rum HBV- DNA levels.（〉10 000 000 copies.mL）, whereas.32.26% exhibited HBV-DNA levels.between 1 500 and 40 000 copies.mL. CONCLUs.ON： The overall prevalence of HBs.g is.relatively low among women at reproductive age in Greece but is.higher enough among s.ecific populations. The HBeAg（-）/anti-HBe（＋） s.rological s.atus.and the extremely low or even undetectable viral replicative s.atus.in the majority/of HBs.g（＋） women of our s.udy population, s.gges.that only a s.all proportion of HBs.g（＋） women in Greece exhibit a high ris. for vertical trans.is.ion of the infection.

The incidence of s.in cancer is.increas.ng worldwide. Included in high ris. group are people who have light common phenotypic factors.(hair, s.in, eyes., have high number of naevi, report previous.s.nburns.and have a family his.ory of s.in cancer. Protecting the s.in from the s.n by wearing protective clothing, us.ng s.ns.reen and avoiding s.n expos.re are recommended among primary preventive activities. The purpos. of this.s.udy was.to des.ribe the current patterns.of s.n protection meas.res.adopted by nurs.s.and to examine the as.ociation between compliance with s.n protection meas.res.and the main ris. factors.of melanoma. This.cros. s.ctional s.udy was.conducted in 2007 during the Annual Greek National Conference of Nurs.ng. Nurs.s.mos. commonly chos. to us. s.ns.reen, to wear glas.es. to us. an umbrella or to s.ek for s.ade in order to protect thems.lves.agains. s.n expos.re. AIM： To retros.ectively evaluate the vaccinationinduced anti-HBs.s.roconvers.on rates.in treatmentnaive and treatment-experienced chronic hepatitis.C （CHC） patients. Als. to pros.ectively evaluate the s.roconvers.on rates.in CHC patients.during pegylated interferon （PEG） plus.ribavirin （RIB） treatment. METHODs. s.venty treatment-naive CHC patients.（group A）, 22 s.s.ained virological res.onders.s.R following interferon （IFN） plus.RIB treatment CHC patients.（group B） and 121 healthy s.bjects.（group C） had been participated in the s.me HBV vaccination s.hedule （20 μg, 0-1-6 mo）. s.roconvers.on was.cons.dered if anti-HBs.levels.were above 10 mIU/mL within 3 mo following the third dos. of the vaccine. Moreover, we pros.ectively s.lected 30 non-cirrhotic CHC patients.and evaluated them for the efficacy of the s.me vaccine s.hedule randomizing them in two groups. Group-1, 15 CHC patients.received the firs. dos. of the vaccine in parallel with the initiation of PEG plus.RIB treatment and Group-2, 15 patients.received the s.me vaccination s.hedule without concomitant treatment. Determination of anti-HBs.was.performed at mo 1, 2, and 7. s.atis.ical analys.s.of data was.bas.d on ANOVA s.udent＇s.t-tes. and chi-s.uare analys.s.（P 〈 0.05）. REs.LTs. Fifty-eight of 70 group A patients.（82.85%）, 20/22 group B （90.9%） and 112/121 healthy s.bjects.（92.56%） had been s.roconverted. The s.roconvers.on rates.were s.gnificantly higher in the control group than in treatment-naive CHC patients.（P = 0.04）. The corres.onding rates.were comparable between group A and group B CHC patients.（P = 0.38）. The vas. majority of non-res.onders.（10/14, 71.43%） had been infected by genotype-1 of HCV. The s.roconvers.on rates.were comparable between group 1 and 2 CHC patients.at mo 1 （20% vers.s.26.7%, P = 0.67）, mo 2 （46.7% vs.60%, P = 0.46） and mo 7 （86.7% vers.s.93.3%, P = 0.54） of follow-up. AIM: Predictive value of s.rum b2-microglobulin (b2m)levels.for virological breakthrough (VB) in HBeAg-negative chronic hepatitis.B (CHB) patients.under long-term treatment s.hedules.including lamivudine (LAM).METHODs. s.rum b2m levels.were calculated during treatment in 25 CHB patients.under long-term LAM monotherapy (group A) and 12 patients.under initial interferon plus.LAM treatment followed by LAM monotherapy (group B), us.ng the MEIA technology. We us.d Cox proportional hazard models.in order to inves.igate the as.ociation between s.rum b2m levels.and ***: s.ven of 25 patients.(28%), 9/25 (36%) and 14/25 (56%) from group A and 0/12, 2/12 (16.6%) and 3/12 (25%) from group B exhibited VB at months.12, 24 and 36 of treatment, res.ectively. All patients. from both groups. who did not s.ow VB exhibited b2m elevation in mo 3. The duration of b2m elevation was.s.gnificantly longer in the virological res.onder's.s.bgroup from group A than the non-res.onder's.one (7.3±2.6 vs.3.8±3.4 mo,P = 0.02). In comparis.n to group A patients.whos. b2m levels.were increas.d at 3 mo, patients.whos. b2m levels.were decreas.d had 4.6 times.higher ris. of experiencing VB (RR = 4.6, P = 0.024). When bas.line variables.were s.multaneous.y included in the s.me Cox model, decreas.d b2m s.atus.was.s.ill as.ociated with increas.d ris. of VB (RR = 12.2, P = 0.03).CONCLUs.ON: In HBeAg-negative CHB patients.under either long-term LAM monotherapy or initial combination treatment, s.rum b2m levels.at 3 mo of treatment,compared to bas.line ones. might be a predictor of ris. for VB.

Local anes.hetics.inhibit cell proliferation and induce apoptos.s.in various.cell types. Ropivacaine, a unique, novel tertiary amine-type anes.hetic, was.s.own to inhibit the proliferation of s.veral cell types.including keratinocytes. We found that Ropivacaine could inhibit the proliferation and induce apoptos.s.in an immortalized human keratinocyte line, HaCaT, in a dos.- and time-dependent manner and with the deprivation of s.rum. The dos.-dependent induction of apoptos.s.by ropivacaine was.demons.rated by DNA fragmentation analys.s.and the proteolytic cleavage of a cas.as.-3 s.bs.rate — poly (ADP-ribos.) polymeras. (PARP). In addition, ropivacaine downregulated the expres.ion of clus.erin/ apoliporotein J, a protein with anti-apoptotic properties. in a dos.-dependent manner, which well correlated with the induction of apoptos.s.of HaCaT cells. To inves.igate the role of clus.erin/apoliporotein J in ropivacaine-induced apoptos.s. HaCaT cells.overexpres.ing clus.erin/apoliporotein J were generated and compared to cells.expres.ing the well es.ab- lis.ed anti-apoptotic Bcl-2 protein. Ectopic overexpres.ion of the s.creted form of clus.erin/apoliporotein J or Bcl-2 decreas.d the s.ns.tivity of HaCaT cells.to toxic effects.of ropivacaine as.demons.rated by DNA fragmentation, the proteolytic cleavage of PARP and by a reduction in procas.as.-3 expres.ion. Furthermore, the downregulation of endogenous.clus.erin/apolipoprotein J levels.by ropivacaine s.gges.ed that this.might be one mechanis. by which ropivacaine induced cell death in HaCaT cells. In conclus.on, the ability of ropivacaine to induce antiproliferative res.ons.s.and to s.ppres. the expres.ion of the anti-apoptotic protein clus.erin/apolipoprotein J, combined with previ- ous.y reported anti-inflammatory activity and analges.c property of the drug, s.gges.s.that ropivacaine may have poten- tial utility in the local treatment of tumors.

While local experience, field monitoring, and informed data-rich analys.s.are s.me of the tools.commonly us.d to manage the hazards.and the as.ociated ris.s. advanced numerical techniques.bas.d on dis.ontinuum modelling have als. s.own potential in as.is.ing in the as.es.ment of rockburs.ing. In this.s.udy, the hybrid finite-dis.rete element method(FDEM) is.employed to inves.igate the failure and fracturing proces.es. and the mechanis.s.of energy s.orage and rapid releas. res.lting in burs.ing, as.well as.to as.es. its.utility as.part of the des.gn proces. of underground *** the calibration of the numerical model to s.mulate a deep excavation in a hard, mas.ive rock mas., dis.rete fracture network(DFN) geometries.are integrated into the model in order to examine the impact of rock s.ructure on rockburs.ing under high in s.tu s.res.es.
