Sporadic ganglioneuromatosis of esophagogastric junction in a patient with gastro-esophageal reflux disorder and intestinal metaplasia

A 58-year-old female with a recurrent history of upper abdominal pain and intermittent dysphagia underwent endoscopic evaluation that demonstrated an irregular and nodular esophago-gastric （EG） junction and grade I erosive esophagitis. Biopsies showed prominent intestinal metaplasia of Barrett＇s type without dysplasia, chronic inflammation and multiple aggregates of large cells within the mucosal lamina propria, some with spindle shaped nuclei. Immunohistochemistry stains for keratins AE-1/ AE-3 were negative, while S-100 and NSE were positive. This, together with routine stains, was diagnostic for mucosal ganglioneuromatosis. The background of chronic inflammation with intestinal type metaplasia was consistent with long-term reflux esophagitis. No evidence of achalasia was seen. Biopsies of gastric antrum and fundus were unremarkable, without ganglioneural proliferation. Colonoscopy was unremarkable. No genetic syndromes were identified in the patient including familial adenomatous polyposis and multiple endocrine neoplasia type Ⅱ b （MEN Ⅱ b）. Iansoprazole （Prevacid） was started by oral administration each day with partial relief of symptoms. Subsequent esophagogastroscopy repeated at 4 mo showed normal appearing EG junction. Esophageal manometry revealed a mild nonspecific lower esophageal motility disorder. Mild motor dysfunction is seen with gastro-esophageal reflux disease （GERD） and we feel that the demonstration of localized ganglioneuromatosis was not likely related etiologically. In the absence of findings that might suggest neural hypertrophy, such as achalasia, the nodular mucosal irregularity seen with this instance of ganglioneuromatosis may, however, have exacerbated the patient＇s reflux.

Improvement of Surface Mechanical and Tribological Characteristics of L-PBF Processed Commercially Pure Titanium through Ultrasonic Impact Treatment

Multi-pass ultrasonic impact treatment(UIT)was applied to modify the microstructure and improve the mechanical and tribological characteristics at the near-surface region of commercially pure Ti(CP-Ti)specimens produced by the laser powder bed fusion(L-PBF)*** considerably refined the L-PBF process-related acicular martensites(α′-M)and produced a well-homogenized and dense surface microstructure,where the porosity content of 1-,3-,and 5-pass UITed samples was reduced by 43,60,and 67%,*** UITed samples showed an enhancement in their near-surface mechanical properties up to a depth of about 300μ*** nanoindentation results for the 3-pass UITed sample revealed an increase of about 53,45,and 220%in its nanohardness,H/E_(r),and H_(3)/E_(r)^(2)indices,*** stylus profilometry results showed that performing the UIT removed the L-PBF-related features/defects and offered a smooth *** roughness average(R_(a))and the skewness(R_(sk))of the 3-pass UITed sample were found to be lower than those of the L-PBFed sample by 95 and 223%,*** the UIT also enhanced the material ratio,where the maximum load-bearing capacity(~100%)in as-L-PBFed(as-built)and 3-pass UITed samples was obtained at 60-and 10-µm depths,*** tribological investigations showed that applying the UIT resulted in a significant reduction of wear rate and average coefficient of friction(COF)of *** instance,under the normal pressures of 0.05 and 0.2 MPa,the wear rate and COF of the 3-pass UITed sample were lower than those of the L-PBFed sample by 65 and 58%,and 20 and 17%,respectively.

Enhanced axonal regeneration and functional recovery of the injured sciatic nerve in a rat model by lithium-loaded electrospun nanofibrous scaffolds

Increasing evidence indicates that engineered nerve grafts have great potential for the regeneration of peripheral nerve injuries (PNIs). While most studies have focused only on the topographical features of the grafts, we have considered both the biophysical and biochemical manipulations in our applied nanoscaffold. To achieve this, we fabricated an electrospun nanofibrous scaffold (ENS) containing polylactide nanofibers loaded with lithium (Li) ions, a Wnt/beta-catenin signaling activator. In addition, we seeded human adipose-derived mesenchymal stem cells (hADMSCs) onto this engineered scaffold to examine if their differentiation toward Schwann-like cells was induced. We further examined the efficacy of the scaffolds for nerve regeneration in vivo via grafting in a PNI rat model. Our results showed that Li-loaded ENSs gradually released Li within 11 d, at concentrations ranging from 0.02 to (3.64 +/- 0.10) mmol/L, and upregulated the expression of Wnt/beta-catenin target genes (cyclinD1 and c-Myc) as well as those of Schwann cell markers (growth-associated protein 43 (GAP43), S100 calcium binding protein B (S100B), glial fibrillary acidic protein (GFAP), and SRY-box transcription factor 10 (SOX10)) in differentiated hADMSCs. In the PNI rat model, implantation of Li-loaded ENSs with/without cells improved behavioral features such as sensory and motor functions as well as the electrophysiological characteristics of the injured nerve. This improved function was further validated by histological analysis of sciatic nerves grafted with Li-loaded ENSs, which showed no fibrous connective tissue but enhanced organized myelinated axons. The potential of Li-loaded ENSs in promoting Schwann cell differentiation of hADMSCs and axonal regeneration of injured sciatic nerves suggests their potential for application in peripheral nerve tissue engineering. Experimental arena settings might lead to misinterpretation of movement properties

Movement is an important animal behavior contributing to reproduction and *** movement is often examined in arenas or enclosures under laboratory *** used the red flour beetle(Tribolium castaneum)to examine here the effect of the arena size,shape,number of barriers,access to the arena's center,and illumination on six movement *** demonstrate great differences among *** example,the beetles moved over longer distances in clear arenas than in obstructed *** along the arena's perimeter was greater in smaller arenas than in larger *** was more directional in round arenas than in rectangular *** general,the beetles stopped moving closer to the perimeter and closer to corners(in the square and rectangular arenas)than expected by *** some cases,the arena properties interacted with the beetle sex to affect several movement *** these suggest that arena properties might also interact with experimental manipulations to affect the outcome of studies and lead to results specific to the arena *** other words,instead of examining animal movement,we in fact examine the animal interaction with the arena *** is therefore advised in interpreting the results of studies on movement in arenas under laboratory conditions and we recommend paying attention also to barriers or obstacles in field *** instance,movement along the arena's perimeter is often interpreted as centrophobism or thigmotaxis but the results here show that such movement is arena dependent.

Enhancing Identity Protection in Metaverse-Based Psychological Counseling System

Non-face-to-face psychological counseling systems rely on network technologies to anonymize information regard-ing client ***,these systems often face challenges concerning voice data leaks and the suboptimal communication of the client’s non-verbal expressions,such as facial cues,to the *** study proposes a metaverse-based psychological counseling system designed to enhance client identity protection while ensuring efficient information delivery to counselors during non-face-to-face *** proposed systemincorporates a voicemodulation function that instantlymodifies/masks the client’s voice to safeguard their ***,it employs real-time client facial expression recognition using an ensemble of decision trees to mirror the client’s non-verbal expressions through their avatar in the metaverse *** system is adaptable for use on personal computers and smartphones,offering users the flexibility to access metaverse-based psychological counseling across diverse *** performance evaluation of the proposed system confirmed that the voice modulation and real-time facial expression replication consistently achieve an average speed of 48.32 frames per second or higher,even when tested on the least powerful smartphone ***,a total of 550 actual psychological counseling sessions were conducted,and the average satisfaction rating reached 4.46 on a 5-point *** indicates that clients experienced improved identity protection compared to conventional non-face-to-face metaverse counseling ***,the counselor successfully addressed the challenge of conveying non-verbal cues from clients who typically struggled with non-face-to-face psychological *** proposed systemholds significant potential for applications in interactive discussions and educational activities in the metaverse.

Efficacy of tadalafil on improvement of men with erectile dysfunction caused by COVID-19:A randomized placebo-controlled trial

Objective: According to the high prevalence of COVID-19 and the subsequent risk of men's sexual health, we decided to investigate the efficacy of tadalafil on improvement of men with erectile dysfunction caused by ***: In this study, 70 outpatients who were recovered from COVID-19 without acute respiratory distress syndrome with negative polymerase chain reaction test and a complaint of erectile dysfunction were divided into two groups: 35 patients who received tadalafil 5 mg daily and 35 who received placebo. For each patient, basic assessment of sexual function was performed using the 5-item version of the International Index of Erectile Function (IIEF-5) questionnaire. Then, treatment was started from 2 months after complete recovery of COVID-19 with negative polymerase chain reaction test for 3 months. At the end of the treatments, the patients were re-evaluated for sexual function using the complete version of IIEF questionnaire. Finally, the results before and after treatment in the intervention group were compared with those of the control ***: Treatment with both tadalafil and placebo improved the patients' sexual function criteria compared to the baseline. However, this improvement was significantly higher in the intervention group with tadalafil than the control group with placebo (p<0.05).Conclusion: Daily administration of tadalafil 5 mg seems to be effective and safe for improvement of erectile dysfunction caused by COVID-19.

Defining the association between the prolonged operative time and 90-day complications in patients undergoing radical cystectomy

Objective:Radical cystectomy is a complex lengthy procedure associated with postoperative *** aimed to assess the operative time(OT)in patients undergoing radical cystectomy and its impact on 90-day postoperative complications and readmission ***:The retrospective cohort study included 296 patients undergoing radical cystectomy and urinary diversion from May 2010 to December 2018 in our *** OT of 369 min was set as a cutoff value between short and long OT *** primary outcome was 90-day postoperative complication *** outcomes were gastrointestinal recovery time,length of hospital stay,and 90-day readmission ***:The overall incidence of 90-day postoperative complications was 79.7%where 43.2%representing low-grade complications according to the ClavieneDindo classification(Grade 1 and Grade 2),and 36.5%representing high-grade complications(Grade3).Gastrointestinal tract and infectious complications are the most common complications in our data set(45.9%and 45.6%,respectively).On multivariable analysis,prolonged OT was significantly associated with odds of high-grade complications(odds ratio 2.340,95%confidence interval 1.288e4.250,p=0.005).After propensity score-matched analysis,a higher incidence of major complications was identified in the long OT group 55(51.4%)compared to 35(32.7%)in the short OT group(p=0.006).A shorter gastrointestinal tract recovery time was noticed in the short OT group(p=0.009).Prolonged OT was associated with a higher 90-day readmission rate on univariate and multivariate analyses(p<0.001,p=0.001,respectively).

Automated Extraction and Analysis of CBC Test from Scanned Images

Health care is an important part of human life and is a right for everyone. One of the most basic human rights is to receive health care whenever they need it. However, this is simply not an option for everyone due to the social conditions in which some communities live and not everyone has access to it. This paper aims to serve as a reference point and guide for users who are interested in monitoring their health, particularly their blood analysis to be aware of their health condition in an easy way. This study introduces an algorithmic approach for extracting and analyzing Complete Blood Count (CBC) parameters from scanned images. The algorithm employs Optical Character Recognition (OCR) technology to process images containing tabular data, specifically targeting CBC parameter tables. Upon image processing, the algorithm extracts data and identifies CBC parameters and their corresponding values. It evaluates the status (High, Low, or Normal) of each parameter and subsequently presents evaluations, and any potential diagnoses. The primary objective is to automate the extraction and evaluation of CBC parameters, aiding healthcare professionals in swiftly assessing blood analysis results. The algorithmic framework aims to streamline the interpretation of CBC tests, potentially improving efficiency and accuracy in clinical diagnostics.
