Indoor Exposure to Mould and Dampness in Infancy and Its Association to Persistent Atopic Dermatitis in school Age: Results from the Greek IsAAC II study

Introduction: The presence of mould as a source of perennial allergens and bacteria products has been related to the appearance of respiratory symptoms in several studies. Yet, its role in eczema has not been elucidated. The aim of this study was to investigate the association between exposure to indoor visible molds/dampness and the manifestation of eczema in children. Methods: The study is part of the Greek contribution to IsAAC IΙ that includes 2023 students of randomly selected public primary schools in Athens and Thessaloniki, aged 9 - 10 years old. The children represented a general population sample and were evaluated according to IsAAC II questionnaire, validated for Greek language. Additionally, skin prick tests to aero-allergens were performed and children were examined for active skin lesions. Results: 13% had suffered from eczema in the past, 9% had current and 2% had atopic eczema (positive at least one skin prick test). Out of the children examined, half reported that eczema first appeared after the age of five years old whereas 70% mentioned persistence of eczema. Dampness was reported in 10.8% and visible mould in 6.4% of all cases during infancy, while continued exposure until the age of 10 years old was reported in 38% and 33% out of them respectively. 10.8% of the sensitized children were positive to house dust mites and Alternaria, however, sensitization was not related to indoor exposure. In logistic regression analysis evaluating 20 environmental risk factors, a significant association was noted between the presence of indoor visible mold and dampness in infancy, and the presence of current eczema OR 1, 89 (95%CI 1.18 - 3.03). This association remained significant irrespective of the family history of eczema and sensitization. Conclusions: Frequently eczema first appears at early school age. simulating Medical Imaging X-Ray Tubes with Various Parameters Using BEAMnrc Monte Carlo software

Context: Medical imaging has a wide range of applications in today’s society. Basic projectional radiography, CT scans, mammograms and a range of other advanced technologies all use x-rays to create a large number of examinations every day across the world. The most essential component of such medical equipment is the x-ray tube, which creates and produces x-rays. Objective: We describe and investigate an abstract model-geometry of a simple x-ray tube utilizing the open-source software package of BEAMnrc of the EGsnrcmp family, which is well validated by several studies over the years, for high and low energy photons generation. Methodology: Our research focuses on two different electron beam energies: 120 keV and 30 keV. The 120 keV is the typical energy for simple projectional radiographic exams and CT examinations, whereas the 30 keV is the typical energy of mammography. Results: Two different anode materials are used for each case, Gold (Au) and Tungsten (W) for 120 keV because these are the most common in projectional radiography and CT;Molybdenum (Mo) and Rhodium (Rh) for 30 keV because with these targets most mammography exams are carried out. The aim of this work is to show how the BEAMnrc software package can simulate effectively x-ray generation of low-energy photons which are utilized in modern medical imaging procedures. We describe useful information on anode-target characteristics, such as anode angle, anode material, and metal filter materials, based on previous quality studies even by using software other than BEAMnrc. Conclusion: We demonstrate that BEAMnrc can be efficiently used for Monte Carlo modeling of low-energy photons.

Modeling and Implementation of a Highly Efficient solar-Powered storage Installation through self-Reconfigurable Batteries

self-reconfigurable batteries represent a new and promising technique of electrochemical storage. The application of self-reconfigurable batteries can resolve the challenge of efficient renewable storage in solar-powered installations. In this paper, the problem of solar panel’s Maximum Power Point (MPP) tracking utilizing self-reconfigurable batteries is explored through modeling. The efficiency of energy storage is improved by removing the intervening DC/DC converter, which is usually necessary for solar PV applications. To make such a system functional, a switching Battery Management system (sBMs) is proposed instead of a traditional couple of DC/DC converter and usual BMs. This system allows the series connection of multiple battery modules of different sizes, states-of-Charge (soC), and states-of-Health (soH). Two main challenges arise by the proposed implementation: tracking MPP of solar panels through battery cell switching and maintaining an equal (balanced) soC of the separate cells/modules. The theoretical investigation includes developing the distinct software parts: digital twins of the battery module and solar PV modules that interact with the sBMs and the algorithm according to which the proposed sBMs will operate. The sBMs algorithm, based on sorting the battery cells according to their soC, resolves both challenges. Having this promising theoretical starting point, a working prototype was developed. The prototype worked as expected and was tested under field conditions, being integrated into the power grid as part of a virtual power plant.

safety of anti-tumor necrosis factor therapy during pregnancy in patients with inflammatory bowel disease

Treatment of inflammatory bowel disease has significantly improved since the introduction of biological agents, such as infliximab, adalimumab, certolizumab pegol, and golimumab. The Food and Drug Administration has classified these factors in category B, which means that they do not demonstrate a fetal risk. However, during pregnancy fetuses are exposed to high anti-tumor necrosis factor(TNF) levels that are measurable in their plasma after birth. since antibodies can transfer through the placenta at the end of the second and during the third trimesters, it is important to know the safety profile of these drugs, particularly for the fetus, and whether maintaining relapse of the disease compensates for the potential risks of fetal exposure. The limited data available for the anti-TNF drugs to date have not demonstrated any significant adverse outcomes in the pregnant women who continued their therapy from conception to the first trimester of gestation. However, data suggest that antiTNFs should be discontinued during the third trimester, as they may affect the immunological system of the newborn baby. Each decision should be individualized, based on the distinct characteristics of the patient and her disease. Considering all the above, there is a need for more clinical studies regarding the effect of antiTNF therapeutic agents on pregnancy outcomes.

Background:Persistent patent ductus arteriosus(pPDA)is a common condition in preterm *** metaanalysis aimed to assess the safety and efficacy of transcatheter closure(TC)when compared to surgical ligation(sL)in preterm infants with ***:A literature search of Ovid Cochrane Library,Medline,Embase,Epub,scopus,PMC Preprints,and *** was conducted from inception to May 06,*** studies reported infants diagnosed with pPDA born at≤2000 g birth weight or at≤37 weeks’who underwent TC or sL as *** review was registered in PROsPERO(CRD42022325944).Results:From 97 studies screened,8 studies met the eligibility criteria,with a total of 756 preterm infants undergoing either TC(n=366)or sL(n=390).Compared to TC,sL had higher mortality rates(OR=0.32,95%CI:0.16,0.66,I^(2)=0%).No difference was seen in post-procedural complication rate(OR=0.90,95%CI:0.18,4.44,I^(2)=79%),mean duration of post-procedural mechanical ventilation(MD=−2.21 days,95%CI:−4.88,0.47,I^(2)=60%),hospital stay length(MD=−8.30 days,95%CI:−17.03,0.44,I^(2)=0%)or neonatal intensive care unit stay length(MD=−3.50 days,95%CI:−10.27,3.27,I^(2)=0%).Conclusion:Our meta-analysis demonstrated TC as a viable alternative option in managing preterm infants with pPDA in the context of *** the promising trends demonstrated in this meta-analysis,further studies with larger sample size and controlled baseline characteristics are needed to evaluate the safety and efficacy of TC and sL for preterm infants with pPDA.

Combined Use of Percutaneous Canulated screws and External or Internal Fixation for Less Invasive Treatment of Tibial Plateaux Fractures

The classic way of treatment of tibial plateau fractures with an extensive approach, opening of the knee and compressive internal fixation can lead to major complications such as infection, skin necrosis and knee stiffness. Here we present a less invasive and thus safer surgical technique, and its indications and results. Twenty patients with various types of fractures according to schatzker’s classification (mainly V and VI) were treated during a time period of seven years. surgical treatment usually consisted of a combination of percutaneous canulated screws with a hybrid external fixator. In three cases canulated screws were combined with a laterally applied anatomic locking plate. Patients were followed up for six months to three years postoperatively. Indications as well as intra and postoperative parameters such as surgical time, stability of fixation, blood loss, wound healing, infection, fracture healing and final result were studied. No major complications were recorded either early or later. The use of external οr less invasive internal fixation in combination with percutaneous canulated screws appears to be an adequate method for the treatment of most types of these fractures.

Verification of Hazard Analysis and Critical Control Point in Hotels and Catering Units: Evaluation of the Cleaning and Disinfection Procedures and Microbiological Monitoring of Hot and Cold Meals

The aim of the present study is to assess the efficacy of the methods used in a HACCP system by measuring the Total Viable Count (TVC) and Coliforms, after cleaning and disinfecting the surfaces used and also by monitoring TVC and enterobacteriaceae of hot and cold meals. In order to evaluate the appropriateness of the surfaces used, samples were obtained from kitchens of 37 Greek hotels/resorts, all of which operated a documented HACCP safety management scheme, while the samples of hot and cold meals were selected from the menu of a catering unit and collected from the actual point of hot and cold dishes banquette at the start time (time 0) and after 4 hours. The developed colonies were counted. The results showed that only 13.5% of the hotels presented surfaces without any TVC or coliforms and 86.5% of the hotels had surfaces with TVC or coliforms or both above the limits. However, 75.8% of the total surfaces examined were acceptable for TVC and 81.9% of the surfaces examined were coliforms. Our microbiological monitoring of food showed that the TVC content of hot meal samples after 4 hours of display (above 60℃) was not significantly different from the TVC content of the aforementioned samples at time 0, while in cold meals TVC and enterobacteriaceae content had significantly risen after 4 hours of display (below 5℃).Conclusively, insufficient application of disinfection and ineffective surface and equipment cleaning programs were found. Additionally, cold meals are more vulnerable than hot meals in developing TVC and enterobacteriaceae.

Testing the value of simple arterial blood pressure measurements in a Greek children’s hospital

Arterial hypertension begins in childhood and may continue, without intervention, throughout adulthood with severe health *** base of timely diagnosis is the systematic blood pressure (BP) measurement. Background: The study aims at revealing the level of parental and community awareness and testing the value of a simple measurement in uncovering children who need further assessment when performing an in-hospital BP screening. Methods: BP was measured in 600 children (0 - 17 yr, mean age 5.1 ± 4.3 yr) at a Children’s hospital. Parents were asked to fill in a specially structured questionnaire. Results: In 47.2% of the subjects (40.1% of them were Greeks and 60.6% foreigners), BP had never been checked before (p arterial BP had never been checked before. surprisingly, a health certificate was issued for school purposes for 81.2% of the schoolchildren of this study, and 75.3% of the enrolled children were allowed to participate in sports clubs without first checking BP. Moreover, parents demonstrated a low level of alertness with respect to paediatric hypertension. Conclusions: simple arterial BP measurements can ensure early detection and thus early referral to specialists.

Case Report of a 12-Year-Old Girl with Imperforate Hymen—Review of the Literature on Incision Techniques and Further Treatment of Patients

Background: Imperforate hymen (IH) is formed if no canal is created between the vaginal plate and the urogenital sinus. Treatment of IH is surgical. Different types of incisions and postoperative treatment are proposed. Case: A 12-year-old girl presented with a 3-day history of pain in the lower abdomen. Examination of the external genitalia disclosed a bulging hymen. Ultrasound examination of her lower abdomen revealed hematocolpos. A diagnosis of hematocolpos caused by IH was established and surgical treatment was decided. Conclusion: Treatment of choice for IH is undeniably surgical. Although different types of procedures have been proposed, the surgeon should choose the one that best suits the patient’s needs without harming anatomical structures closely related to the hymen.
