enterococcus faecalis and enterococcus faecium rank among the leading causes of nosocomial bacteremia and urinary tract infections. They often persist on hospital surfaces due to their ability to withstand adverse environmental conditions (low or high temperatures, high pH, and high salinity). The global enterococcus faecalis-enterococcus faecium ratio is currently shifting towards enterococcus faecium. enterococci present variable levels of resistance to certain families of antibiotics. This is the case for aminoglycosides, beta-lactams and cephalosporins. In 2017, WHO ranked enterococci among priority pathogens for research and development of new antibiotics. The objective of our study was to determine the antibiotic resistance profile of enterococcus faecalis and enterococcus faecium isolates from urine and pleural fluid in two hospitals in Cameroon. This cross-sectional and analytic study was carried out between June to August 2023 on hospitalized and day patients in which a cytobacteriological test of urine and pleural fluid was done. The samples were inoculated on CLeD Agar for urine and on Chocolate + polyvitex and blood agar (prepared from Columbia agar) for pleural fluid samples and incubated at 37℃ for 18 to 24 hours. Identification of isolates was carried out using the API 20 STReP micro gallery (Biomerieux, France) and tested for antimicrobial susceptibility. The data on socio-demographical and potential risk factors were recorded using self-administered questionnaires and data from laboratory analyses of the specimen were collected in a data capture sheet. Potential risk factors associated with the presence of enterococci, were evaluated using the logistic regression in univariate and multivariate analysis. P value ered as significant. 自从1989年国际抗癫痫联盟(ILAe)最后批准的分类以来,ILAe已经更新了癫痫的分类,以反映我们在获得巨大科学进步后对癫痫及其基本机制理解方面的收获。作为执业医师的重要工具,在我们的思想中癫痫分类必须有恰当和动态的变化,并且应全面的翻译到全球所有地区。癫痫分类最初的目的是对患者进行诊断,但是对于癫痫研究、抗癫痫治疗的发展和全世界的交流也非常重要。这次新分类起源于2013年提交以供公开评论的一份草拟文件,该草拟文件在经过国际癫痫界几轮讨论广泛反馈后得以修订。此分类呈现了3个层次,首先是发作类型,假设患者已经有最新的2017年ILAe癫痫发作分类所确定的发作类型;诊断了发作类型后,下一步就是诊断癫痫类型,包括局灶性癫痫、全面性癫痫、全面性及局灶性癫痫两者兼有,以及分类不明的癫痫;第三层次是癫痫综合征,此处可以做出特定综合征的诊断。这一新分类在每一层次均包含了病因,强调在每一步诊断时都要考虑病因,因为病因将会对治疗产生重要影响。选择将病因分为6个亚组是基于其潜在的治疗因果关系。该分类引入了新的术语如"发育性和癫痫性脑病"等。"自限性"和"药物有效性"取代"良性"一词。希望这一新的框架将有助于提高21世纪癫痫的关爱和研究。

AIM： In the present study, antibody and peripheral blood mononuclear cells （PBMC） proliferative responses against hepatitis C virus （HCV） antigens were evaluated in HCV chronically infected patients. MeTHODS： Paired serum and PBMC samples were taken six months apart from 34 individuals, either treated or not, and tested by enzyme-linked immunosorbent assay （eLISA） and carboxyfluorescein succinimidyl ester staining. RSULTS： Over 70% of the patients showed specific IgG and IgM against capsid, e1 and NS3, while HVR-1 was recognized by half of the patients. An increase in the levels of the anti-capsid IgM （P = 0.027） and IgG （P = 0.0006） was observed in six-month samples, compared to baseline. Similarly, a significantly higher percent of patients had detectable IgA reactivity to capsid （P = 0.017） and NS3 （P = 0.005） after six months, compared to baseline. Particularly, IgA against structural antigens positively correlated with hepatic damage （P = 0.036）. IgG subclasses evaluation against capsid and NS3 revealed a positive recognition mediated by IgG1 in more than 80% of the individuals. On the contrary, less than 30% of the patients showed a positive proliferative response either of CD4＋ or CD8＋ T cells, being the capsid poorly recognized. CONCLUSION： These results confirm that while the cellular immune response is narrow and weak, a broad and vigorous humoral response occurs in HCV chronic infection. The observed correlation between IgA and hepatic damage may have diagnostic significance, although it warrants further confirmation.

Background and Aims:High-intensity interval training(HIIT)is a therapeutic option for people with nonalcoholic steatohepatitis(NASH).However,the perspectives and experiences of HIIT for people with NASH are unknown,limiting translation of *** explored the experiences and perspectives of both professionally supervised and self-directed HIIT in people with NASH and evaluated participant-reported knowledge,barriers,and enablers to commencing and sustaining ***:Twelve participants with NASH underwent 12 weeks of supervised HIIT(3 days/week,4×4 minutes at 85–95%maximal heart rate,interspersed with 3 minutes active recovery),followed by 12-weeks of selfdirected(unsupervised)***-on-one,semistructured participant interviews were conducted by exercise staff prior to HIIT and following both supervised and self-directed HIIT to explore prior knowledge,barriers,enablers,and outcomes at each *** were audio-recorded,transcribed,coded,and thematically analyzed by two independent ***:Four dominant themes were identified:(1)no awareness of/experience with HIIT and ambivalence about exercise capabilities;(2)multiple medical and social barriers to commencing and continuing HIIT;(3)exercise specialist support was a highly valued enabler,and(4)HIIT was enjoyed and provided holistic ***:People with NASH may lack knowledge of and confidence for HIIT,and experience multiple complex barriers to commencing and continuing *** specialist support is a key enabler to sustained *** factors need to be addressed in future clinical programs to augment the uptake and long-term sustainability of HIIT by people with NASH so they can experience the range of related benefits.

Objective:The dopaminergic nigrostriatal neurons(DA cells)in healthy people present a slow degeneration with aging,which produces cellular debris throughout *** 2%-5%of people present rapid cell degeneration of more than 50%of DA cells,which produces Parkinson's disease(PD).Neuroinflammation accelerates the cell degeneration and may be critical for the transition between the slow physiological and the rapid pathological degeneration of DA cells,particularly when it activates microglial cells of the medial forebrain bundle near dopaminergic *** synaptic debris produced by DA cell degeneration may trigger the parkinsonian neuroinflammation,this study investigated the removal of axonal debris produced by retrograde degeneration of DA cells,paying particular attention to the relative roles of astrocytes and ***:Rats and mice were injected in the lateral ventricles with 6-hydroxydopamine,inducing a degeneration of dopaminergic synapses in the striatum which was not accompanied by non-selective tissue damage,microgliosis or *** possible retrograde degeneration of dopaminergic axons,and the production and metabolization of DA-cell debris were studied with immunohistochemical methods and analyzed in confocal and electron microscopy ***:The selective degeneration of dopaminergic synapses in the striatum was followed by a retrograde degeneration of dopaminergic axons whose debris was found within spheroids of the medial forebrain *** spheroids retained mitochondria and most(e.g.,tyrosine hydroxylase,the dopamine transporter protein,and amyloid precursor protein)but not all(e.g.,α-synuclein)proteins of the degenerating dopaminergic *** showed initial(autophagosomes)but not late(lysosomes)components of autophagy(incomplete autophagy).These spheroids were penetrated by astrocytic processes of the medial forebrain bundle,which provided the lysosomes needed to continue the degradation of dopaminergic debri
