Background the time of onset of myocardial infarction shows seasonal and daily variation. We aimed to investigate whether the number of hours with daylight has an effect on the seasonal variation of heart attack, and whether the time of sunrise has an effect on the diurnal rhythm of myocardial infarction. Methods We carried out a retrospective database study covering all patients admitted to any acute care hospital with the diagnosis of myocardial infarction in Hungary between January 2004 and December 2005 （n=32 329）. Data were collected from the National Health Insurance Fund Administration （OEP） according to the International Classification of Diseases （ICD 121, 122） and National Meteorology Service （OMSZ）. In case of patients who occurred in the database several times the events have been considered as a separate case. Results With consideration to seasonal variation, the peak period of heart attack was found in the spring, with the lowest number of events in the summer. the number of hours with daylight showed a weak negative correlation with the occurrence of myocardial infarction （r=-0.108, P 〈0.05）. With respect to diurnal variation, the peak period of daily events was between 6-12 in the morning （35.57%）. We have found a positive correlation between the time of sunrise and sunset and the occurrence of myocardial infarction （P 〈0.01）. Conclusion Based on our findings, the number of hours with daylight and the time of sunrise may be connected with the chances of having heart attack; however other factors, such as high blood pressure, diabetes or smoking may also have an influence.

As the relevance between left and right brain neurons when transmitting electrical signals of umami taste is unknown,the aim of this work was to investigate responsive regions of the brain to the umami tastant monosodium glutamate(MSG)by using scalp-electroencephalogram(EEG)to identify the most responsive brain regions to *** concentrations of MSG(0.05,0.12,0.26 g/100 mL)were provided to participants for tasting while recoding their responsive reaction times and brain *** results indicated that the most responsive frequency to MSG was at 2 Hz,while the most responsive brain regions were T4 CzA2,F8 CzA2,and Fp2 ***,the sensitivity of the brain to MSG was significantly higher in the right brain *** study shows the potential of using EEG to investigate the relevance between different brains response to umami taste,which contributes to better understanding the mechanism of umami perception.

Mucotomy is one of the most frequently applied surgical techniques for the management of inferior turbinate hyperplasia. Mucotomy guarantees patent airway, however, it might lead to the emergence of sicca syndrome. In contrast, KTP (potassium titanyl phosphate) laser treatment spares the medial part of the inferior turbinate mucosa, contributing to maintenance of physiological nasal function. A retrospective comparative clinical study was performed to reveal the advantages and side-effects of both surgical methods in medium- and long-term in allergic rhinitis and non-allergic patients. Furthermore, we wished to determine the exact indications of the up-to-date laser treatment. Ninety-one of the 117 patients who underwent bilateral turbinate surgery during an 8-year period (2000-2007) responded to our questionnaire focusing on subjective postoperative changes. Patients were separated into 6 groups, based on the type of operation they underwent, the length of the follow-up and whether they suffered from allergies. the major complaint, nasal obstruction, improved in all 6 groups, which reached significance (p ? 0.05) in 4 groups. the most pronounced improvement was observed in the group of non-allergic patients with medium-term follow-up who underwent mucotomy. Nevertheless, a serious side- effect: crusting also increased significantly (p ? 0.05) in the latter group, while it was absent in allergic patients with medium-term follow-up, who underwent mucotomy. these results lead us to propose the following protocol for the treatment of inferior turbinate hyperplasia: 1) after unsuccessful conservative treatment, laser treatment is suggested for non-allergic patients;2) following unsuccessful conservative and even repeated laser treatment in the allergic group, mucotomy or turbinoplasty should be attempted.

AIM： To characterize H^＋ and HCO3^- transporters in polarized CFPAC-1 human pancreatic duct cells, which were derived from a cystic fibrosis patient with the AF508 CFTR mutation. MEthODS： CFPAC-1 cells were seeded at high density onto permeable supports and grown to confluence. the cells were loaded with the pH-sensitive fluorescent dye BCECF, and mounted into a perfusion chamber, which allowed the simultaneous perfusion of the basolateral and apical membranes. Transmembrane base flux was calculated from the changes in intracellular pH and the buffering capacity of the cells. RESULTS： Our results showed differential permeability to HCO3^＋/CO2 at the apical and basolateral membranes of CFPAC-1 cells. Na^＋/HCO3^- co-transporters （NBCs） and Cl^-/HCO3^- exchangers （AEs） were present on the basolateral membrane, and Na＋/H＋ exchangers （NHEs） on both the apical and basolateral membranes of the cells. Basolateral HCO3 uptake was sensitive to variations of extracellular K^＋ concentration, the membrane permeable carbonic anhydrase （CA） inhibitors acetazolamide （100 μmol/L） and ethoxyzolamide （100 μmol/L）, and was partially inhibited by H2-DIDS （600 μmol/L）. the membrane-impermeable CA inhibitor 1-N-（4-sulfamoylphenylethyl）-2,4,6-trimethylpyridine perchlorate did not have any effect on HCO3^- *** basolateral AE had a much higher activity than that in the apical membrane, whereas there was no such difference with the NHE under resting conditions. Also, 10 μmol/L forskolin did not significantly influence Cl^-/HCO3^- exchange on the apical and basolateral membranes. the administration of 250 μmol/L H2-DIDS significantly inhibited the basolateral AE. Amiloride （300 μmol/L） completely inhibited NHEs on both membranes of the cells. RT-PCR revealed the expression of pNBC1, AE2, and NHE1 mRNA. AIM： To establish whether there are fundamental differences in the biochemistries of adenocarcinomas of the gastroesophageal junction （GEJ） and the squamous cell carcinomas of the lower third of the esophagus （LTE）. MEthODS： Between February 1, 1997 and February 1, 2000, we obtained tissue samples at the moment of resection from 54 patients for biochemical analysis. the full set of data could be comprehensively analyzed in 47 of 54 patients＇ samples （81%）. Of these, 29 were adenocarcinomas of the GEJ Siewert type Ⅰ （n = 8）, type Ⅱ （n = 12）, type Ⅲ （n = 9）, and 18 presented as squamous cell carcinomas of the LTE. We evaluated the mean values of 11-lysosomal enzyme and 1-cytosol protease activities of the tumorous and surrounding mucosae as well as their relative activities, measured as the ratio of activity in tumor and normal tissues from the same patient. these data were further analyzed to establish the correlation with tumor localization, TNM stage （lymph-node involvement）, histological type （papillary, signet-ring cell, tubular）, state of differentiation （good, moderate, poor）, and survival （≤24 or ≥24 mo）. RESULTS： In adenocarcinomas, the activity of α-mannosidase （AMAN）, cathepsin B （CB） and dipeptidyl-peptidase Ⅰ （DPP Ⅰ） increased significantly as compared to the normal gastric mucosa. In squamous cell carcinomas of the esophagus, we also found a significant difference in the activity of cathepsin L and tripeptidyl-peptidase Ⅰ in addition to these three. there was a statistical correlation of AMAN, CB, and DPP Ⅰ activity between the level of differentiation of adenocarcinomas of the GEJ and lymph node involvement,because tumors with no lymph node metastases histologically confirmed as well-differentiated, showed a significantly lower activity. the differences in CB and DPP Ⅰ activity correlated well with the differences in survival rates, since the CB and DPP Ⅰ values of those who died within 24 mo following surgical intervention were significantly higher tha

Climate research produces a wealth of multivariate data. these data often have a geospatial reference and so it is of interest to show them within their geospatial context. One can consider this configuration as a multifield visualization problem, where the geo-space provides the expanse of the field. However, there is a limit on the amount of multivariate information that can be fit within a certain spatial location, and the use of linked multivariate information displays has previously been devised to bridge this gap. In this paper we focus on the interactions in the geographical display, present an implementation that uses Google Earth, and demonstrate it within a tightly linked parallel coordinates display. Several other visual representations, such as pie and bar charts are integrated into the Google Earth display and can be interactively manipulated. Further, we also demonstrate new brushing and visualization techniques for parallel coordinates, such as fixed-window brushing and correlation-enhanced display. We conceived our system with a team of climate researchers, who already made a few important discoveries using it. this demonstrates our system＇s great potential to enable scientific discoveries, possibly also in other domains where data have a geospatial reference.

Cardiovascular diseases(CVDs) continue to represent the number one cause of death and disability in industrialized countries. the most severe form of CVD is acute myocardial infarction(AMI), a devastating disease associated with high mortality and disability. In a substantial proportion of patients who survive AMI, loss of functional cardiomyocytes as a result of ischaemic injury leads to ventricular failure, resulting in significant alteration to quality of life and increased mortality. therefore, many attempts have been made in recent years to identify new tools for the regeneration of functional cardiomyocytes. Regenerative therapy currently represents the ultimate goal for restoring the function of damaged myocardium by stimulating the regeneration of the infarcted tissue or by providing cellsthat can generate new myocardial tissue to replace the damaged tissue. Stem cells(SCs) have been proposed as a viable therapy option in these cases. However, despite the great enthusiasm at the beginning of the SC era, justified by promising initial results, this therapy has failed to demonstrate a significant benefit in large clinical trials. One interesting finding of SC studies is that exosomes released by mesenchymal SCs(MSCs) are able to enhance the viability of cardiomyocytes after ischaemia/reperfusion injury, suggesting that the beneficial effects of MSCs in the recovery of functional myocardium could be related to their capacity to secrete exosomes. Ten years ago, it was discovered that exosomes have the unique property of transferring miRNA between cells, acting as miRNA nanocarriers. therefore, exosomebased therapy has recently been proposed as an emerging tool for cardiac regeneration as an alternative to SC therapy in the post-infarction period. this review aims to discuss the emerging role of exosomes in developing innovative therapies for cardiac regeneration as well as their potential role as candidate biomarkers or for developing new diagnostic tools.

Metabolic syndrome(Met S), as a chronic inflammatory disorder has a potential role in the development of inflammatory and cancerous complications of the colonic tissue. the interaction of DNA damage and inflammation is affected by the insulin-like growth factor 1 receptor(IGF1 R) signaling pathway. the IGF1 R pathway has been reported to regulate autophagy, as well, but sometimes through a bidirectional context. Targeting the IGF1 R-autophagy crosstalk could represent a promising strategy for the development of new antiinflammatory and anticancer therapies, and may help for subjects suffering from Met S who are at increased risk of colorectal cancer. However, therapeutic responses to targeted therapies are often shortlived, since a signaling crosstalk of IGF1 R with other receptor tyrosine kinases or autophagy exists, leading to acquired cellular resistance to therapy. From a pharmacological point of view, it is attractive to speculate that synergistic benefits could be achieved by inhibition of one of the key effectors of the IGF1 R pathway, in parallel with the pharmacological stimulation of the autophagy machinery, but cautiousness is also required, because pharmacologic IGF1 R modulation can initiate additional, sometimes unfavorable biologic effects.
