Driving Skills in HIV-Infected Patients Well Controlled with Antiretroviral Therapy

A study was made to determine whether HIV-infected patients with prolonged virological control suffer subclinical neurocognitive problems capable of interfering with driving skills, compared with the general population, and to explore the possible existence of differences between those treated with and without efavirenz. Material and Methods: We included 40 patients without history of neoplasm, psychiatric disorders or infections of the central nervous system associated or not to HIV, with stable and effective antiretroviral therapy during at least 48 months. Use was made of the ASDE DRIVER TEST N-845 standardized by the Spanish traffic authorities, and for which data corresponding to the Spanish general population were obtained from the manufacturer of the test battery. The Student t-test was used to compare the different variables with the population standards, and the comparison of proportions Z-statistic was used to determine the proportion of subjects above the accepted limit of normality cutoff point. These analyses were replicated for the two sub-samples (with or without efavirenz therapy), with a 95% confidence level. The SPSS version 15 statistical package and Epidat 3.1 program were used. Results: The scores obtained in the HIV group were significantly poorer in the anticipation speed tests and in one of the multiple reactions test, though better results were obtained in the bimanual visual-motor coordination test. There were no differences in the percentages of patients with scores below the recommended limits. On comparing the treatment subgroups (efavirenz versus protease inhibitor), no differences were recorded in any of the study variables, and the differences with respect to the general population were the same as those described for the global group. Purification, Immobilization and Characterization of Lipase Isoenzyme from <i>Aspergillus niger</i>with C8 Magnetic Particles

The purification of a lipase isoenzyme from an Aspergillus niger lipase A is reported in this manuscript. Purification was carried out in a simple adsorption step, in which the lipase was offered at low ionic strength to the commercially available C8 modified magnetic particles, MaKProt C8. When the isoenzyme was desorbed with a 0.2% solution of Triton X-100, the SDS-PAGE gel showed a single pure band with a molecular weight of 35 KDa. The purified fraction showed 66.75-fold purification compared with the crude extract. The pure fraction was characterized along with the crude extract and the lipase adsorbed on the MaKProt C8. The purified and the adsorbed lipase showed better activity for the tested substrates (p-nitrophenyl acetate, decanoate, myristate and palmitate) than the crude extract, the preferred substrates being myristate (26.7 μmol·min-1·mg-1) and decanoate (17.4 μmol·min-1·mg-1), respectively. The temperature and pH profiles showed no change for the three enzymes, the optimum temperature being 37°C and the best pH 7.0.

Association of objectively measured physical activity and sedentary time with health-related quality of life in women with fibromyalgia:The al-ándalus project

Purpose: To examine the association of physical activity(PA) intensity levels and sedentary time with health-related quality of life(HRQoL) in women with fibromyalgia and whether patients meeting the current PA guidelines present better ***: This cross-sectional study included 407 women with fibromyalgia aged 51.4 ± 7.6 years. The time spent(min/day) in different PA intensity levels(light, moderate, and moderate-to-vigorous physical activity(MVPA) and sedentary time were measured with triaxial accelerometry. The proportion of women meeting the American PA recommendations(>150 min/week of MVPA in bouts >10 min) was also *** domains(physical function, physical role, bodily pain, general health, vitality, social functioning, emotional role, and mental health), as well as physical and mental components, were assessed using the 36-item Short-Form Health ***: All PA intensity levels were positively correlated with different HRQoL dimensions(r_(partial) between 0.10 and 0.23, all p < 0.05). MVPA was independently associated with social functioning(p < 0.05). Sedentary time was independently associated with physical function, physical role, bodily pain, vitality, social functioning, and both the physical and mental component summary score(all p < 0.05). Patients meeting the PA recommendations presented better scores for bodily pain(mean = 24.2(95%CI: 21.3-27.2) vs. mean = 20.4(95%CI: 18.9-21.9), p = 0.023)and better scores for social functioning(mean = 48.7(95%CI: 43.9-44.8) vs. mean = 42.3(95%CI: 39.8-44.8), p = 0.024).Conclusion: MVPA(positively) and sedentary time(negatively) are independently associated with HRQoL in women with fibromyalgia. Meeting the current PA recommendations is significantly associated with better scores for bodily pain and social functioning. These results highlight the importance of being physically active and avoiding sedentary behaviors in this population.

Effects of a Cardiac Rehabilitation Program in Patients with Congenital Heart Disease

The beneficial effects of the programs of cardiac rehabilitation in adults are widely known. Nevertheless, the accomplishment of these programs in patients with congenital heart is scarcely developed. In this study, we want to evaluate the preliminary results of a program of cardiac rehabilitation. Patients and methods: Thirty-three patients aged 8 to 40 completed the rehabilitation program. Thirty-one of them had an operated congenital heart disease, one had an implantable cardioverter defibrillator, and another was a patient with arterial hypertension and obesity. Data before and after the program were compared. Results: Improvements were found in all of the measured parameters. We found a statistically significant difference in maximum heart rate, exercise duration, foreseen heart rate’s percentage and increase of metabolic equivalents. Twenty-seven of 33 patients noticed a significant improvement in their quality of life at the end of the program. Conclusion: Initial results seem to be encouraging and we consider the experience very positive since the program achieved an improvement in the functional capacity of our patients. In our opinion, these observations and the results of our study provide strong arguments for the incorporation of cardiac rehabilitation into the care of many patients with congenital heart disease.

The present work reports the discovery and the complete characterisation of an ancient cultivated rose variety found growing in a private garden in the southwest of the Principality of Asturias(northern Spain).The variety is here given the name *** majority of roses currently cultivated belong to the so-called group of‘Modern Roses’,all of which were obtained after 1867 via artificial crosses and improvement *** are destined for ornamental *** the 19th century,the great majority of the many ancient cultivated roses in Europe were used in perfumery and cosmetics,or had medicinal *** damascena and Rosa centifollia are still grown and used by the French and Bulgarian perfume *** Asturian Massif of the Cantabrian Mountain Range provides a natural habitat for some 75%of the wild members of the genus Rosa,but until now there was no evidence that this area was home to ancient cultivated roses.A complete botanical description is here provided for a discovered ancient *** is also characterised according to a series of sequence tagged microsatellite sites,and its agronomic features are *** addition,a histological description(optical and scanning electronic microscope studies)of the petals is offered,along with an analysis of the volatile compounds present in these organs as determined by solid phase microextraction and gas chromatography-mass *** results reveal the uniqueness of this ancient type of rose and suggest it may be of interest to the perfume industry.

Spatial pattern of landscape changes and consequence changes in species diversity between 1956-1999 of Pinus halepensis Miller plantations in Montes de Malaga State Park (Andalusia, Spain)

Changes in forest diversity and the transforma- tion of coniferous plantations into more natural woodlands are of special interest in the Medi- terranean. The aim of this study was to investi- gate changes in land use patterns and vegeta- tion cover from 1956 to 1999 in Pinus halepensis Miller plantations in Montes de Málaga State Park in southern Spain. At the landscape level, a FRAGSTAT analysis was performed using aerial photographs. At the community level, a total of 47 sample plots were analyzed. Field measure- ments indicated the presence of a total of 70 species in the different vegetation types sam- pled. Shannon-Wiener (H), and Simpson’s (D) diversity indices showed the greatest species richness in Aleppo pine plantations (H = 1.54 ± 0.2;D = 3.88 ± 0.6), while Holm oak stands in the forested areas (H = 1.37 ± 0.1;D = 3.35 ± 0.6) and agricultural land (H = 1.33 ± 0.01;D = 2.80 ± 0.03) had lower values. FRAGSTATS analyses at land- scape and community levels indicated increases in patch richness, patch number, and percent of landscape mainly for pine plantations, Holm oak forests and shrub habitats. Replacement of agri- culture by conifer plantations led to the homo- genization of landscape structure. This change has modified the landscape by facilitating the recovery of indigenous tree species including Holm oak, which will likely lead to the future establishment of native forests in this area. Our findings suggest that pine plantations could fa- vour woody species diversity by increasing abundance and species richness in contrast to the situation in non-forested agricultural land.

In the treatment of patients with symptomatic cholelithiasis and choledocholithiasis (CBDS) detected during intraoperative cholangiography (IOC),or when the preoperative study of a patient at intermediate risk for CBDS cannot be completed due to the lack of imaging techniques required for confirmation,or if they are available and yield contradictory radiological and clinical results,patients can be treated using intraoperative endoscopic retrograde cholangiopancreatography (ERCP) during the laparoscopic treatment or postoperative ERCP if the IOC finds *** choice of treatment depends on the level of experience and availability of each option at each *** ERCP has the advantage of being a single-stage treatment and has a significant success rate,an easy learning curve,low morbidity involving a shorter hospital stay and lower costs than the two-stage treatments (postoperative and preoperative ERCP).Intraoperative ERCP is also a good salvage treatment when preoperative ERCP fails or when total laparoscopic management also fails.
