BACKGROUND Intrahepatic sarcomatoid chonalgiocarcinoma(s-CCC) is an extremely rare disease, accounting for less than 1% of hepatobiliary system malignancies, and its pathophysiology is not well known. AIM To evaluate the use of fully covered self-expandable metal stents(FCSEMSs) for pancreatic duct strictures in children with chronic *** Eight patients with refractory benign dominant stricture of the main pancreatic duct(MPD) were enrolled through chart reviews between December 2014 and June 2017 in a single center. Endoscopic retrograde cholangiopancreatography(ERCP) with placement of a 6-mm FCSEMS with dual flaps was performed. Endoscopic removal of FCSEMSs was performed with a snare or rat-tooth forceps. All procedures were performed by a pediatric gastroenterologist. For the assessment of outcomes, technical and clinical success, adverse events, and stent patency were evaluated *** The placement and removal of the FCSEMSs were successful in all 8 patients. Five patients were boys and 3 were girls. The median age at initial FCSEMS placement was 12 years(range, 5-18 years). The diameters of all the inserted stents were 6 mm, and the lengths were 4-7 cm. The median indwelling time was 6 mo(range, 3-10 mo). No pancreatic sepsis, pancreatitis, cholestasis, or mortality occurred. There was no proximal and distal migration. All subjects showed a patent stent. On follow-up ERCP, the mean diameter of the stricture improved from 1.1 mm to 2.8 mm(P < 0.05), whereas that of upstream dilation improved from 8.4 mm to 6.3 mm(P < 0.05).CONCLUSION This initial experience showed that temporary FCSEMS placement is feasible and safe for the management of refractory benign MPD stricture in children.

AIM To clarify the prevalence of occult hepatitis B virus(HBV) infection(OBI) and the association between OBI and liver disease progression, defined as development of liver cirrhosis or hepatocellular carcinoma(HCC), worsening of Child-Pugh class, or mortality in cases of chronic hepatitis C virus(HCV) infection. METHODS This prospective cohort study enrolled 174 patients with chronic HCV infection(chronic hepatitis, n = 83; cirrhosis, n = 47; HCC, n = 44), and evaluated disease progression during a mean follow-up of 38.7 mo. OBI was defined as HBV DNA positivity in 2 or moredifferent viral genomic regions by nested polymerase chain reaction using 4 sets of primers in the S, C, P and X open reading frame of the HBV genome. RESULTS The overall OBI prevalence in chronic HCV patients at enrollment was 18.4%, with 16.9%, 25.5% and 13.6% in the chronic hepatitis C, liver cirrhosis and HCC groups, respectively(P = 0.845). During follow-up, 52 patients showed disease progression, which was independently associated with aspartate aminotransferase > 40 IU/L, Child-Pugh score and sustained virologic response(SVR), but not with OBI positivity. In 136 patients who were not in the SVR state during the study period, OBI positivity was associated with neither disease progression, nor HCC development. CONCLUSION The prevalence of OBI in chronic HCV patients was 18.4%, and OBI was not associated with disease progression in South Koreans.

The Yacon tubers extracts,chlorogenic acid and ferulic acid were administrated to rats once daily by oral gavage. The number of sperm heads in the testis was assessed by counting the number of sperm heads using slight modifica- tion of the procedure reported by Toth et al(1989). RESULTS:After administration of chlorogenic acid and Yacon tuber extracts to rat for 5 weeks,the number of sperm in epididymis were increased by 20.33% and 34.48%,*** femlic acid which was report- ed as a metabolite of chlorogenic acid and constituent of Yacon to estimate the spermatogenic activity was also ad- *** administration of ferulic acid and Yacon tubers extracts,the sperm number were increased by 37.87% and 43.33%,*** ef- fects of chlorogenic acid and ferulic acid were 0.59 times and 0.87 times of Yacon tubers extracts,respectively. CONCLUSION:The spermatogenic effect of Yacon tu- bers extracts may result from the combined effect of chloro- genic acid or its analogues and their metabolites.

Background In order to bind or fix bioactive materials directly to the surface of a Ti implant, the prior binding process of functional groups （FGs, -COOH and -OH） to the implant surface is necessary. Conventional binding processes are so high-cost and complex, so it is essential to find a simple and effective procedure for Ti-FG *** Various electrolyte compositions and electrochemical processing were adopted in this study to develop a relatively simple and effective Ti-FG binding process. The ability of Ti-FG binding and calcium （Ca）/phosphorous （P）absorption and corrosion resistance were evaluated according to various titanium surface treatment in electrolyte involving -COOH and -OH ion by using X ray photoelectron spectroscopy （XPS）, field emission scanning electron microscope （FE-SEM） and potentiodynamic scan method *** In cases of -COOH, the anodic oxidation process （AN） showed an effective binding ability between -COOH and Ti surface. On the other hand, in cases of -OH, there were no significant differences in the result between the conditions used. In regard to the absorption of Ca and P on Ti surface, there was a minimal amount of Ca absorbed but no P was absorbed. The anodic oxidation series showed homogenous corrosion, whereas the electrolyte immersion （EL）series showed unstable corrosion. Although EL-OH showed a novel corrosion potential, the EL-COOH series showed good corrosion resistance over the anodic potential *** The ability of binding between FG and the Ti surface and Ca/P absorption were strongly associated with the surface potential （ξ, potential）, which was dependent on the pH of the electrolyte. Accordingly, in order to achieve the effective absorption of various FGs on the Ti surface, it is needed to develop the combination process in addition to the electric affinity, relation with the ξ, potential.

Actinomycosis is an uncommon chronic infectious disease. Common sites of involvement include the cervicofacial, thoracic and abdominopelvic regions. In abdominopelvic actinomycosis, the ileocecal region, including the appendix, is the most commonly involved site. In some reports, limited appendiceal actinomycosis has revealed a thickened appendiceal wall with peri-appendiceal inflammation as acute appendicitis or perforated appendicitis. We experienced pathologically confirmed intraluminal limited appendiceal actinomycosis without peri-appendiceal infiltration. Here, we report the computed tomography and ultrasound findings.
