Efficacy of Mometasone Furoate Nasal Spray in Delayed Antibiotic Prescriptions for Acute Rhinosinusitis: A Multicenter, Randomized, Open-Label, Comparative, Prospective, Parallel Group Study

While their clinical presentation is similar, post-viral rhinosinusitis does not indicate the possible development of bacterial rhinosinusitis which occurs in only 0.5% to 5% of all cases of ARS. However, unreasonable antibacterial therapy is prescribed in 54% to 77% of cases. The perspective is to use delayed antibacterial therapy management. The purpose of the study was to investigate the efficacy of mometasone furoate nasal spray within the delayed prescription approach. Methods: A multicenter, randomized, open-label, comparative, prospective, parallel group study enrolled a total of 156 patients with ARS aged 18 to 60 years, of them 155 patients completed the study. The patients were randomized into the main group of Mometasone Furoate (Flix) Nasal Spray or into the control group of amoxicillin clavulanate tablets while both groups were administered a standard ARS therapy. Findings: The use of MF nasal spray within the delayed antibiotics prescription approach has a significantly higher efficacy compared to the immediate antibiotics prescription in the first days of treatment (p ibacterial therapy was prescribed to 42.3% of patients in the main group compared to 100.0% of patients in the control group (p it was shown that there were statistically significant differences in the dynamics of changes in the total symptom score between the patients administered antibiotics and the antibiotic-naive patients. Conclusion: The use of MF within the delayed antibiotics prescription approach reliably improves the pattern of symptoms over time at the disease onset and significantly reduces the need for NSAiDs administration and antibiotics prescription frequency.

Effect of CeO_2 and Al_2O_3 on the activity of Pd/Co_3O_4/cordierite catalyst in the three-way catalysis reactions(CO/NO/C_nH_m)

The present article studies the effect of CeO2 and A1203 on the activity of Pd/Co304/cordierite catalyst in conversion of NO, CO, CnHm. The catalysts were characterized by temperature programmed reduction with hydrogen, X-ray diffraction, X-ray photoelectron spectroscopy and transmission electron microscopy. it is shown that the effect of CeO2 on the properties of Pd/C03 O4/cordierite catalyst depends on preparation method. The catalyst obtained by co-deposition of cerium and cobalt oxides has higher activity in CO oxidation （CO ＋ 02 and CO ＋ NO） and total hexane oxidation （C6H14 ＋ 02）. Such phenomenon is probably caused by more than stoichiometric amount of formed oxygen vacancies, an increase in both mobility of surface oxygen and dispersity of components in the catalytic composition. it is demonstrated that CeO2 addition promotes the SO2 resistance of Pd/C03 O4/cordierite. The second support decreases the activity of Pd/Co3Oa/cordierite catalyst in the reactions of CO and C6H14 with oxygen because of COA1204 formation.

Symmetry Analysis of Barotropic Potential Vorticity Equation

Recently F. Huang [Commun. Theor. Phys. 42 （2004） 903] and X. Tang and P.K. Shukla [Commun. Theor. Phys. 49 （2008） 229] investigated symmetry properties of the barotropic potential vorticity equation without forcing and dissipation on the beta-plane. This equation is governed by two dimensionless parameters, F and β, representing the ratio of the characteristic length scale to the Rossby radius of deformation and the variation of earth＇ angular rotation, respectively. in the present paper it is shown that in the case F ≠ 0 there exists a well-defined point transformation to set β = 0. The classification of one- and two-dimensional Lie subalgebras of the Lie symmetry algebra of the potential vorticity equation is given for the parameter combination F ≠ 0 and β = 0. Based upon this classification, distinct classes of group-invariant solutions are obtained and extended to the case β ≠0.

Diagnostic Accuracy and Predictive Value of Clinical Symptoms for the Diagnosis of Mild COViD-19

Objective: To assess the diagnostic accuracy and predictive values of clinical symptoms in patients with suspected mild COViD-19 to identify target groups for self-isolation and outpatient treatment without additional testing. Methods: We conducted an open-label prospective study in patients aged 18 to 72 years with suspected mild COViD-19. The clinical diagnosis was based on the acute onset of such symptoms as olfactory dysfunction, hyperthermia, myalgia, nasal congestion, nasal discharge, cough, rhinolalia, sore throat, without pneumonia in persons in contact with a confirmed case of COViD-19. The physician assessed clinical symptoms using a 4-point scale. The patient self-assessed clinical symptoms using a ten-point visual analogue scale (VAS). All enrolled patients underwent laboratory testing to confirm the diagnosis of COViD-19. Results: Of the 120 patients underwent testing, the diagnosis of mild COViD-19 was confirmed in 96 patients and ruled out in 24 patients. When assessing symptoms by a physician according to the correlation analysis, hyperthermia, myalgia, nasal congestion and rhinolalia have a positive predictive value with a significance level of more than 0.6. When self-assessing symptoms by a patient, fever, myalgia and nasal congestion have a diagnostic accuracy with a significance level of more than 0.5. Nasal discharge, cough and sore throat have negative predictive values. Conclusion: The presence of these symptoms in patients with an acute onset of the disease can help to make a clinical diagnosis of coronavirus disease and identify target groups for self-isolation and outpatient treatment without additional testing. Highly suspect asymptomatic patients are not considered as those who have possible mild COViD-19 infection.
