Risk scores for allograft failure: Are they still useful in liver recipients from donation after circulatory death?

BACKGROUND Liver grafts from donation after circulatory death(DCD)are associated with a higher risk of early graft dysfunction,determined by the warm ischemia and cold ischemia *** is essential to have precise criteria to identify this complication in order to guide therapeutic *** To validate different graft and recipient survival scores in patients undergoing liver transplantation(LT)with DCD *** A retrospective and observational unicentric study was conducted on 65 LT patients with grafts obtained from controlled DCD donors from November 2013 to November *** United Kingdom(UK)risk score,early allograft dysfunction(EAD)Olthoff score,and model for early allograft function(MEAF)score were used to evaluate the risk of graft and recipient survival *** survival analysis purposes,we used the Kaplan-Meier method,and the differences between subgroups were compared using the log-rank(Mantel-Cox)*** Sixty-five patients were included in the *** UK risk score did not demonstrate predictive capacity for recipient or graft ***,in donors aged over 70 years old(18.4%),it significantly predicted graft survival(P<0.05).According to Kaplan-Meier survival curves,graft survival rates at 6 months,2 years,and 5 years in the futility group dramatically decreased to 50%compared to the other groups(log-rank 8.806,P<0.05).The EAD Olthoff and MEAF scores did not demonstrate predictive capacity for recipient or graft *** on Kaplan-Meier survival curves,patients with a MEAF score≥7 had a lower graft survival rate at 6 months,2 years,and 5 years compared to patients with a lower MEAF score(log-rank 4.667,P<0.05).CONCLUSION In our series,both UK DCD risk score and MEAF score showed predictive capability for graft survival.

Association of objectively measured physical activity and sedentary time with health-related quality of life in women with fibromyalgia:The al-ándalus project

Purpose: To examine the association of physical activity(PA) intensity levels and sedentary time with health-related quality of life(HRQoL) in women with fibromyalgia and whether patients meeting the current PA guidelines present better ***: This cross-sectional study included 407 women with fibromyalgia aged 51.4 ± 7.6 years. The time spent(min/day) in different PA intensity levels(light, moderate, and moderate-to-vigorous physical activity(MVPA) and sedentary time were measured with triaxial accelerometry. The proportion of women meeting the American PA recommendations(>150 min/week of MVPA in bouts >10 min) was also *** domains(physical function, physical role, bodily pain, general health, vitality, social functioning, emotional role, and mental health), as well as physical and mental components, were assessed using the 36-item Short-Form Health ***: All PA intensity levels were positively correlated with different HRQoL dimensions(r_(partial) between 0.10 and 0.23, all p < 0.05). MVPA was independently associated with social functioning(p < 0.05). Sedentary time was independently associated with physical function, physical role, bodily pain, vitality, social functioning, and both the physical and mental component summary score(all p < 0.05). Patients meeting the PA recommendations presented better scores for bodily pain(mean = 24.2(95%CI: 21.3-27.2) vs. mean = 20.4(95%CI: 18.9-21.9), p = 0.023)and better scores for social functioning(mean = 48.7(95%CI: 43.9-44.8) vs. mean = 42.3(95%CI: 39.8-44.8), p = 0.024).Conclusion: MVPA(positively) and sedentary time(negatively) are independently associated with HRQoL in women with fibromyalgia. Meeting the current PA recommendations is significantly associated with better scores for bodily pain and social functioning. These results highlight the importance of being physically active and avoiding sedentary behaviors in this population.

A novel porous and crystalline two-dimensional(2D)electrochemically active covalent organic framework(COF)based on ortho-quinone units has been prepared as an innovative approach towards the development of organic cathode materials with multiple redox sites as an efficient electrocatalyst for the oxygen reduction reaction(ORR).In contrast with most of the previously reported COFs as electrocatalysts for the ORR,the electrocatalytic application of this material towards ORR has been investigated without adding any metal or conductive supporting material and avoiding any additional carbonization ***,the electrochemical properties of the COF material have been compared with two analogue amorphous frameworks with similar chemical composition,which points out the important role of the enhanced crystallinity and porosity of the COF network in its superior performance as an electrocatalyst towards ORR.

Motility and Functional State of the Membrane of Caprine Capacitated Spermatozoa under Different Chemical Agents

The objective of this study was to test the use of a commercial extender (Triladyl) as a diluent in caprine semen refrigerated at 15℃, using caffeine (CF), heparin (HP), synthetic oviductal fluid (SOF) andtriladyl (TRY) as capacitating chemical agents at different times. Twenty ejaculates of caprine semen were collected using an artificial vagina. The ejaculates were diluted and refrigerated by three days. Evaluated the progressive motility (PM) and the functional state of the sperm plasma membrane trough fluorescent CTC staining, counting 200 spermatic cells (non-capacitated spermatozoa NCS, capacitated CS and reacting spermatozoa REA) of caprine in two capacitating agents: CAF and HEP;one culture medium:SOF and a commercial extender: TRY at 60, 120, 180 and 240 min of incubation, during 24, 48 and 72 hs. PM was high under TRY, and CS was high under the HEP treatment. TRY could be an alternative to capacitate caprine spermatozoa, keeping PM for a longer time than HEP or CAF.

Efficacy and safety of oral solution dosed misoprostol versus misoprostol vaginally in labour induction

Background: Labour induction is one of the most common medical procedures in obstetrics. The aim is to end the pregnancy when continuity is a risk to mother or fetus. Its main side effect is the increase in the cesarean rate, compared to spontaneous onset deliveries. On the other hand, mortality and morbidity in cesareans are higher. The most common pharmacological drugs used for induction are prostaglandins: dinoprostone and misoprostol. The “gold standard” for labour induction is vaginal misoprostol. The oral route is also effective and also has several benefits like faster onset and easear administration. In recent years several publications state that the administration of misoprostol oral solution, given in doses gradually, is associated with a lower cesarean and hyperstimulation rate than the cases where vaginal misoprostol has been used in pregnant women with unripe cervix. Furthermore, being its half life shorter, it may be very useful in case of uterine hyperstimulation and, probably, a high percentage of women prefer this oral administration to the vaginal one. The objective of this study is to compare the efficacy, safety and side effects on mother and fetus on use of oral versus vaginal administration for induction of labour for prolonged gestation (41 weeks) and premature rupture of membranes, both with live fetus. Methods/Design: Design: double blind controlled trial. Study population: Pregnant women whose labour will be induced due to premature rupture of membranes or prolonged gestation. Inclusion Criteria: 1) Bishop Test equal to or less than 7;2) Single pregnancy;3) Pregnancy at term (37 - 42 weeks);4) No history of uterine surgery;5) Cephalic presentation;6) Live fetus;7) No prostaglandins contraindications. Discussion: Nowadays induction rates are very high, ranging from 25% to 30% approximately. In these cases caesarean rates are higher than when the delivery starts spontaneously. Loss of genetic variability is an increasing challenge in tree breeding programs due to the repeated use of a reduced number of founder ***,in almond,little is known about the genetic variability in current breeding stocks,although several cases of inbreeding depression have been *** gain insights into the genetic structure in modern breeding programs worldwide,marker-verified pedigree data of 220 almond cultivars and breeding selections were *** coefficients,pairwise relatedness,and genetic contribution were calculated for these *** results reveal two mainstream breeding lines based on three cultivars:“Tuono”,“Cristomorto”,and“Nonpareil”.Descendants from“Tuono”or“Cristomorto”number 76(sharing 34 descendants),while“Nonpareil”has 71 *** mean inbreeding coefficient of the analyzed genotypes was 0.041,with 14 genotypes presenting a high inbreeding coefficient,over *** programs from France,the USA,and Spain showed inbreeding coefficients of 0.075,0.070,and 0.037,*** to their genetic contribution,modern cultivars from Israel,France,the USA,Spain,and Australia trace back to a maximum of six main founding *** the group of 65 genotypes carrying the S f allele for self-compatibility,the mean relatedness coefficient was 0.125,with“Tuono”as the main founding genotype(24.7%of total genetic contribution).The results broaden our understanding about the tendencies followed in almond breeding over the last 50 years and will have a large impact into breeding decision-making process *** current genetic variability is required in almond breeding programs to assure genetic gain and continuing breeding progress.
